Differences in Utilization of Health Care Services by the Type of Disability (장애 유형별 의료서비스 이용의 차이)

  • The disabled population is a vulnerable group, having very complex medical conditions, but little is known about differences in the level of access by type of disability. This study was performed to investigate the differences of health care utilization by the type of disability. The database was constructed from registry of the disabled and health insurance and medical aid claims data submitted to the Korea Health Insurance Cooperation during in the year 2003. The disability classified three groups according to the Disabled Welfare Act; physically disability with external dysfunction, physically disability with organic disease, and mentally disability. There were huge differences in health care utilization by the type of disability. For the inpatient care, those with a mental disability were more likely to utilize health care services in terms of average visit number of medical facilities and visit days per case, but the treatment amount per case was the highest in physically disabled with organic disease. For the outpatient care, those who the physically disabled with organic disease were more likely to utilize health care services in terms of average visit number of medical facilities, treatment amount per case, and the treatment days per case. Also, those who physically disabled with organic disease were more likely to utilize general hospital for both inpatient and outpatient care, and spent more out-of-pocket expenditure. As the number of persons with disabilities rises, the need to consider new approaches to protecting their health grows increasingly. Especially, Korean health care system should be refined to be more responsive to the needs of the type of disability.

Relationship between Occupational Accident Service, Pain, and Life Satisfaction of Industrial Accident Compensation Insurance Beneficiaries (산업재해보험 급여수급자의 통증과 산업재해서비스 및 생활만족간의 관계)

  • This study aims to provide basic data on socioeconomic and social activities from the point of view that disability or disease accompanying the suffering of industrial accident subjects is directly related to individual socioeconomic activity and is a major variable affecting current health and daily life satisfaction. The purpose of this study is to investigate the relationship between pain and life satisfaction and to suggest basic data for economic and psychological rehabilitation of industrial accident insurance beneficiaries by linking factors affecting life satisfaction with industrial accident services. As a result of the study, it was found that workers' compensation insurance benefit recipients had an effect on their daily life satisfaction after becoming an industrial accident patient due to industrial accident pain. There was a positive(+) relationship between rehabilitation services and medical counseling services. Occupational accident pain lowers self-esteem, which is a characteristic of an individual, and even if current health improves, satisfaction with daily life decreases. The importance of industrial accident-related services can be seen in that the satisfaction of daily life is increased through rehabilitation services and medical consultation services. In addition, it was indirectly confirmed that satisfaction with rehabilitation services and health can be improved as a result of continuing attempts to improve self-esteem suitable for patients in various problems and situations after an industrial accident.

A Survey on the voice symptoms and vocal-health service related experience of occupational voice users (직업적 음성사용자의 음성증상 및 '음성건강' 관련 서비스 인지도 조사)

  • This survey was to identify voice symptoms and vocal-health service related experiences of occupational voice users(teachers, telemarketers, speech therapists). The 91.8% of teachers, 97.9% of telemarketers, 86% of speech therapists surveyed reported more than one voice symptom. The symptoms were classified as 9 categories(running a temperature, getting dry, dry and cough, pain, phlegm, tingled, hoarseness, cracks, swollen) and the most frequently reported from 3 groups was 'getting dry'. The 85.7% of teachers, 87.8% of telemarketers, 66% of therapists surveyed had no experience of vocal-health related services. The 19.6%, 19.9%, and 72% of each group reported they have heard both of 'voice/speech therapist'. The 36.8% of teachers and 43.6% of telemarketers answered they don't know how to use their voice efficiently and 45.3% of the teachers, 43.6% of the telemarketers, 28% of the therapists surveyed asked professional help for their voice. The result showed that most of the occupational voice users surveyed experienced voice symptoms but rarely knew professional vocal-health related services.

