A Study on the Change of Clinical Self-Confidence according to the Number of Clinical Dental Hygiene Practices of Students in the Department of Dental Hygiene - Focusing on scaling practices -

  • The purpose of this study was to provide the basic data needed to plan the effective teaching design of scaling education and to measure the changes in students' confidence in performing scaling according to the number of scaling practices. Scaling education is presented as a core basic hygiene competency of dental hygienists and evaluated as the practical competency of dental hygienists. This study selected 48 third-year students from the department of dental hygiene at S University in Asan, and analyzed data from completed participant surveys. The degree of "confidence in facing the subject in clinical practice," "confidence in applying hand instrument to the subject," and "confidence in applying ultrasonic instruments to the subject" was assessed on a five-point Likert scale. The difference in confidence was analyzed during five practice sessions. Each response was compared using frequency analysis, chi-square test, and repeated measurement ANOVA. Students who complained of a lack of confidence in the 14.6%, 25.0%, and 12.5%, respectively, in face-to-face practice, hand instruments, and ultrasonic instrument application responses. The more the practice was repeated, the more confident students were in all three categories, and the more statistically significant (p<0.001). As the number of scaling practice sessions increased, students' confidence in performing scaling also improved. In particular, the level of self-confidence was higher after the third practice session when compared to the first session. Therefore, it is necessary to design effective courses for teaching scaling practices so that at least three repetitive practice periods can be provided in clinical dental hygiene practices.

서비스직 유무에 따른 아르바이트 종사자 대학생의 자가구취인식도가 구강보건관리실천도에 미치는 영향 (The effects of self-perception of halitosis on oral health behaviors of college students)

  • 본 연구는 수도권에 거주하는 서비스직 아르바이트 중인 대학생 150명, 비서비스직인 대학생 150명을 대상으로 2018년 4월 26일부터 5월 5일까지 자가구취인식도가 구강보건관리실천도에 미치는 영향력 알아보고자 설문조사를 실시한 연구결과는 다음과 같다. 1. 서비스직군에서 칫솔질을 하는 목적은 '충치와 잇몸병 예방'과 '구취' 항목이 주를 이룬 반면, 비서비스직군에서는 '충치와 잇몸병 예방'과 '상쾌한 기분' 항목에 가장 많은 응답을 한 것으로 나타났다(x2=18.21, p<0.001). 2. 서비스직군과 비서비스직군 모두에서 구취의 원인은 백태와 음식물이 영향을 미치는 요인으로 나타났다(x2=10.95, p=0.027). 3. 서비스직 유무에 따른 자가구취인식도와 구강보건관리실천도의 평균분석에 따르면, 서비스직군에서 모든값이 높은 것으로 나타났다(p<0.001). 4. 자가구취인식과 구강보건관리실천도는 음의 상관관계가 있는 것으로 확인되었다(γ=-0.11, p<0.05). 5. 성별과 학년을 보정한 회귀분석모델에서, 자가구취인식도가 한 단위 증가하면 구강보건관리실천도는 증가하는 것으로 나타났다(Beta=-0.185, p=0.02). 이상과 같이 서비스직 유무에 따른 아르바이트 종사자 대학생을 대상으로 자가구취인식도가 구강보건관리 실천도에 미치는 영향을 확인해 본 결과, 서비스직 아르바이트에 종사하는 대학생의 자가구취인식도가 높을수록 구강보건관리실천도에 미치는 영향이 증가하는 것을 근거로 서비스직 종사자의 구강건강향상 및 증진을 위한 구강건강 프로그램 및 홍보등의 체계적인 개입이 필요할 것으로 사료된다.

Knowledge and Opinions Regarding Oral Cancer among Yemeni Dental Students

  • Background: Oral cancer presents with high mortality rates, and the likelihood of survival is remarkably superior when detected early. Health care providers, particularly dentists, play a critical role in early detection of oral cancers and should be knowledgeable and skillful in oral cancer diagnosis. Purpose: The aim of the present study was to assess the current knowledge of future Yemeni dentists and their opinions on oral cancer. Materials and Methods: A pretested self-administered questionnaire was distributed to fourth and fifth year dental students. Questions relating to knowledge of oral cancer, risk factors, and opinions on oral cancer prevention and practices were posed. Results: The response rate was 80%. The vast majority of students identified smoking and smokeless tobacco as the major risk factors for oral cancer. Most of the students (92.6%) knew that squamous cell carcinoma is the most common form of oral cancer, and 85.3% were aware that tongue and floor of the mouth are the most likely sites. While the majority showed willingness to advise their patients on risk factors, only 40% felt adequately trained to provide such advice. More than 85% of students admitted that they need further information regarding oral cancer. As expected, students of the final year appeared slightly more knowledgeable regarding risk factors and clinical features of the disease. Conclusions: The findings of the present study suggest that here is a need to reinforce the undergraduate dental curriculum with regards to oral cancer education, particularly in its prevention and early detection.

