Associations Between Thyroid Hormone Levels and Urinary Concentrations of Bisphenol A, F, and S in 6-Year-old Children in Korea

  • Objectives: Bisphenol A (BPA) is used in the electrical, mechanical, medical, and food industries. Previous studies have suggested that BPA is an endocrine disruptor. Regulation of BPA has led to increased use of bisphenol F (BPF) and bisphenol S (BPS). However, few studies have investigated the associations of BPF and BPS with thyroid dysfunction in children. Our study investigated the associations of prenatal BPA and early childhood BPA, BPF, and BPS exposure with thyroid function in 6-year-old children. Methods: Prenatal BPA concentrations were measured during the second trimester of pregnancy in an established prospective birth cohort. We measured urinary BPA, BPF, and BPS concentrations and thyroid hormone levels (thyroid-stimulating hormone, total T3, and free T4) in 6-year-old children (n=574). We examined the associations between urinary bisphenol concentrations and percentage change of thyroid hormone concentrations using multivariate linear regression. We also compared thyroid hormone levels by dividing the cohort according to BPA, BPF, and BPS concentrations. Results: The associations between prenatal BPA and total T3 levels were statistically significant in all models, except for girls when using a crude model. The associations between urinary BPA and BPS concentrations and levels of all thyroid hormones were not statistically significant. However, we observed that lower free T4 levels (-1.94%; 95% confidence interval, -3.82 to -0.03) were associated with higher urinary BPF concentrations in girls only. Conclusions: Our findings identified significant associations between prenatal BPA exposure and total T3 levels in all children and between BPF exposure and free T4 levels in girls only.

Association Between Local Government Social Expenditures and Mortality Levels in Korea

  • Objectives: We examined the association between social expenditures of the local government and the mortality level in Korea, 2004 to 2010. Methods: We used social expenditure data of 230 local governments during 2004 to 2010 from the Social Expenditure Database prepared by the Korean Institute for Health and Social Affairs. Fixed effect panel data regression analysis was adopted to look for associations between social expenditures and age-standardized mortality and the premature death index. Results: Social expenditures of local governments per capita was not significantly associated with standardized mortality but was associated with the premature death index (decline of 1.0 [for males] and 0.5 [for females] for each expenditure of 100 000 Korean won, i.e., approximately 100 US dollar). As an index of the voluntary effort of local governments, the self-managed project ratio was associated with a decline in the standardized mortality in females (decline of 0.4 for each increase of 1%). The share of health care was not significant. Conclusions: There were associations between social expenditures of the local government and the mortality level in Korea. In particular, social expenditures per capita were significantly associated with a decline in premature death. However, the voluntary efforts of local governments were not significantly related to the decline in premature death.

한국 성인의 혈청 잔류성 유기오염물질 농도와 연령 및 체질량지수와의 관련성 (Association of Persistent Organic Pollutants (POPs) with Age and Body Mass Index in Korean Adults)

  • Objectives: Persistent organic pollutants (POPs) are known to be the detrimental chemicals in the body, even at low levels, and are stored in adipose tissue. Recently, POPs have been reported to be associated with chronic diseases, including cancer and cardiovascular disease, and aging and obesity are reported as common factors in chronic disease. However, there have been only a few studies on the associations of POPs with age and body mass index (BMI) in Korea. Therefore, we analyzed the associations of serum POPs levels with age and BMI in Korea. Methods: This cross-sectional study includes 444 subjects (253 men and 191 women) from the Korean Cancer Prevention Study-II (2004-2011). Serum levels of 33 polychlorinated biphenyls (PCBs) and 19 organochlorine pesticides (OCPs) were measured by a gas chromatographer (Agilent 6890) coupled to high resolution mass spectrometer (JEOL JMS-800D). Results: Concentrations of PCB 153 (men: 12.26 ng/g lipid, women: 10.50 ng/g lipid) and p,p'-DDE (men: 94.66 ng/g lipid, women: 96.66 ng/g lipid) were the highest among serum PCBs and OCPs, respectively. PCBs and OCPs were significantly positively correlated with age in both sexes. After adjustment for age, non-dioxin like PCBs were significantly negatively correlated with body mass in women. However, cis-heptachlor epoxide was significantly positively correlated with body mass index in both sexes. When analyzed by sex, stronger associations were shown between POPs and age in men and POPs and BMI in women. Conclusion: These results may provide baseline data for the study of POPs and for the health management field in Korea.

