A Study on the Improvement of Preventive Measures for Improving the Safety of Chemical Reactor (화학반응기의 안전성 향상을 위한 예방조치 개선에 관한 연구)

  • Based on the cases of fire and explosion accident in the chemical reactor, thr problems of preventive measures installed in the chemical reactor were analyzed. The chemical reactors produce a variety of chemicals and install rupture disk to relieve the pressure that rises sharply in the event of a runaway reaction. In order to maintain the function of the rupture disk, the emissions was allowed to be discharged into the atmosphere, resulting in fire and explosion accidents. As a way to improve this, safety instrumented system based on the safety integrity level(SIL3) was applied as a preventive measures for chemical reactor. Two emergency shur-off valves are installed in series on pipe dropping raw materials for chemical reactor so that the supply of raw materials can be cut off even if only one of the two emergency shut-off valves is operated during the runaway reaction. The automatic on/off valve is installed in parallel in the supply pipe of the reaction inhibitor so that the reaction inhibitor can be injected even if only one valve is opened at the time of the runaway reaction.

Necessity of Adverse Event Reporting System through the Trend of Internet News about Safety of Herbal Medicine (한약의 안전성에 대한 인터넷 보도의 특성을 통해 본 한약 부작용 관리 체계 확립의 필요성)

  • Objective : The aims of this study are to investigate the trend of internet journalism about the toxicity and safety of the herbal medicine, and to suggest the regulatory solution of the issue. Method : In this study, we had searched the internet news article published from 2001 to 2011 in the five major portal sites-NAVER, DAUM, Nate, Google Korean, and Yahoo Korean. The search terms were 'herbal medicine', 'adverse event', 'toxicity'. If the articles described the same event in the same form and tone, the articles were considered overlapping. The overlapped articles were excluded. The articles were categorized by the form and tone. The form categories were straight news, interpretative story, editorial, interview, and the tone categories are the positive, the negative, and the neutral. The regulations were searched about the negative issue. Result : Total 56 articles were reviewed. There were 19 positive articles, 29 negative articles, 8 neutral articles. Most negative issues have the proper regulations, but insufficient measures for the adverse event reporting system. Conclusion : The herbal medicine specified adverse event reporting system is essential.

Effect of the family dentist system on oral health status of children and adolescents in Busan, Korea (치과주치의 사업이 아동·청소년의 구강건강에 미친 변화: 부산지역을 중심으로)

  • Objectives : The purpose of this study was to evaluate the effect of the family dentist system on oral health status of children and adolescents of Community Children's Centers in Busan, Korea. Methods : The subjects of this study were 81 children and adolescents, 6 to 14 years old, using Community Children's Centers. The oral health survey was conducted on the subjects from the starting stage of family dentist program in 2009 to the evaluation stage in 2010. Dental health status was examined by a trained dentist according to the guideline proposed by the World Health Organization. In addition, the information on the oral health knowledge, belief, and process of dental care were obtained using questionnaires. Data were analyzed using the paired samples t-test. Results : Percentages of subjects with fissure sealants on permanent teeth and filling rate among DMF teeth of 2010 year were higher than those of 2009 year (P<0.05). However, the rate of decayed teeth among DMF teeth of 2010 year was lower than those of 2009 year (p=0.049). Conclusions : These findings showed that the family dentist system brought a positive effect on caries prevention and proper dental care of children and adolescents of the Community Children's Centers.

Development of a public health care linkage model within the community care system in Daejeon City (대전광역시 지역사회 통합돌봄 체계내에서 공공보건의료 연계 모델 개발)

  • Objectives: This study aimed to establish a linkage model involving regional responsible medical institutions after analyzing the existing conditions and deriving problems through qualitative analysis within the community care system. Methods: A total of 14 participants of this study were selected through the snowball sampling method, including 7 community care service providers and 7 service users. As for the research data, primary data were collected through interviews, and as a result of analyzing according to Aday&Anderson' model, a total of 5 catergories, 8 topics, and 22 sub theme were derived. Results: The problem derived from the interview is that division services are provided for each institution due to the absence of a key central institution of community care system, and users' commercial institutions is unclear. The second is the inconsistency between the needs and supply for community care, resulting in a possibility of delay in returning to the community after discharge. Based on these problems, it is necessary to unify it as an community care window of the Dong-community center. In addition, there is a need for public health centers to play an active role, and to establish a public-private joint system with the Health and Living Support Center to establish a model that can play a certain role. Conclusions: Therefore, based on the results of this study, it can be used as basic data when constructing community care model and applying it as an expanded model in the future.

