치근면 활택술후 2% minocycline gel의 치료효과 (Effects of 2% minocycline gel as an adjunct to scaling and root planing on the treatment of adult periodontitis)

  • The objective of the present study was to evaluate the clinical and microbiological effects of scaling and root planing combined with local application of 2% minocycline gel to patients with moderate to advanced chronic adult periodontitis. 27 healthy patients with moderate to advanced chronic adult periodontitis were enrolled in the study. The quadrants that had 2 or more teeth with $5{\sim}8mm$ probing pocket depth and radiographic evidence of alveolar bone loss were selected and divided into test side and control side according to the splitmouth design. All patients received standardized oral hygiene instructions at the beginning of the study. Subsequently scaling and root plaining was performed on all remaining teeth until 0 week. The 2% minocycline gel was applied to periodontal pocket at 0, 1, 2, 3week in the test side. The normal saline was irrigated subgingivally for about 30 seconds in the control side. The clinical and microbiological analysis was carried out at 0, 4, 8, and 12weeks. The results of this st udy were as follows; 1.2% minocycline gel delivered subgingivally as an adjunct to scaling and root planing provided benefit in reducing sulcular bleeding index and pocket depth than the use of normal saline. 2. The relative proportion of cocci and non-motile bacteria was increased in the test and control groups with time, and there was no statistically significantdifference between two groups. 3. The proportion of spirochetes was slowly reduced in the control group, but, inthe test group, they were remarkably reduced from the 4th week, and there was a statistically significant difference between two groups. 4. In both groups, the relative proportion of motile rods was notably decreasedat the beginning of the study, and remained until 12th week in the test group,but, in the control group, they were slowly increased from the 4th weekand finally similar to that of the initial examination. In conclusion, local application of 2% minocycline gel may be effective in the clinical and microbiological aspects as an adjunct to scaling and root planing in periodontal disease sites.

난치성 치주염의 질환진행 예견 인자에 관한 분석 (ANALYSIS ON THE PREDICTOR OF DISEASE PROGRESSION IN REFRACTORY PERIODONTITIS)

  • Refractory periodontitis manifest progressive attachment loss in a rapid and unrelenting manner regardless of the type or frequency of therapy applied. The purpose of this study was ta evaluate the relation between the level of cytokines in GCF and periodontopathic microflora with disease activity of refractory periodontitis. Selection of patients with refractory periodontitis (7 males, 3 females) were made by long term clinical observation including conventional clinical history and parameters. Teeth that showed pocket depth greater than 6mm were selected as sample teeth. Subjects were examined at baseline and after 3 months. Prior to baseline test, individual acrylic stent was fabricated. Reference grooves were made on each sample tooth site. Pocket depth and attachment loss were measured by Florida Probe. Gingival index was measured at 4 sites each sample teeth. Disease activity was defined as attachment loss of ${\ge}$ 2.1mm, as determined by sequential probing and tolerance method. The pattern and amount of alveolar bone resorption was observed with quantitative digital subtraction image processing radiography. Morphological analysis of subgingival bacteria was taken by phase contrast microscopy. Predominant cultivable bacterial distribution and frequency were compared between disease-active and disease-inactive site using immunofluorescence microscopy and selective microbial culturing. Levels of $interleukin-l{\beta}$, 2, 4, 6 and $TNF-{\alpha}$ in GCF and blood serum sample were quantified by ELISA. In active sites, P. intermedia was significantly increased to compare with inactive site. $IL-1{\beta}$, IL-2, IL-6 and $TNF-{\alpha}$ in GCF were increased in active sites and IL-2 in serum was increased in active patients significantly. Alveolar bone loss in active site was correlated with $IL-1{\beta}$, IL-2 in GCF. And loss of attachment in active site was correlated with IL-2 in GCF. These results demonstrate that IL-2 in serum, $IL-1{\beta}$, IL-2, IL-6 and $TNF-{\alpha}$ in GCF, P, intermedia might be used as possible predictors of disease activity in refractory periodontitis before it is clinically expressed as attachment loss and quantitative alveolar bone change.

