Objective : Our purpose of this study is to compare insertion angles and screw lengths from Roy-Camille, Magerl, and our designed method for cervical lateral mass screw fixation in the Korean population by quantitative measurement of reformatted two dimensional (2D) computed tomography (CT) images. Methods : We selected thirty Korean patients who were evaluated with thin section CT scans and reconstruction program to obtain reformatted 2D-CT images of the transversal plane passing the cranio-caudal angle using three different techniques. We measured the minimum angle to avoid vertebral artery (VA) injury, the ideal angle and depth for bicortical screwing of cervical lateral mass. Morphometric measurements of the lateral masses from C3-C7 were also taken. Results : In all three techniques, the mean safety angles from the VA were less than 8 degrees and the necessary depth of the screw was about 14 mm for safety to the VA and for the bicortical purchase. In our designed technique, the mean $\beta$ angles of each level from C3 to C7 were 29.0. 29.8. 29.5. 26.3. and 23.9 degrees, respectively. Conclusion : Results of this study and data from the literature indicate that differences may exist between the Korean and Western people in the length and angle for ideal lateral mass screw fixation. In addition, our technique needs further cadaveric and clinical study for safety and efficacy for being performed as alternative method for cervical lateral mass fixation.
In this study, the images of specific prompt gamma (PG)-rays of 719 keV emitted from proton-boron reactions were analyzed using single-photon emission computed tomography (SPECT). Quantitative evaluation of the images verified the detection of anatomical changes in tumors, one of the important factors in daily adaptive proton therapy (DAPT) and verified the possibility of application of the PG-ray images to DAPT. Six scenarios were considered based on various sizes and locations compared to the reference virtual tumor to observe the anatomical alterations in the virtual tumor. Subsequently, PG-rays SPECT images were acquired using the modified ordered subset expectation-maximization algorithm, and these were evaluated using quantitative analysis methods. The results confirmed that the pixel range and location of the highest value of the normalized pixel in the PG-rays SPECT image profile changed according to the size and location of the virtual tumor. Moreover, the alterations in the virtual tumor size and location in the PG-rays SPECT images were similar to the true size and location alterations set in the phantom. Based on the above results, the tumor anatomical alterations in DAPT could be adequately detected and verified through SPECT imaging using the 719 keV PG-rays acquired during treatment.

Background: Chronic obstructive pulmonary disease (COPD) is a common chronic respiratory disease with increased prevalence in the elderly. Telomeres are repetitive DNA sequences found at the end of the chromosome, which progressively shorten as cells divide. Telomere length is known to be a molecular marker of aging. This study aimed to assess the relationship between telomere length and the risk of COPD, lung function, respiratory symptoms, and emphysema index in Chronic Obstructive Pulmonary Disease in Dusty Areas (CODA) cohort. Methods: We extracted DNA from the peripheral blood samples of 446 participants, including 285 COPD patients and 161 control participants. We measured absolute telomere length using quantitative real-time polymerase chain reaction. All participants underwent spirometry and quantitative computed tomography scan. Questionnaires assessing respiratory symptoms and the COPD Assessment Test was filled by all the participants. Results: The mean age of participants at the baseline visit was 72.5±7.1 years. Males accounted for 72% (321 participants) of the all participants. The mean telomere length was lower in the COPD group compared to the non-COPD group (COPD, 16.81±13.90 kb; non-COPD, 21.97±14.43 kb). In COPD patients, 112 (75.7%) were distributed as tertile 1 (shortest), 91 (61.1%) as tertile 2 and 82 (55%) as tertile 3 (longest). We did not find significant associations between telomere length and lung function, exacerbation, airway wall thickness, and emphysema index after adjusting for sex, age, and smoking status. Conclusion: In this study, the relationship between various COPD phenotypes and telomere length was analyzed, but no significant statistical associations were shown.

Background Three-dimensional (3D) model printing improves visualization of anatomical structures in space compared to two-dimensional (2D) data and creates an exact model of the surgical site that can be used for reference during surgery. There is limited evidence on the effects of using 3D models in microsurgical reconstruction on improving clinical outcomes. Methods A retrospective review of patients undergoing reconstructive breast microsurgery procedures from 2017 to 2019 who received computed tomography angiography (CTA) scans only or with 3D models for preoperative surgical planning were performed. Preoperative decision-making to undergo a deep inferior epigastric perforator (DIEP) versus muscle-sparing transverse rectus abdominis myocutaneous (MS-TRAM) flap, as well as whether the decision changed during flap harvest and postoperative complications were tracked based on the preoperative imaging used. In addition, we describe three example cases showing direct application of 3D mold as an accurate model to guide intraoperative dissection in complex microsurgical reconstruction. Results Fifty-eight abdominal-based breast free-flaps performed using conventional CTA were compared with a matched cohort of 58 breast free-flaps performed with 3D model print. There was no flap loss in either group. There was a significant reduction in flap harvest time with use of 3D model (CTA vs. 3D, 117.7±14.2 minutes vs. 109.8±11.6 minutes; P=0.001). In addition, there was no change in preoperative decision on type of flap harvested in all cases in 3D print group (0%), compared with 24.1% change in conventional CTA group. Conclusions Use of 3D print model improves accuracy of preoperative planning and reduces flap harvest time with similar postoperative complications in complex microsurgical reconstruction.
