Standardized Body Size Indexes of 10 Canine Breeds Vulnerable to Intervertebral Disc Disease

  • The intervertebral disc is a crucial structure located between each vertebral body, except C1 and C2 and the fused sacral vertebrae, which provides cushioning and stability. However, sometimes these discs may bulge or extrude, causing painful conditions and various neurological problems such as ataxia, paresis, motor or sensorimotor paralysis. As a result, dogs affected by intervertebral disc disease (IVDD) can suffer a significant decrease in their quality of life. The main objective of this study was to provide comprehensive data on the body size of several IVDD-susceptible dog breeds commonly bred in South Korea, which could help develop supportive devices for IVDD patients. Using standardized measurements, the study aimed to design appropriate support tools, taking into account the mobility challenges faced by IVDD patients. The findings provide valuable data to improve the quality of life of patients with IVDD. In addition, it may lay the foundation for research to establish the correlation between body size and IVDD through future research.

  • Narcolepsy is a central neurologic system disease. It begins early in life with disabling symptoms including excessive daytime sleepiness, cataplexy, sleep paralysis, hypnagogic hallucination and nocturnal sleep fragmentation. Patient with typical symptoms of narcolepsy is diagnosed by objective data from nocturnal polysomnography and multiple sleep latency tests. Narcolepsy is controlled with various medications. Nowadays, modafinil with favorable side effects profiles compared with traditional stimulant is mainly used. Gamma hydroxyl butyrate is effective in cataplexy. Cataplexy is also controlled with antidepressant such as Venlafaxine, SSRI, and TCA. As the knowledge of pathophysiology of narcolepsy expands, new treatment including immunological method, application of hypocretin and histamine systems have been tried.

A Phenomenological Study on Illness Experience of Patients with Pressure Ulcer (욕창 환자들의 질병 체험에 관한 현상학적 연구)

  • Purpose: The purpose of this study was to describe the illness experience of patients with pressure ulcer. Methods: A phenomenological methodology was used for the study. The data were collected by individual in-depth interview with seven participants with pressure ulcer during 2013~2014. All interviews were audio-taped and verbatim transcripts were made for the analysis. The data were analyzed using Colaizzi's phenomenological method. Results: All participants had underlying disease, such as spinal paralysis and diabetes. Average period of having pressure ulcer was 18 months, ranged from 3 to 36 months. A total of seven theme clusters were derived from the analysis; unexpected wound, inherent vulnerability to infection, reversal of the treatment policy, unpleasant and strange feeling of wound, sweeping fear and helplessness, socioeconomic burden, and healing through specific actions and reflection. The participants faced various contradictory and paradoxical situations in managing their pressure ulcers as well as underlying diseases in their everyday life. However, they slowly overcome these situations by strictly practicing concrete action-oriented strategies that they have learned through suffering and appreciating miraculous wound healing. Conclusion: The results of this study can help developing a patient-specific intervention program with sufficient emotional support by providing insights of the paradoxical illness experience of patients with pressure ulcer.

Systematic Review on the Customers' Use of and Satisfaction with Oriental Medical Services (한방의료서비스 이용과 만족도의 영향요인에 대한 계통적 고찰)

  • Objectives: This study aimed to find determinants of customers' use of and satisfaction with oriental medical services in Korea. Methods: A total of 33 articles which consist of 9 articles published in academic journals, 20 master's theses, and 4 doctoral dissertations were included in the systematic review. Among the articles used in the study, 29 used primary survey data, while 4 used secondary data produced by the Korean National Health and Nutrition Evaluation Survey. Results: First, it was found that people who prefer to use oriental medical services usually have one or more of the following diseases: musculoskeletal disorders, paralysis, or diseases or injuries without clear diagnosis. Other variables of gender, age, expectation of treatment outcome, and kindness of doctor and other staff were found to be significant determinants of using the oriental medical services. Second, the determinants of customers' satisfaction with oriental medical services were found to be age, gender, level of education, chronic disease with long length of stay, kindness of staff, medical cost, clinical environment, doctor's reputation, and public image of the institutions. Conclusion: The results of this study can be used to develop marketing strategies for oriental medical institutions in Korea.

