Epidemiology of trigeminal neuralgia: an electronic population health data study in Korea

  • Background: Trigeminal neuralgia (TN) is one of the most painful disorder in the orofacial region, and many patients have suffered from this disease. For the effective management of TN, fundamental epidemiologic data related to the target population group are essential. Thus, this study was performed to clarify the epidemiological characteristics of TN in the Korean population. This is the first national study to investigate the prevalence of TN in Korean patients. Methods: From 2014 to 2018, population-based medical data for 51,276,314 subscribers to the National Health Insurance Service of Korea were used for this study. Results: The incidence of TN was 100.21 per 100,000 person-years in the year of 2018 in Korea, and the male to female ratio was 1:2.14. The age group of 51-59 years had the highest prevalence of TN. Constant increases in medical cost, regional imbalance, and differences in prescription patterns by the medical specialties were showed in the management of TN. Conclusions: The results in this study will not only help to study the characteristics of TN, but also serve as an important basis for the effective management of TN in Korea.

Genetic Diversity and Population Genetic Structure of Black-spotted Pond Frog (Pelophylax nigromaculatus) Distributed in South Korean River Basins

  • The objective of this study was to analyze the genotype of black-spotted pond frog (Pelophylax nigromaculatus) using seven microsatellite loci to quantify its genetic diversity and population structure throughout the spatial scale of basins of Han, Geum, Yeongsan, and Nakdong Rivers in South Korea. Genetic diversities in these four areas were compared using diversity index and inbreeding coefficient obtained from the number and frequency of alleles as well as heterozygosity. Additionally, the population structure was confirmed with population differentiation, Nei's genetic distance, multivariate analysis, and Bayesian clustering analysis. Interestingly, a negative genetic diversity pattern was observed in the Han River basin, indicating possible recent habitat disturbances or population declines. In contrast, a positive genetic diversity pattern was found for the population in the Nakdong River basin that had remained the most stable. Results of population structure suggested that populations of black-spotted pond frogs distributed in these four river basins were genetically independent. In particular, the population of the Nakdong River basin had the greatest genetic distance, indicating that it might have originated from an independent population. These results support the use of genetics in addition to designations strictly based on geographic stream areas to define the spatial scale of populations for management and conservation practices.

Age Distribution of Breast Cancer from a Thailand Population-Based Cancer Registry

  • Background: Breast cancer is a common cancer worldwide. With the establishment of Thailand's population-based cancer registry and availability of complete data from 2002-2011, it is of interest to investigate the epidemiologic and clinic-pathological profiles of breast cancer based on the population-based registry data. Methods: The data of all breast cancer patients in the registry for the period of 2002-2011 were included. All medical records of the patients diagnosed from documents of National Cancer Registry of Thailand were retrieved and the following information abstracted: age, clinical characteristics, and histological variables. Thailand census data for the period of 2002-2011 were used to provide the general population's statistics on age, gender, and other related demographic factors. Results: Over the 10 year-period, 7,711 breast cancer cases were included. The disease incidence under age 40 years was relatively low (4.13/$10^5$) while the incidence in the age groups 40 and older was very high (39.2/$10^5$). The vast majority of breast cancer cases (88.8%) were diagnosed by histology as primary lesions in the breast. The most common of patients with breast cancer (36.4%) had regional lymph node involvement and the most common of histopathology diagnosed in patients (84.2%) was an infiltrating duct carcinoma. Conclusions: This study showed a high incidence of breast cancer in older subjects, and high rate of breast cancer in Thailand. Future studies should explore clinical and molecular disease patterns.

