Burkitt lymphoma (BL) is an aggressive form of non-Hodgkin's B-cell lymphoma found primarily in the pediatric population. In the oral cavity, this tumor can grow rapidly and often brings about facial swelling or development of an exophytic mass involving the jaws. A 5-year-old boy was referred for swelling and pain in the left mandibular area. The patient showed diffuse swelling on the left side of the mandible and firm-moderate tenderness upon palpation. An intraoral examination showed moderate mobility and sensitivity to percussion on the left primary first and second molars, without severe caries. A radiographic examination revealed complete loss of the lamina dura on the left primary second molar and permanent first molar. There was a radiolucent osteolytic lesion and destruction of the cortical bone of the left mandibular body. Based on the clinical, radiographic, and immunohistochemical findings, the patient was diagnosed with BL, and was referred to a pediatrician for systemic evaluation and intensive chemotherapy. Even before the completion of chemotherapy, the swelling resolved and the displaced teeth were relocated to a normal position. This patient showed a good prognosis due to prompt diagnosis and intensive chemotherapy. Early diagnosis and referral for treatment can prevent the development of BL.

Cleidocranial Dysplasia(CCD) is an autosomal dominant human bone disease characterized by abnormal clavicles, patent sutures and fontanelles, and dental anomalies. Among dental anomalies, it is characterized that permanent dentition is severly disturbed due to multiple supernumerary teeth and abnormalities of tooth morphology. A eight-year-old female patient diagnosed as cleidocranial dysplasia visited in our hospital. Upon clinical oral exam, retained deciduous teeth, constriction of dental arch, anterior cross bite, and multiple dental caries were observed. In the dental panoramic radiograph, retained deciduous teeth and multiple supernumerary teeth in the maxilla and the mandible were found. In the cephalometric radiograph, open sutures and wormian bones were seen. In the chest P-A view absence of clavicles was observed. The cleidocranial dysplasia patients have eruption problems in permanent dentition both in regions with and without supernumerary teeth. The severely delayed or arrested eruption of permanent teeth has been ascribed to various factors : 1) The presence of multiple supernumerary teeth, 2) malformed roots with lack of cellular cementum, 3) the jaw bone being too dense, and 4) abnormal resorption of bone and primary teeth. Formation and maturation of primary teeth in cleidocranial dysplasia are normal, whereas the permanent dentition has various anomalies. Therefore, dentists should understand the development of dentition in cleidocranial dysplasia, and treat them in proper time.

Infraocclusion is defined as tooth whose relative occlusal movement was blocked during the period of active eruption due to ankylosis and so on. Then infraoccluded tooth remains under the occlusal plane composed by adjacent structures showing normal eruption patterns. Untreated infraocclusion may cause: prolonged retention of infraoccluded teeth; extrusion of apposed teeth; destruction of periodontal tissues by occlusal force and food packing; increased sensitivity for dental caries; and disturbances on eruption pathway of succedaneous teeth. Therefore, periodic check-ups and proper treatments are required. There are many treatment options on infraoccluded deciduous molars such as periodic observation, conservative method, restoration and space regaining with extraction of the teeth. The choice of treatment may depend on the presence of succedaneous teeth, time of diagnosis and degree of infraocclusion. In this case report, three patients showing displacement of the second premolars due to infraocclusion of upper second primary molars, were treated by means of space regaining with removable orthodontic appliances and extraction of ankylosed primary molars. All malpositioned permanent premolars in the 3 cases showed ordinary eruption pathways after treatment.

