복식호흡이 조기진통 임부의 불안, 혈압, 말초 피부온도와 산소 포화도에 미치는 효과 (Effects of Abdominal Breathing on Anxiety, Blood Pressure, Peripheral Skin Temperature and Saturation Oxygen of Pregnant Women in Preterm Labor)

  • Purpose: This study was done to examine the effects of abdominal breathing on VAS-Anxiety (VAS-A), blood pressure, peripheral skin temperature and saturation oxygen in pregnant women in preterm labor. Method: The study design was a matched control group interrupted time series. Forty-six women matched to gestational age were assigned to either the experimental group (26) or control group (20). Data were collected between March 2007 and May 2008. For the experimental treatment the women performed abdominal breathing 30 times, which took 5 minutes, and did one set of 5-minute abdominal breathing daily for three days. Data collection was done before and after the abdominal breathing to measure VAS-A, blood pressure, peripheral skin temperature and oxygen saturation. Descriptive, $X^2$, Mann-Whitney U tests were used to analyze the data with the SPSS/PC+Win 15.0 program. Result: For the experimental group there were significant decreases in VAS-A (Z=-4.37, p=.00), systolic blood pressure (Z=-3.38, p=.00), and an increase in skin temperature (Z=-4.50, p=.00) and oxygen saturation (Z=-3.66, p=.00). Conclusion: These findings suggest that abdominal breathing in pregnant women in preterm labor results in decreases in anxiety(VAS-A) including biological evidences such as systolic blood pressure, and increases in peripheral skin temperature and oxygen saturation. Further longitudinal study is needed on the lasting effects and obstetric and neonatal outcomes following abdominal breathing.

  • 혈액에서의 산소 포화도는 Hb와 HbO$_2$의 광학 스펙트라에서 흡수계수의 차이에 의해서 측정할 수 있다. 본 연구는 하지 조직에서 발생하는 말초혈관계 질환진단을 위하여 파장이 660nm 와 940nm의 LED를 이용하여 산소포화도를 측정하는 장치를 개발하였다. 산소측정장치는 광 탐촉자와 광 신호처리부, LED 구동회로, 컴퓨터와 인터페이스로 구성하고, 데이터의 수집과 분석을 위한 프로그램 을 개발하였다. 구현된 산소측정장치에 대한 임상적인 평가를 위하여 하지조직에서 운동 부하에 따른 실험을 하여 생체조직내 생리적변화에 따른 산소량의 변화를 측정하고, 기기의 성능을 평가하였다. 실험결과 생체조직내의 산소포화도는 광원과 검출기의 간격에 따라 측정이 가능함을 보여 주었다.

라인댄스 프로그램이 노인의 산소포화도와 행복정도에 미치는 영향 (The Effect of Line Dance Program on Oxygen Saturation and Happiness of Elderly)

  • Purpose: The research was to investigate the effect of line dance programs on oxygen saturation level and happiness of elderlies. Methods: Researchers had conducted a series of test to measure peripheral blood vessel oxygen saturation level, pulse and happiness level of 47 elderlies from the D district of G city. Collected data were analyzed with ${\chi}^2$-test and independent t-test, equipped with SPSS WIN 17.0. Results: As a result of serial tests, Pre-test and Post-test of line dance performance showed that oxygen saturation of the Experimental group had significant increase following the program and happiness of the Experimental group also had significant increase following the program. Conclusion: In conclusion, with many advantages such as its easiness to access and emulate as well as its ubiquitousness as an aerobic exercise, line dance cannot only be very beneficial to cardiovascular function and relation of elderlies, but also to emotional and psychological effect, considered as a great breakthrough to improve comprehensive well-being in both physical and mental aspects.

노래부르기가 노인의 생리적 변화에 미치는 효과 (Effects of Singing of Physiologic Changes in the Elderly Women)

  • Recently, music therapy is widely used for various kinds of diseases. Music therapy has beneficial effects on emotional disorder and neuropsychiatric diseases in particular. This study was performed to evaluate the effect of singing on physiologic changes. We checked peripheral oxygen saturation and heart rate as indices of physiologic changes. The subjects were 19 control and 30 test group who were registered on the D welfare center for the elderly and agreed to join this study. They had been singing regularly for 6 months. The data were collected just before and after the singing. Data were analyzed with mean, t-test, and paired t-test using SPSS $PC^+$ program. The results were as follows: 1. Heart rate of the singing group decreased significantly after singing.(p<0.05) 2. Peripheral oxygen saturation of the singing group increased significantly after singing.(p<0.05) In conclusion, singing, a kind of aerobic exercise, has beneficial effects on cardiopulmonary system.

