합곡(合谷) 침자극(鍼刺戟)이 동통억제(疼痛抑制)에 미치는 영향(影響) (Effects of Acupuncture at HapGuk$(LI_4)$ on Pain in Rats)

  • The purpose of this study was finding the pain inhibitory effect of acupuncture at $LI_4$. The pain at dentes incisivi was evoked by noxious electric stimulation and EMG changes based on the interval were measured. To do this, the opioid antagonist was administered intraperitoneally. Following are some of results. 1. The pain inhibitory effect of acupuncture at $LI_4$ was 94.35%, 84.56%, 57.62%, 54.40%, at 10, 20, 30 and 40 minutes respectively. All the data were calculated based on 100% at 0 minute. The effect was very significant at 40 minutes. 2. The continuous effect of acupuncture at $LI_4$ after taking out the needle was 58.58%, 57.62%, 66.22%, 73.18%, 83.70%, 92.68%, at 50, 50, 70, 80, 90 and 100 minutes respectively. The overall continuous effect was shown its maximum 20 minutes after taking out the needle and reached to the initial value at 60 minutes. 3. The pain inhibitory effect following the naloxone administration and acupuncture application at $LI_4$ was 95.96%, 96.04%, 94.86%, 94.92% at 10, 20, 30 and 40 minutes respectively. All values showed similar tendency to the initial data. 4. The continuous effect of acupuncture at $LI_4$ after taking out the needle which was preceded by the naloxone administration was 94.48%, 96.02%, 96.02%, 98.00%, 98.46%, 97.18% at 50, 60, 70, 80, 90 and 100 minutes respectively. This trend was not a significant fluctuation. It was concluded that effect of acupuncture at $LI_4$ was shown in conjunction with secretion of analgesic substance. Therefore it is implied that the acupuncture application will play a major role in treating many diseases with more revelation of scientific acupuncture mechanism. Further studies of acupuncture manipulation are needed in the future based on our study.

  • The purpose of this study was finding the pain inhibitory effect of acupuncture based on rataining time at LI -4. The pain at dentes incisor was evoked by noxious electric stimulation and digastric electromyogram(dEMG) changes based on time interval were measured. To do this, the opioid antagonist was administered intraperitoneally and four groups were made for convenience. Without naloxone, dEMG was changed by either retaining the needle for 40 minutes (Group I) or by lifting and thrusting the needle (Group II). With naloxone administration, dEMG was changed by either retaining the needle for 40 minutes (Group III) or by lifting and thrusting the needle (Group IV). The results are as following 1. The pain inhibitory effect of acupuncture at LI -4 was expressed best in Group I. 2. The pain inhibitory effect was somewhat expressed in Group II but the effect was smaller than Group I. 3 .In Groups III and IV, the pain inhibitory effect was not expressed. The overall result should be the foundation for the further studies to figure out the underlying mechanism of acupuncture. In addition, it is assumed that the results will be useful for optimal retaining time of acupucture for its maximal effect.

족삼리(足三里) 침자(鍼刺)가 동통억제(疼痛抑制)에 미치는 영향(影響) (Effects of Acupuncture at Chok-samni(ST36) on Pain in Rats)

  • The purpose of this study was finding the pain inhibitory effect of acupuncture at ST-36. The pain at dentes incisor was evoked by noxious electric stimulation and EMG changes based on time interval were measured. To do this, the opioid antagonist was administered intraperitoneally. Followings are some of the results. 1. The pain inhibitory effect of acupuncture at ST-36 was 93.82%, 87.00%, 75.30%, 69.76% at 10 20, 30 and 40 minutes respectively. All the data were calculated based on 100% at 0 minute. The effect was very significant at 40 minutes. 2. The continuous effect of acupuncture at ST-36 after taking out the needle was 77.46%, 79.66%, 87.60%, 91.50%, 95.14%, 99.48% at 50, 60, 70, 80, 90 and 100 minutes respectively. The overall continuous effect was shown its maximum 20 minutes after taking out the needle and reached to the initial value at 60 minutes. 3. The pain inhibitory effect following the naloxone administration and acupuncture application ST-36 was 93.44%, 94.58%, 90.80%, 88.04% at 10, 20, 30 and 40 minutes respectively. All values showed similar tendency to the initial data. 4. The continuous effect of acupuncture at ST-36 after taking out the needle which was preceded by the naloxone administration was 91.26%, 91.90%, 92.06%, 93.66%, 94.12%, 93.50% at 50, 60, 70, 80, 90 and 100 minutes respectively. This trend was not a significant fluctuation. It Was concluded that effect of acupuncture at ST-36 was shown in conjunction with secretion of analgesic substance. Therefore it is implied that the acupuncture application will play a major role in treating many diseases with more revelation of scientific acupuncture mechanism. 금주사액약침(金注射液藥鍼)의 Adjuvant 관절염 유발 흰쥐의 진통효과(鎭痛效果) 및 그 기전(機轉)에 관한 연구 (The Experimental Study on the Analgesic Effect and Mechanism of Gold Injection Aqua-acupuncture to Adjuvant Induced Arthritis in Rats)