XPC 939A>C and 499C>T Polymorphisms and Skin Cancer Risk: a Meta-analysis

  • The xeroderma pigmentosum complementation group C gene (XPC) has been identified as important for repairing UV-related DNA damage. Some subtle changes in this gene may impair repair efficiency and influence susceptibility to human cancers, including skin cancer. Two polymorphisms in XPC, 939A>C (rs2228001) and 499C>T (rs2228000), are considered to have possible associations with the risk of skin cancer, but the reported results have been inconsistent. Here we performed a meta-analysis of the available evidence regarding the relationship between these two polymorphisms and the risk of skin cancer. All relevant studies were searched using PubMed, Embase and Web of Science before February 2012. A total of 8 case-control studies were included in this analysis, and no convincing associations between the two polymorphisms and risk of skin cancer were observed in any of the genetic models. Stratified analyses by skin cancer type also did not detect significant associations in any subgroup. This meta-analysis suggested that the XPC 939A>C and 499C>T polymorphisms may have little involvement in susceptibility to skin cancer.

지역사회 자발적 결사체의 연결망과 지역사회 역량 (The Network Analysis for Community Voluntary Organizations and Its Implication for Community Capacity Building Toward Health Promotion)

  • The paradigm of health promotion requests community participation and its active problem-solving. Community is conceptualized as a resource pool to be organized. Such resource is called community capacity. Community participation is a process of capacity building. Community voluntary associations are considered as valuable resource to be used for health promotion. This paper tried to identify the network structure among community voluntary associations and to infer the possibility to make such network of organizations participate in health promotion programs. Two survey data were used for this research: 1) Measurements and Evaluations of Community Capacity on Dobong-gu (N=94) 2) A development plan of health medicine service to be Healthy Gangdong-gu (N=69). The questionnaire included such variables measuring community capacity as leadership, membership, organizational resources, and inter-organizational network, etc. Both regions had the following common characteristics: 1) There were positive correlations between the organization's budget and membership. 2) Organizational types were associated with their founded years. Two regions showed the following differences: Dobong displayed the high density of community organizations, but Gangdong showed the low density. Dobong community organizations were able to be classified into three network clusters such as women & environments, youth & adolescent, and sports organizations. Each cluster of organizations favored the different type of health promotion programs. Gangdong community organizations were less developed, and not possible to be clustered. Depending upon the level of community capacity or community organizations' differentiation, the strategy of community participation could be settle down in different ways. Particularly the health agency had to pay more attention to support the growth of civil organizations.

Short-term Associations of Air Pollution with Postneonatal Infant Death in Seoul, Korea, 1999-2003

  • Objective to assess whether exposure to air pollutants is associated with postneonatal infant death, using a timeseries methodology, between 1999 and 2003 in Seoul, Korea.. Methods We investigated the short-term effects of air pollution for 548,725 live births during the study period. The daily count of postneonatal infant deaths from all causes and from SIDS (sudden infant death syndrome) by birth order was analyzed by a Generalized Additive Poisson model, with controlling for the effects of seasonal trends, air temperature, relative humidity, barometric pressure, and day of the week as covariates. Results During the study period, we observed 699 deaths from all causes and 47 deaths from SIDS. We did not find any significant associations between daily mortality and ambient levels of air pollutants except for CO and $NO_2$. The estimated relative risk of postneonatal infant death from all causes was 1.17 (95% CI=1.04-1.32) and 1.16 (95% CI=1.03-1.29) by IQR (interquartile range) for CO and $NO_2$ respectively. Also, we observed no clear trend of the mortality effects of air pollution by birth orders. Conclusion In conclusion, our findings suggest that air pollution, in general, influenced adversely postneonatal infant death from all-cause and SIDS although it was not statistically significant. This study may support that the rationale.

