흉막강액에서 진단된 류마티스성 흉막염 - 세포학적 소견 1예 보고 - (Cytologic Findings of Rheumatoid Pleuritis in Pleural Effusion - A Case Report -)

  • Patients with rheumatoid arthritis of any degree of severity or duration may develop necrotizing granulomatous pleuritis, a morphologic replica of the inflammatory reaction characteristic of rheumatoid synovitis and rheumatoid nodules. The principal feature is the background composed of granular, amorphous, particulate material or debris of various hues. The material is sometimes eosinophilic sometimes more cyanophilic, or even green in the Papanicolaou stain. Within this background are elongated, fibroblast-like epithelioid cells, numerous multinucleated giant cells and degenerating leukocytes. The combination of the debris, spindle epithelioid ceils, and multinucleated giant cells in fluid is pathognomonic for rheumatoid pleuritis. We experienced a hcase of rheumatoid pleuritis showing these characteristic cytologic findings. The patient was a 63 year-old man with positive rheumatoid factor. The pleual fluid specimen revealed elongated epithelioid cells and multinucleated giant cells on a background of amorphous granular material.

Pleuritis and pericarditis associated with Klebsiella pneumoniae in a Eurasian beaver (Castor fiber)

  • An adult one-year-old male Eurasian beaver (Castor fiber) died of a traumatic injury to its right leg from a fall. At necropsy, fibrinopurulent exudates were observed in the thoracic cavity. Histopathologic examination showed that the pericardium, thoracic wall, pulmonary pleura and the lungs were markedly thickened due to mixtures of necrotic cellular debris, neutrophils, fibrin, red blood cells, and bacterial aggregates. Pure culture of Klebsiella (K.) pneumoniae was isolated from the thoracic exudates, pleura and heart tissues. Based on these findings, this is the first report describing pleuritis and pericarditis associated to K. pneumoniae in a beaver.

  • Leopard cat (Prionailurus bengalensis euptilurus) is a small wild cat assessed as an endangered wildlife in Korea. There have been very few reports of their diseases. Herein, we describe fibrinous pleuritis caused by Streptococcus canis infection with excessive pleural effusion, hydropericardium, mild ascites, and liver fibrosis in a leopard cat. S. canis is a commensal microflora in domestic cats and often affects the upper respiratory tract inducing chronic and severe respiratory diseases. However, there is no literature regarding the S. canis in leopard cats. Therefore, we first report fibrinous pleuritis associated with an S. canis infection in a leopard cat.

다람쥐 원숭이의 Bordetella bronchiseptica 감염 예 (A Case Report of Bordetella Bronchiseptica Infection in Squirrel Monkey(Saimiri sciureus))

  • A dead, female, 3 years old, squirrel monkey (Saimiri sciureus) was submitted and examined. Before death the monkey has showed lethargy, recumbency and inappetence since November 14, 2001 and died in November 17. Grossly much fibrin was deposited on the pleura of right lung, pericardium, and diaphragm(pleural part). And reddening of right lung was seen. Histopathologically lung showed severe fibrinous pleuritis, severe edema, thrombosis, and focal necrosis in parenchyma. Also much fibrin and mononuclear cells were deposited on the pericardium. In bacterial culture on the pleura and parenchyma of lung, and pericardium, B. bronchiseptica was isolated. Therefore we confirmed this case as the fatal case by B. bronchiseptica in squirrel monkey.

중심정맥포트 삽입 후 의인성 속목정맥 천공에 의한 화학적 늑막염과 혈흉의 중재적 치료: 증례 보고 (Interventional Treatment of Chemical Pleuritis and Hemothorax Caused by Iatrogenic Internal Jugular Vein Perforation after Central Venous Port System Implantation: A Case Report)

  • 항암치료를 위해 흉강 내 속목정맥의 천자를 통한 피하매몰 중심정맥 케모포트(implantable central venous chemoport) 도관의 설치 중 발생할 수 있는 의인성 속목정맥 천공은 매운 드문 합병증 중의 하나로 혈흉이나 출혈성 쇼크를 일으킬 수 있으며, 부적절한 항암제 주입으로 인한 늑막삼출이 발생할 수 있다. 따라서 항암제 주입 전 조기에 진단하여 응급 개흉술을 통해 천공된 속목정맥을 봉합하는 것이 치료 원칙이다. 저자들은 우측 속목정맥을 통한 피하매몰 중심정맥 케모포트의 설치 후 발생한 속목정맥 천공 환자에서 부적절한 항암제 주입으로 인해 발생한 늑막삼출과 혈흉을 개흉술을 시행하지 않고 경피적 배액술 후 코일과 N-butyl cyanoacrylate를 이용한 색전술을 통해 성공적으로 치료한 1예를 경험하였기에 이를 보고하고자 한다.