There have been efforts that inhibit development of dental caries by sugar substitution. But, it is controversial if xylitol has anticariogenic effect in the presence of sucrose. And there are few papers dealing with the combined action of xylitol and sucrose. For the purpose of resolving this controversy, the author investigated the effect of xylitol on enamel demineralization and on adhesiveness of S. mutans to hydroxyapatite in the presence of sucrose. Five experimental solutions were prepared as follows: (S: sucrose, X: xylitol) Group 1: BHI broth Group 2: BHI+1% S Group 3: BHI+0.75% S+0.25% X Group 4: BHI+0.5% S+0.5% X Group 5: BHI+0.25% S+0.75% X Group 6: BHI+1% X Each solution was inoculated with $100{\mu}l$ of S. mutans JC-2. And saliva coated hydroxyapatite beads were put into each experimental solution. And then each solution was incubated at $37^{\circ}C$ under anaerobic condition. After incubation, the adhesiveness of S. mutans on hydroxyapatite was evaluated. The Vickers hardness numbers were measured on extracted human primary teeth, and these teeth were dipped into the same experimental solution and incubated at $37^{\circ}C$ under anaerobic condition for 48hours. Surface microhardness were measured again after incubation. The obtained results were as follows; 1. In the presence of sucrose, xylitol can reduce the adhesiveness of S. mutans on hydroxyapatite surface from the ratio of 25% sucrose to 75% xylitol(P<0.05). 2. In the presence of sucrose, xylitol can reduced demineralization of primary teeth enamel surface from the ratio of 50% sucrose to 50% xylitol(P<0.01).

This study evaluated the influence of chemomechanical caries removal agent $Carisolv^{TM}$(MediTeam, Sweden) for composite resin adhesion to sound human permanent and primary dentin. The buccal/labial surfaces of 80 permanent molars and 80 primary incisors were used. Four types of adhesives and one composite resin were used; AQ Bond(Sun Medical, Japan), Clearfil SE Bond(Kuraray, Japan), Single Bond(3M, USA), Scotchbond Multi-Purpose(3M, USA) and Z100(3M, USA). One drop of $Carisolv^{TM}$(MediTeam, Sweden) was pretreated on the dentin for 0 second(control) and 60 seconds. The specimens were thermocycled for 1,000 times in baths kept 5 degrees C and 55 degrees C with a 30 seconds dwell time. Shear bond strengths were tested and the data was statistically analyzed using one-way ANOVA with subsequent post hoc Scheffe test at p<0.05. $Carisolv^{TM}$ treatment significantly decreased the shear bond strength. Shear bond strength of permanent dentin was significantly higher than that of primary dentin. Clearfil SE Bond treatment groups showed the highest shear bond strength and AQ Bond treatment groups showed the lowest shear bond strength.

The presence of a "prismless" layer on the enamel surface particularly on deciduous teeth has been reported by a number of workers. This structure, which appears to lack the normal prism delineations, could interfere with tag formation and hence, reduce bonding to such surfaces. The purpose of this study was to investigate the relationship of etching times on the effect of acid etching on primary enamel with respect to the quality of etching patterns. Labial surfaces of 32 extracted or exfoliated caries-free primary anterior teeth were used. 35% phosphoric acid gel was used only cervical regions of labial surfaces for each etching time group, 15, 30, 45 and 60 seconds. The surfaces were then washed with water for 20 seconds and dried with air spray for 20 seconds. 1. The Type 3 is 75% when the 15 seconds acid etching time was used. 2. The Type 1 is 38% and Type 2 is 75% when the 30 and 45 seconds acid etching time was used. 3. The Type 1 is 25% and Type 2 is 75% when the 60 seconds acid etching time was used. 4. An etching time of 60 seconds produced a constant and regular etching pattern. 5. There is a significant difference between the groups with respect to the patterns of etch achieved(p<0.05). 6. We confirmed that the acid induced patterns(type 1, 2) became more pronounced when the application time increased(p<0.05). $45{\sim}60$ seconds was the optimal time for etching on the primary enamel.