치은염 환자에서 저출력 이산화탄소 레이저의 염증완화 효과에 관한 연구 (Effects of resolution of inflammation for low-power $CO_2$ laser treatment on gingivitis patients)

  • Purpose: In this study, we compared low-power $CO_2$ laser treatment to ultrasonic scaling, which is generally approved as a power-driven mechanical instrumentation, and evaluated both of these treatments regarding their clinical effectiveness and change in the volume of in GCF. Material and methods: 20 patients who had gingivitis were selected. all of patients has no systemic problems. Randomly selected, one quadrant received ultrasonic scaling only, another quadrant received ultrasonic scaling and $CO_2$ laser irradiation, the other quadrant received $CO_2$ laser irradiation only. Clinical parameters measured at baseline, 1 weeks, 2weeks, 4weeks and 8weeks. Result: Pocket probing depth and clinical attachment level were not changed during study period. Gingival index of all group were improved after treatment. At 1 weeks after treatment, Gingival index of ultrasonic scaling group was only significantly different compared to control group. At 2 weeks after treatment, gingival index of all experimental group were significantly different compared to control group. At 4 and 8 weeks after treatment, gingival index of all group were increased, but experimental group were lower than control group. Sulcus bleeding index was similar to the results of gingival index. At 1 weeks after treatment, all experimental group were significantly different compared to control group and it maintained during study. At 2 weeks after treatment, sulcus bleeding index of all group were lowest during study. Gingival crevicular fluid were measured with $Periotron^{(R)}$ 8000($Oraflow^{(R)}$, Inc. USA). At baseline, all group were showed moderately severe condition. At 1 week after treatment, laser treatment only group was reduced quantity of gingival crevicular fluid mostly, and all group were reduced quantity of gingival crevicular fluid. At 2 weeks after treatment, all group were health state. At 4 and 8 weeks after treatment, all group were showed recurrent of inflammation, and control group was the most significantly increased. Conclusion: This study showed that the effects of $CO_2$ laser treatment were similar to conventional ultrasonic scaling and this result remained longer than plaque control only. These results suggest possibility of $CO_2$ laser treatment for altered periodontal therapy.

Impact of scaling and root planing on C-reactive protein levels in gingival crevicular fluid and serum in chronic periodontitis patients with or without diabetes mellitus

  • Purpose: The present study was conducted to evaluate the impact of scaling and root planing (SRP) on the C-reactive protein (CRP) levels of gingival crevicular fluid (GCF) and serum in chronic periodontitis patients with type 2 diabetes mellitus (T2DM-CP) or without type 2 diabetes mellitus (NDM-CP). Methods: Forty-eight human participants were divided into two groups: an experimental (T2DM-CP) group (group I, n=24) comprising chronic periodontitis patients with random blood sugar ${\geq}200mg/dL$ and type 2 diabetes mellitus, and control (NDM-CP) group (group II, n=24) of those with chronic periodontitis and random blood sugar <200 without T2DM for the study. All subjects underwent nonsurgical periodontal therapy (NSPT) including complete SRP and subgingival debridement. Periodontal health parameters, plaque index (PI), gingival index (GI), probing pocket depth (PPD), clinical attachment level (CAL), GCF volume (GCF vol), GCF-CRP, random blood glucose (RBS), glycated hemoglobin, and systemic inflammatory markers, serum CRP, total leukocyte count (TLC), neutrophil count (Neutr) and lymphocyte count (Lymph), were evaluated at baseline, 1 month, and 3 months after SRP. Results: NSPT resulted in statistically significant improvement in periodontal health parameters (PI, GI, PPD, CAL, GCF vol), CRP levels in serum as well as GCF of both groups I and II. The mean improvement in periodontal health parameters (PI, GI, PPD, CAL, GCF vol), CRP levels in serum and GCF was greater in group I than group II after NSPT. There was nonsignificant increase in GCF-CRP, TLC, Lymph, and RBS, and a significant increase in Neutr and Serum CRP in group II at 1 month. The Serum CRP level of 20 out of 24 group II patients had also increased at 1 month. Conclusions: The CRP levels in both GCF and serum were higher in T2DM-CP patients than in NDM-CP patients. Although there was a significant improvement in both the groups, greater improvement was observed in both GCF and serum samples of T2DM-CP patients.