Estimated Number of Korean Adults with Back Pain and Population-Based Associated Factors of Back Pain : Data from the Fourth Korea National Health and Nutrition Examination Survey

  • Objective : We estimated the number of Korean adults with back pain and evaluated population-based associated factors of back pain from a representative sample data from the Fourth Korea National Health and Nutrition Examination Survey. Methods : The number of Korean adults who experienced back pain (experienced patients), those who experienced back pain lasting for three or more months during the past year (chronic patients), and those who were currently suffering from back pain (current patients) were estimated by analyzing the data from the fourth Korea National Health and Nutrition Examination Survey conducted in 2007 using surveyfreq procedure of the SAS statistical package. Population-based odds ratios for being experienced, chronic, and current patient according to demographic (age and gender), socioeconomic (education and occupation), and lifestyle factors (smoking, drinking, and exercise) were estimated using surveylogistic procedure. Results : It was estimated that there were 5,554,256 (proportion, 15.4%; 95% CI, 4,809,466 - 6,299,046) experienced patients, 2,060,829 (5.7%; 1,557,413-2,564,246) chronic patients, and 3,084,188 (8.5%; 2,600,197 - 3,568,179) current patients among 36,107,225 Korean adults aged 20-89 years in 2007. Each of explanatory variables was significantly associated with at least one of the response variables for back pain. Conclusion : Based on our study results, further efforts to investigate epidemiology of back pain, to evaluate associated factors, and to improve treatment outcomes are needed.

취약계층의 주거환경 실태조사 -보건소 방문보건사업 대상을 중심으로- (A Survey on Housing Environment of Vulnerable Population - Focused on the Recipients of Visiting Health Care Services -)

  • Purpose: The aim of this study is to provide baseline data on the housing environment of a vulnerable population. Methods: This study provides a descriptive survey on the housing environment of a vulnerable population of a city in Gyeong-gi province. A total of 9,061 households were selected as research participants. Data were collected through face-to-face interviews by visiting nurses from February 2014 to March 2014. Results: Interviewees answered "poor" on items including cold drafts, illumination, daylight penetration, inflow of polluted air, house odor, stairs safety, noise, mold, pests, and ventilation. The score for housing environment problems showed a significant group difference due to gender, age, economic status, family type, type of housing, and type of ownership. Conclusion: These results indicate that the vulnerable population is exposed to a poor housing environment. Thus, there is an urgent need to improve the housing environment for the vulnerable population. Based on this study, linking a visiting health care service program and housing environment improvement projects from relevant administrative agencies might be recommended in order to effectively improve the housing environment.

국내외 공공도서관 개관시간 기준 연구 (A Study on the Standards for Public Library Opening Hours in Foreign Countries and Korea)

  • 도서관은 지역주민이 장서 및 서비스를 가장 효과적이고 편리하게 이용할 수 있는 시간에 개관해야 한다. 달리 말해서 모든 공공도서관은 지역주민에게 적절한 개관시간을 제공해야 한다. 이에 본 연구는 국제기구(IFLA/UNESCO)와 주요 국가(미국, 캐나다, 영국, 프랑스, 호주, 중국과 대만, 일본, 한국)의 공공도서관기준을 대상으로 최소 개관시간을 분석하고 봉사대상인구에 기초하여 국내 공공도서관 자료실의 바람직한 주당 최소 개관시간 기준(1만명 미만은 45시간, 1만명~2만명 미만은 50시간, 2만명~5만명 미만은 55시간, 5만명~10만명 미만은 60시간, 10만명~30만명 미만은 65시간, 30만명 이상은 70시간)을 제안하였다.

  • Korean goral (Nemorhaedus caudatus raddeanus) is an endangered species in Korea, and the rugged terrain of the Sorksan National Park $(373km^2)$ is a critical habitat for the species. Since the goral population is threatened by habitat fragmentation, it is essential to establish a reserve for the isolated goral population. The objective of this study was to propose a reserve for Korean goral in the national Park We employed habitat suitability model, habitat capability model, and the concept of minimum viable population. The results of the study were as follows. First, the carrying capacity and optimal density of gorals in the national park were projected to be 449 gorals, and 251 gorals, respectively Second, since only one patch was projected to satisfy the criteria of minimum viable population (50 individuals/during 50 years), the long term extinction possibility of gorals in the site would be very high. Finally, the patch that satisfy the minimum viable population of goral was proposed as the core zone of the goral reserve and adjacent patches were included as buffer zones.