  • Objective: In this study, the analgesic effect of gold injection aqua-acupuncture on CFA induced arthritis in rats was inverstigated. Furthermore, to determine the antinociceptive mechanism of gold injection aqua-acupuncture, naloxone and yohimbine were administrated intraperitoneally 15min prior to the gold injection aqua-acupuncture. Methods: The experimental groups were divided into 6 groups. Control group, NSI, AMI, AMI+SL : AMI+Nx, AMI+Yb. Thereafter we measured the chages of TFL between control and experimental groups. Results: 1. The tail flick latency induced by CFA was significantly increased on AMI as compared with the NSI group. 2. Effect of sodium aurothiomalate aqua-acupuncture in TFL on CFA was decreased after application of naloxone. The anti-nociceptive effect of AMI was completely antagonized by naloxone(an opioid receptor antagonist). 3. Effect of sodium aurothiomalate aqua-acupuncture in TFL on CFA was decreased after application of yohimbine. The anti-nociceptive effect of AMI was completely antagonized by yohimbine(${\alpha}2$-adrenoceptor antagonist). Conclusions: According to the results, gold injection aqua-acupuncture has significant analgesic effects on adjuvant induced arthritis in rats and the effect of anti-nociception was mediated by opiodergic system and ${\alpha}2$-adrenergic system.

내관 혈 지압이 통증 자가 조절(PCA)을 하는 부인과 수술환자의 오심과 구토 억제 및 환자만족도에 미치는 효과 (Effects of Nei-Guan Acupressure on Nausea, Vomiting and Level of Satisfaction for Gynecological Surgery Patients Who Are Using a Patient-Controlled Analgesia)

  • Purpose: This study was done to examine effects of Nei-Guan acupressure on nausea, vomiting and level of satisfaction for gynecological surgery patients who were using a patient-controlled analgesia (PCA). Methods: For this study, 51 patients were assigned to one of three groups, a control group (17 patients), experimental group 1 (finger acupressure group) (17 patients), and experimental group 2 (relief band group) (17 patients). The data were collected for 24 hr in the recovery room of a university hospital located in Seoul. The 6 hr-intervals including the time of leaving the recovery room were taken into consideration. Results: The occurrence of nausea between the experimental group with Nei-Guan acupressure treatment and the control group was different. However, there was no difference in nausea and vomiting control or level of patient satisfaction between the finger acupressure group and the relief band group. Conclusion: Nei-Guan acupressure is recommended for nursing practice as a way for alleviating the opioid-induced nausea and accelerating the recovery of patients who are using PCA after surgery.

Analgesic activity of three Channa spp. fish extracts

  • In the present study, three Malaysian Channa spp. fish Channa striatus, Channa micropeltes and Channa lucius were assessed for their analgesic activity. Distilled water and mixture of chloroform: methanol were used for extraction. The analgesic or antinociceptive activity was investigated by abdominal writhing and hot plate test. The water extract of Channa striatus and the chloroform: methanol extract Channa lucius produced potent antinociceptive effect when assessed with the abdominal writhing test. The activity was compared to morphine where the activity of the extracts was less potent than the opioid. In the hote plate test, water extract of Channa striatus revealed significant activity and chloroform:methanol extract of Channa micropeltes had moderate activity. However, these activities were statistically lower than morphine. Collectively, this study also showed that Channa striatus extract was more potent analgesic agent when compared to the other closely related snakehead Channa micropeltes and Channa lucius.

Vitamin E Potentiates the Anti-nociceptive Effects by Intraperitoneal Administration of Lidocaine in Rats

  • The present study was to evaluate effects of vitamin E on intravenous administration of lidocaine-induced antinociception. Experiments were carried out using male Sprague-Dawley rats. Orofacial formalin-induced nociceptive behavioral responses were used as the orofacial animal pain model. Subcutaneous injection of formalin produced significant nociceptive scratching behavior. Intraperitoneal injection of 5 and 10 mg/kg of lidocaine attenuated formalin-induced nociceptive behavior in the 2nd phase, compared to the vehicle-treated group. Intraperitoneal injection of 1 g/kg of vitamin E also attenuated the formalin-induced nociceptive behavior in the 2nd phase, compared to the vehicle-treated group. However, low dose of vitamin E (0.5 g/kg) did not affect the nociceptive behavioral responses produced by subcutaneous injection of formalin. The present study also investigated effects of intraperitoneal injection of both vitamin E and lidocaine on orofacial formalin-induced behavioral responses. Vehicle treatment affected neither formalin-induced behavioral responses nor lidocaine-induced antinociceptive effects. However, intraperitoneal injection of 0.5 g/kg of vitamin E enhanced the lidocaine-induced antinociceptive effects in the 2nd phase compared to the vehicle-treated group. Intraperitoneal injection of naloxone, an opioid receptor antagonist, did not affect antinociception produced by intraperitoneal injections of both vitamin E and lidocaine. These results suggest that treatment with vitamin E enhances the systemic treatment with lidocaine-induced antinociception and reduces side effects when systemically treated with lidocaine. Therefore, the combined treatment with vitamin E and lidocaine is a potential therapeutic for chronic orofacial pain.