Deep caries in primary molars without early intervention frequently induce a pulpal disease and consequent furcation lesion with fistulous openings Up to now, majority of the textbooks on pediatric dentistry and literatures have described that extraction of the inflicted teeth is indicated for these cases and in reality these teeth have usually been extracted in the dental clinics. However when we recognize the excellent capacity of bone regeneration in children and the presence of numerous accessory canals at furcation areas, the removal of infection source in pulp by pulpectomy and inflammatory granulation tissues at furcation areas by furcal curettage might open the possibility of rapid healing at the furcation regions. In this report, 10 cases of primary molars in 3 to 6-year-old children with fistulous openings and furcation lesions in moderate size of 2 to 4mm in depth radiolucency at furcation lesion have been chosen. After pulpectomy and furcal curettage, evident bone regeneration was detected radiographically in all cases. Through the cases, we came to realize that all the cases previously described are not the indications of extraction and this approach could make many cases with pulp and furca combined lesions survive and remain healthy in the children's dental arches. However, in order for this approach to acquire objective appropriateness, it is thought that more scrupulous evaluation is desirable on the various factors regarding the indication such as the extent of furcation lesions, absorption status of teeth, amount of covering bone on succeeding teeth and so on.
group($22\%\;vs.\;38\%$, p=0.24). The 5YSRs of 21 patients of primary tumor extension to adjacent sites and the other 13 patients of tonsillar proper site were $28\%\;and\;38\%$, respectively but the difference was not significant statistically(p=0.52) There was a statistically significant difference in 5YSRs between the groups of the Patients who received radiotherapy in less than 61days vs more than 60days($60\%\;vs.\;18\%$, p=0.027). All living Patients without any tumor progression(n=11) had suffered from serious late sequelae such as xerostomia, edentia, dental caries and one patient had the osteoradionecrosis of mandible. On univariate analysis. the duration of radiotherapy and T-stage were the significant prognostic factors affecting 5YSR. On multivariate analysis, also the duration of radiotherapy was the only significant Prognostic factor(p=0.01). Conclusion : There was no survival difference between the radiotherapy alone and with neoadiuvant chemothe groups. Although it was a retrospective study, the role of conventional radiotherapy alone could be effective as the local treatment modality only for the early stage of tonsillar carcinomas. But for the purpose of more improved survivals and better quality of lives of living patients, other altered fractionation such as hyperfractionated radiotherapy with shorter treatment time and smaller fraction size rather than conventional radiotherapy might be beneficial and these prospective studies are needed.

The purpose of this study was to compare the effect of sodium fluoride(NaF) varnish and potassium iodide(KI) on remineralization efficacy of silver diamine fluoride(SDF) by measuring microhardness and evaluating surface morphology by scanning electron microscope(SEM). Artificial caries lesions were induced on extracted primary molars and vickers microhardness was measured. Specimens were randomly separated into 4 groups for treatment. The specimens in group I were treated with SDF, group II with NaF varnish after SDF, group III with KI after SDF and group IV with distilled water. After 8 days of pH cycling, vickers microhardness was measured and difference before and after treatment was calculated. For SEM, 2 samples were evaluated respectively after enamel polishing, lesion formation and after pH cycling. Group III showed highest increase in microhardness. Group I showed higher increase in microhardness than Group II but without statistical difference. Group IV showed lowest increase in microhardness value among 4 groups. On SEM image, group I, II and III showed smoother and less irregular surface compared to group IV. Amorphous crystal pellicles were observed in group III. In conclusion, SDF, SDF and NaF, SDF and KI groups showed smoother surface and increase in microhardness suggesting the possibility that remineralization effect might take place in oral conditions. In addition, in limited conditions of this study, applying NaF varnish after SDF did not increase the remineralization efficacy of SDF while KI significantly increased the remineralization efficacy of SDF. However, additional study considering various conditions that might affect demineralization and remineralization in clinical situations need to be conducted.

Dental caries and periodontal disease continue to present unique problems in the dental management of the persons with disabilities because the chronicity of oral diseases complicates the primary physical or mental disability. The increased prevalence of dental disease in most persons with disabilities is probably not due to any inherent proclivity for dental disease but more likely evolves because dental care receives less attention. Prosthetic dentistry procedures are not contraindicated for most patients with physical and mental disabilities. Fixed bridges may be feasible if the patient or care provider can maintain adequate oral hygiene and the patient's disability dose not preclude this type of prosthesis. Removable partial or full dentures may be indicated if the patient or care provider can easily remove the prosthesis and care for it. Although most persons with disabilities need no additional behavior management modalities to complete dental care, some persons require professionally recognized behavior management techniques during treatment, such as physical restraint, pharmacologic agents, or general anesthesia. Hospitalization and the use of general anesthesia are sometimes required to deal effectively with the extreme management problem patient. This patient with mild mental retardation was fearful of dental treatment. Routine restorative, surgical and prosthetic dentistry procedures were performed under general anesthesia.