Attitude and Practices Among Dentists and Senior Dental Students in Iran Toward Tobacco Cessation as an Effort to Prevent Oral Cancer

  • Background: Oral health professionals are responsible in Iran for providing a brief tobacco cessation program to smoker patients. The aim of this study was to assess Iranian dental student and dentist practice, knowledge and attitudes toward smoking cessation programs. Materials and Methods: A valid and reliable self-administered questionnaire was designed and distributed to 150 dentists working in Isfahan-Iran and 60 dental students. Some questions were developed based on the expected 5A tobacco cessation protocol. Statements on attitudes focused on professional responsibility towards smoking cessation and its effectiveness. Chi-square, ANOVA, and t test were used for statistical analysis. Results: The cessation program in dental settings covers a small group of patients (18%). Some 69.1% (n=96) of dentists reported asking their patients about tobacco use, 64% (n=83) advising their patients to quit, 33.8% (n=47) assessing their patients willingness to quit and 20% (n=28) reported helping their patients in changing their behavior. A far lower percentage reported active involvement in arranging assistance for smokers to quit (4.3%, n=5). Some 22% of students and 26% of dentists disagreed that the tobacco cessation programs should be as part of dentists' professional responsibility and 70% of them were willing to follow the protocol of tobacco cessation for patients. Conclusions: Iranian dentist performance regarding tobacco cessation is weak. Dentists and students indicated their lack of knowledge as the major reason for non-adherence to the protocol. Therefore, planning to encourage dentist to follow the protocol needs continuous educational programs.

Breast and Cervical Cancer Related Practices of Female Doctors and Nurses Working at a University Hospital in Turkey

  • Background: Breast and cervical cancers are among the most frequent and most fatal cancers in women. Life span of patients may be increased and quality of life improved through early diagnosis and treatment. This investigation was performed in order to determine knowledge and practices of female health personnel working at a university hospital regarding breast and cervical cancers. Materials and Methods: This descriptive investigation was performed in Erciyes University Hospitals in 2014. A total of 524 female health personnel were included in the study. Data were collected through a questionnaire of 36 questions prepared by the researchers. The Chi square test and logistic regression were used for statistical analyses. Results: The mean age of the study group was $32.8{\pm}6.9$ years, 18.3% being doctors and 81.7% nurses. Of the study group, 60.5% stated that they performed self breast-examination, 4.4% underwent HPV testing, 26.3% thought about taking an HPV test, 34.7% of those who are 40 years and over had mammography regularly and 19.5% of those who were married had a Pap smear conducted regularly. Most important causes of not performing the methods for early diagnosis of breast and cervical cancers are "forget and neglect". Conclusions: It was concluded that female doctors and nurses do not pay sufficient attention to screening programs for breast and cervical cancers. The importance of early diagnosis and treatment should be emphasized during the undergraduate education and in-service training programs. Health condition of personnel and their utilization of preventive health care should be followed by occupational physicians.

치과위생사의 구강보건지도 실천분석 -진료실내의 성인환자를 중심으로- (An Analysis of the Practices of Dental Hygienists in Offering Oral Health Education -In Case of Adult Patients Visiting Dental Clinics-)