Perception on the Traditional Korean Medicine According to the Existence of a Chronic Disease

  • Objectives: Chronic diseases (CDs) continue to increase due to advances in medicine and increase in lifespan, affecting quality of life and resulting in economic loss through treatment costs. This study addressed the perception characteristics of the effectiveness of Korean medicine (KM) based on the existence of a CD to determine the national preference for KM treatment. Methods: Using data from the 2017 National Survey of KM Usage, we evaluated the perception on treatment effect of 16 diseases by dividing them into the CD group and the non-CD group. Response reliability was verified by applying the chi-square test (χ2-test) analysis method of the Statistical Package for the Social Sciences (SPSS) statistical program. Results: The analysis of the perception on effectiveness of KM for patients with CD (n = 1,050, 21.0%) and for patients with non-CD (n = 3,950, 79.0%) showed an overall similar trend for all the 16 diseases. The response rates of having some treatment effect were high for nine diseases namely, disc-related disease (CD: 70.7%, non-CD: 73.1%), osteoarthritis (CD: 72.3%, non-CD: 72.4%), frozen shoulder and shoulder pain (CD: 79.6%, non-CD: 81.4%), back pain (CD: 84.6%, non-CD: 85.0%), sprain (CD: 84.8%, non-CD: 84.1%), facial nerve paralysis (CD: 73.5%, non-CD: 71.7%), stroke (CD: 66.2%, non-CD: 62.8%), digestive disease (CD: 53.3%, non-CD: 50.0%), and common cold and rhinitis (CD: 44.7%, non-CD: 44.8%). Conclusion: The present results found that there was little difference in the perception on effectiveness of KM for each of the 16 diseases in patients with CD and non-CD, however, 70% or more of the respondents recognized some treatment effect on musculoskeletal disorders, regardless of the existence of CD. Preferential political support for KM treatment of chronic musculoskeletal disorders is recommended.

Brain Injury Patients's Disability Degree and Educational Needs of Family Caregivers (뇌손상 환자의 장애정도와 간병가족의 교육요구도)

  • Purpose: This study was conducted in order to identify the brain injury patients's disability degree and educational needs of family caregivers. Methods: A convenience sample of 94 families with brain injury patients, who have been receiving treatment at the neurological intensive care unit and neurosurgery ward, were used. Data was collected with a self-report questionnaire from September 5 to November 28, 2011, and was analyzed using SAS program. Results: 'Defecation/urination' disability was the highest score of patient's physical disability and the next ranking was 'paralysis'. 'Memory impairment' disability was the highest score of patient's cognitive disability, and the next ranking was 'personality changes'. Overall, educational needs of family caregivers scored 4.15 out of the perfect score of 5. The factor, which scored highest, was 'information related with disease'. In addition, educational needs of family caregivers were positively related with patient's degree of. Conclusion: Educational needs of family caregivers are distinct, according to the disability degree of brain injury patient. Therefore, the study suggests the development of individualized educational program for family with brain injury patient.

Trend Analysis of Complex Disasters in South Korea Using News Data (뉴스데이터를 활용한 국내 복합재난 발생 동향분석)

  • As the diversity of disasters continues to increase, the concept of "complex disasters" has gained prominence in various policies and studies related to disaster management. However, there has been a certain limitation in the availability of the systematic statistics or data in advancing policies and research initiatives related to complex disasters. This study aims to analyze the macro-level characteristics of the complex disasters that have occurred domestically utilizing a 30-year span of a news data. Initially, we categorize the complex disasters into the three types: "Natural disaster-Natural disaster", "Natural disaster-Social disaster", and "Social disaster-Social disaster". As a result, the "natural diaster-social disaster" type is the most prevalent. It is noted that "natual disaster-natural disaster" type has increased significantly in recent 10 years (2011-2020). In terms of specific disaster types, "Storm and Flood", "Collapse", "Traffic Accident", "National Infrastructure Paralysis", and "Fire⋅Explosion" occur the most in conjunction with other disasters in a complex manner. It has been observed that the types of disasters co-ocuuring with others have become more diverse over time. Parcicularly, in recent 10 years (2011-2020), in addition to the aforementioned five types, "Heat Wave", "Heavy Snowfall⋅Cold Wave", "Earthquake", "Chemical Accident", "Infectious Disease", "Forest Fire", "Air Pollution", "Drought", and "Landslide" have been notable for their frequent co-occurrence with other disasters. These findings through the statistical analysis of the complex disasters using long-term news data are expected to serve as crucial data for future policy development and research on complex disaster management.