Job Analyses of Health Care Managers in Group Health Care System (보건관리대행사업에서 보건관리자의 직무분석)

  • For developing the Group Health Care System, health managers' job structure were analysed in the aspects of content, amount, and process. As a trial research, data were collected by a standardized job analysis table to 6 doctors, 40 nurses, and 11 industrial hygienists of Group Health Care System. Health care managers were performing complex and intellectual jobs such as healh education for workers, managing health care, conference as well as more simple jobs like as filling diary. Especially, job was consisted of general job and health care management job in the proportion of 1:2.18. The major general job were data management related with the health statistics, and major health care management jobs were managing health care, health counselling, environmental management of working sites. Each specific jobs were required differentiated intellectual capacity, creativity, autonomy, psychic stress, and physical work; most respondents perceived that health care management jobs should require more inputs than general jobs. Additionally job satisfaction and perceived need on specific Job items were anzlysed. Results of this research, suggested through the field experiences in working sites, should be considered for improving the Group Health Care System.

  • Background: In this study, we aimed to investigate the recent trends for health care indicators including maternal mortality ratio, infant mortality rate, under-five mortality rate, life expectancy, years of life lost, and healthcare resources in South Korea, North Korea, Germany, Russian Federation, Mongolia, Vietnam, China, Czech Republic, Poland, and Hungary. Methods: We used data from five sources: World Health Organization, Federal Institute for Population Research, World Bank, Organization for Economic Cooperation and Development health statistics, and national statistics. Results: In the early 1990s, health indicators continued to improve in countries that switched to the health insurance system, but the gap widened in North Korea as health indicators worsened. Conclusion: The establishment of a sustainable health care system after unification of the Korean peninsula requires substantial changes in the health care system and efforts to improve the health of North Koreans.

  • To investigate perception and attitude toward Group Health Management and Service System for Small and Medium Industries, a survey using self-administered questionnaire was conducted to a part of industries in Inchon, at October 1992. Major dependent variables were perception, attitude, and practice related with Group Health Management and Service System; these variables were measured by 3-point Likert like scale consisted of 7, 5, and 5 items, respectively. Data were collected in 149 industries,72.7% out of 205. Perception nab slightly high, 1.25; attitude was some positive, 1.46; and practice was some passive,0.94. Major determinants of perception, attitude, and practice were sex and age of industry's health manager; perception influenced upon attitude, and perception and attitude had positive effect upon practice. To activate Group Health Management and Service System, it is necessary to develop education and promotion programs for industry's health managers of small and medium industries.

Content Analysis of the Health Counseling by the Intranet in University : 2000-2004 (일개 대학교 5년간 인트라넷 건강상담 내용분석)

  • Objectives: The research was aimed at analyzing the contents of university intranet for systematically execution of the healthy information provision and healthy consultation services from 2000 January to 2004 December. Methods: We have analyzed 300(28.3%) the instances of accomplished health consultation cases from the whole 1,059 instances which were the replied in a university intranet. Results: According to the contents of health consultation in ICPC code, a general symptom 91 cases(30.3%) was most, muscle-skeletal system 44 cases(14.7%), and digestive system 43 cases(14.3%) in order of cases of health counselling. The symptoms and complaint with 155 cases(51.7%) were most in the distribution of the health counselling contents by 17 charter of ICPC. The most common reason of counselling by 17 charter of ICPC were as follows; questions about the symptom and diagnosis(118 cases, 39.3%), the preventive and treatment methods(91 cases, 30.0%), and medical fee(20cases, 6.7%) in order. We mainly answer on the content of health counselling were as follows; make an offer of medical information and knowledge(48.3%), recommend visit clinic or hospital(23.7%), guide to treatment(12.7%), and so on. Conclusions: This research showed that the program of health counselling may not meet completely the high quality and adequate distribution of health counselling by the intranet in a university by content analysis. The finding suggests that health counselling by intranet in a university may be used to supplement of systemic improvement on the intranet Q/A format from current lack of essential health information and security for the quality of the health counselling.