엘크의 폐장 아스퍼질러스증 및 신장 옥살산증 (Pulmonary Aspergillosis and Renal Oxalosis in an Elk)

  • A four-year-old male elk (Cervus elaphus nelsoni) was diagnosed as pulmonary aspergillosis and renal oxalosis. Clinical signs were coughing, sneezing, respiratory distress, salivation, moderate anorexia, and progressive emaciation. Main gross lesions were fibrinopurulent tonsillitis, diffusely fibrinous pleuritis, and distinct lobar pneumonia with purple red in color. Most of the pulmonary lobes had numerous well demarcated 0.5 to 2 cm yellowish white discrete or confluent nodules that were surrounded by pale red zones. Histopathologically, the affected lungs were disseminated necrotizing pyogranulomas including fungal hypae, vasculitis, and diffusely fibronecrotic pleuritis. The renal lesions were composed of extensive tubular necrosis with large numbers of rosette-formation by birefringent oxalate crystals. Aspergillus fumigatus was isolated from lesions of the lungs. It seems to be a first report for pulmonary aspergillalis and renal oxalosis of a farmed elk in Korea.

전북지역 도축돈 적내장 육안병변 조사 (Survey on the red internal organs gross lesions of slaughtered pigs in Jeonbuk)

  • Respiratory diseases are common in swine industry and have great economic importance. Respiratory disorders cause substantial losses to the swine industry. Losses associated with respiratory disease vary considerably between herds and seasons. In a survey was carry out to investigate the lesion of red internal organs in slaughtered pigs and provided assistant data which are useful for each farm. From november 2012 to december 2013, 1,680 pigs out of 84 farms were sampled in Jeonbuk province. Gross lesions such as swine enzootic pneumonia (SEP), pleuritis, pleuropneumonia, pericarditis, liver milk spot were examined for the pigs. Overall prevalence of SEP was 55.7%. According to season, the incidence occurred higher in fall than winter, spring, and summer. The mean SEP score was 0.91, the highest incidence occurred in fall. The prevalence of pleuropneumonia, pleuritis, pericarditis, and milk spot was 36.4%, 49.7%, 2.3%, and 8.8%, respectively. The positive rate of PRRS and PCV2 was 2.9% and 70.0% by PCR analysis.

Risk factors predicting development of complications in 72 dogs with esophageal foreign bodies

  • The aim of this study was to determine predictive risk factors implicated in complications in dogs with esophageal foreign bodies. Medical records of 72 dogs diagnosed with esophageal foreign bodies by endoscopy were reviewed retrospectively. Factors analyzed included age; breed; gender; body weight, location, dimension, and type of foreign body; and duration of impaction. To identify risk factors associated with complications after foreign body ingestion, categorical variables were analyzed using the chisquare or Fisher's exact tests and multivariate analysis, as appropriate. Complications secondary to esophageal foreign body ingestion included megaesophagus, esophagitis, perforation, laceration, diverticulum, and pleuritis. Univariate analysis revealed that the location and duration of impaction after foreign body ingestion were associated with an increased risk of esophageal laceration and perforation. Multivariate analysis showed that age, duration of impaction, and foreign body dimension were significant independent risk factors associated with the development of complications in dogs with esophageal foreign bodies. In conclusion, these results showed that longer duration of impaction and larger foreign body dimensions may increase the risks of esophageal laceration, perforation, and plueritis in dogs.

Pleuroperitoneal communication-associated pleuritis as an uncommon cause of fever of unknown origin in a child on peritoneal dialysis: a case report

  • Pleuroperitoneal communication (PPC) is a rare mechanical complication of peritoneal dialysis (PD), which causes dialysate to move from the peritoneal cavity to the pleural cavity, resulting in pleural effusion. Typically, PPC is discovered through pleural effusion in PD patients who are not in volume overload status. A unique characteristic of the pleural effusion caused by PPC is that it is not resolved by increasing ultrafiltration by dialysis. In this report, we present a 7-year-old girl with PD after birth with the history of various infectious PD-related complications, presenting with fever ongoing for 6 months. PPC-associated pleuritis was suspected as the cause of fever, which eventually developed after long-term PD and induced complicated pleural effusion, lung inflammation, and prolonged fever for 6 months.

전북지역 도축 출하돈 병변 조사 (Survey on the gross lesions of slaughtered pigs in Jeonbuk area, Korea)

  • Respiratory disease in pigs is common in modern pork production worldwide and is often referred to as porcine respiratory disease complex (PRDC). PRDC is polymicrobial in nature, and results from infection with various combinations of primary and secondary respiratory pathogens. The control of swine respiratory disease requires an understanding of the interactions between the organisms that can cause this illness, the pig and management of the environment. This study was carried out to investigate the lesion of red internal organs in slaughtered pigs and provided assistant data for pig farms. A total of 900 lung samples, 45 farms were collected randomly from slaughtered pigs in Jeonbuk province from April to December in 2014. Gross lesions such as swine enzootic pneumonia (SEP), pleuritis, pleuropneumonia, pericarditis, liver white spots were examined for the pigs. Overall prevalence of SEP was 70.8%. According to season, the incidence occurred higher in summer than winter, fall and spring. The mean SEP score was 1.4, the highest incidence occurred in fall. The prevalence of pleuropneumonia, pleuritis, pericarditis, and milk spot was 26.1%, 71.4%, 2.8%, 21.6%, respectively. In the detection of pathogens, PRRS was not detected, PCV2 was positive in 87.6%.