Currently, the naked eyes-based diagnosis of bone metastases on CT images relies on qualitative assessment. For this reason, there is a great need for a state-of-the-art approach that can assess and follow-up the bone metastases with quantitative biomarker. Radiomics can be used as a biomarker for objective lesion assessment by extracting quantitative numerical values from digital medical images. In this study, therefore, we evaluated the clinical applicability of non-invasive and objective bone metastases computer-aided diagnosis using radiomics-based biomarkers in CT. We employed a total of 21 approaches consist of three-classifiers and seven-feature selection methods to predict bone metastases and select biomarkers. We extracted three-dimensional features from the CT that three groups consisted of osteoblastic, osteolytic, and normal-healthy vertebral bodies. For evaluation, we compared the prediction results of the classifiers with the medical staff's diagnosis results. As a result of the three-class-classification performance evaluation, we demonstrated that the combination of the random forest classifier and the sequential backward selection feature selection approach reached AUC of 0.74 on average. Moreover, we confirmed that 90-percentile, kurtosis, and energy were the features that contributed high in the classification of bone metastases in this approach. We expect that selected quantitative features will be helpful as biomarkers in improving the patient's survival and quality of life.
과학 기술의 발달로 인해 의료 방사선 장비 또한 첨단화 되고 있으며 정확한 진단과 치료를 할 수 있게되어 의료의 질적 향상은 물론 양적인 면에서도 그 사용이 날로 증가하고 있는 추세이다. 특히 환자 피폭선량에 대부분을 차지하는 CT(Computed Tomography) 촬영의 건수는 크게 증가하고 있다. 치과 방사선 검사의 경우 정확한 병변 진단을 위한 CT 및 CBCT(Cone Beam Computed Tomography) 의 도입으로 과거에 비해 환자는 많은 선량을 받게 되었다. 본 연구에서는 인체 팬텀과 TLD-100H를 이용하여 치과 방사선 검사별 조직의 흡수선량을 측정하고 ICRP 60, 103에 따라 유효선량을 계산하였다. ICRP 60, 103에 따른 유효선량 값은 파노라마 검사의 경우 $5.1{\mu}Sv$, $29.5{\mu}Sv$, 세팔로 검사의 경우 $11.2{\mu}Sv$, $14.4{\mu}Sv$, 이며 CBCT는 상악골의 경우 $53.7{\mu}Sv$, $209.6{\mu}Sv$, 하악골은 $129{\mu}Sv$, $391.5{\mu}Sv$ 그리고 CT에서는 상악골의 경우 $93.3{\mu}Sv$, $139.5{\mu}Sv$이며 하악골은 $282.7{\mu}Sv$, $489.7{\mu}Sv$로 평가 되었다. CBCT 검사의 경우 유효선량은 CT 검사에 비해서는 적지만 파노라마 검사 및 세팔로 검사에 비해서는 높게 평가되었다.
본 연구는 소아 심장 전산화단층촬영 검사에서 조영제의 과다 사용을 최소화하며 피폭 선량 저감화를 목적으로 조영제의 주입속도 및 심장 도달 시간 등의 변수를 고려한 스캔 지연 시간을 경험론적 방법으로 수식화함으로써 4개의 심방과 심실이 동시 조영증강 영상을 획득할 수 있는 최적화 방안을 제시하고자 하였다. 소아 심장전산화단층 검사를 시행한 소아 환자 30명을 대상으로 정성적 평가, 정량적 평가, 선량평가를 실시하였다. 결론적으로 본 연구에서 개발한 체중에 따른 조영제 양과 주입속도에 따른 스캔 지연 시간을 계산하는 경험론적 방법을 적용함으로써 왼 오른 심방과 심실의 동시 조영증강에서 소아 심장 판독에 적합한 300 HU 값 이상으로 나타났고 명확성, 선예도, 노이즈에 대한 정성적 영상 평가에서 질적 우수성을 보였으며 선량평가에서도 소아가 받는 피폭선량도 명확하게 감소되었다. 따라서 본 연구는 임상에서 조영제 과다 사용으로 인한 부작용 감소 효과를 기대 할 수 있었으며 방사선 피폭 저감화를 기대 할 수 있는 이점을 가지고 최적의 영상을 만드는데 중요한 역할을 할 것으로 판단되었다.
