Comparison of immediate complete denture, tooth and implant-supported overdenture on vertical dimension and muscle activity

  • PURPOSE. To compare the changes in the occlusal vertical dimension, activity of masseter muscles and biting force after insertion of immediate denture constructed with conventional, tooth-supported and Implant-supported immediate mandibular complete denture. MATERIALS AND METHODS. Patients were selected and treatment was carried out with all the three different concepts i.e, immediate denture constructed with conventional (Group A), tooth-supported (Group B) and Implant-supported (Group C) immediate mandibular complete dentures. Parameters of evaluation and comparison were occlusal vertical dimension measured by radiograph (at three different time intervals), Masseter muscle electromyographic (EMG) measurement by EMG analysis (at three different positions of jaws) and bite force measured by force transducer (at two different time intervals). The obtained data were statistically analyzed by using ANOVA-F test at 5% level of significance. If the F test was significant, Least Significant Difference test was performed to test further significant differences between variables. RESULTS. Comparison between mean differences in occlusal vertical dimension for tested groups showed that it was only statistically significant at 1 year after immediate dentures insertion. Comparison between mean differences in wavelet packet coefficients of the electromyographic signals of masseter muscles for tested groups was not significant at rest position, but significant at initial contact position and maximum voluntary clench position. Comparison between mean differences in maximum biting force for tested groups was not statistically significant at 5% level of significance. CONCLUSION. Immediate complete overdentures whether tooth or implant supported prosthesis is recommended than totally mucosal supported prosthesis.

Three-Dimensional Finite element analysis of Canine and Carnassial for Observation of Dog Bite forces (개(犬)의 교합력 관찰을 위한 견치와 열육치의 3차원 유한요소 분석)

  • Purpose: This study is for the prosthesis of dogs. Observe the occlusal relation between the dog's canine and carnassial teeth. The strength and the direction of the occlusal by 3D FEM analysis. Methods: The mandibular canine and carnassial of dogs were tested. The dog's skull was contact point confirmed by dental CAD. The skull of the dog was 3D modeled by CT. The 3D model was analyzed by ABAQUS. Opening and closing movement has been a force of 100N, 200N, 300N, 500N, 1000N, 1,500N. The peak von Mises stress distribution was confirmed. Results: As occlusal force increased, stress appeared to 1.34 MPa, 3.32 MPa, 5.00 MPa, 6.19 MPa, 5.58 MPa, 5.47 MPa in left canine. and Stress was seen at 2.10 MPa, 3.08 MPa, 3.89 MPa, 5.50 MPa, 7.04 MPa, 7.18 MPa in the right canine. Stress appeared at 2.41 MPa, 3.53 MPa, 5.15 MPa, 7.28 MPa, 31.26 MPa, 67.22 MPa in the left carnassial. and Stress was seen at 1.57 MPa, 2.96 MPa, 3.76 MPa, 6.01 MPa, 20.94 MPa, 64.38 MPa in the right carnassial. Conclusion: Peak von Mises stress values were found at the peak of the canine, the buccal of the central cusp of the carnassial, and the occlusal surface of the distal cusp.

  • Endosseous implants have been used to provide anchorage control in orthodontic treatment without the need for special patient cooperation. However these implants have limitation like space requirement, cost, equipments. Recently titanium micro-implant for orthodontic anchorage was introduced. Micro-implants are small enough to place in any area of the alveolar bone, easy to implant and remove, and inexpensive. In addition, orthodontic force application can begin almost immediately after implantation. The mandibular first, maxillary first, mandibula second, and maxillary second molars were the four most commonly missing teeth in adult sample. In case of posterior molar teeth missing, deflective contacts in any position, over time, has produced pathologic change of occlusal scheme because of extrusion of opposing teeth. This case had interocclusal space deficiency by mandibular right molars missing over time. The micro-implants had been used for intrusion of maxillary right molars for interocclusal space. The micro-implant would be absolute anchorage for orthodontic movement. Therefore, the micro-implant would be effective method for correction of occlusal plane.

  • Occlusal loading is considered as the main factor of noncarious cervical lesions. The goal of this study is to identify stress distribution using three dimensional finite element analysis, when occlusal loading is applied on the cervical lesion of human tooth. A finite element model was constructed from micro-CT image and three kinds of static force(500 N) were assumed. In all cases stress concentrates on the same area in the cement-enamel junction. This finding is consistent with published experimental results.