TERT rs2736098 Polymorphism and Cancer Risk: Results of a Meta-analysis

  • Objective: Several studies have demonstrated associations between the TERT rs2736098 single nucleotide polymorphisms (SNPs) and susceptibility to cancer development. However, there are conflicting results. A systematic meta-analysis was therefore performed to establish the cancer risk associated with the polymorphism. Methods: In this meta-analysis, a total of 6 case-control studies, including 5,567 cases and 6,191 controls, were included. Crude odds ratios with 95% confidence intervals were used to assess the strength of associations in several genetic models. Results: Our results showed no association reaching the level of statistical significance for overall risk. Interestingly, in the stratified analyses (subdivided by ethnicity), significantly increased risks were found in the Asian subgroup which indicates the TERT rs2736098 polymorphism may have controversial involvement in cancer susceptibility. Conclusions: Overall, this meta-analysis indicates that the TERT rs2736098 polymorphism may have little involvement in cancer susceptibility.

Bridge the Gap Between Local Governments and Communities: Key Factors in Generating Community Involvement in the Historic Preservation District in Japan

  • Since 1795, 106 districts in Japan have been selected as Important Historic Preservation Districts (Juuyo dentouteki kenzoubutsugun hozon chiku [Juudenken]). The system for protection of cultural properties enables the local government to name a "Preservation District" and allows for the development of a preservation plan based on local ordinances. Moreover, the well-organized, bottom-up networks, which are groups for community development activities on the basis of local participation, play an important role in raising awareness and conducting several preservation projects in their own towns. This study mainly focused on cultural resources management in the local community. The system, which possibly bridged the gap between the local authorities and the community, was revealed. Fifty non-profit groups and active citizens, who were engaged in an advanced stage of community participation in Juudenken, were selected to be interviewed. The results then were analyzed using STAT program. The significant associations were shown by mapping the associations related to the public process of community involvement. Each variable had its own significant meaning and contributed credible indirect association to community involvement. The network mapping indicated that balancing the local economy and technical conservation was important in generating community involvement, which provided a model on how local authorities and communities could articulate and maintain their own cultural resources.

Socioeconomic Status and Number of Children Among Korean Women: The Healthy Twin Study

  • Objectives: This study aimed to evaluate whether the birth rate is associated with socioeconomic status in the women of the Republic of Korea, where the birth rate is rapidly decreasing. Methods: This study included 732 females from the Healthy Twin Study, a family-twin cohort. The participants were classified into 3 socioeconomic groups according to their average income, education, and occupation. The association between socioeconomic status and number of children was assessed using gamma regression analysis with a generalized linear mixed model, adjusting for the age group, smoking/alcohol status, and family relationships. Results: The group with the highest education level had significantly fewer children compared with the group with the lowest education level (p=0.004). However, no significant associations were found according to household income level. The non-manual labor group had significantly fewer children compared with those working as homemakers (p=0.008). Conclusions: This study aimed to explain the causal relationship between socioeconomic status and number of children. Associations between some socioeconomic status and number of children were found in Korea.

Associations Between Parental Depression and Early Childhood Development in Indonesia: A Cross-sectional Study

  • Objectives: This study aimed to determine the associations between parental depression and early childhood development among children aged 36 months to 59 months in Indonesia. Methods: From Indonesia's Basic Health Survey (RISKESDAS) 2018, this study included 6433 children aged 36 months to 59 months and their parents. Maternal and paternal depression was examined using the Mini International Neuropsychiatric Interview survey instrument, which was previously translated into Indonesian. The study also used the Early Child Development Index to measure child development and its 4 domains (cognitive, physical, socio-emotional, and learning). Multivariate logistic regression analysis was performed to determine the association between parental depression and early childhood development. Results: Overall, 10.3% of children aged 36 months to 59 months were off-track for development. After adjusting for biological, parental, and social characteristics, children born to parents with depression were found to be 4.72 times more likely to be off-track for development (95% confidence interval, 1.83 to 12.15). Conclusions: Children of depressed parents were more likely to be off-track for development. The findings highlight the need for early diagnosis and timely intervention for parental depression to promote early childhood development.