본 연구는 전산화단층촬영에서 간 질환의 자동 인식으로 질감특징분석(texture feature analysis. TFA) 알고리즘을 제안하고자 하였으며, 간세포암(Hepatocellular carcinoma. HCC)에 대한 컴퓨터보조진단(computer-aided diagnosis. CAD) 시스템을 설계하고, 제안하는 각 알고리즘의 성능을 평가하고자 하였다. HCC 영상에서 분석영역($40{\times}40$ 픽셀)을 설정하고 각 부분영상에 통계적 특징을 이용한 6가지 TFA 파라메터(평균 밝기, 평균 대조도, 평탄도, 왜곡도, 균일도, 엔트로피)비교하여 간세포암 인식률(recognition rate)을 구하였다. 결과적으로 TFA는 간세포암 인식률을 나타내는 척도로 유의함을 알 수 있었으며 6가지 파라메터에서 균일도가 가장 인식률이 높았으며 평균 대조도, 평탄도, 왜곡도가 비교적 높았고 평균 밝기와 엔트로피는 상대적으로 낮은 인식률을 나타내었다. 이와 관련하여 높은 인식률을 보인 알고리즘(최대 97.14%, 최소 82.86%)을 간세포암 영상의 병변을 판별하여 임상의 조기 진단을 보조하여 치료를 시행한다면 진단의 효율성이 높아 질 것으로 판단되었으며, 향후 효율적이고 정량적인 분석을 추가함으로써 질병인식의 일반화에 대한 기준 연구가 필요 할 것으로 사료되었다.
영상 획득과 재구성 방법에 따라 CT 감약계수는 다양성을 보이고 관심 영역의 노이즈는 정밀도에 영향을 준다. 인체에서 간 실질조직과 위장의 물의 CT 감약계수와 노이즈를 커널에 따라 측정하였다. 다중채널 CT 스캐너를 이용하여 복부를 스캔 하였고, 커널은 B10 (very smooth), B20 (smooth), B30 (medium smooth), B40 (medium), B50 (medium sharp), B60 (sharp), B70 (very sharp), B80 (ultra sharp)으로 재구성하여 간의 실질 조직과 물이 들어 있는 위장 부위를 ROI 기능을 이용하여 평균의 CT감약계수와 표준편차인 노이즈를 측정하여 영상을 비교하였다. 간의 실질 조직에서 CT감약계수는 커널에 따라 60.4에서 69.2 HU사이에서 분포하여 차이가 없었으나, 노이즈는 커널(7.6$\sim$63.8 HU)이 높아질수록 증가하였다. 물의 CT감약계수는 -2.2 HU에서 0.8 HU사이에서 측정되었고, 노이즈는 커널(10.1$\sim$82.4 HU)이 높아질수록 증가하였다. 영상의 질을 높이기 위해서는 검사 부위에 따라 노이즈를 감소하기 위해 적절한 커널을 선택하여 CT 검사를 하여야 한다.
치과 방사선 검사 중 파노라마(panorama) 촬영은 왜곡현상이 발생하여 정량적인 평가 시 부정확한 결과를 야기할 수 있다. 최근 임상에 적용되고 있는 새로운 영상화 기법인 cone beam형 전산화단층촬영(이하 CBCT)은 고해상도의 영상과 위치 정보를 제공한다. 본 실험에서는 DCT-90-P IMPLAGRAPHY Dental CT system (Vatech, Korea)을 사용하여 20명의 환자를 검사하였고, 촬영조건은 관전압 85kVp, 관전류 7mA, 촬영시간은 24초이다. CBCT 데이터를 3차원 컴퓨터 프로그램을 이용하여 재구성하고, 형태 계측학적으로 평가 분석하였다. 그 결과 하악체에 기울기에 따라 변화를 주었을 때, 하악관의 직경의 왜곡이 감소하는 것을 알 수 있었다. 결론적으로 피사체와 필름간의 거리가 클수록 확대가 많이 나타나고, 초점에서 벗어난 거리에 따라 영상이 차이가 있을 것으로 예상되기 때문에 재구성시 해부학적 위치의 구조에 맞게 부분적으로 알맞은 방법을 채택을 해야 하는 것으로 사료된다.