Analysis of the Genetic Diversity and Population Structure of Amaranth Accessions from South America Using 14 SSR Markers

  • Amaranth (Amaranthus sp. L.) is an important group of plants that includes grain, vegetable, and ornamental types. Centers of diversity for Amaranths are Central and South America, India, and South East Asia, with secondary centers of diversity in West and East Africa. The present study was performed to determine the genetic diversity and population structure of 75 amaranth accessions: 65 from South America and 10 from South Asia as controls using 14 SSR markers. Ninety-nine alleles were detected at an average of seven alleles per SSR locus. Model-based structure analysis revealed the presence of two subpopulations and 3 admixtures, which was consistent with clustering based on the genetic distance. The average major allele frequency and polymorphic information content (PIC) were 0.42 and 0.39, respectively. According to the model-based structure analysis based on genetic distance, 75 accessions (96%) were classified into two clusters, and only three accessions (4%) were admixtures. Cluster 1 had a higher allele number and PIC values than Cluster 2. Model-based structure analysis revealed the presence of two subpopulations and three admixtures in the 75 accessions. The results of this study provide effective information for future germplasm conservation and improvement programs in Amaranthus.

Genetic Susceptibility to Esophageal Cancer due to CYP1A1 Gene Variant rs4646903 in Tobacco Addicted Patients of Pashtun Ethnicity: a Case Control Study in Khyber Pakhtunkhwa Province of Pakistan

  • The purpose of this study was to evaluate associations of the CYP1A1 gene variant rs4646903 polymorphism with the risk of developing esophageal cancer (EC). A case-control study was carried out in Pashtun population of Khyber Pakhtunkhwa province of Pakistan in which 140 hospital based EC cases and 196 population based healthy controls exposed to similar environmental conditions were included. A specific method based on the real time polymerase chain reaction (RT-PCR) was used to detect genotypes in case and control groups and results were then analyzed with SPSS version 20. In our population, individuals with CC and TC genotypes of the CYP1A1 rs4646903 polymorphism had significantly higher risk of EC (adjusted odds (OR): 15.709, 95%CI: 6.065-40.686, OR: 3.256 95%CI: 1.902-5.574 respectively). The 'C' allele was strongly associated with the disease (p< 0.0001). Adjusted OR was higher (1.5 times in C/C) in case of variant alleles that show the contribution of environmental and nutritional factors towards the development of EC. Our findings suggest that presence of the 'C' allele of rs4646903 (T>C) may be one of the risk alleles for EC susceptibility in Pashtun population.

Age-Specific Thyroid Internal Dose Estimation for Koreans

  • Background: The International Commission on Radiological Protection is preparing to provide reference dose coefficients for environmental radioiodine intake based on newly developed age-specific biokinetic models. However, the biokinetics of iodine has been reported to be strongly dependent on the dietary intake of stable iodine; for example, the thyroidal uptake of iodine may be substantially lower in iodine-rich regions than in iodine-deficient regions. Therefore, this study attempted to establish a system of age-specific thyroid dose estimation for South Koreans, whose daily iodine intakes are significantly higher than that of the world population. Materials and Methods: Korean age-specific biokinetic parameters and thyroid masses were derived based on the previously developed Korean adult model and the Korean anatomical reference data for adults, respectively. This study complied with the principles used in the development of age-specific biokinetic models for world population and used the ratios of baseline values for each age group relative to the value for adults to derive age-specific values. Results and Discussion: Biokinetic model predictions based on the Korean age-specific parameters showed significant differences in iodine behaviors in the body compared to those predicted using the model for the world population. In particular, the Korean age-specific thyroid dose coefficients for 129I and 131I were considerably lower than those calculated for the world population (25%-76% of the values for the world population). Conclusion: These differences stress the need for Korean-specific internal dose assessments for infants and children, which can be achieved by using the data calculated in this study.
