Valve Sparing Aortic Root Replacement in Children with Loeys-Dietz Syndrome

  • Loeys-Dietz syndrome (LDS) is an autosomal dominant connective tissue disorder that is characterized by aggressive arterial and aortic disease, often involving the formation of aortic aneurysms. We describe the cases of two children with LDS who were diagnosed with aortic root aneurysms and successfully treated by valve-sparing aortic root replacement (VSRR) with a Valsalva graft. VSRR is a safe and suitable operation for children that avoids prosthetic valve replacement.

Analysis of Elementary Students' Intake of Dietary Supplements (초등학생들의 일반적 특성에 따른 영양보충제 섭취실태 분석)

  • The principal objective of this study was to assess the current status of elementary students' intake of dietary supplements, in an effort to diagnose any overnutrition, and to employ the results as basic data for the establishment of a balanced nutritional education. A survey was conducted by using a questionnaire administered to 734 elementary students’ parents in Seoul, and the analyzed results were as follows: Most parents believed that their nutritional knowledge was better than the norm(91.5%). However, almost half of the parents(49.2%) were feeding certain dietary supplements to their children, despite their perception that their children were already healthy, and were not recognizing any significant beneficial effects from those supplements. The most popular dietary supplements being fed were vitamin complexes. The majority of children took at least one type of dietary supplements at a monthly cost of approximately 10,000${\sim}$30,000 won. These results revealed the potential for overnutrition in some of the elementary students as the result of dietary supplement intake; thus, further studies involving assessments of risk for dietary supplement overconsumption in children and proper dietary education for the parents are suggested.

Systematic Review and Meta-Analyses of Diagnostic Accuracy of Infrared Thermometer when Identifying Fever in Children (아동에서의 적외선 체온 측정의 진단정확도 연구 : 체계적 문헌 고찰과 메타분석)

  • Purpose: Infrared thermometers are increasingly used as a convenient, non-invasive assessment method for febrile children. However, the diagnostic accuracy of the infrared thermometer for children has been questioned, particularly in relation to sensitivity and specificity. The aim of this study was to evaluate diagnostic accuracy of infrared thermometers in febrile children. Methods: Articles published between 1966 and 2012 from periodicals indexed in the Ovid Medline, Embase, CINAHL, Cochrane, KoreaMed, NDSL, KERIS and other databases were selected, using the following keywords: 'infrared thermometer'. The QUADAS-II was applied to assess the internal validity of the diagnostic studies. Selected studies were analyzed using meta-analysis with MetaDisc 1.4. Results: Nineteen diagnostic studies with high methodological quality, involving 4,304 children, were included. The results of meta-analysis showed that the pooled sensitivity, specificity and AUC (Area Under the Curve) of infrared tympanic thermometers in children over 1 year were 0.80 (95% CI 0.78, 0.81), 0.94 (95% CI 0.93, 0.95) and 0.95 respectively. However the diagnostic accuracy of infrared tympanic thermometers in children with hyperthermia was low. Conclusion: The diagnostic accuracy of infrared tympanic thermometer was similar to axillary and rectal thermometers indicating a need for further research to substantiate these findings in children with hyperthermia.

The Effects of Mothers' Parenting Stress and Parental Satisfaction on Attachment Formation with Their Children (어머니의 양육스트레스 및 부모역할만족도가 0-3세 자녀와의 애착형성에 미치는 영향)

  • The purpose of this study was to examine the factors related to mothers' attachment formation with their children. For this research, mothers' attachment formation with their children was examined based on the relationship involving three factors: personal variables, parenting stress, and parental satisfaction. A survey instrument was administered to 114 mothers who had 0~3 year-old children attending day care centers in Seoul. Data was analyzed using descriptive statistics and multiple regression using the SPSS+12 version. Major findings were as follows: First, the degree of mothers' parental stress rated relatively low and the degree of parental satisfaction rated relatively high. Among the three variables that were measured, parental satisfaction was the most influential factor on mothers' attachment formation. Finally, 'burden and stress of parenting', 'feelings of guilt for others taking care of their children', 'general satisfaction', and 'parent-child relationship satisfaction' were powerful factors in explaining attachment formation.

Development of a Violence Prevention Educational Program for Elementary School Children Using Empathy (VPEP-E)

  • Purpose: This study describes the development of a violence prevention educational program for elementary school children using empathy (VPEP-E) that teachers can use during class. Methods: Hoffman's theory of empathy and Seels and Richey's (1994) ADDIE model were applied to develop this program. Results: The developed program consisted of eight sessions: "Orientation/definition of violence and empathy", "Types and boundaries of violence", "Look into my feelings", "Say it with a facial expression", "Preventing non-empathic violence due to social prejudice", "Preventing physical violence", "Verbal and online violence prevention: empathic conversation", and "I can do well: review of the whole curriculum". The program was evaluated by 15 elementary school teachers, who considered it to be easily accessible to elementary school students. The final VPEP-E, which will be provided in eight times for 40 minutes each for fifth-grade students, will provide a basis for preventing violence by fostering empathy. Conclusion: We expect the developed educational program to be effective in preventing violence among elementary school students. However, further research involving children from various age groups is needed.

