중환자실 간호사가 경험한 사람-중심의 관계 돌봄에 대한 해석 현상학적 분석 연구 (Person-Centered Relational Care Experienced by Critical Care Nurses: An Interpretative Phenomenological Analysis Study)

  • Purpose: The aim of the study was to explore nurses' experience of person-centered relational care in the context of critical care. Methods: Key interview questions were developed based on the human-to-human relationship model suggested by Travelbee. Data were collected through indepth interviews with a purposive sample of 11 nurses having more than 2 years of working experience in intensive care units. An interpretative phenomenological analysis was conducted to analyze the data. Results: Four super-ordinate and nine sub-ordinate themes were identified. Emerged super-ordinate themes were as follows: (1) encountering a live person via patient monitoring systems; (2) deep empathic connection; (3) humanistic and compassionate care, and (4) accompanying the journey to the end. Study findings revealed that nurses in intensive care units experienced 'balancing emotions' and 'authenticity' in caring when entering human-to-human relationships with dying patients. The phenomenon of person-centered relational care in intensive care units was found to subsume intrinsic attributes of empathy, compassion, and trust, similar to the central concepts of Travelbee's theory. Conclusion: The interpretative findings in this study provide deeper understanding of Travelbee's human-to-human relationship model. The technological environment in intensive care units did not hinder experienced nurses from forming human-to-human relationships. These themes need to be emphasized in critical care nursing education as well as in nursing management. The results of this study will contribute to understanding nurse-patient caring relationships in depth, and help improve the quality of nursing care in intensive care units.

Sick or sickness? The importance of person centred healthcare and medicine paradigm

  • When you act for a good or just when you make a choice, you should ask yourself if the result of your action coincides with the principle that has generated it. Health and respect for our own individuality are two essential goods for the human being and also values that medicine itself should guard carefully remembering that its goal is to take care for a person and not only to cure a sick body. The means to achieve all this can only be a person-centered medicine which has the honor and the burden of considering the person (in the Kantian sense) and his health as an end, not a means. The current mainstream concept of health defined in 1948 by the World Health Organization as a state of complete mental, physical and social wellbeing and not merely the absence of disease and infirmity remind us that medical reasoning, based on the pathogenesis operator, is no longer sufficient in dealing with requests from the citizens: is now the right time to shift the focus from sickness to health giving emphasis to the salutogenic medicine approach.

중환자실 환자가 경험한 인간중심 간호에 관한 질적 내용분석 (A Qualitative Content Analysis of the Person-Centered Care Experienced by Critical Care Patients)

  • Purpose : To explore the person-centered care (PCC) experienced by critical patients. Method : This qualitative study deductively examined the attributes of the PCC model proposed by Jakimowicz and Perry. The participants were 16 patients who were recently discharged from intensive care units at a university hospital. Data were collected through in-depth interviews and were analyzed using the deductive method of content analysis. Results : A total of 4 categories, 16 subcategories, and 33 codes were generated from 171 meaningful statements. The final 4 categories were "compassionate presence," "professional interaction," "outstanding competency," and "patient identity." These were consistent with the main attributes of Jakimowicz and Perry's model. However, most of the codes belonged to the "compassionate presence" and "patient identity" categories. Among the attributes of the model, "continuity of nursing," "therapeutic relationship," "expert knowledge," "clinical knowledge," "evidence-based intervention," and "patient's rights" were not derived as codes. Conclusion : These findings deepen the understanding of the PCC model from the patient's point of view. The main attributes of PCC identified in the current study can be applied to the development of practical guidelines for intensive care nursing. In addition, we recommend the development of a PCC measurement tool for critical care patients.

4차 산업혁명시대 노인 간호 (Gerontological Nursing in the Era of the Fourth Industrial Revolution)

  • Purpose: There is a need to examine changes in the health care environment and the impact on gerontological nursing care in the era of the Fourth Industrial Revolution. In this article recent healthcare paradigm changes, gerontechnology, high tech and high touch, person-centered approaches are discussed. Methods: A narrative review was used. Results: Cyber physical system, artificial intelligence, advance and convergence of bioengineering, and information communication technology are changing the health care paradigm to "precision", "prediction" and "personalization". Entry into the global aging society and the surge in the elderly population worldwide has led to searches for a new means to prepare for projected demands of this growing population. Thus, efforts such as gerontechnology have been made to apply and utilize recent innovative science and technology in order to promote the health and life of elders. There is a great emphasis on the convergence of high tech and high touch, in which humanistic and artistic approach are critical in order to assure that technology is beneficial to human beings rather than harmful. Conclusion: Positive healthcare experiences among patients and their families are emphasized by utilizing new technology and employing high touch while providing personalized care with a person-centered approach.