Quantitative analysis of the effect of fraction of inspired oxygen on peripheral oxygen saturation in healthy volunteers

  • Background: The international organization for standardization (ISO) 80601-2-61 dictates that the accuracy of a pulse oximeter should be assessed by a controlled desaturation study. We aimed to characterize the relationship between the fraction of inspired oxygen (FiO2) and peripheral oxygen saturation (SpO2) using a turnover model by retrospectively analyzing the data obtained from previous controlled desaturation studies. Materials and Methods: Each volunteer was placed in a semi-Fowler's position and connected to a breathing circuit to administer the hypoxic gas mixture containing medical air, oxygen, nitrogen, and carbon dioxide. Volunteers were exposed to various levels of induced hypoxia over 70-100% arterial oxygen saturation (SaO2). The study period consisted of two rounds of hypoxia and the volunteers were maintained in room air between each round. FiO2 and SpO2 were recorded continuously during the study period. A population pharmacodynamic analysis was performed with the NONMEM VII level 4 (ICON Development Solutions, Ellicott City, MD, USA). Results: In total, 2899 SpO2 data points obtained from 20 volunteers were used to determine the pharmacodynamic characteristics. The pharmacodynamic parameters were as follows: kout = 0.942 1/min, Imax = 0.802, IC50 = 85.3%, γ = 27.3. Conclusion: The changes in SpO2 due to decreases in FiO2 well explained by the turnover model with inhibitory function as a sigmoidal model.

Cerebral Oxygenation during Apnea in Preterm Infants: Effects of Accompanying Peripheral Oxygen Desaturation

  • Purpose: Premature infants have immature respiratory control and cerebral autoregulation. We aimed to investigate changes in cerebral oxygenation during apnea with and without peripheral oxygen desaturation in premature infants. Methods: This prospective observational study was conducted at Inha University Hospital. Near-infrared spectroscopy (NIRS)-monitored regional cerebral oxygen saturation (rScO2) and pulse oximeter-monitored peripheral oxygen saturation (SpO2) were assessed during the first week of life in 16 stable, spontaneously breathing preterm infants. Apneic episodes that lasted for ≥20 seconds or were accompanied by desaturation or bradycardia were included for analysis. The average rScO2 value during the 5-minute prior to apnea (baseline), the lowest rScO2 value following apnea (nadir), the time to recover to baseline (recovery time), the area under the curve (AUC), and the overshoot above the baseline after recovery were analyzed. Results: The median gestational age and birth weight of the infants were 29.2 weeks (interquartile range [IQR], 28.5 to 30.5) and 1,130 g (IQR, 985 to 1,245), respectively. A total of 73 apneic episodes were recorded at a median postnatal age of 2 days (IQR, 1 to 4). The rScO2 decreased significantly following apneic episodes regardless accompanied desaturation. There were no differences in baseline, nadir, or overshoot rScO2 between the two groups. However, the rScO2 AUC for apnea with desaturation was significantly higher than that for apnea without desaturation. Conclusion: Cerebral oxygenation can significantly decrease during apnea, especially when accompanied by reduced SpO2. These results add the evidence for the clinical utility of NIRS in monitoring premature infants.

Effects of Oxygen Saturation Fluctuations on Retinopathy in Infants Younger than 30 Weeks' Gestational Age

  • Purpose: To evaluate the risk factors and effects of fluctuations in oxygen saturation on the occurrence and severity of retinopathy of prematurity (ROP). Methods: From January 1, 2013 to January 31, 2020, 260 patients hospitalized in the neonatal intensive care unit of Ajou University Hospital were retrospectively analyzed. Results: Sixty-six patients (25%) were diagnosed with ROP; of them 39 required treatment. In the multivariate regression analysis of ROP severity, the odds ratio (OR) of gestational age was 0.32 (95% confidence interval [CI], 0.22 to 0.47; P<0.0001). The OR of saturation of peripheral oxygen (SpO2) fluctuations at the 4th week after birth was 1.02 (95% CI, 1.02 to 1.12; P=0.041). Conclusion: SpO2 fluctuations in premature infants younger than 30 weeks' gestational age affects ROP severity, especially at the 4th week of life.