Listeria monocytogenes Serovar 4a is a Possible Evolutionary Intermediate Between L. monocytogenes Serovars 1/2a and 4b and L. innocua

  • The genus Listeria consists of six closely related species and forms three phylogenetic groups: L. monocytogenes-L. innocua, L. ivanovii-L. seeligeri-L. welshimeri, and L. grayi. In this report, we attempted to examine the evolutionary relationship in the L. monocytogenes-L. innocua group by probing the nucleotide sequences of 23S rRNA and 16S rRNA, and the gene clusters lmo0029-lmo0042, ascB-dapE, rplS-infC, and prs-ldh in L. monocytogenes serovars 1/2a, 4a, and 4b, and L. innocua. Additionally, we assessed the status of L. monocytogenes-specific inlA and inlB genes and 10 L. innocua-specific genes in these species/serovars, together with phenotypic characterization by using in vivo and in vitro procedures. The results indicate that L. monocytogenes serovar 4a strains are genetically similar to L. innocua in the lmo0035-lmo0042, ascB-dapE, and rplS-infC regions and also possess L. innocua-specific genes lin0372 and lin1073. Furthermore, both L. monocytogenes serovar 4a and L. innocua exhibit impaired intercellular spread ability and negligible pathogenicity in mouse model. On the other hand, despite resembling L. monocytogenes serovars 1/2a and 4b in having a nearly identical virulence gene cluster, and inlA and inlB genes, these serovar 4a strains differ from serovars 1/2a and 4b by harboring notably altered actA and plcB genes, displaying strong phospholipase activity and subdued in vivo and in vitro virulence. Thus, by possessing many genes common to L. monocytogenes serovars 1/2a and 4b, and sharing many similar gene deletions with L. innocua, L. monocytogenes serovar 4a represents a possible evolutionary intermediate between L. monocytogenes serovars 1/2a and 4b and L. innocua.

수분 부족 스트레스 처리시 Monodehydroascorbate Reductase (MDHAR)의 반응 (Response of Monodehydroascorbate Reductase (MDHAR) in Lettuce (Lactuca sativa L.) Leaves Subjected to Water Deficit Stress)

  • 수분 공급을 제한하여 수분 부족 스트레스를 처리한 상추식물에서 산화적 스트레스와 관련된 monodehydroascorbate reductase (MDHAR)의 활성도, 엽록소 함량, 과산화수소의 함량 등과의 상관관계를 조사한 결과, 생육배지의 수분의 함량이 감소함에 따라 식물체내 과산화수소의 생성량이 증가($R^2$=0.8851)하였으며, 수용성단백질 함량은 점차 감소($R^2$=0.9826)하는 경향을 나타내었다. 총 엽록소함량은 수분 부족 스트레스를 처리한 공시작물에서의 함량이 정상 생육시 보다 그 함량이 대체적으로 낮은 경향을 보였으며, 엽록소 a와 엽록소 b함량 변화도 총 엽록소의 함량변화와 비슷한 경향을 보였다. 그러나 총 엽록소에 대한 카로티노이드의 비율은 수분 부족 스트레스를 처리한 식물에서 정상생육 시 보다 더 높은 경향을 보였다. 수분 부족 스트레스가 진행됨에 따라 아스코브산의 함량은 정상 생육 시 보다 더 높은 경향을 보였으나 환원형인 디하이드로아스코브산의 함량은 수분 부족 스트레스를 처리한 초기에 정상생육 시 보다 더 낮은 경향을 보였다. MDHAR의 활성도는 사이토졸(cytosolic) 분획과 엽록체(chloroplastic) 분획에서 공히 크게 증가하였으며 MDHAR의 mRNA 전사 정도도 수분 부족 스트레스가 진행됨에 따라 크게 증가하였다. 수분함량이 감소함에 따라 MDHAR의 활성도가 크게 증가하였으며, 또한 과산화수소의 함량이 증가함에 따라서도 MDHAR의 활성도가 크게 증가($R^2$=0.9937과 0.8645)되어 수분 부족 스트레스로 나타나는 요인들과 MDHAR 사이에 밀접한 관련이 있음을 확인할 수 있었다.

