The Outcomes of an e-Wellness Program for Lupus Patients in Thailand: A Participatory Action Research Approach

  • Objectives: Systemic lupus erythematosus (SLE) or lupus patients usually experience various physical and psychological challenges. Since the coronavirus disease 2019 pandemic, these challenges have become even harsher. Using the participatory action research approach, this study evaluated how an e-wellness program (eWP) impacted SLE-related knowledge and health behaviors, mental health, and quality of life among lupus patients in Thailand. Methods: A 1-group, pretest-posttest design study was conducted among a purposive sample of lupus patients who were members of Thai SLE Foundation. The 2 main intervention components were: (1) online social support, and (2) lifestyle and stress management workshops. Sixty-eight participants completed all the study requirements, including the Physical and Psychosocial Health Assessment questionnaire. Results: After being in the eWP for 3 months, participants' mean score for SLE-related knowledge increased significantly (t=5.3, p<0.001). The increase in sleep hours was statistically significant (Z=-3.1, p<0.01), with the percentage of participants who slept less than 7 hours decreasing from 52.9% to 29.0%. The percentage of participants reporting sun exposure decreased from 17.7% to 8.8%. The participants also reported significantly lower stress (t(66)=-4.4, p<0.001) and anxiety (t(67)=-2.9, p=0.005). The post-eWP quality of life scores for the pain, planning, intimate relationship, burden to others, emotional health, and fatigue domains also improved significantly (p<0.05). Conclusions: The overall outcomes showed promising results of improved self-care knowledge, health behaviors, mental health status, and quality of life. It is recommended that the SLE Foundation continues to use the eWP model to help the lupus patient community.

Comparative Study for Analgesic and Adverse Effects of 8% and 10% Hypertonic Saline in Epidural Adhesiolysis (경막외 유착용해술시 투여되는 8%와 10% 고장성 식염수농도의 제통효과 및 부작용의 비교)

  • Background: Epidural Adhesiolysis is an interventional pain management technique for patients with low back pain. In addition to local anesthetics and corticosteroid, hypertonic saline (NaCl) are used for the technique. Present study was aimed to compare analgesic and adverse effects of two different concentration of hypertonic saline in Epidural Adhesiolysis. Methods: Fifty-three subjects with low back pain with radiculopathy were assigned to one of two epidural adhesiolysis treatment groups: 8% (Group I, n=26) or 10% (Group II, n=27) hypertonic saline. 17 G epidural needle was inserted at sacral hiatus and catheter was advanced untill its tip was located at lesion site under fluoroscopic guidance. Subjects in all treatment groups received epidural corticosteroid and local anesthetic. And then, hypertonic saline injection via catheter were carried out daily for 3 days. Evaluation included assessment of pain relief (Numerical Rating Scale; NRS) at post-epidural adhesiolysis 1, 2, 3 days, 1 week, and 1, 3 months. We also looked for complications of epidural adhesiolysis at different concentration of hypertonic saline. Results: Statistical analysis demonstrated that NRS was not significantly different between two groups during 3 months after epidural adhesiolysis. There were no major differences of complications between two groups and disappeard after a few months without residual sequelae. Conclusions: We conclude that epidural adhesiolysis using 8% hypertonic saline is effective for relief of low back pain with proven lumbosacral fibrosis without any residual sequelae compared with 10%.

Development of Web-based Multimedia Contents for the Critical Care Practice of Nursing Students through Inter-College Collaboration (대학 간 통합 웹기반 중환자간호실습 콘텐츠 개발 및 적용)

  • Purpose: This study was conducted to develop Web-based multimedia contents for supporting student nurses' clinical practice on critical care, and to evaluate learners' responses. Methods: Based on the steps of Assessment, Design, Development, Implementation, & Evaluation(ADDIE) model, a total of 13 self-directed learning modules including live lectures and real video clips were developed through faculty collaboration of nine nursing colleges in Gwangju and Chonnam province. The finally developed multimedia contents were published on the Web of the learning management system at a local e-learning center. Results: The Web contents were evaluated after self-learning by 81 junior college nursing students who were encouraged to study it at their own pace during their two-week clinical practice at a medical or surgical intensive care unit of a university hospital and two hospitals. The knowledge (t = -27.66, p < .001) and self-evaluated clinical performance level(t = 7.54, p < .001) were significantly increased after learning of the Web contents and clinical practice, and satisfaction level that measured post-test only was 4.0 out of 5 point. Conclusion: The use of Web contents for critical care need to be extended as a complimentary material in a class room lecture or clinical practice of students to increase their self-learning ability and understandings of clinical knowledge and situation.

Effects of Computer Based Virtual Reality Program on Clinical Rehabilitation in Korea: A Meta-analysis (컴퓨터 기반 가상현실 프로그램의 국내 임상재활 적용 효과: 메타분석)

  • Meta-analysis was conducted to examine effects of computer based virtual reality on clinical rehabilitation in Korea. The data were analyzed as the ICF categorizations. The papers used in this study were located through domestic data search engines. The fifteen studies were selected among the studies published between 1995 and 2014. The PEDro score was used for qualitative assessment. The sample size and the average and standard deviations of pre and post tests were analyzed through a meta-analysis. The PEDro scores of the selected studies were 6 points higher and 338 participants were involved. The effect size of combined data was 1.030, activities and participation 1.120, sensory function 1.199 as indicated by a "large effect size" and the effect size of mental function was 0.557 as indicated by a "medium effect size." As a result, virtual reality program as a adjunctive therapeutic methods has proven the effect. In particular, the effects were demonstrated in activities and participation, sensory function. However there was no evidence of the effectiveness of mental function.