  • The purpose of this study was to serve as a basis for the planning of oral health education and the development of an oral health-promotion program for patients who visited dental clinics by examining how much dental hygienists offered oral health education to adult patients at dental clinics. A parent group was selected, being made up of 1,600 dental hygienists who registered with the Dental Hygienist Association and worked in Seoul. The questionnaire survey was carried out and an ${\chi}^2-test$ was made using the data collected from 218 subjects to determine how their practice of oral health education was different according to certain general characteristics(the sort of organization for which they worked, age, the term of their service, and the mean number of patients per day). As a result, the following findings were obtained: 1. Thees general characteristics made the following differences to the content of oral health, education: The sort of organization for which the subjects worked made a significant difference in the following tooth brushing instruction (p<0.05), the effect of oral prophylaxis or education about aftereffects (p<0.05), the regular examination of prosthesis (p<0.05), smoking-prohibition education (p<0.05), and the prevention poor-quality fillings (p<0.01). The mean number of patients per day made significant differences to the regular examination of prosthesis (p<0.05) and the prevention poor-quality fillings (p<0.01). But no significant disparity was generated by age or the term of service. 2. The general characteristics made the following differences to education about nutrition and diet counseling: The sort of organization for which the subjects worked had a significant effect just on the importance of a balanced menu (p<0.05). Age made significant differences in advice for vitamin, mineral, protein or other nutrients (p<0.01), and the importance of balanced menu (p<0.001). The term of service made significant differences in the importance of balanced menu (p<0.01), and advice for nutrients including vitamin, mineral or protein (p<0.01). 3. The general characteristics made the following differences to the recommendation and use of oral hygiene aids: The sort of oragnization for which they worked made significant differences only to a gingival massager and water pick (p<0.05). No significant difference was produced by age, the term of service or the mean number of patients per day. 4. The use of educational media for oral health was different according to the general characteristics: The use of pamphlets or booklets significantly varied depending on the organization for which they worked and with the mean number of patients per day (p<0.05). The use of slides or slide projectors was significantly affected by age (p<0.05). But no significant disparity was yielded by the term of service. 5. The general characteristics made the following difference as to whether a continued oral management system was carried out or not: The sort of organization for which they worked had very a significant effect on this result (p<0.001), and no significant disparity was made by age, the term of service or the mean number of patients per day. 6. The place where oral health education was giver differed according to the following general characteristics: The sort of organization for which they worked made very a significant difference as to the use of an examination room's dental unit chair or waiting room (p<0.01), and to the use of an oral health education room or reception counter (p<0.001). The term of service had a significant effect on the use of a counseling room (p<0.01). And the mean number of patients per day made significant differences in the use of a dental unit chair or reception counter (p<0.05), and to the use of an oral health education room or waiting room (p<0.01).

Cross Sectional Survey on Association between Alcohol, Betel-Nut, Cigarette Consumption and Health Promoting Behavior of Industrial Workers in Ghaziabad

  • Background: The work force in industries are at risk of developing unduly high rates of health and behaviour related problems including abuse of alcohol, betel nut and cigarette (alcohol, betel nut and cigarette consumption). This study describes the relationships between alcohol, betel nut and cigarette consumption and health promoting behaviour among industrial workers. Materials and Methods: A cross sectional survey was conducted on workers in various industries of Ghaziabad city with concerned authority permission. A sample size of 732 workers was calculated based on pilot study. Through Simple random sampling 732 workers in 20 to 50 years age group with informed consent were interviewed through structured, pretested, validated questionnaire in vernacular language by one calibrated investigator. Data on socio demography, alcohol, betel nut and cigarette consumption pattern and health behaviour were collected. The association between health promoting behaviour and alcohol, betel nut and cigarette consumption was analysed by Logistic regression and Chi-square test through SPSS 16 at p<0.05 and 95%CI as significant. Results: Total prevalence of alcohol, betel nut and cigarette consumption in study population was 88%. The prevalence of individual alcohol, betel nut and cigarette consumption were 82%, 68% and 79% respectively. Combined alcohol, betel nut and cigarette prevalence in study population was 58%. Alcohol and cigarette users were significantly higher (p<0.001) in 30 to 40 years age group with lower level of education having poor attitude towards health promoting behaviour, poor oral hygiene practices and rare indulgence in regular physical exercise. Conclusions: This study stimulate further research on exploring methods to prevent initiation of health risk behaviour and promote healthy behaviour with cessation help for the current alcohol, betel nut and cigarette users.

도시 저소득층 고령 주민의 구강건강실태 (Oral Health Status of Needy Old Residents in Urban Area)

  • 도시 고령 영세민들의 구강건강을 증진하기 위한 구강보건진료사업을 개발하는 데에 기초자료를 마련할 목적으로, 부산광역시 해운대구 운봉영구임대아파트 단지 주민들 중 44세이상의 인구를 대상으로 구강건강행동과 구강건강실태를 조사분석 검토한 결과를 요약하면 다음과 같다. 1. 1일 잇솔질횟수는 44-54세에서 1,9회, 55-64세에서 1.7회, 65세이상에서 1.7회이었고, 식후 잇솔질률은 44-54세에서 73.7%, 55-64세에서 식후 잇솔질률은 70.6%, 65세이상에서 식후 잇솔질률은 76.5%이었다. 2. 우식경험영구치지수는 44-54세에서 15.5개, 55-64세에서 16.4개, 65세이상에서 26.6개이었다. 3. 우식영구치율은 44-54세에서 23.9%, 55-65세에서 11.6%, 65세이상에서는 11.7%이었고, 상실영구치율은 44-54세에서 55.5%, 55-65세에서 62.8%, 65세이상에서는 77.4%이었으며, 처치영구치율은 44-54세에서 20.0%, 55-65세에서 25.0%, 65세이상에서는 10.9%이었다. 4. 건전영구치수는 44-54세에서 16.4개, 55-64세에서 15.6개, 65세이상에서 5.4개이었고, 현존영구치수는 44-54세에서 23.7개, 55-64세에서 21.6개, 65세이상에서 10.9개이었다. 5. 치면세마필요자율은 44-54세에서 75.0%, 55-64세에서 83.3%, 65세이상에서 76.9%이었으며, 복합치주병치료 필요자율은 44-54세에서 16.7%, 55-64세에서 13.3%, 65세이상에서 15.4%이었다. 6. 치면세마 필요분악률은 44-54세에서 59.3%, 55-64세에서 71.5%, 65세이상에서 71.5%이었으며, 복합치주병치료 필요분악를은 44-54세에서 5.6%, 55-64세에서 4.1%, 65세이상에서 5.7%이었다. 7. 도시 저소득층 고령 주민들의 구강건강향상을 위해서 체계적이고 포괄적인 구강보건진료사업을 개발하여야 한다.