A prescription study in 『Dongeuibogam』 for the Applications of Cnidii Rhizoma (천궁(川芎)의 활용(活用)을 위한 『동의보감(東醫寶鑑)』 처방연구(處方硏究))

  • Objectives : Until now the study of Cnidii Rhizoma, hemorrhage, brain waves, such as ischemic brain injury, analgesic, effect overcome of the stress from pregnancy melanin formation and inhibiting effects skin whitening have been published regarding this article. Cnidii Rhizoma demonstrates its different abilities depending on the characteristics. This paper reported that effect of Cnidii Rhizoma in Dongeuibogam blended prescriptions as main medicine. In addition, by analyzing data, we studied about utilizing of Cnidii Rhizoma. Methods : Cnidii Rhizoma in Dongeuibogam Prescriptions as the main ingredient was built with database of 202 prescriptions. Thus analyzed data was summarized in detail.(Table-1) If there is no difference in the title of the prescription but in other case the configuration information is different, formulations 1 and 2 were divided by the table. Results : The following results were reached through investigations on the prescriptions usikng Cnidii Rhizoma as a key component. 1. Prescriptions taking Cnidii Rhizoma as a monarch drug are utilized for 40 therapeutic purposes. In particular, 12.3% of prescriptions appear in the chapter of head, and 10.8% of those appear in the chapter of women, and 9.4% of eye, 8.9% of child, 6.4% of wind disease respectively. 2. Prescriptions utilizing Cnidii Rhizoma as the main ingredient are used in the treatment of headache, dizziness and pregnancy hemorrhage fetal movement, premature birth and they are also used for treating 131 different types of disease. 3. The dosage of Cnidii Rhizoma in formulas is from 2pun(about 0.75g) to 5don(nearly 18.75g), however 1don(nearly 3.75g) has been taken the most for clinical application. 4. We find out that according to herbs or prescriptions, Cnidii Rhizoma has a variety of functions such as ascending & descending of energy. Samultang is the most useful base prescription which used the Cnidii Rhizoma as the main component. Conclusion : These results suggest that, Cnidii Rhizoma once-amount use (don nearly 3.75g) 4g in head, gynecology, ophthalmology, pediatrics and paralysis disease associated with oriental medicine resource development can be considered to be widely used These results suggest that Cnidii Rhizoma was used most with 1 don(4g) and can be widely used for the resource development to the disease such as brain, gynecology, ophthalmologhy, pediatrics and wind-associated symptoms.

A Basic Study on the Differential Diagnostic System of Laryngeal Diseases using Hierarchical Neural Networks (다단계 신경회로망을 이용한 후두질환 감별진단 시스템의 개발)

  • The objectives of this Paper is to implement a diagnostic classifier of differential laryngeal diseases from acoustic signals acquired in a noisy room. For this Purpose, the voice signals of the vowel /a/ were collected from Patients in a soundproof chamber and got mixed with noise. Then, the acoustic Parameters were analyzed, and hierarchical neural networks were applied to the data classification. The classifier had a structure of five-step hierarchical neural networks. The first neural network classified the group into normal and benign or malign laryngeal disease cases. The second network classified the group into normal or benign laryngeal disease cases The following network distinguished polyp. nodule. Palsy from the benign laryngeal cases. Glottic cancer cases were discriminated into T1, T2. T3, T4 by the fourth and fifth networks All the neural networks were based on multilayer perceptron model which classified non-linear Patterns effectively and learned by an error back-propagation algorithm. We chose some acoustic Parameters for classification by investigating the distribution of laryngeal diseases and Pilot classification results of those Parameters derived from MDVP. The classifier was tested by using the chosen parameters to find the optimum ones. Then the networks were improved by including such Pre-Processing steps as linear and z-score transformation. Results showed that 90% of T1, 100% of T2-4 were correctly distinguished. On the other hand. 88.23% of vocal Polyps, 100% of normal cases. vocal nodules. and vocal cord Paralysis were classified from the data collected in a noisy room.

Statistical Techniques based Computer-aided Diagnosis (CAD) using Texture Feature Analysis: Applied of Cerebral Infarction in Computed Tomography (CT) Images

  • The brain is the body's most organized and controlled organ, and it governs various psychological and mental functions. A brain abnormality could greatly affect one's physical and mental abilities, and consequently one's social life. Brain disorders can be broadly categorized into three main afflictions: stroke, brain tumor, and dementia. Among these, stroke is a common disease that occurs owing to a disorder in blood flow, and it is accompanied by a sudden loss of consciousness and motor paralysis. The main types of strokes are infarction and hemorrhage. The exact diagnosis and early treatment of an infarction are very important for the patient's prognosis and for the determination of the treatment direction. In this study, texture features were analyzed in order to develop a prototype auto-diagnostic system for infarction using computer auto-diagnostic software. The analysis results indicate that of the six parameters measured, the average brightness, average contrast, flatness, and uniformity show a high cognition rate whereas the degree of skewness and entropy show a low cognition rate. On the basis of these results, it was suggested that a digital CT image obtained using the computer auto-diagnostic software can be used to provide valuable information for general CT image auto-detection and diagnosis for pre-reading. This system is highly advantageous because it can achieve early diagnosis of the disease and it can be used as supplementary data in image reading. Further, it is expected to enable accurate medical image detection and reduced diagnostic time in final-reading.
