Sequential treatment for a patient with hemifacial microsomia: 10 year-long term follow up

  • Hemifacial microsomia (HFM) is the most common craniofacial anomaly after cleft lip and cleft palate; this deformity primarily involves the facial skeleton and ear, with either underdevelopment or absence of both components. In patients with HFM, the management of the asymmetries requires a series of treatment phases that focus on their interception and correction, such as distraction osteogenesis or functional appliance treatment during growth and presurgical orthodontic treatment followed by mandibular and maxillary surgery. Satisfactory results were obtained in a 9-year-old girl with HFM who was treated with distraction osteogenesis. At the age of 19, genioplasty and mandible body augmentation with a porous polyethylene implant (PPE, $Medpor^{(R)}$, Porex) was sequentially performed for the functional and esthetic reconstruction of the face. We report a case of HFM with a review of the literature.

  • This study evaluated the influence of smooth and brushite-coated implant surfaces in dogs. The first through fourth mandibular premolars were extracted from eight young adult dogs. Twelve weeks after extraction. implantation was performed at the extraction sites. In total, 40 implant fixtures were implanted in the dog mandibles. Twenty machined implants served as controls and twenty brushite-coated surfaces served as tests. Dogs were sacrificed 2 and 4 weeks after implantation. The hemi-mandibles were obtained and processed histologically to obtain non-decalcified sections. Longitudinal sections of each implant were made and analyzed using light microscopy. The overall implant success rate was 83.3%. Histomorphometrically. the experimental group had a better percentage of bone-implant contact than the control group (p<0.05) and there was a significant difference between the 2- and 4-week groups after implantation (p<0.05) Our results suggest that the implant surface morphology influences the increase in peri-implant osteogenesis in the early period of peri-implant healing.

A case report about the reconstruction procedures of the previously failed cylinderical implants site using distraction osteogenesis

  • We report the eventually successful treatment of a huge bone defect and peri-implantitis following reconstruction of a previously failed intra-mobile cylinder implant system (IMZ) implant site using distraction osteogenesis (DO). In the anterior mandible, two IMZ implants failed and surgical debridement was performed in accordance to the patient's needs. Thereafter, mobility and suppuration were decreased and the patient visited the dental clinic on a regular basis for oral health maintenance. However, the inflammation did not resolve, and the bone destruction around the implants progressed for 4 years. Finally, the implants failed and a severe bone defect remained after implant removal. To reconstruct the bone defects, we attempted bone graft procedures. Titanium mesh was unsuccessfully used to obtain bone volume regeneration. However, DO subsequently was used to obtain sufficient bone volume for implant placement. The new implants were then installed, followed by prosthetic procedures. In conclusion, progression of peri-implantitis could not be arrested despite surgical intervention and repeated maintenance care for 3 years. Reconstruction of the peri-implantitis site was complicated due to its horizontal and vertical bone defects. Lesions caused by implant failure require an aggressive regenerative strategy, such as DO. DO was successful in reconstruction of a peri-implantitis site that was complicated due to horizontal and vertical bone defects.

수직 치조골 신장술 후 증대된 치조골과 임플란트의 예후 평가 (Evaluation of augmented alveolar bone with vertical alveolar distraction osteogenesis and implant installation)

  • Introduction: The purpose of this study was to evaluate the clinical result of vertical alveolar distraction, especially the distracted alveolar bone and installed implants. Materials and Methods: Twenty-one patients who have been received the vertical alveolar distraction and implant installation on 22 areas (3 maxilla and 19 mandible) using intraoral alveolar distraction device were examined. After consolidation period of 3-4 months, distraction devices were removed and 91 implants were installed in the distracted alveolar bone. The distracted bone and implants were evaluated clinically and radiographically. Results: Mean height of distracted alveolar bone was $7.5{\pm}3.2$ mm (range: 2.5-15.0 mm). Mean follow-up period after completion of the distraction was 3.1 years (range: 1.4-11.5 years). Mean resorption of distracted alveolar bone was $1.6{\pm}1.8$ mm. The success and survival rates of implants was 95.3% and 100%, respectively. Conclusion: Results of this study indicate that vertical alveolar distraction procedure is a useful and stable method for alveolar ridge augmentation and implantation.