유아의 구강관리에 관한 보호자의 인지도 조사 (A STUDY OF PARENTAL KNOWLEDGE AND ATTITUDE ABOUT INFANT ORAL HEALTH CARE)

  • 전반적인 유아의 구강관리에 관한 보호자의 인지도를 알아 보고, 이를 통한 구강관리에 대한 동기유발을 목적으로 1998년 7월부터 1998년 8월까지 삼성서울병원 소아과에 내원한 아동과 강남구 일원동 일대 유치원 및 어린이집 8곳의 만 3세 이하아동 350명의 보호자를 대상으로 시행한 설문 조사를 통해 다음과 같은 결과를 얻었다. 1. 남성보다는 여성이, 그리고 교육정도가 높을수록 치과 지식정도가 높았으며, 연령이나 경제 수준과는 무관하게 나타났다. 2. 치아의 생성시기에 대해 응답자의 59%가 태아기라고 답하였다. 3. 구강관리 시작시기는 응답자의 76%가 맹출 전이라고 하였으며, 63%가 실제로 만 1세 미만에 구강관리를 시작한다고 하였다. 4. 구강관리 방법에 대해 응답자의 70%가 거즈를 사용한다고 하였다. 5. 응답자의 57%가 만 1세 이후에도 계속 수유한다고 하였다. 6. 우유병 우식증에 대한 인지도는 45%로 나타났다. 7. 유치수복의 필요성에 대하여서는 90%가 필요하다고 하였으며, 31%에서 최초의 치과내원시기를 만 2세 경이라고 하였다. 8. 47%의 보호자가 치아우식이 감염성 질환임을 인식하지 못하였다. 9. 응답자의 33%만이 임신 중 치과 검진을 받았다고 하였고, 소아과나 산부인과에서 구강관리에 관한 지도를 받은 것은 11% 뿐이었다. 10. 주로 육아서적에서 유아의 구강관리에 관한 정보를 얻는 것으로 나타났다. 11. 보호자들의 전반적인 치과지식과 실천정도는 유아들에게 적절한 구강관리를 제공하기에 충분치 않은 것으로 나타났다.

A comparative study of the prevalence of Helicobacter pylori in the oral biofilms of a group of dental and non-dental undergraduates from Sri Lanka

  • Dental health care workers (DHCW) are at a risk of occupational exposure to Helicobacter pylori from the aerosolized oral biofilms and saliva of patients. We designed this study to investigate the prevalence of H. pylori in the oral biofilms of a group of dental and non-dental undergraduates from Sri Lanka. After obtaining informed consent, oral biofilms were collected from 38 dental undergraduates (19 males and 19 females) undergoing clinical training and 33 non-dental undergraduates (14 males and 19 females). The participants were in the age range of 22-27 years and had healthy periodontium. Total DNA from the oral biofilms were extracted, and H. pylori DNA was detected using polymerase chain reaction (PCR) amplification of 16S rRNA gene of H. pylori using JW22-JW23 primers, and the results were confirmed using PCR amplification of H. pylori-urease specific HPU1-HPU2 primers. Out of 71 participants, 11 (28.95%) dental and 3 (9.09%) non-dental undergraduates had H. pylori in their oral biofilms indicating an overall prevalence rate of 19.72% (14/71). Thus, the prevalence of H. pylori in oral biofilms was significantly higher in dental undergraduates than in non-dental undergraduates (p < 0.05). An odds ratio of 4.07 indicated that dental undergraduates were four times more likely to harbor H. pylori in their oral biofilms than non-dental undergraduates. Foregoing data support the fact that there may be greater occupational risk of exposure to H. pylori for dental undergraduates during clinical training than that for non-dental undergraduates, warranting meticulous infection control practices during clinical dentistry.
