Aortic Valve Sparing Operations: A Review

  • Aortic valve sparing operations were developed to preserve the native aortic valve during surgery for aortic root aneurysm as well as surgery for ascending aortic aneurysms with associated aortic insufficiency. There are basically two types of aortic valve sparing oprations: remodeling of the aortic root and reimplantation of the aortic valve. These operations have been performed for over two decades and the clinical outcomes have been excellent in experienced hands. Although remodeling of the aortic root is physiologically superior to reimplantation of the aortic valve, long-term follow-up suggests that the latter is associated with lower risk of developing aortic insufficiency. Failure of remodeling of the aortic root is often due to dilatation of the aortic annulus. Thus, this type of aortic valve sparing should be reserved for older patients with ascending aortic aneurysm and normal aortic annulus whereas reimplantation of the aortic valve is more appropriate for young patients with inherited disorders that cause aortic root aneurysms. This article summarizes the published experience with these two operations. They are no longer experimental procedures and should be part of the surgical armamentarium to treat patients with aortic root aneurysm and ascending aortic aneurysms with associated aortic insufficiency.

Radix mesiolingualis and radix distolingualis: a case report of a tooth with an unusual morphology

  • Variation in the root and canal morphology of the maxillary first molars is quite common. The most common configuration is 3 roots and 3 or 4 canals. Nonetheless, other possibilities still exist. The presence of an additional palatal root is rather uncommon and has been reported to have an incidence of 0.06 - 1.6% in varying populations studied. Whenever two palatal roots exist, one of them is the normal palatal root, the other is a supernumerary structure which can be located either mesiolingually (radix mesiolingualis) or distolingually (radix distolingualis). This case report describes successful endodontic treatment of a maxillary first molar with radix mesiolingualis and radix distolingualis. Identification of this variation was done through clinical examination along with the aid of multiangled radiographs, and an accurate assessment of this morphology was made with the help of a cone-beam computed tomography imaging. In addition to the literature review, this article also discusses the epidemiology, classifications, morphometric features, guidelines for diagnosis, and endodontic management of a maxillary first molar with extra-palatal root.

Comparison between dental and basal arch forms in normal occlusion and Class III malocclusions utilizing cone-beam computed tomography

  • Objective: The purpose of this study was to investigate the relationship between the mandibular dental and basal arch forms in subjects with normal occlusion and compare them with those of Class III malocclusion using cone-beam computed tomography (CBCT). Methods: CBCT images of 32 normal occlusion (19 males, 13 females; 24.3 years) and 33 Class III malocclusion subjects (20 males, 13 females, 22.2 years) were selected. Facial axis and root center points were identified from the left to right mandibular first molars. Distances between the facial axis and root center points for each tooth were calculated, and 4 linear and 2 ratio variables were measured and calculated for each arch form. The variables were compared between groups by independent t-test. Pearson correlation coefficient was applied to assess the relationships between dental and basal variables within each group. Results: The mandibular dental and basal intercanine widths were significantly greater in the Class III group than in normal occlusion subjects (p < 0.05). The dental and basal intercanine widths as well as the dental and basal intermolar widths were strongly correlated in normal occlusion and moderately correlated in Class III malocclusion. Conclusions: The dental arch form demon strated a strong positive correlation with the basal arch form in the normal occlusion group and moderate correlation in the Class III malocclusion group. These results might be helpful for clinicians to have a better understanding of the importance of basal arch form in the alveolar bone.

  • This study was performed to investigate the anti-obesitic effect of mulberry root-bark on male and female Zucker FA/FA or FA/fa rats. Obesity in the rats was induced by feeding high-lipid diet contained 3% corn oil and 1% cholesterol. Experimental groups in male and female rats were assigned to normal diet group (normal control), high-lipid diet group(positive control) and 3% mulberry root-bark powder in high-lipid diet group (MRC). The mulberry treated-group showed decreases of body weight, FER (food efficiency ratio) value and lipid peroxidation in the liver and increase of high density lipoprotein cholesterol (HDL-C) value, compared with positive control groups. Our findings suggest that mulberry root-bark has a potential role in preventing or improving obesity from the following points of view, body weight, serum lipids and antioxidant enzyme activities.

습지 수제부에서 삽목방법에 따른 갯버들 생장율 및 토양 유실 억제 효과 (The Growth Rate of Salix Gracilistyla Miq. and its Effect of Protecting Soil from Dispersion Depending on the Planting Method Applied to Shore-marginal Slope)

  • The main purpose of this study was to verify the shore margin protection effect of the root system of Salix gracilistyla Miq. developed from direct sticking cuttings on wetland, through the measurement of root growth and comparison of soil slaking rate depending on the planting method applied to shore-marginal slope. Comparison of growth rate and soil dispersion rate was made between five planted slope and one naked slope. The planting methods applied to the planted slope were (a) horizontally layed burying of stick(45cm) bundle (b) horizontally layed covering the slope with sticks (c) horizontally fencing with normal cuttings(20cm) (d) elected sticking of normal cutting at equal distances (e) random scattering short cuttings(3-4cm). As results, the most effective planting method was horizontally layed burying, and in order to increase its efficiency scattering the live stem chips in 2-3cm on the slope is recommended. The growth of root was negatively regressive to the distance from water floor.