상급종합병원 간호사의 환자중심간호 예측모형 (A Predictive Model on Patient-Centered Care of Hospital Nurses in Korea)

  • Purpose: Patient-centered care is a widely utilized concept in nursing and health care. However, the key components of patient-centered nursing have not yet been reported. Moreover, previous studies on patient-centered care have mostly focused on components of nursing rather than organizational factors. Therefore, a comprehensive understanding of influential factors of patient-centered care is required. Methods: The purpose of this study was to develop a theoretical model based on person-centered care theory, and the relevant literature and to test the developed model with covariance structure analysis in order to determine the causal paths among the variables. Results: The model fit indices for the hypothetical model were suitable for the recommended level (goodness of fit index=.87, standardized root mean residual=.01, root mean square error of approximation=.06, Tucker-Lewis index=.90, comparative fit index=.92, parsimonious normed fit index=.75). In this study, five of the six paths established in the initial hypothetical model were supported. The variables of teamwork, self-leadership, and empathy accounted for 56.4% of hospital nurses' patient-centered care. Among these, empathy was the strongest predictor of patient-centered care. Conclusion: These results suggest that it is necessary to use strategies to improve self-leadership and empathy. In addition to enhancing the personal factors of nurses, nursing organizations should strive for effective multidisciplinary cooperation with active support for patient-centered care and openness to change.

노인시설 종사자의 업무스트레스, 업무만족, 치매 및 인간중심보호 인식 연구: 사회복지사, 간호사, 생활지도원의 비교 (A Study on Work Stress, Satisfaction, and Dementia Attitudes of Social Care Work Force of Dependent Elders)

  • 이 연구에서는 부산과 대구 지역의 노인시설에 근무하는 사회복지사, 간호사, 생활지도원 502명을 대상으로, 노인보호 관련 종사자들의 업무특성을 반영하여 고안된 측정도구들을 활용하여 이들의 일반적인 특성과 업무 스트레스, 업무 만족, 치매 및 인간중심 보호와 관련된 인식과 태도를 조사하여 그 결과를 분석하였다. 분석 결과 노인시설 종사자들의 급여수준이 낮고 근무시간이 길며 업무 부담이 과중한 상태인 것으로 나타났다. 치매 및 인간중심 보호와 관련된 종사자들의 인식수준은 높은 편이었으나, 업무 만족 수준이 낮았고 노인에 대한 보호 제공 및 업무환경과 관련된 스트레스가 높게 나타났다. 노인장기요양보험제도 시행을 앞두고 노인에게 사회적 보호를 제공할 전문 인력의 원활한 수급과 전문성 향상, 보호의 질 제고를 위하여 노인시설 종사인력에 대한 처우 개선과 인력 확충, 중장년기 인력 활용, 노인시설 종사인력에 대한 전문적 훈련과 교육 강화, 노인시설에서의 인간중심 보호 방식의 도입과 실천이 필요함을 제시하였다.

성인 중환자실 간호사의 인간중심간호 수행과 영향요인 (Predictors of Person-Centered Care among Nurses in Adult Intensive Care Units)

  • Purpose: The purpose of this study was to examine the level of Person-centered Critical Care Nursing (PCCN) and the factors influencing PCCN for nurses in Intensive Care Units (ICU). Methods: This study was designed by cross-sectional descriptive correlational study. The participants included 147 ICU nurses in two general hospitals in Seoul, Korea. Demographic characteristics, PCCN, communication skills, professionalism, and work environment were measured. The collected data were analyzed using descriptive statistical analysis, independent t-test, One-way ANOVA, Pearson's correlation coefficient, and stepwise multiple linear regression with the SPSS/Win 25.0 program. Results: The average age of the participants was 29.6±4.7 years and the mean work experience in the ICU was 4.67±3.52 years. The level of PCCN was 3.70±0.41, which was moderate to high, and it significantly showed a positive correlation with therapeutic communication skills (r=.66, p<.001), global interpersonal communication competence (r=.42, p<.001), professionalism (r=.38, p<.001), and work environment (r=.16, p=.048). The factors influencing PCCN were identified as therapeutic communication skill and global interpersonal communication competence (Adj R2=.45, p<.001). Conclusion: The findings of this study were confirmed that the strategies to promote PCCN are necessary to enhance therapeutic communication skill and global interpersonal communication competence. In addition, they may be particularly meaningful in providing basic data for nursing education and future intervention development research to promote PCCN for the ICU nurses. For improving PCCN for healthcare providers in ICU, further studies should be conducted to develop education and intervention programs.