  • Attrition is the loss of tooth hard tissue due to contact between teeth, and in severe cases, dentin is exposed, accompanied by selective corrosion and excessive wear of teeth, which is called cupping. If these lesions are left untreated, the size of the lesion gradually increases, breaking the unsupported enamel, resulting in a decrease in aesthetics and chewing function. In this case report, patients with cupping and enamel fracture due to severe attrition were directly restored using a resin with soft properties containing organic fillers. In the follow-up observation six years later, most of the filling of the occlusal surface was eliminated, but the filling on the buccal surfaces remained relatively intact, and it was confirmed that this type of resin was suitable for the area where the occlusal force was relatively weak rather than the area where the occlusal force was greatly applied.

Evaluation of relationship between cognitive function and occlusal status in elderly individuals using the T-scan III® system (T-scan III® system을 이용한 일부지역 노인의 교합양상과 인지기능의 관련성)

  • Objectives: The aim of this study was to investigate the relationship between cognitive function and occlusal status in elderly individuals. Methods: A total of 162 individuals aged 65 years and older, who attended the senior citizen center in Daegu city, were included after consent for participation in the study was obtained. The Korean version of the Mini-Mental State Examination (MMSE-K) was used to evaluate the level of cognitive function. Occlusal status was determined using the T-scan $III^{(R)}$ system. All collected data were analyzed by ${\chi}^2$ test, t-test, one way ANOVA, and linear regression analysis using SPSS version 23.0 for Windows. Results: There was a significant relationship between the cognitive function and the use of dentures. Individuals with posterior occlusal status had higher MMSE-K scores than those with anterior occlusal status. There was a positive correlation between the cognitive function and posterior occlusal force. Conclusions: These results suggest that occlusal status was associated with cognitive function in the elderly individuals. Active national policies to improve occlusal condition in the elderly population are needed.

A STUDY OF HOLOGRAPHIC INTERFEROMETRY ON THE INITIAL REACTION OF MAXILLOFACIAL COMPLEX TO THE MAXILLARY PROTRACTION USING THE ANTENNA TYPE MODIFIED PROTRACTION HEAD GEAR (Modified Protraction Headgear를 이용한 상악골 전방 견인시 악안면골의 초기반응에 관한 Holographic Interferometry 연구)

  • The majority of the commonly used protraction headgears for the protraction of small and/or retropositioned maxilla not allow a change in the point of force application or direction of the force delivery to attain predictable results because of the position of the upper and lower lips to avoid discomfort to the patient. The purpose of this study was to investigate the initial reaction of maxillofacial complex according to the change of force variables such as direction and point of force application with designing an antenna type-modified protraction head gear. A macerated human skull with well aligned upper teeth was used to experimental model and the investigation was done by double exposure holographic interferometry. Fringe patterns of each protraction conditions were compared and analized. The results were as follows. (Frontal view) 1. The Counterclockwise rotation of the maxilla was showed by parallel protraction to occlusal plane and the fringe was decreased in number as higher point of force application. 2. Generally, the number of fringe was increased in 500gm of protraction force than in 300gm. 3. When apply the protraction force to the maxilla with rapid palatal expansion, the direction of fringe patterns was differed from the protraction without expansion. 4. In most of cases, the counterclockwise rotation was decreased in case of the direction of the force is $20^{\circ}$ downward to occlusal plane compared to the parallel direction. 5. At the point of force application is 15mm above and the direction of force is 20 downward to occlusal plane , the translation of the maxillary complex was showed. (Lateral view) 6. The direction of fringe patterns of the facial bones were differed each other by the sutures, and showed almost parallel when apply the 300gm and 500gm of protraction force. 7. In case of rapid palatal expansion with protraction of the maxilla, the fringe patterns between the maxillary area and the area from the posterior of the maxillary first molar to the pterygomaxillary fissure were differed. In case without rapid palatal expansion, the changes of direction and point of the force application did not affect to the direction and the number of the fringe patterns.