인체기관지내의 입자펄스 확산 실험 (Dispersion of Particle Pulse in Human Lung Airway)

  • 세기관지의 구조와 이상유무의 신속 정확한 측정을 위해 세계적으로 활발히 연구되고 있는 에어로졸볼러스기법을 확립하기 위하여, 호흡과정 중 흡기에 공급된 입자펄스가 호흡의 진행에 따라 인체기관지내에서 대류분산된 결과로 호기중 입자농도의 증가된 분포폭을 측정하는 장치를 개발하고 이를 이용한 검증실험을 수행하였다. 개발된 장치는 인체에 무해한 sebacic acid를 이용하여 평균입경이 1 $\mu$m이고 기하표준편차가 1.2정도인 임자를 $10^{9}$개/cc 정도의 고농도로 발생하고, 솔레노이드밸브를 이용하여 호흡중의 원하는 시점에 정확히 짧은 펄스형태로 입자를 공급하며, 레이저광산란을 이용하여 흡기와 호기중의 입자농도의 시간변화를 측정한다. 흡기중의 농도분포는 매우 매끈한 분포를 보임으로써 발생장치와 측정장치의 정강작동은 잘 검증되었다. 농도의 시간변화 측정치로부터 계산된 분산도를 1차원 대류확산방정식 이론치 및 외국에서 보고된 실험결과와 비교한 격과, 입자도달깊이에 따른 분산도의 변화에 대한 정성적인 실험결과는 이론치와 상당히 잘 일치하고 기존의 결과보다도 견과의 산포가 개선되었다. 다만 아직도 산포가 상당히 남아있어 개선의 여지를 가지고 있으며, 외국 결과와의 분산도의 타이는 신체조건의 타이 등으로 파악되었다. 산포도의 개선을 위해서는 호흡시에, 특히 최고농도점 부근에서, 호흡유량이 일정하게 유지되어야만 불규칙 이차파형의 형성을 방지하여 측정결과의 해석에 오차를 줄일 수 있음을 확인하였다.

Modification of a Smoking Motivation Questionnaire for Chinese Medical Students

  • Introduction: Smoking prevalence among the medical students is high in China. Therefore, understanding the smoking motivations of medical students is crucial for smoking control, but currently there are no scales questionnaires customized for probing the smoking motivations of medical students. This aim of study was to test and modify a questionnaire for investigating smoking motivations among medical students. Methods: A cross-sectional survey was conducted among 1,125 medical students at Xuzhou Medical College in China in 2012.The model fit and validity was assessed by confirmatory factor analysis (CFA) and the reliability was tested by single-item reliability, composite reliability, and item-total correlation. Results: The prevalence of smoking was 9.84 % among study population. In the modified scales, the global fit indices identified a CFI value of 0.96, TLI was 0.96, and the RMSEA was 0.063. CFA supported the two dimensional structure of the instrument. The average variance extracted ranged from 0.45 to 0.62. All single-item reliability scores were greater than 0.20, and the composite reliability ranged from 0.74 to 0.91. Conclusion: Modified scales could be the preliminary instrument used in evaluating the smoking motivations of medical students. However, it should be further assessed using other forms and methods of validity and reliability, additional motivations of smoking, and the survey of other medical colleges in China.