Midazolam의 비강내 투여시 의식진정효과에 관한 연구 (SEDATIVE EFFECT OF INTRANASAL ADMINISTRATION WITH MIDAZOLAM IN SEDATING PEDIATRIC DENTAL PATIENTS)

  • The purpose of this study was to assess the sedative effect of intranasal spray with midazolam for management of the uncooperative 20 children aged from 24 months to 92 months who required extensive treatment. The patients were given randomly a dose of 0.2mg/kg of intranasal placebo, intranasal spray with midazolam, and intranasal drop with midazolam. All the children were restrained in a pediwrap and were monitored with pulse oximeter for assessing the pulse rate and peripheral oxygen saturation. According to Fukuta's behavior rating scale, behavior was checked for evaluation of the clinical sedative effect. The obtained results were as follows: 1. Behavior score of intranasal spray with midazolam was lower than intranasal drop with midazolam(P<0.01). 2. Pulse rate was a significant change as a function of dental procedure(P<0.001), however the peripheral oxygen saturation was not influenced significantly by either adiministration route of drug or dental procedure. Clinically, intranasal spray with midazolam were safe and effective sedation in young children undergoing pediatric dental procedures.

만성폐쇄성폐질환자에서 기류제한 및 COPD 복합지수와 말초산소포화도의 연관성 (Association of Airflow Limitation and COPD Composite Index with Peripheral Oxygen Saturation in Patients with Chronic Obstructive Pulmonary Disease)

  • Objective: Chronic obstructive pulmonary disease (COPD) is characterized by progressive airflow obstruction that is only partly reversible, inflammation in the airways, and systemic effects. This study aimed to investigate the association between low peripheral oxygen saturation levels (SpO2), and composite indices predicting death in male patients with (COPD). Method: A total of 140 participants with post-bronchodilator FEV1/FVC ratio less than 0.7 were included. Three composite indices (ADO, DOSE, BODEx) were calculated using six variables such as age (A), airflow obstruction (O), body mass index (B), dyspnea (D), exacerbation history (E or Ex), and smoking status (S). Severity of airflow limitation was classified according to Global Initiative for Obstructive Lung Disease (GOLD) guidelines. SpO2 was measured by pulse oximetry, and anemia and iron deficiency were assessed based on blood hemoglobin levels and serum markers such as ferritin, transferrin saturation, or soluble transferrin receptor. Results: Participants with low SpO2 (<95%) showed significantly lower levels of %FEV1 predicted (p=0.020) and %FEV1/FVC ratio (p=0.002) compared to those with normal SpO2 levels. The mMRC dyspnea scale (p<0.001) and GOLD grade (p=0.002) showed a significant increase in the low SpO2 group. Receiver Operating Characteristic analysis revealed higher area under the curve for %FEV1 (p=0.020), %FEV1/FVC(p=0.002), mMRC dyspnea scale (p=0.001), GOLD grade (p=0.010), ADO (p=0.004), DOSE (p=0.002), and BODEx (p=0.011) in the low SpO2 group. Conclusion: These results suggest that low SpO2 levels are related to increased airflow limitation and the composite indices of COPD.

운동과 음악을 이용한 노래부르기가 노인의 생리적 변화, 인지기능 및 우울에 미치는 효과 (The Effects of Singing Program Combined with Physical Exercise of Physiologic Changes, Perception Function and Degree of Depression in the Elderly Women)

  • This study was conducted for the evaluation of the effects of singing program combined with physical exercise on the physiologic changes, perception function and degree of depression. The subjects were the members of elderly women's glee club in D care center for the elderly, who have been singing for more than 6 months. 30 members were allocated to study group and 30 to control group. The singing program designed for both physical therapy and music therapy was consisted of initial physical exercise, singing art songs and classical song and the finishing physical exercise. This program was performed twice a week and about forth minutes was consumed for one session. We checked the heart rate, peripheral arterial oxygen saturation, perception function and degree of depression before and after the program. We used a pulse oxymeter to check the heart rate to oxygen saturation and a questionnaire for the evaluation of perception function and degree of depression. We need SPSS program for data analysis. The results of the investigated personnel complying with general characteristics were analyzed by frequency, two groups by t-test, data before and after the program by paired t-test, respectively. The results were as follows. 1) Heart rate after the program was significantly lower than that before program in test group(p<0.05). 2) Peripheral oxygen saturation after the program was significantly higher than that before the program(p<0.05). 3) Ability to match the right sign with a certain predetermined number was improved after the program. The frequency of wrong matching the sign with number before program was 30. But the frequency was decreased to 8 after the program. 4) Ability to calculate was improved after the program. The frequency of wrong calculation before the program was $1.10{\pm}1.94$. But the frequency after the program was decreased to $0.97{\pm}1.84$. 5) The degree of depression after the program($2.07{\pm}0.49$) was significantly lower than that before program(p<0.001). These results show that singing program combined with physical exercise improves the oxygen delivery to peripheral circulation, stability of heart function, the perception function(calculating and matching ability) and decreases the degree of depression. In conclusion, singing program combined with physical exercise can be used for the effective measure to improve the health of elderly and prevent dementia.