EFFECT OF MANDIBULAR SET BACK SURGERY ON VOLUMETRIC CHANGE AND BITE FORCE OF MASSETER MUSCLE (하악전돌증환자에서 악교정수술후 저작근의 부피 및 교합력의 변화)

  • Purpose: The purpose of our study was to evaluate the volume of pre- and post operative masseter muscle and bite force in mandibular prognathic patients treated with SSRO with the use of the 3D CT imaging technique and occlusal force meter. Materials and methods: The study group consisted of 12 patients with mandibular prognathism (5 males and 7 females) who underwent mandibular setback surgery (BSSRO) in the Department of Oral and Maxillofacial Surgery, Samsung medical center. Bite force was measured at pre op, post op 3, 6 and 12 months by occlusal force meter(GM10, Nagano Keiki, Japan) The preoperative CT examination of subjects was performed between one month prior to operation and one year after to operation. And muscle volume was measured. Result: As compared to preoperative measurements at 1 year postoperatively the masseter & internal pterygoid muscle volume were diminished (p<0.05) The bite force steadily recovered, so at postoperatively 6 months reached the preoperative level. And at 1 year after operation, the maximum bite force was significantly greater than preoperative levels. No significant correlation was presented between masseter muscle and bite force (p>0.05), internal pterygoid muscle and bite force (p>0.05). Conclusion: In this study, the results showed that volume and bite force of the masticatory muscles decreased significantly immediate after orthognathic surgery for mandibular set-back. However, reduction of maximum bite force disappears within 6 months after surgery.

EFECTS OF LATERAL GUIDANCE AND CHEWING PATTERN ON MASTICATORY MUSCLE ACTIVITY AND OCCLUSAL CONTACT (측방유도형태 및 저작습관이 저작근 활성도 및 교합접촉에 미치는 영향)

  • This study was performed to investigate the influence of lateral guidance pattern and chewing pattern on masticatory muscle activity and occlusal contact pattern. Twenty-five patients with temporomandibular disorders and thirty students without temporomandibular disorders were selected for this study. Electromyographic examination and occlusal contact examination were performed simultaneously with Bio-Pak System (Bioresearch Inc.,U.S.A.) and T-Scan System(Tekscan Co., USA). The obtained results were as follows : 1. The EMG activity of masseter and anterior temporal muscle in patient group with canine guidance was higher than with non-canine guidance. but there was no significant difference (p>0.05). 2. The EMG activity of anterior temporal muscle in normal group with non-canine guidance was higher at preferred chewing side than at opposite side (p<0.05). 3. The EMG activity of masseter muscle was higher in normal group than in patient group (p<0.05). 4. The number of occlusal contact in patient group with canine guidance was higher than with non-canine guidance, but there was no significant difference (p>0.05). 5. The occlusal contact force at preferred chewing side was higher in patient group than in normal group(p<0.05).

A Histochemical Study of the periodontal Ligament Fibers in Trauma from Occlusion (실험적 외상성교합시 치근막섬유의 조직화학적 관찰)

  • Occlusal force is a critical factor affecting the condition and structure of the periodontium. When the occlusal forces exceed the physiologic adaptive capacity of the tissues, tissue damage ensues. Such damage is referred to as trauma from occlusion. Excessive pressure causes compression, degeneration and realignment of the periodontal ligament fibers so that they are paralleled perpendicular to the tooth and bone. By inducing excessive occlusal forces with a high amalgam filling on rat's molar, the author observed histologic alterations of the periodontal ligament fibers by means of Hematoxylin-eosin, Van Gieson and Aldenyde fuchsin stainings. The results of the study were observed as follows: 1) The excessive occlusal forces altered arrangement of the collagenous fibers. 2) The arrangement of the oblique fibers showed appreciable differences between the control group and the group subjected to 10 days experimental trauma from occlusion. 3) The realignment of the transseptal fibers was not found. 4) The arrangement of the oblique fibers after 15 days of trauma from occusion was similar to that of 10 days experimental group. 5) The oxytalan fibers were more abundant at the cementum rather than at the alveolar bone. 6) The excessive occlusal forces produced funnel-shaped widening of the oxytalan fibers, which followed wavy course. 7) The oxytalan fibers appeared to be distributed mainly around the middle third of the root rather than that of the apical third of the root during the experimental trauma from occlusion.