Cost-Benefit Analysis of Back School Program for Occupational Low Back Pain Patients (직업성 요통환자에서 재활 프로그램(Back School Program) 도입의 비용-편익분석)

  • Although occupational low back pain accounts for $20\sim40%$ of all occupational illness and injury, there are limited numbers of studies regarding the effectiveness of back school program. The objective of this study was to evaluate the economic benefit of back school program for early return to work of occupational low back pain patients in the current occupational injury compensation and management system. The cost-benefit analysis in this study was conducted to evaluate the relative magnitude of benefit to cost. The total cost was estimated by calculating the value of components in back school program according to governmental budget protocol. The back school program was consisted of three major approaches, pain center, work-hardening program and funcional restoration program and each of components had various facilities and experts. The total amount of cost was estimated as 250,866,220 won per year. The most promising type of back school program were quite intensive (a 3 to 5-week stay in a specialized center), therefore, if we adopted the 5-week stay course, 10 courses could be held in a year. Following to the medical act, 20 patients per doctor could participate in a each course, ie, total 200 patients in a year. As a result, we could estimate the cost of 1,254,331 won a patient. We estimated the benefit by using data of a few local labor offices about average medical treatment beneficiary and off-duty beneficiary of 46 occupational low back pain patients in 1994. Ullman and Larsson (1977) mentioned that the group of chronic low back pain patients who participated in back school program needed less time to recover by 48.4% of beneficiary duration. And in the trying to estimate the benefit, we asked 10 rehabilitation board certificate doctors about reduction proportion of treatment cost by introducing back school program. The answered reduction proportions were in the range of $30\sim45%$, average 39%. As a final result, we could see that the introduction of back school program in treatment of chronic occupational low back pain patients could produce the benefit to cost ratio as 3.90 and 6.28. And we could conclude that the introduction of back school program was beneficial to current occupational injury compensation and management system.

The Effects of Temperature on Heat-related Illness According to the Characteristics of Patients During the Summer of 2012 in the Republic of Korea

  • Objectives: This study was conducted to investigate the relationship between heat-related illnesses developed in the summer of 2012 and temperature. Methods: The study analyzed data generated by a heat wave surveillance system operated by the Korea Centers for Disease Control and Prevention during the summer of 2012. The daily maximum temperature, average temperature, and maximum heat index were compared to identify the most suitable index for this study. A piecewise linear model was used to identify the threshold temperature and the relative risk (RR) above the threshold temperature according to patient characteristics and region. Results: The total number of patients during the 3 months was 975. Of the three temperature indicators, the daily maximum temperature showed the best goodness of fit with the model. The RR of the total patient incidence was 1.691 (1.641 to 1.743) per $1^{\circ}C$ after $31.2^{\circ}C$. The RR above the threshold temperature of women (1.822, 1.716 to 1.934) was greater than that of men (1.643, 1.587 to 1.701). The threshold temperature was the lowest in the age group of 20 to 64 ($30.4^{\circ}C$), and the RR was the highest in the ${\geq}65$ age group (1.863, 1.755 to 1.978). The threshold temperature of the provinces ($30.5^{\circ}C$) was lower than that of the metropolitan cities ($32.2^{\circ}C$). Metropolitan cities at higher latitudes had a greater RR than other cities at lower latitudes. Conclusions: The influences of temperature on heat-related illnesses vary according to gender, age, and region. A surveillance system and public health program should reflect these factors in their implementation.