The Characteristics of Intellectual and Psychological in the Children with Moyamoya Disease (모야모야 질환 아동의 인지기능 및 정서적인 특성 분석)

  • Purpose: The purpose of this study was to understand the intellectual and psychological features of children with Moyamoya disease who were patients in the Department of Pediatric Neurosurgery of Severance Hospital. Methods: From December 2010 through December 2012, 63 patients with diagnoses of Moyamoya disease and 59 children in a normal group were enrolled. This study was conducted using the Korean-Wechsler Intelligence Scale for Children, Rey-Kim Memory Test for children, Korean Child Behavior Checklist and Pediatric Quality of Life Inventory$^{TM}$4.0. Results: The results showed that the intellectual and psychological profiles of children with Moyamoya disease were lower than the average of the normal control group. The tested patients showed significantly lower scores for Performance Intelligence Quotient cognition level. Also, in terms of quality of life, children with Moyamoya disease had lower levels of physical and school functionality. The results were in line with those of previous studies involving psychological tests of children with chronic diseases. Conclusion: Considering the intellectual and psychological characteristics of children with Moyamoya disease, integrated psychological intervention plans including elements such as supportive therapy for patients and programs for parental education are required.

Analysis of Research Trends in Papers Published in the Journal of Korean Academy of Child Health Nursing (2005-2009) (아동간호학회지 게재논문의 연구동향 분석(2005-2009년))

  • Purpose: The purpose of this study was to analyze papers published in the Journal of Korean Academy of Child Health Nursing (JKACHN) to identify the current state of child health nursing research and recommend directions for future research. Methods: Using analysis criteria developed by the researchers, 233 papers published in JKACHN between 2005 and 2009 were reviewed. Results: Both quantity and quality of these papers showed significant improvement compared to before 2005. The number of published papers, the proportion of intervention studies, and studies that involved children directly as participants had increased. However, published papers still leave much to be desired. Quantitative studies were dominant (91.4%) and many of these studies were survey designs (65.7%). Children, especially young children were still less likely to be direct participants. All experimental studies were quasi or pre-experimental studies. There were few qualitative research studies. Conclusion: Findings of this study indicate that strengthening peer review according to review guidelines will improve the quality of published papers and promote JKACHN as an international journal. Instead of proxy accounts from parents or nurses, research involving direct accounts by children is needed. Child-friendly data collection methods need to be developed and used by child health nurse researchers.

Effects of Computer-Based Cognitive Rehabilitation on the Attention of Children with Autism Spectrum Disorder: A Single-Subject Study (전산화 인지재활이 자폐스펙트럼장애 아동의 주의력에 미치는 효과: 단일대상연구 )

  • Objective : The purpose of this study was to investigate the effects of computerized cognitive rehabilitation on the attention of children with autism spectrum disorder (ASD). Methods : A multiple-baseline design was used in combination with a single-subject experimental research design. A total of 15 sessions were conducted three times a week, each lasting for 30 minutes. The maintenance and selective attention tests in the attention area of the computerized cognitive rehabilitation program were evaluated in random order after each session was completed. To evaluate the participants' selective attention, star cancellation tests was performed as pre- and post-tests. Results : After the computerized cognitive rehabilitation intervention, the accuracy of the participants' attention improved in terms of the maintenance and selective attention tests. In the analysis involving the two standard deviation bands, participants 1 and 2 showed a significant improvement, whereas participant 3 did not show a significant improvement, although the average value improved. Conclusion : The results of this study confirmed that cognitive rehabilitation involving a computerized cognitive rehabilitation program had a positive effect on the attention of three children with ASD.

Health Education Needs of Mothers who are Caring for Children with Disabilities (장애아동 양육을 위한 어머니의 건강관련 교육요구)

  • Purpose: The purpose of this study was to investigate the health education needs of mothers who are nurturing children with disabilities. Method: A descriptive study was done and the participants were 108 mothers of children with disabilities such being mentally challenged, developmentally delayed or having a disability involving brain damage. The questionnaire was a health education need assessment with 11 categories(58 items) developed by Han et al. The data were analyzed using SPSS program. Results: The mean score for health education needs of the mothers of children with disabilities was 3.83 (SD=0.58) out of a maximum 5. The health education need for acquisition of knowledge and information had the highest score (4.40±0.54) followed by health education needs for cognitive development and learning (4.31±0.64), interpersonal relationships (4.04±0.65) and behavior and emotion (4.04±0.79). There were significant differences between the children's sex (t=2.08, p=.04), birth order (t=2.17, p=.03), grade of disability (F=3.32, p=.02) and sex education suitable to the child's in age. Conclusion: The health education needs of mothers of children who are disabled were very high and varied. Therefore, it was important to develop comprehensive education programs which include this content and provide opportunities for mothers of children with disabilities to receive this education.

Clinical Applications of Functional Near-Infrared Spectroscopy in Children and Adolescents with Psychiatric Disorders

  • The purpose of this review is to examine the clinical use of functional near-infrared spectroscopy (fNIRS) in children and adolescents with psychiatric disorders. Many studies have been conducted using objective evaluation tools for psychiatric evaluation, such as predicting psychiatric symptoms and treatment responses. Compared to other tools, fNIRS has the advantage of being a noninvasive, inexpensive, and portable method and can be used with patients in the awake state. This study mainly focused on its use in patients with attention-deficit/hyperactivity disorder and autism spectrum disorder. We hope that research involving fNIRS will be actively conducted in various diseases in the future.