ASIC 설계의 효과적인 검증을 위한 에뮬레이션 시스템 (An Emulation System for Efficient Verification of ASIC Design)

  • 본 논문에서는 ASIC 설계 회로를 빠른 시간 내에 구현 및 검증할 수 있는 에뮬레이션 시스템 ACE(ASIC Emulator)를 제안한다 ACE는 EDIF 번역기, 라이브러리 변환기, 기술 맵퍼, 회로 분할기, LDF 생성기를 포함하는 에뮬레이션 소프트웨어와 에뮬레이션 보드, 논리 분석기를 포함하는 에뮬레이션 하드웨어로 구성된다. 기술 맵퍼는 회로 분할과 논리 함수식 추출, 논리 함수의 최소화, 논리 함수식의 그룹핑의 세 과정으로 이루어지며, 같은 기본 논리 블록에 할당되는 출력의 적항과 변수들을 많이 공유하게 하여 기본 논리 블록 수와 최대 레벨 수를 최소화한다. 에뮬레이션 보드의 배선 구조와 FPGA 칩이 갖는 제한 조건들을 만족시키면서 서로 다른 칩 사이에 연결된 신호선 뿐만 아니라 서로 다른 그룹 사이에 연결된 신호선 수의 최소화를 목적 함수로 하는 새로운 회로 분할 알고리듬을 제안한다 여러 FPGA 칩으로 구성된 에뮬레이션 보드는 완전 그래프와 부분 그래프를 결합한 새로운 배선 구조로 회로의 크기에 관계없이 칩 사이의 지연 시간을 최소화하도록 설계하였다. 논리 분석기를 이용하여 구현된 회로에서 검증을 원하는 내부신호에 대한 파형을 PC의 모니터로부터 관측할 수 있다. 제안한 에뮬레이션 시스템의 성능을 평가하기 위하여 상용 회로중 하나인 화면4분할기 회로를 에뮬레이션 보드상에 설계하여 동작 시간과 기능을 확인한 결과, 14.3MHz의 실시간 동작과 함께 기능이 완전함을 확인할 수 있었다.

일부 대학 스켈링센터 방문환자의 흡연 및 음주와 치주낭 형성의 관련성 (Relationship between Smoking, Drinking and Periodontal Pocket Formation of Patient was Visited at College Scaling Center)

  • 본 연구는 일개 대학 치위생과 스켈링센터에 방문한 환자 177명을 대상으로 흡연 및 음주와 치주질환과의 관련성에 관한 후속연구가 지속적으로 진행되어야 할 필요성이 있다고 사료되어 치주낭 측정과 설문지를 이용하여 조사하였고, 흡연 및 음주여부와 치주질환의 관련성을 분석하여 얻은 결과는 다음과 같다. 1. 성별에 따른 치주낭 보유 분악수는 남자와 여자 모두 1.09로서 유의한 차이가 없었다. 2. 연령에 따른 치주낭 보유 분악수는 19세 이하 0.81, 20-29세 1.08, 30세 이상 2.46으로 연령이 증가됨에 따라 유의하게 높게 나타났다(p<0.01). 3. 결혼여부에 따른 치주낭 보유 분악수는 기혼 2.42로 미혼 0.98보다 유의하게 높게 나타났다(p<0.05). 4. 경제적 상태에 따른 치주낭 보유 분악수는 '상' 1.68, '중' 1.15, '하' 0.68로서 경제적 상태가 어렵다고 생각할수록 낮았지만 유의하지 않았다. 5. 흡연여부에 따른 치주낭 보유 분악수는 흡연자 1.11로서 비흡연자 1.07보다 유의하게 높았다(p<0.05). 6. 음주여부에 따른 치주낭 보유 분악수는 음주자 1.03 으로서 비음주자 1.24보다 낮게 나타났으나 유의하지 않았다. 7. 치주낭 보유 분악수에 유의한 영향을 미치는 독립변수는 연령, 결혼상태, 흡연여부로 나타났다. 결론적으로 흡연은 치주질환과 관련성이 있다는 결과를 도출하여 기존연구의 연장선상에 있지만 음주와 치주질환의 연관성은 없는 것으로 또 다른 연구결과를 나타냈다. 이로써 구강보건전문가는 환자들에게 구강건강을 유지 및 증진시키기 위해서 금연지도가 필수로 이루어져야할 것이며, 지속적인 관리를 위해서는 금연상담프로그램이 마련되어야 할 것이다.
