A Clinical Study on 3 Cases of Complications after Total Thyroidectomy and Neck Dissection in the Papillary Thyroid Carcinoma (유두상 갑상선암 환자의 갑상선절제술 후 합병증에 대한 임상경과 보고 3례)

  • Objectives: Thyroid cancer is the most common cancer for female in Korea. This study aims to report the effects of Korean medicine on complications after total thyroidectomy and neck dissection in the papillary thyroid carcinoma. Methods: The 3 patients with papillary thyroid carcinoma were treated by Korean medicine such as acupuncture, moxibustion and pharmacopuncture therapy. We evaluated the results of treatments by change of symptoms. Results: After treatments such as acupuncture, moxibustion and pharmacopuncture therapy except herb medicine, the symptoms of complications after total thyroidectomy and neck dissection were improved. Conclusions: This study shows that Korean medicine has effects on complications after total thyroidectomy and neck dissection in the papillary thyroid carcinoma.

  • Objective: To report the efficacy of vaginally administered bromocriptine. Material and Method: Case report. Results: The prolactin level was significantly decreased after the administration of bromocriptine vaginally. Conclusions: The vaginal administration of bromocriptine can be an alternative to oral administration in patients with hyperprolactinemia who show severe side effects.

Systematic Review on Complementary and Alternative Medicine for Female Sexual Dysfunction (여성 성기능장애의 보완대체의학적 치료에 관한 논문 연구)

  • Objectives: The aim of this study was to review systemically clinical trials on the trends of studies for Complementary and Alternative Medicine in the treatment of female sexual dysfunction and provide basic resource for future treatment and suggestions for improving research methods. Methods: Through medical website 'Pubmed', foreign clinical literatures about female sexual dysfunction were searched and domestic clinical literatures about female sexual dysfunction were searched using internet websites 'National assembly library', 'KISS', 'RISS', 'Korean traditional knowledge portal'. Results: Total 19 literatures were selected from January 2000 to June 2012. 15 foreign literatures were selected through medical website and 4 domestic literatures were chosen using internet websites or hand-searching. 5 literatures were published in 2008 and 13 literatures mentioned about age group of patients. FSFI is most common tool for female sexual dysfunction. 9 of 19 literatures were using Herbal medicine for treatment. 5 literatures were about CBT, 6 literatures were about Biofeedback and other one literatures are about Perineal massage. There were 14 Randomized controlled trials and 5 Non-randomized controlled trials. And randomized controlled trials were relatively low in risk of bias than non-randomized controlled trials. Conclusions: Upon these results, in similar future more clinical trials should be done and accumulate Evidence-based literatures.

Effects of Herbal Medicine on Breastfeeding - Analysis of Marker Substances in Saenghwa-tang by HPLC and LC/MS/MS - (산모의 한약 복용이 모유에 미치는 영향 - HPLC와 LC/MS/MS를 이용한 생화탕 지표성분 분석 -)

  • Objectives: We took breast milk samples and analyzed them using HPLC and LC/MS/MS, to evaluate the effects of taking Saenghwa-tang during breastfeeding on breast milk. Methods: The study participants were 20 lactating women who admitted in Korean medical postpartum care center. Breast milks were collected from paticipants who have been administrated Saenghwa-tang for more than 3 days. We used HPLC and LC/MS/MS for the determinations of amygdalin, liquiritins, 6-gingerol, decursin and decursinol angelate in Saenghwa-tang. Results: 1. Participants' $Mean{\pm}S.D$ (standard deviation) of age is $31.05{\pm}1.96$, and 15 participants had normal delivery and 5 participants had cesarean delivery. 12 participants were primipara and 8 participants were multipara. $Mean{\pm}S.D$ of lactating date is $9.4{\pm}0.94$. 2. Using HPLC, we learned LOQ level peak that matches the peak retention time of standard components of Saenghwa-tang was not detected from 20 breast milk samples. 3. Using LC/MS/MS, decursin of Angelicae Gigantis Radix was detected from HMSP 02, HMSP 04, HMSP 06, HMSP 11, and the each concentrations are 16, 2, 64, 11 ppb. Liquiritin of Glycyrrhizae Radix was not detected from HMSP 13~HMSP 18. Conclusions: Data obtained by this approach shows that this method is reliable and suitable for determining the safety of taking Saenghwa-tang during breastfeeding.

  • Objective: To report the pedigree of Kallmann syndrome inherited in autosomal dominant mode with variable expressivity. Material and Method: Case report. Results: The patient had amenorrhea and anosmia but did not have a sign of absolute hypo gonadotropic hypogonadism. Her father had an anosmia and her two elderly sisters also had an anosmia but delivered babies uneventfully. Her two male siblings did not show any signs of hypogonadotropic hypogonadism. Conclusion: Kallmann syndrome has many different modes of inheritance such as autosomal dominant, autosomal recessive, and X-linked form. So the careful investigation of family pedigree is required.

  • Objectives: The purpose of this study is to report the effect of Traditional Korean Medicine (TKM) on hyperprolactinemia Methods: The patient in this case was a 26-year-old female. The patient was diagnosed as hyperprolactinemia with oligomenorrhea a month ago. We treated by Ickibohyoultang-gami and evaluated treatment effect by serum prolactin and basal body temperature. Results: After treatment, the Hyperprolactinemia value reduced and returned normal approach. And disorder of menstruation and basal body temperature became stable. Conclusions: TKM treatment is effective in patients with Hyperprolactinemia and disorder of menstruation.