일개 병원의 수술실 약제관련 업무 적정화 방안연구: 설문조사를 중심으로 (Evaluating Appropriateness of Medication Use in the Operating Rooms of a Tertiary Hospital: Based on Survey)

  • Background: Since the use of opioid analgesics is frequent in operation rooms (OR), the risk of medication error is high; however the use of medication in the OR has been operating independently with the hospital pharmacy. Therefore, the assessment on management of medication use in operation and the pharmacist's role is needed. Methods: We conducted the literature review and survey from anesthesiologists, operating nurses at Chung-Ang Hospital on management of medication for operation use, awareness on need for medication management efficiency, need for satellite pharmacy in the operating room and its effect. Results: 56% of medical staffs responded that management of medication in the operating room is efficient; however, 82.6% responded that they felt the inconvenience in medication delivery to the OR when additional prescription was ordered. 51.5% also responded that extra time was required for management of narcotics and inventory/record keeping. 80% agreed that there could be lost costs due to prescription missed. Medical staffs responded improving the drug management system could increase the OR efficiency (87%), and eventually bring the increase in hospital revenue (80.4%). Those who responded that implementation of OR satellite pharmacy was needed include physicians (84.6%), nurses (63.6%), and also responded that it'd bring more profit to the hospital by increasing the efficiency in OR (60.9%). Conclusion: For efficient management of medications, implementation of OR satellite pharmacy would lead to improved drug management and increased efficiency in OR and reduced cost and improved patient care.

외과계 중환자실의 섬망 발생 위험요인 조사연구 (Risk Factors related to Delirium Development in Patients in Surgical Intensive Care Unit)

  • Purpose: The purpose of this study was to examine the frequency and the course of delirium, and identify risk factors for the development of delirium in surgical intensive care unit (SICU). Methods: Subjects older than 19 years admitted to the SICU were recruited. After informed written consent, enrolled subjects had baseline cognitive and functional assessments. Subjects were assessed daily for delirium using the Confusion Assessment Method-ICU (CAM-ICU). Results: During the study period, 110 patients were enrolled. The overall incidence of delirium was 20% (22/110). The average time to onset of delirium was 3.04(${\pm}1.25$) days. Several variables were associated with an increased risk of delirium including older age (p<.05), higher admission APACHE II score (p<.001), use of opioid and analgesics (p<.01), using physical restraints (p<.001), and intraoperative hypotension (p<.05). In a multivariate logistic regression model, using physical restraints (p<.001), intraoperative hypotension (p<.05), and older age (p<.05) remained significant predictors of the delirium development. Conclusion: Using physical restraints, intraoperative hypotension and older age was strongly associated with development of delirium in the SICU. Prevention measures need to focus on identifying patients at higher risk for delirium development.

슬관절경 수술 후 관절강 내 주입한 Bupivacaine 및 Morphine이 통증에 미치는 영향 (Postoperative Analgesic Effect of Intraarticular Bupivacaine or Morphine After Arthroscopic Knee Surgery)

  • 전신 흡입 마취로 슬관절경 수술을 받은 건강한 환자 60명을 대상으로 슬관절경내 Morphine 3 mg을 주입한 군(20명)과 0.25% Bupivacaine 20 ml을 주입한 군(20명)과 약제를 주입하지 않은 대조군(20명)의 통증 정도를 술후 1, 2, 4, 6, 12, 24시간 동안 Visual Analogue Pain Scale을 이용하여 비교 평가하여 다음과 같은 결론을 얻었다. 1) 슬관절강내 0.25% Bupivacaine 20 ml을 주입한 군에서 술후 첫 1, 2시간 동안 통계학적으로 의의있는 통증 점수를 나타내었다. 2) 슬관절강내 Morphine 3 mg을 주입한 군에서는 술후 4시간부터 통계학적으로 의의있는 통증 점수가 나타난 뒤 실험이 끝날 때까지 지속적인 진통효과를 나타내었다. 3) 구역, 구토, 소양증, 뇨정체, 호흡억제 등의 전신적인 부작용은 나타나지 않았다. 이상과 같이 소량의 Morphine을 관절강내 주입함으로써 술후 통증을 감소시켜 진통제의 사용량을 줄일 수 있었다.
