Surgical Outcomes of Type A Aortic Dissection at a Small-Volume Medical Center: Analysis according to the Extent of Surgery

  • Background: Despite progress in treatment, Stanford type A aortic dissection is still a life-threatening disease. In this study, we analyzed surgical outcomes in patients with Stanford type A aortic dissection according to the extent of surgery at Daegu Catholic University Medical Center. Methods: We retrospectively analyzed 98 patients with Stanford type A aortic dissection who underwent surgery at our institution between January 2008 and June 2018. Of these patients, 82 underwent limited replacement (hemi-arch or ascending aortic replacement), while 16 patients underwent total arch replacement (TAR). We analyzed in-hospital mortality, postoperative complications, the overall 5-year survival rate, and the 5-year aortic event-free survival rate. Results: The median follow-up time was 48 months (range, 1-128 months), with a completion rate of 85.7% (n=84). The overall in-hospital mortality rate was 8.2%: 6.1% in the limited replacement group and 18.8% in the TAR group (p=0.120). The overall 5-year survival rate was 78.8% in the limited replacement group and 81.3% in the TAR group (p=0.78). The overall 5-year aortic event-free survival rate was 85.3% in the limited replacement group and 88.9% in the TAR group (p=0.46). Conclusion: The extent of surgery was not related to the rates of in-hospital mortality, complications, aortic events, or survival. Although this study was conducted at a small-volume center, the in-hospital mortality and 5-year survival rates were satisfactory.

Settlement Behavior of Soft Ground Reinforced by Stone Columns (쇄석말뚝으로 보강된 연약지반의 침하거동)

  • Stone columns is ground improvement method which is composed of compacted gravel or crushed stone inserted into the soft ground consisting of loose sand and clay by replacement method. Generally stone columns are constructed in silty clay, above 70% replacement rate for increasing the bearing capacity and shear strength. Low replacement stone columns method is limited below 30% at replacement rate-premising strength increase of clay ground is estimated efficiently. This study, laboratory model tests were conducted to investigate the consolidation drainage promotion and shear strength increase effect in soft ground with replacement rate by stone columns. The settlement reduction effect and settlement reduction coefficients increase with increasing the replacement rate in composite ground. The results of model tests indicate that consolidation promotion effect is proved. The increasing strength of composite ground was verified by vane shear tests.

  • This study has Performed to develop rehabilitation model for determination of optimal rehabilitation time on the Cast Iron Pipes (CIP) with diameter less than 300 mm in water pipeline network and to analysis the sensitivity on the rehabilitation times of developed model. In the result from the application of the field, the renovation time was faster about 10 years than the replacement time. Especially, as the difference between rehabilitation and replacement time on E-CIP was about from 3 to 5 years, and it was thought that the replacement was effective on E-CIP. To sensitivity analysis, the discount rate of coefficient was fixed at 0.08, and the values of initial year break rate(N($t_o$) and growth rate coefficient(A) were adjusted in values of 0.0009, 0.0018, 0.0027, and 0.05, 0.10, 0.15 respectively. When the values of N($t_o$) and A was increased, the results from the time of rehabilitation and replacement was faster It was thought that N($t_o$), 0.018 was reliable values on the applied pipeline through the result of the study. In case of A, the values of A above 0.1 was thought to be Proper.

Estimating Retirement Consumption Needs Using Target Replacement Rate (목표 소득대체율을 통한 은퇴소비의 추정)

  • This study estimates the consumption needs of preretired households through target replacement ratio approach. Based on the Life Cycle Model, this study used the household expenditure function to derive the target replacement ratio appropriate for each household. The target replacement ratio is estimated using the 1996 National Survey of family Income and Expenditure by National Statistical Office. The estimated target replacement ratio was 82.4% for married couple households, and 85.1% for single households. Total retirement consumption needs during entire retirement period was 161,620,000 won for married couple households, and 50,532,039 won for single households.

  • The effects of differences in the rate and composition of intravenous fluid replacement for urine loss on the pharmacokinetics and pharmacodynamics of torasemide were evaluated using rabbits as the animal model. Each rabbit received 2-h constant intravenous infusion of 1 mg/kg ∼ 1 of torasemide with 0% replacement (treatment I, n = 6), 50% replacement (treatment II, n = 9), and 100% replacement with lactated Ringer's solution (treatment III, n = 8) as well as with 100% replacement with 5% dextrose in water(D-5-W, treatment IV, n=6).   • In this paper, it is assumed that a system is composed of an essential unit and a nonessential unit. During the running of the system, an essential unit is replaced at periodic replacement time T or at nth failure of essential unit whichever occurs first. Nonessential unit is replaced at its failure and at the replacement of essential unit. This paper derive optimal replacement period which minmises the total expected cost for replacement. The unimodality of totoal maintenance cost function is proved under the assumption that hazard rate of each component is continuous and monotone increasing failure rate(IFR). Based on this condition, it is shown that the optimal replacement period is finite and unique.

  • Web cache, which is functionally another word of proxy server, is located between client and server. Web cache has a limited storage area although it has broad bandwidth between client and proxy server, which are usually connected through LAN. Because of limited storage capacity, existing objects in web cache can be deleted for new objects by some rules called replacement algorithm. Hit rate and byte-hit rate are general metrics to evaluate replacement algorithms. Most of the replacement algorithms do satisfy only one metric, or sometimes none of them. In this paper, we propose two replacement algorithms to achieve both high hit rate and byte-hit rate with great satisfaction. In the first algorithm, the cache is appropriately partitioned according to file types as a basic model. In the second algorithm, the cache is composed of two levels; the upper level cache is managed by the basic algorithm, but the lower level is collectively used for all types of files as a shared area. To show the performance of the proposed algorithms, we evaluate hit rate and byte-hit rate of the proposed replacement algorithms using the trace driven simulation.

Review of Changes in Mechanical Properties of Concrete According to Recycled Coarse Aggregate Replacement Rate_Case Study (순환 굵은 골재 치환율에 따른 콘크리트의 역학적 특성 변화 검토_사례 연구)

  • In this study, it was determined that it was necessary to consider the replacement rate when applying recycled coarse aggregate to concrete, so data on existing research trends and results were collected and the mechanical properties of concrete according to the replacement rate of recycled coarse aggregate were analyzed. In collecting data on recycled coarse aggregate, data without processes such as compressive strength and removal of residual mortar attached to recycled coarse aggregate were collected among the concrete measurement items. In the case of concrete with 50 % and 100 % replacement of recycled coarse aggregate, it was confirmed that the mechanical properties were lower or higher than ordinary concrete by -36.0 ~ 9.9 % and -40.0 ~ 10.4 %, respectively, on average. Accordingly, it is judged that additional water should be mixed in consideration of the absorption rate when mixing, and the replacement rate of recycled coarse aggregate, which has mechanical properties of 80 % or more compared to ordinary concrete, should be less than 50 %.

Wavelets have good time-frequency localization, fast algorithms and parsimonious representation.   • In this paper, we consider the periodic preventive maintenance model for a repairable system following the expiration of extended warranty under replacement-repair warranty. Under the replacement-repair warranty, the failed system is replaced or minimally repaired by the manufacturer at no cost to the user. Also, under extended warranty, the failed system is minimally repaired by the manufacturer at no cost to the user during the original extended warranty period. As a criterion of the optimality, we utilize the expected cost rate per unit time during the life cycle from the user's perspective. And then we determine the optimal preventive maintenance period and the optimal preventive maintenance number by minimizing the expected cost rate per unit time. Finally, the optimal periodic preventive maintenance policy is given for Weibull distribution case.