치조골 소실과 심한 양악전돌을 동반한 성인환자에서의 피질골 절단술과 Compression osteogenesis를 이용한 교정치료 (Severe bimaxillary protrusion with adult periodontitis treated by corticotomy and compression osteogenesis)

  • 본 증례보고는 성인형 치주염으로 인해 전반적인 치조골 소실을 보이고 양악 전돌을 동반한 II급 부정교합으로 진단된 50세 10개월 된 여자환자의 치료를 소개하고자 한다. 치주 치료를 진행한 후 양악 전돌을 해소하기 위해 양악 제1소구치를 발치하고, 상악 전치부는 피질골 절단술 시행 후 악정형적 견인을, 하악 전치부는 6전치의 전방부 분절골 절단술[Anterior segment osteotomy(ASO)]을 국소마취하에 시행하였다. 총 치료기간은 9개월이 소요되었고 안정적인 교합관계와 안모의 개선이 이루어졌다. 하지만 치료 후에 하악 전치부에 약간의 치근 흡수 소견이 관찰되었다. 치료 27개월 후에도 안정적인 치료결과가 유지되었다.

구순구개열 환자에서 골신장술을 이용한 전상악골의 재건 (Premaxillary Reconstruction by Distraction Osteogenesis for Cleft Lip/Palate)

  • Patients with cleft lip and palate usually present midfacial depression and anterior cross-bite. This dentofacial deformity has been believed due to the undergrowth of maxilla and/or the collapse of premaxilla. But, in the case that the collapsed premaxilla exists only, the reconstruction of the premaxilla has to be required for the correction of that deformity. These cases show the surgical treatment of midfacial depression and anterior cross-bite in the cleft lip and palate. After the careful diagnosis for the collapse of premaxilla, the reconstruction using distraction osteogenesis was done successfully. As a result, the anterior overbite / overjet, and facial esthetics were improved remarkably, and the occlusion was also recovered to normal state. In conclusion, the premaxillary reconstruction by distraction osteogenesis in cleft lip and palate patients is a good treatment method based on the pathophysiology.

$Synthes^{(R)}$상악골 신장기를 이용한 성인 구개구순열 환자의 치험례 (An Adult Cleft Lip and Plate Patient Using a Maxillary Distractor by $Synthes^{(R)}$ : Report of a case)

  • Generally, an adult cleft lip or/and palate patient shows some amount of maxillary deficiency due to limitation of bony growth caused by heavy scars resulted from previous operations such as a cheiloplasty and/or a palatoplasty at an early child age. To solve the problem, advancement of the maxilla is usually required during orthognathic surgery. However, severe tensional force resulted from heavy scars on the palate and/or the lip, as well as the bony defect at the cleft area limited sufficient advancement of the maxillary segment and finally caused relapse of the reposed maxilla. Therefore, distraction osteogenesis of the maxilla was introduced for the successful maxillary advancement inthose kinds of patients. As both hard and soft tissues can be simultaneously and gradually extended with this technique, tensional force caused by heavy scars opposed to forward movement of the maxilla can be reduced to an extent not to develop severe relapse of the advanced maxilla. Since distraction osteogenesis of the maxilla was applied as one of standard protocols for the treatment of the patients with severe maxillary hypoplasia dueto cleft lip and/or palate, the devices for the distraction was improved to control the vectors of distraction with better and more stable. We have treated a 23-year-old male cleft patient with a severe maxillary hypoplasia using a newly developed a maxillary distraction device and a RP model for a pre-operative simulation surgery. As a result, we could successfully move the maxilla as we designed pre-operatively and also reduce much of operation time. Therefore, we report of the case to share our experience with colleagues.