Practice of Health Managers and Workers according to Guidance of Work Environment Management of Specialized Health Management Institution (보건관리전문기관의 작업환경관리 지도에 따른 일부 지역 사업장 보건업무담당자와 근로자의 실천도)

  • Objectives: This study aimed to provide basic data for the improvement of the guidance services for the management of the workplace environment of the specialized health management institution, by making inquiries on the perception and extent of practice by health managers and workers in the workplace. Methods: Workplaces with officially noted environmental hazards of noise, organic compounds and/or dust-metals were selected in a South Korean metropolitan city in 2015. The workplace health managers(hereinafter referred to as 'manager') and workers at 97 workplace were interviewed. Managers are those who are in charge of health management at the corresponding workplace and the workers were subjected to in-depth interviews by sampling one worker for each of the workplace. Results: The majority of the managers acknowledged the guidance services of the specialized health management institution affirmatively. Regarding the extent of practice in accordance with the guidance on engineering improvement, only 23 managers(37.1%) responded. With respect to education, 40 managers(41.2%) responded that it was implemented along with 35 workers(36.1%) who responded that they received education, showing no significant difference between manager and worker. Regarding the actual wearing of the protective equipment, however, 83 of the manager(85.6%) insisted that workers wore the protective devices while only 44 workers(49.5%) responded that they actually wore the devices, thereby illustrating a significant difference between managers and workers. Conclusions: These results suggest the urgent necessity of improvement in the mechanism of guidance services of the specialized health management institution for the workplace environment management.

  • The Korea Post plays a public role in providing postal services such as mail, savings, and insurance to the public. Although postal machines are becoming automated, workers are still exposed to various industrial accidents caused by being caught in the mail, and musculoskeletal disorders. Therefore, occupational safety and health activities are needed to prevent serious accidents. In this study, the level of safety and health consciousness was analyzed for employees working in the postal logistics center The tasks such as regulations and procedures, organization composition, and safety and health education method improvement, were classified into five items and proposed, and efficient industrial accident prevention was presented.

Health Promoting Lifestyles of Korean Employees (근로자의 건강증진 생활양식 실천에 관한 연구)

  • Introduction : In Korea, national attention to the workplace health promotion programs (HPP) for employees began in early 1990s. Governmental supports for the HPP and education programs have given to the employees. The purpose of this study is to find the performance degree of employees' health promoting lifestyle(HPL). Subjects and Methods : For this study, 615 employees who attended governmental educations were selected as research samples. The tool for measuring HPL used in this study was developed for Korean by In-Sook Park in 1995. It is composed of 4-point scaled 60 items and divided into 11 subcategories. The data were collected by self reporting questionnaires from June to December in 1997. Those data were analyzed percentile, mean, standard deviation, ANOVA, t-test with SAS program. Major findings are as follows; 1. The total mean score of the employees' HPL performance was 2.66. The average scores of 'harmonious relationships' category were the highest as 3.11, whereas the one of 'professional health maintenance' were the lowest as 2.02. The remains were 'sanitary life'(2.90), 'emotional support(2.85), 'regular diet'(2.84), 'self achievement'(2.78), 'healthy diet'(2.56), 'rest and sleep'(2.56), 'exercise and activity'(2.54), 'diet control'(2.53), 'self-control'(2.52). 2. The factors affecting HLP were category of industries and sex, age, marital status, education level, major, educational experience of health promotion, among personal characteristics. The employees of service industries, female, older age, married, nurse, educated for health promotion, graduated from junior college performed HLP more than the others. 3. The participation rates of employees for HPP were 12.4%, because of limited time and facilities and equipments. Recommendations; 1. The regulation for performing the health promotion programs in the industries is essential for activating industrial health promoting movement. 2. More governmental supports for educations and services for health promotion programs in the industries are needed. 3. For behavioral changes of the employees, the contents of educations have to consist of exercise and activity, rest and sleep, diet and smoking habits. 4. The evaluating studies for ready made health promotion programs in the industries are expected.

The Difference of the Nurses' Perception on the Importance and Performance of Rehabilitation Nursing Service (산업장 재활간호서비스의 중요도에 대한 간호사의 지각과 수행의 차이)

  • Purpose: The purpose of this study was to compare the rehabilitation nursing services what nurses recognize to be important and what they do in practice of the industrial workers. Method: The subjects were 150 nurses who were working at the industrial hospital. The questionnaire was made up with 43 items about the perception on importance and performance of rehabilitation services. Result: The highest services in the perception on importance and performance subscale were physical and therapeutic services. The highest difference between perception on importance and performance was the social and occupational services. The perception on importance of rehabilitation services had positive correlation with performance. The group take care of general patients had a higher level of perception on importance than the other group, and level of performance had significant difference with age and position. Conclusion: We need to develop the strategy for promoting the performance of services that recognize it will be important services, but not to do. Further study on system for promoting the nurse practitioner who responsible for rehabilitation nursing.

