Evaluation of the Appropriateness of Empirical Antibiotic Prescription after Implementation of Antibiotic Treatment Guidelines for Pneumonia in a Hospital (원내 폐렴 진료 지침 수립 후 경험 항생제 선택의 적절성 평가)

  • Background : The Antimicrobial Stewardship Program promotes interdisciplinary interventions and targeted recommendations for the proper utilization of antibiotics. In particular, the aim of the program is to avoid indiscriminate use of broad-spectrum antibiotics based on the documented literature on the significant impact of unsystematic usage of antibiotics on the distribution of antibiotic-resistant microorganisms. To improve the care process for pneumonia treatment using antimicrobial agents, institution-level guidelines were established and disseminated at the Seoul National University Bundang Hospital in April 2016. In this study, we evaluated changes in the physicians'antibiotic prescribing patterns both before-and after-the implementation of the guidelines. Methods : The electronic medical records of inpatients who were prescribed with one or more antibiotics in May 2014 (Group A) and May 2016 (Group B) were reviewed. Data on demographic characteristics, clinical outcomes, and antibiotic prescriptions were collected and the prescription records were compared both before- and after- the implementation of the guidelines. Results : A total of 180 patients were included in the study: 77 patients in group A and 103 patients in group B. The baseline characteristics of the patients were not significantly different between the two groups. Community-acquired pneumonia was the most common diagnosis in both the groups and the difference was not significant (68.8% vs. 67.9%; p=0.67). The type of antibiotic prescriptions used for empirical treatment was not different between the two groups. The most commonly prescribed empirical antibiotics were cephalosporins, with no significant difference (p=0.31). One of the most inappropriately used antibiotics was piperacillin/tazobactam and the rate of prescription was similar in both the groups (p=0.68). The rates of appropriate empirical selection of antibiotics remained unaltered between the two groups (67.5% vs. 71.8%; p=0.53). Conclusions : Implementation of the guidelines only exhibited no significant effect on the antibiotic prescribing patterns of physicians for the treatment of pneumonia. To improve the adequate use of empiric antibiotics, more active interventions and closer monitoring of the feedbacks should be additionally considered and evaluated in future studies.

Effects of Forest Walking Based on Exercise Prescription on Body Composition of Older Individuals (운동처방을 적용한 숲길 걷기가 노인의 신체조성에 미치는 영향)

  • This study aimed to investigate the effects of forest walking based on an exercise prescription on body composition of older adults. Forty-four older adults (average age: 69.3 ± 3.1 years) participated in this study. The experimental group engaged in forest walking based on a prescribed exercise intensity considering the participants' respective health conditions. The participants walked three times a week for more than an hour each time for 8 weeks. By contrast, the participants in the control group spent their days according to their usual lifestyle. The analysis involved the following: ① a comparison of the measurements taken before and after the 8-week period of forest walking in the experimental group, ② a comparison between pre- and post-study measurements in the control group, and ③ a comparison of the changes (post-study minus pre-study values) between the experimental and control groups. The results were as follows: ① the experimental group showed significant decreases in weight, body fat mass, and body mass index; ② the control group exhibited significant decreases in muscle mass and ratio in right arm and left leg, muscle ratio in trunk, and body water mass in right arm; and ③ weight and body fat index decreased in the experimental group but increased in the control group. In conclusion, forest walking based on an exercise prescription had a positive impact on body composition of older adults.

Effects of Guaruhaebaekbanha-tang Extract on Beating Rate and LDH Activity in Cultured Rat Myocardial Cells (과루해백반하탕 추출물이 배양 심근세포의 박동수와 LDH 활성도에 미치는 영향)