Glyoxide의 근관정화효과에 관한 연구 (A STUDY ON THE CLEANSING EFFECT OF GLYOXIDE ON THE DENTINAL WALLS OF ROOT CANAL)

  • The purpose of this study was to evaluate the cleansing effect of Glyoxide on the dentinal walls of the root canal. Fourty teeth were divided into four groups and the canals in each group were individually enlarged with K-file by step-back technic and irrigated with one of four irrigants. The four used irrigants were Glyoxide (Marion lab. U.S.A.) in combination with 3.5% sodium hypochlorite, 3.5% sodium hypochlorite, 3.5% sodium hypochlorite in combination with 3% hydrogen peroxide and normal saline solution. All the irrigants were used in conjunction with instrumentation as they would be during clinical conditions. After final irrigation, the canals were dried with paper points and the teeth were split longitudinally. The cleaness of canal walls according to the size and the level of canals were evaluated under steroscope by t analysed statistically The results were as follows. 1. The use of 3.5% sodium hyphochlorite in combination with 3.0% hydrogen peroxide revealed the most clean canal surface regardless of the size of canal at apical third of root canal (p < 0.05). 2. Glyoxide in combination with 3.5% sodium hyphchlorite showed no significant difference in cleaning effect of canal surface compared with 3.5% sodium hypochlorite and normal saline solution at the apical third of narrow canal. 3. Glyoxide in combination with 3.5% sodium hypochlorite revealed no significant difference in debridement of canal walls compared with 3.5% sodium hypochlorite in combination with 3% hydrogen peroxide and saline solution at the middle third of narrow and large canals.

전자근관장측정기 Root ZX의 정확도에 관한 실험적 연구 (AN IN VITRO EVALUATION OF THE ACCURACY OF ROOT ZX ELECTRONIC APEX LOCATOR)

  • The purpose of this study was to evaluate the in vitro accuracy of Root ZX(Morita Co., Japan) which is the ratio type electronic apex locator. The 86 extracted human palatal roots of maxillary molar with fully formed apices were used. File lengths with the file tip just visible at the foramen were compared to those measured with Root ZX. For length measuring with Root ZX, saline test model with which the apical 1/3 of each root was submerged into normal saline were designed. The root canal lengths were determined with Root ZX and the radiographs were taken with a file in the canal. The distances from file tips of Root ZX lengths to apecies in radiographs also were measured with Profile projector PJ311(Mitutoyo Co., Japan). The results were as follows : 1. The root canal length determined with electronic apex locator was $0.78{\pm}0.53mm$ shorter than the length with visual measurement. 2. The file tip of Root ZX lengths was located at $0.85{\pm}0.49mm$ away from the apex in radiograph. 3. The accuracy of the Root ZX was 79.1% within 0.5mm of visual working length and 96.5% within 1.0mm.

상악 견치의 저항 중심에 관한 Laser speckle interferometry와 holographic interferometry볼 이용한 실험적 연구 (EXPERIMENTAL STUDY OF THE CENTER OF RESISTANCE OF A MAXILLARY CANINE USING LASER SPECKLE INTERFEROMETRY AND HOLOGRAPHIC INTERFEROMETRY)

  • The center of resistance is a important determining factor of tooth movement pattern. Laser speckle interferometry, recently developed for noninvasive measurement of small displacements (microns), was used to detect the center of resistance of a maxillary canine which has normal tooth axis and distal curved root in dry human skull. Laser holographic interferometry was used to verify the results of laser speckle interferometry The following result were obtained; 1. In measurement of the degree of rotation, center of resistance was localized when the traction line passed 4.4/18.0 level from alveola crest to root apex. 2. In measurement of the degree of tipping, center of resistance was localized when the traction line passed 4.6/18.0 level from alveola crest to root apex. 3. In holographic determination, the center of resistance was observed when the traction line passed between 3mm to 6mm level from alveola crest to root apex, therefore the results using laser speckle interferometry was coincided with holographic results.

홍삼 내백부의 수삼 미세구조 (Ultrastructure of Fresh Root Turned into Inside White of Red Ginseng)

  • The pith and xylem parts of fresh root that turned into inside-white during processing for red ginseng was investigated under scanning electron microscope in comparison with the sa31e position of fresh root processed into normal reddening. In the inside-white part starch storage cells remain mostly in vacancy or with small number of starch granules and with large hollow by missing cell membranes between cells. Many starch seed granules appeared on the surface of storage cell wall in the inside-white part. Fresh root sample showed better picture than dried powder.

Expression of a Functional Type-I Chalcone Isomerase Gene Is Localized to the Infected Cells of Root Nodules of Elaeagnus umbellata

  • A putative type-I chalcone isomerase (CHI) cDNA clone EuNOD-CHI was previously isolated from the root nodule of Elaeagnus umbellata [Kim et al. (2003)]. To see if it encodes a functional CHI, we ectopically overexpressed it in the Arabidopsis (Arabidopsis thaliana) transparent testa 5 (tt5) mutant, which is defective in naringenin production and has yellow seeds due to proanthocyanidin deficiency. Ectopic overexpression of EuNOD-CHI resulted in recovery of normal seed coat color. Naringenin produced by CHI from naringenin chalcone was detected in the transgenic lines like in the wild-type, whereas it was absent from the tt5 mutant. We conclude that EuNOD-CHI encodes a functional type-I CHI. In situ hybridization revealed that EuNOD-CHI expression is localized to the infected cells of the fixation zone in root nodules.