A Study Evaluating a Program for the Interaction between Children in Institutional Care and their Original Families (시설아동과 원가족 상호작용 프로그램 평가 연구)

  • The purpose of this study is to evaluate the program for enhancing the interaction capacity between the child care institutions and the original family (associate) and propose a program suitable for the child care institutions. The program was conducted at two institution located in Seoul and Cheongju, and the process evaluation and outcome evaluation were conducted. First, the problems that appeared in the process evaluation were recruiting and maintaining participants. As a result of the satisfaction of each program, it was found that the satisfaction of the program sharing the various experiences or sharing the small daily life was high, and it was positive for the improvement of the relationship. Parents showed improvement in the post -test rather than the pre-test. Through qualitative assessment, positive evaluations were made in programs where the family participated in activities that could not be done together in the camp or everyday life, rather than just one-time activity programs.

Development of Positive Behavior Support Model for Children in Child Care Institution (양육시설 아동을 위한 보편적 긍정적 행동지원 모형개발)

  • This study was designed to develop a positive behavior support model for children in child care institutions. For this purpose, a demand survey for PBS was conducted with 55 child care institution staff members. 76% of the respondents responded that PBS is needed to prevent problem behavior and can be a good alternative for personality education, and approximately 70% responded that they are willing to implement PBS. The specifications of the model are suggested as follows. First, the preparation step would consist of establishing a support team, educating staff members about PBS, selecting expected behaviors, assessing the current baseline behavior, and setting up a universal PBS environment. The application step would consist of instructing social skills, implementing reinforcement, personal goal-setting and assessing behavior, educating trouble-making students, and monitoring. Finally, at the outcome assessment period, measuring the change in target behavior from the pre-intervention to the post-intervention stage, change in social skills and academic achievement, and social validity is suggested. It is expected that application of this model to children in child care institution will decrease problem behaviors of students, enhance desirable behaviors, and boost the staff members' efficacy.

  • 2018 aged patient dataset (HIRA-APS), 33,821 patients admitted to long-term care hospitals were classified by major disease groups to identify the characteristics of medical use and provide effective policy data. First, it is necessary to operate specialized long-term care hospitals such as dementia, rehabilitation, hospice, and recovery specialization tailored to the characteristics of the patient's disease group. Second, the paralysis group, cerebrovascular disease group, and dementia group were the longest LOS and the cognitive impairment group is the longest by patient classification group, requiring medical demand and quality management for long-term patients. Third, the musculoskeletal disease group was the highest normal discharge, and support for the post-discharge home program is required in paralysis and cerebrovascular disease.

Effect of early robot-assisted training using virtual reality program in patient with stroke (가상현실을 이용한 조기 로봇보조 보행 훈련이 뇌졸중 환자에 미치는 영향)

  • Purpose: The purpose of this study was to determine the effect of early Robot-assisted training on gait ability, function and ADL in patients with stroke. Methods: 26 patients with stroke were recruited for this study. The subjects were randomly assigned to either the experimental group (EG) or the control group (CG), with 13 patients in each group. All subjects received a routine physical therapy. The robot-assisted training was for 30 min in the case of the EG subjects. The assessment tools of this study involved the gait ability, balance ability, function and ADL. The measurements were recorded before the intervention and after the intervention. Results: EG subjects and CG subjects, the variables measured after the intervention significantly differed from gait ability, balance ability, function and ADL without the FMA (p<0.05). The FMA was only effective experimental group after intervention. Also, there were significant differences in gait ability, balance ability, function and ADL without the FMA at post-test between the 2 groups (p<0.05). Conclusion: The findings indicate that early robot-assisted training exerts a positive effect on gait ability, balance ability, function and ADL in patients with stroke. This result indicates the possibility of application of the early Robot-assisted training to the management for stroke patients. Further studies are required to generalize the result for this study.

  • Most damages of civil infrastructures under natural disasters are frequently occurred at surrounding areas of the river or the road. Every year, Recovery for these disaster damages are performed by the government. Recently, the government decide to change current recovery plan system because current recovery plan which doesn't consider future disaster impacts at a site has been proved to be ineffective. Accordingly, new permanency recovery plan system is needed and its corresponding ideas are presented in this research considering more detailed disaster damage classifications and cause assessments. The proposed permanency recovery plan would also provide more systematic and diverse recovery response strategies including both two concepts, for example Preparedness considered by risk assessment and management, and Mitigation investigated by hazard impact analyses.

The Effects of SBAR Reports Education on Communication Clarity, Communication Skill and Report Confident for Nursing Students -Adult Nursing Clinical Practicum- (SBAR 보고교육이 간호대학생의 의사소통명확성, 의사소통능력 및 보고자신감에 미치는 효과 - 성인간호학 실습 -)

  • The purpose of this study was to investigate the effect of SBAR(Situation-Background-Assessment-Recommendation) reports on communication clarity, communication skill and report confident for nursing students. This study used a one group pre-post test design. The subjects of this study were 44 nursing students from G city, and collected by self - report questionnaires from march 6 to april 20, 2018. Data were analyzed by frequencies, paried t-test using SPSS Win 23.0. After SBAR reports education, levels of nursing student's communication clarity(t=-8.12, p<.001), communication skill(t=-4.37, p<.001) and report confident(t=-7.67, p<.001) were significantly higher than levels before SBAR reports education. SBAR reports education improved the communication clarity communication skill and report confident. It is recommended to use a lot of SBAR in clinical practice.