  • To certify the protective effect of herbal medicine against oxygen free radical-induced myocardiotoxicity, cytotoxicity was measured using MTT, LDH activity and Beating rate assay in the presence of Guaruhaebaekbanha-tang(GHBT) extracts or single constituents of this prescription. Myocardial toxicity was evaluated in neonatal rat myocardiocytes in cultures. In the present study, xanthine oxidase/hypoxanthine(XO/HX) resulted in a decrease in cell viability, increases in LDH activity in culture medium and decreases in beating rate in cultured myocardial cells. In the effect of GHBT extract, it showed the prevention from the XO/HX-induced cardiotoxicity by the increases of cell viability and beating rate as well as the decrease of LDH activity. In the protective effect of Fructus Trichosanthis(FT), Bulbus Allii Macrostemi(BAM) and Rhizoma Pinelliae(RP), all the extracts were significantly effective in the protection of XO/HX-induced cardiotoxocity in cultured myocardial cells by the increase of beating rate as well as th decrease of LDH activity. From these results, they show that XO/HX is cardiotoxic in cultured myocardial cells derived from neonatal rat, and it suggests that GHBT, FT, SAM, RP extracts are positively effective in the blocking in XO/HX-induced cardiotoxicity.

Development of customized patient data analysis process for quality of care improvement : focused on foreign patients (진료 품질 향상을 위한 환자 데이터 맞춤형 분석 프로세스 개발: 외국인 환자를 중심으로)

  • Purpose: The purpose of this study was to find meaningful patient groups of disease using foreign patients data and analyze implemented test of the patient groups. Methods: The data was collected by foreign patients' EMR data of K university hospital. The author proposed tree-form patients' characteristic diagram through statistical methods that association rule, proportion test, clustering using prescription information and questionnaire information. Results: This study's analysis process was applied high blood data and diabetes data. Analysis showed other characteristic of meaningful patient groups in high blood and diabetes. In high blood, test implementation rate of patient group showed the differences. And in diabetes, test implementation rate of patient group and implemented test list showed differences. Conclusion: The result of this study can play a role as basic data that can be clinical testing standard in preventive aspect. Eventually, 5 dimensions of SERVQUAL will be improved by this study's process.

Late Biological Effect of Gamma Radiation and the Effect of Bu-Zhong-Yi-Qi-Tang on Radiation-Induced Late Biological Effect in Mice (감마선 조사 마우스의 만성장해와 보중익기탕 투여 효과)

  • We performed this study to determine the late biological effect of gamma radiation and effect of Bu-Zhong-Yi-Qi-Tang (BZYQT), a prescription of traditional Oriental medicine, on radiation-induced late biological effect (survival, hematological change, carcinogenesis) of mice irradiated with 3 Gy of gamma-radiation. There were little difference in body weights between normal and irradiated mice. Survival rate were decreased in irradiated mice and the survival rate and mean survival time of the groups treated with BZYQT were far better than the irradiation control group. A significant elevation of total leukocyte or lymphocyte counts was seen at week 4 and 12 of the group treated with BZYQT. Stimulated recovery by the extract from BZYQT was also observed in thrombocyte. Main gross findings of irradiated mice were appeared as enlargement of spleen, thymus and liver, tumorous nodules of lung and cyst or mass of ovary. Microscopically, there were various findings including hematopoietic and lymphoid tumor, lung cancer, ovarian cancer and cancer of other lesions. BZYQT reduced the incidence of tumor development. Further studies are needed to characterize better the protective nature of ingredients and active compounds.

Follow-up Study on Chronic Idiopathic Urticaria Patients Treated with Pyungweesangamibang (평위산가미방으로 치료한 만성 특발성 두드러기 환자의 추적조사)

  • Objective : The purpose of this study is to investigate the recurrence rate of Chronic Idiopathic Urticaria patients treated with Pyungweesangamibang through long-term follow-up survey.Methods : This tracing study was conducted for sixty-seven Chronic Idiopathic Urticaria patients who received and have finished treatment with Pyungweesangamibang at Woobo Korean Medicine Clinic in 2013 and 2014.Result : 81.4% of the patients, who had spent over 1 year since the finish of treatment with Pyungweesangamibang, did not relapse. In case of more than 2 years elapse, 66.7% of the patients did not suffer relapse.Conclusion : The result of tracing recurrence of the Chronic Idiopathic Urticaria patients treated with Pyungweesangamibang describes that the Korean medical treatment (Pyungweesangamibang prescription) shows lower recurrence rate than other general treatment shows.