키틴 유도체와 하이드록시아페타이트 복합체가 개의 뼈에 미치는 영향 (The Effects of Chitin Derivative and Hydroxyapatite Compound in Canine Bone)

  • The aim of this study was to investigate bioactivities of 50% Chitin - hydroxyapatite (Chitin-H) compound and 50% Chitosan - hydroxyapatite (Chitosan-H) compound in canine bone. Ten healthy mongrel dogs (1-5 years old, 1.7 - 6.9 kg) were used in this study. These compounds had been transplanted into bilateral femur separately, and then the changes of femur were observed through the examinations of hemato-biochemical profiles, radiology, and histological profiles for 42 days. After 3 weeks, expanded radiolucent changes were observed in both areas transplanted the compounds. After 6 weeks, the area transplanted the Chitin-H compound did not observe any changes of bony tissue, while the area inserted the Chitosan-H compound was observed changes of increasing bone formation. In histological examination, infiltrations of inflammatory cells and bone absorptions were observed at both transplanted sites. However an increasing of active osteogenesis was observed at the transplanted site with Chitosan-H compound. In conclusion, Chitosan-H compound had an function of active osteogenesis as compared with Chitin-H compound. From this study, it is indicated that Chitosan-H compound would be used in dogs with severe bone defect.

Ergostane-Type Steroids from Korean Wild Mushroom Xerula furfuracea that Control Adipocyte and Osteoblast Differentiation

  • As part of our current work to discover structurally and/or biologically novel compounds from Korean wild mushrooms, we isolated five ergostane-type steroids (1-5) from the fruiting bodies of Xerula furfuracea via repeated column chromatographic separations and HPLC purification. The chemical structures of the isolated steroids were shown to be (22E,24R)-24-methylcholesta-4,22-diene-3,6-dione (1), ergosta-7,22-diene-3β,5α,6β-triol (2), ergosta-7,22-diene-3β,5α,6β,9α-tetraol (3), (22E,24R)-5α,8α-epidioxyergosta-6,22-diene-3β-ol-3-O-β-D-glucopyranoside (4), and (22E,24R)-5α,8α-epidioxyergosta-6,9,22-triene-3β-ol-3-O-β-D-glucopyranoside (5)based on comparison of the data regarding their spectroscopic and physical properties with those of previous studies. Notably, this is the first report on the presence of the identified steroids (1-5) in this mushroom. We tested compounds 1-5 to determine their effects on adipogenesis and osteogenesis in the mouse mesenchymal stem cell line C3H10T1/2 and found that compounds 4 and 5 suppressed the differentiation of stem cells into adipocytes. Notably, in addition to its suppressive effect on adipogenesis, compound 5 was also shown to promote the osteogenic differentiation of stem cells. These findings demonstrate that the bioactive compounds isolated might be effective for the treatment of menopause-associated syndromes, such as osteoporosis and obesity, as the isolated compounds were shown to suppress adipogenesis and/or promote osteogenesis of stem cells.

  • Pleiotrophin or osteoblast-specific factor 1(HOSF-1) is a growth-associated protein present in bone matrix. This study was designed to study pleiotrophin expression in osteoblastic cells. Pleiotrophin was expressed by osteoblast-like cell line. Pleiotrophin expression increased following the proliferative phase and was minimal at the terminal phases of the induced differentiation of cultured MC3T3-E1 cells. Pleiotrophin expression represents another autocrine factor that may contribute to the physiologic control of induced bone formation. In this study, induced osteogenesis will be examined in the context of the osteoblast expression of and regulation by PTN. I hypothesized that PDGF-BB stimulation of PTN expression represents an important paracrine signal during the induced osteogenesis associated with periodontal and implant surgeries. The possible mediation by PTN of anabolic effects attributed to PDGF-BB stimulation was examined in cell culture models of osteoblast differentiation. These studies will contribute fundamental insights to osteoblast biology and insights regarding the potential use of factors such as PTN in the clinical environment.