The Effect of Serum Obtained before and after Treatment for Endometriosis on in vitro Fertilization Rate of Mouse Oocyte (자궁내막증 치료 전후 환자의 혈청이 생쥐 난자의 수정률에 미치는 영향)

  • Objective: To evaluate the effect of serum obtained before and after treatment for endometriosis on in vitro fertilization and development of two cell mouse embryo. Design: Pretreatment and posttreatment comparoson of fertilization of mouse oocyte and embryo development in serum supplement from patients with endometriosis; result were compared using Stuent T-test analysis. Method: Infertility Clinic, Department of Obstetrics and Gynecology, Collage of Medicine, Won kwang university, Korea. Patients was chosed eleven consecutive women with endometriosis. Interventions was all patient underwent laparoscopic or conservative surgery. This was followed by a 6-month course of burserelin acetate $900{\mu}g/d$. Main outcome was measured total number of fertilization and embryo that was fertilization after 24 hours and reached blastocyst stage after 72 hours of incubation were compared before and after treatment. Result: Before treatment, 47% of the oocyte were fertilized and 31% of the embryo reached blastocyst stage. After treatment, Significantly more fertilized and Significantly more embryo developed to blastocyst on the stage I and II of endometriosis. Conclusion: The fertilization and embryo toxicity of serum samples from patients with endometriosis is lost after treatment.

A Study on the Relationship between the Obstetric Variables and Granulocyte/Lymphocyte Ratio in Some Postpartum Women (일부 산모의 산과적 지표와 과립구/림프구 비와의 관계)

  • Purpose: The aim of this study was to investigate the relation between peripheral blood leukocyte differential - Granulocyte rate, Lymphocyte rate and its calculated Granulocyte/Lymphocyte ratio - and the obstetric variables. Methods: From 270 cases of women who were in the postpartum care center attached to Won-Kwang Korean Medical Hospital, 36 cases met exclusion criteria. On the obstetric variables of 234 cases, peripheral blood leukocyte differential - Granulocyte rate, Lymphocyte rate and its calculated Granulocyte/Lymphocyte ratio - was analyzed. In the postpartum care center, the obstetric variables were asked of 234 cases of women, and who measured leukocyte differential. Results: Mean granulocyte rate and lymphocyte rate of peripheral blood leukocyte differential showed statistically significant difference from those in healthy people and was high. There was statistically significant difference in granulocyte/lymphocyte ratio according to mood of delivery, parity and period passed from childbirth. But whether postpartum women with anemia or not, maternal age and gender of neonate were not associated with granulocyte/lymphocyte ratio, and which was not significantly correlated with gestational age and neonatal birth weight. Conclusions: These findings suggest that activation of the sympathetic nervous system in postpartum women is higher than in healthy people. And granulocyte/lymphocyte ratio was influenced by mood of delivery, parity and period passed from childbirth. In future, more studies or surveys, with less bias, for the relation between peripheral blood leukocyte differential and the obstetric variables are required.

  • Objective: Extrauterine formation of decidua of stromal cells has been well described, particularly in the cervix and ovary. The apparent hormonal mechanisn of this phenomenon suggestes a relationship to endometriosis. Whether formation of ectopic decidua represents a marked progestational response of endometriosis or an independent peritoneal-stromal reaction to pregnancy is unclear. This study was designed to determine the prevalence of deciduosis in the patients whose fertility were proven. Design: Prospective study of patients who had undergone cesarean delivery without history of endometriosis. Materials and Methods: The study was performed in 179 full tenn pregnant women. During the cesarean section, the pelvic organs were thoroughly investigated and the biopsies were collected at the lesions suspicious endometriosis. And then microscopic examination of removed tissues were done. Results: Of the 179 patients who underwent cesarean delivery, 48 women (26.8%) had the lesions suspicious endometriosis such as adhesion, pigmented spots. The ovary was the most frequently ocurred site (79.2%). Microscopically, decidual cells were observed in 34 cases (70.8%) of 48 biopsed patients. Conclusion: Endometriosis has been known to be associated with subfertility. Our observations found the prevalence of deciduosis was 19.0% (34/179) in tenn pregnant women whose fertilites were proven. We suggests that the deciduosis maya manifestation of endometriosis during pregnancy. However, further follow up study should be done to confirm this clinicopathologic process.

Evaluation of Health Information on Internet Sites Dealing with Oriental Obstetrics & Gynecology about Menstrual Pain (월경통으로 검색한 한방부인과 관련 인터넷사이트의 건강정보에 대한 평가)

  • Purpose: The purpose of this study was to analyze current status of the web-sites dealing with oriental obstetrics & gynecology in the internet, and to evaluate their quality and confidence of health information. Methods: It was selected 54 local sites by searching for such key words as menstrual pain. The evaluation tool consisted of six major classification (Content, Authorship, Purpose, Functionality, Feedback and Privacy). Results: In the evaluation of web-sites, the lowest and the highest scores were 7 and 19 out of 26, and the average score was 13.94. The scores of Purpose, Content, Feedback were higher than the average, but the scores of Authorship, Functionality, Privacy were lower than the average. Conclusion: There is a need to develop a high-quality information about latest, confidence, authorship of documents, and to prevent a exposure of privacy.