Metabolic Syndrome Prevalence and Lifestyle by Age and Metabolic Syndrome Status in Women Religious (여성수도자의 연령별 대사증후군 유무별 대사증후군 위험요인과 생활습관 차이)

  • Purpose: The purpose of this study was to analyze the prevalence, risk factors of metabolic syndrome, and lifestyle in religious women by age and metabolic syndrome status between the metabolic syndrome group and the normal group. Methods: As the subjects for this study, 125 religious women in the city of D, H, S, Y, participated in this study. The diagnostic criterion of metabolic syndrome used was the National Cholesterol Education Program Adult Treatment Panel III. Results: The prevalence of metabolic syndrome was higher as the religious women got older. The metabolic syndrome group in their forties showed higher waist circumference, triglycerides, and lower HDL-cholesterol than the normal group. Among those in their fifties, the metabolic syndrome group had higher waist circumference, fasting glucose, triglyceride and lower HDL-cholesterol than the normal group. In their sixties, the metabolic syndrome group had higher fasting glucose, triglyceride and systolic blood pressure than the normal group. Conclusion: The metabolic syndrome group in their forties showed that their practice rate of 'trying to avoid stresses at work', 'taking prescription medicines' was low. For those in their fifties, the practice rate of 'reducing overeating' and 'choice of low fat meats' was low. Finally, in the group of those in their sixties, 'reducing fried foods' was low.

Development of a smart pillbox and improvement of the medication adherence for the efficient management of medicine administration (효율적인 복약 관리를 위한 스마트 약상자의 개발과 약물순응도의 향상)

  • As Korea is expected to enter an aging society in near future, the number of patients who need a long-term prescription is expected to increase as well. In particular, Korea shows the highest in both the incident rate and the death rate among OECD member nations. The current situation requires more strict monitoring and management for patients' taking medicine, there are a few practical problems such as personnel expenses. In order to realize the efficient management of medicine administration, this paper proposes a smart pillbox that is developed based on IT technologies. Through the experiments in which actual patients participate, it is proved that the smart pillbox is beneficial to the patients taking medicine.

The Study on Exercise Prescription of Patients with NIDDM (인슐린비의존형 당뇨환자의 운동처방에 관한 연구)

  • The purpose of this study was to investigate the glucose and lipid metabolic changes physical exercise of patients with NIDDM. The physical exercise consisted of 15 minutes per day on a bicycle ergometer at $70\%$ maximum heart rate. The results of this study which were calculated from the level of glucose and lipid metabolism of the preexercise and after 2weeks exercise were as follows. 1. Blood glucose was siginificiantly decreased after physical exercise of two weeks(P<0.005). 2. Total cholesterol showed a tendency to decrease after physical exercise of two weeks, but the difference was statistically insignificiant. 3. Triglyceride showed a tendency to decrease on female, but triglyceride on man was inclose after exercise. 4. HDL showed a tendency to increase after physical exercise of two weeks, but the results were statistically insigificiant. 5. LDL showed a tendency to decrease after physical exercise of two weeks, but the results were statistically insignificiant. 6. HDL/cholesterol showed a tendency to increase aftre physical exercise of two weeks, but the results were statistically insignificiant.

Hypoparathyroidism in a diabetic dog (당뇨병 이환견에서 발생한 부갑상샘기능저하증 증례)

  • A miniature schnauzer (8 years old, male castrated), diagnosed as diabetes mellitus 6 months ahead, were referred to the Veterinary Medical Teaching Hospital of our University with hindlimb stiffness, facial rubbing and tetany. Serum chemistry showed severe hypocalcemia, hypomagnesemia, and measured iPTH (intact pharathyroid hormone) concentration was low (< 3.0 pmol/L). Consistent rate injection of calcium gluconate (282 mg/kg over 30 min) resulted in prompt relief of the tetany. Then typical treatment for hypoparathyroidism (calcium gluconate and dihydrotachysterol per os) with magnesium administration could control the neuromuscular signs. Diabetes mellitus is being controlled by insulin therapy and prescription diet. Hypomagnesemia in diabetic dog may be induced hypoparathyrodism.