The Need and Perception about Volunteer of Student from Department of Health Care for the Old in the Community : Focus on Ulsan City (보건계열 대학생의 봉사활동에 대한 지역사회 노인의 욕구 및 인식차이 조사 : 울산광역시 중심으로)

  • Objective : The objective of this research tried to know the differences about the perception of volunteering between the elderly group of seniors with experience of college's Department of Health Care Services and the elderly group of seniors without it. Method : We researched 79 people aged using the facilities for the elderly and the facilities for welfare for the aged from August 2010 to November 2010. Result : As a result of perception of the old according to whether to experience in services of students in the college of department of health care or not, the old (89%) of the whole person was positive. The perception of the volunteer of student in the department of health care was not a statistical difference between the old group with experience in volunteer service and the old group without it. The field that two groups want to be serviced was the highest in the department of rehabilitation. Conclusion : It is necessary for the university with the functions of social service to provide health and medical service to the old in the community through systematic training to meet their needs.

Association Between Burnout and Intention to Emigrate in Peruvian health-care Workers

  • Background: Emigration of health-care workers is a problem within global health systems which affects many countries, including Peru. Several factors have caused health-care workers to emigrate, including burnout syndrome (BS). This study aims to identify the association between BS and its dimensions with the intention of physicians and nurses to emigrate from Peru in 2014. Methods: A cross-sectional study, based on a secondary analysis of the National Survey of Health Users (ENSUSALUD - 2014) was conducted. Sampling was probabilistic, considering the 24 departments of Peru. We include the questionnaire for physicians and nurses, accounting for 5062 workers. BS was measured by the Maslach Burnout Inventory-Human Services Survey. Adjusted odds ratio (OR) was calculated using multiple logistic regression. Results: Of the study population, 44.1% were physicians, 37.7% males, and 23.1% were working in Lima. It was found that 2.8% [95% confidence interval (CI): 2.19-3.45] of health-care workers had BS. The overall prevalence of intention to emigrate among health-care workers was 7.4% (95% CI: 6.36-8.40). Association was found between BS and intention to emigrate in Peruvian health-care workers (OR = 2.15; 95% CI: 1.05-4.40). Emotional exhaustion was the BS dimension most associated with intention to emigrate (OR = 1.80; 95% CI: 1.16-2.78). Conclusion: Physicians and nurses from Peru who suffered from BS were more likely to have intention to emigrate. Policies should be established to reduce BS as a strategy to control "brain drain" from health-care workers of Peru.

Planning of Nuclear Medicine in Turkey: Current Status and Future Perspectives

  • Background and Purpose: An analysis of the current nuclear medicine (NM) status and future demand in Turkey in line with the international benchmarks was conducted to establish a comprehensive baseline reference. Methods: Data from all NM centers on major equipment and manpower in Turkey were collected through a survey and cross-checked with the primary research and governmental data. Data regarding manpower currently working were obtained from the relevant academic centers and occupational societies. Results: The current numbers of NM laboratories, NM specialists, gamma cameras, PET/CT scanners, radioiodine treatment units for thyroid cancer are 217, 474, 287, 75 and 39, respectively. There was personnel and equipment need underestimated in the field compared to developed countries. Equipment insufficiency was more significant in the Ministry of Health (MoH) hospitals. These gaps should be eliminated with strategic planning of equipment and NM laboratories. Currently, the number of the PET/CT devices is at the level of the developed countries. The number of specialists in the field should reach the expected goal in 2023. By 2023, Turkey will need around 820 NM specialists, 498 gamma cameras and 99 PET/CT devices. In addition, further studies should be made regarding other related staff, particularly for health physicians, radiopharmacists and NM technicians. Conclusion: There is an insufficiency of personnel and equipment in Turkey's NM field. Comprehensive strategic planning is required to allocate limited resources and the purchase of the equipment and employment policies should be structured as part of "National Special Feature Requiring Health Service Plan".