중환자실 간호사의 근무환경과 인간중심 간호의 관계 (The Relationship between the Work Environment and Person-centered Critical Care Nursing for Intensive Care Nurses)

  • Purpose : The purpose of this study was to investigate the effect of the perceived nursing work environment (NWE) on person-centered critical care nursing (PCCN) practices among nurses in intensive care units (ICUs). Methods : A cross-sectional research design was adopted for this study; participants were 126 ICU nurses working in five hospitals in B city. From December 2018 to February 2019, a survey questionnaire on NWE and PCCN were conducted. The collected data were analyzed with a multiple hierarchical regression analysis. Results : The mean score for NWE was $2.98{\pm}0.39$. Among the factors, basic work system was the highest ($3.39{\pm}0.41$), followed by leadership of the head nurse ($3.31{\pm}0.55$), interpersonal relationships ($2.77{\pm}0.62$), and institutional support ($2.58{\pm}0.52$). The mean score for PCCN was $3.52{\pm}0.46$. Among the factors, comfort was the highest ($3.77{\pm}0.62$), followed by respect ($3.55{\pm}0.56$), compassion ($3.43{\pm}0.60$), and individuality ($3.38{\pm}0.56$). The interpersonal relationship (${\beta}=.31$, p=.001) and the number of patients per shift (${\beta}=-.23$, p=.005) had a significant effect on PCCN, accounting for 25.9% of the total variance. Conclusion : These results suggest that it is necessary to enhance the relationship-oriented culture and reduce the workload of nurses in order to improve the quality of nursing care in ICUs.

고학년 간호대학생의 인간중심간호역량 영향요인: King의 역동적 상호작용 체계모형을 중심으로 (Factors affecting Person-Centered Care Competence for senior nursing students: Focused on King's dynamic interaction system model)

  • 본 연구는 King의 역동적 상호작용체계를 기반으로 고학년 간호대학생의 인간중심간호역량에 영향을 미치는 요인을 파악하기 위한 서술적 조사연구이다. 임상실습경험이 있는 4학년 간호대학생 242명을 대상으로 2021년 3월 20일부터 4월 3일까지 google 설문지를 통해 윤리적 민감성, 동료돌봄행위, 대인관계유능성, 사회적지지, 인간중심간호역량을 조사하였다. 수집된 자료는 SPSS 26.0 program을 사용하여, 기술통계, T-test, ANOVA, Pearson's correlation coefficient, Multiple regression을 통해 분석하였다. 연구결과, 대상자의 인간중심간호역량에 영향을 미치는 요인은 대인관계유능성(β=0.42, p<.001)과 사회적지지(β=0.29, p<.001)였으며, 이들의 설명력은 44.1%였다(F=23.77, p<.001). 본 연구는 개념적 기틀을 바탕으로 고학년 간호대학생의 인간중심간호역량에 영향을 미치는 개인적, 대인관계적, 사회적요인을 파악함으로써 이를 증진시키기 위한 방안을 모색하고 나아가 중재 프로그램 개발의 기초자료를 제공하였다는 점에서 의의가 있다.

Testing Reliability and Validity of the Person-centered Climate Questionnaire-staff version in Korean for Long-term Care Facilities

  • Purpose: To test the reliability and validity of the Korean version of the Person-centered Climate Questionnaire-staff version (KPCQ-S) in long-term care institutions. Methods: A total of 297 staff in long-term care institutions including nine nursing homes (NHs) and 4 long-term care hospitals (LTCHs) were included. The KPCQ-S was developed following the WHO guidelines of the process of translation and adaptation of instruments. An internal consistency using Cronbach's ${\alpha}$ was tested for reliability. Exploratory factor analysis (EFA) was used to examine the construct validity. Convergent and discriminant validity were examined using Pearson correlation. Results: EFA demonstrated the construct validity of the 14-item KPCQ-S with three-factor solutions, specifically three factors (safety, everydayness, and community) in NHs and four factors (safety, everydayness, community, and comprehensibility) in LTCHs. Convergent validity was found in the correlation with the work satisfaction (r=.55). The KPCQ-S showed satisfactory internal consistency reliability (Cronbach's ${\alpha}=.91$). Conclusion: The KPCQ-S is found to be a reliable and valid tool for measuring staff perceptions of the person centeredness of long-term care environments.
