Isolated Ventricular Inversion and Anatomically Corrected Malposition of the Great Arteries Associated with Right Juxtaposition of Left Atrial Appendage: A case of Successful surgical repair

  • A seven month old female infant with isolated ventricular inversion and anatomically corrected malposition of the great arteries in situs solitus, associated with ventricular septal defect, patent ductus arteriosus, right-sided juxtaposition of left atrial appendage, is reported. The patient showed usual atrial arrangement with somewhat superoinferior relation, a discordant atrioventricular connection, and a concordant ventriculoarterial connection with aorta in the right-sided position. A normal sized left atrium was connected to the left superiorly positioned morphologic right ventricle through a tricuspid valve, which crossed the left ventricular outflow tract anteriorly. Well developed bilateral[subaortic and sub-pulmonary]conus was documented at operative field. successful surgical repair was done by performing the Senning procedure and by closing the ventricular sepal defect with a patch through the right ventriculotomy. The infant’s postoperative course was uneventful with normal sinus rhythm. Postoperative cardiac catheterization revealed no hemodynamic obstruction or residual shunt.

  • Congenital pulmonary vein stenosis is a rare anomaly and related to high mortality due to progressive pulmonary hypertension and heart failure in infancy. Aggressive anti-failure medication and surgical treatment is recommended. Surgical options are balloon dilatation, endovascular stent, pneumonectomy, lung transplantation, patch grafting, and sutureless repair. We report a case of congenital pulmonary vein stenosis with normal anatomical connection successfully treated with sutureless technique and using pulmonary vasodilators, such as Sildenafil, lloprost and iNO postoperatively.

Influence of viscous phenomena on steel-concrete composite beams with normal or high performance slab

  • The aim of the paper is to present some results about the influence of rheological phenomena on steel-concrete composite beams. Both the cases of slab with normal and high performance concrete for one and two-span beams are analysed. A new finite element model that allows taking into account creep, shrinkage and cracking in tensile zones for concrete, along with non-linear behaviour of connection, steel beam and reinforcement, has been used. The main parameters that affect the response of the composite beam under the service load are highlighted. The influence of shrinkage on the slip over the supports is analysed, together with the cracking along the beam. At last, by performing a collapse analysis after a long-term analysis, the influence of rheological phenomena on the ductility demand of connection and reinforcement is analysed.

  • The Fisher information matrix plays a significant role in statistical inference in connection with estimation and properties of variance of estimators. In this paper, the parameter space of the normal distribution using its Fisher's matrix is defined. The Riemannian curvature and J-divergence to parameter space are calculated.

  • Let T be Gauss transformation on the unit interval defined by T (x) = ${\frac{1}{x}}$ where {x} is the fractional part of x. Gauss transformation is closely related to the continued fraction expansions of real numbers. We show that almost every x is mod M normal number of Gauss transformation with respect to intervals whose endpoints are rational or quadratic irrational. Its connection to Central Limit Theorem is also shown.

  • In [2] we proved that if the minimum of the sectional curvature of a compact real minimal hypersurface of CP$^{m}$ is 1/(2m-1), then M is the geodesic hypersphere. This result was generalized in [8] to the case of M is a generic submanifold with flat normal connection. Effect of cleansing methods on the bone resorption due to repeated dis/re-connection of implant abutment (지대주 풀림과 조임시 지대주 세척방법에 따른 임플란트 주변 골소실의 양에 대한 평가)

  • Background : Repeated dis/re-connection of implant abutment caused bone loss around implant fixtures due to the new formation of biologic width of the mucosal-implant barrier. The aim of this clinical study was to evaluate whether the repeated dis/re-connection of implant abutment cause bone loss clinically and the effect of cleansing methods on a bone loss during the early healing period. Methods : A total 50 implants were installed in 20 patients and repeated dis/re-connection of abutment was performed at the time of surgery and once per week for 12 weeks. 0.9% normal saline solution as group1 and 0.1% chlorhexidine solution as group 2 was used to clean abutments. All patients had radiographs taken at the placement of implant and 4, 8, and 12 weeks postoperatively. The data for bone loss around implant were analyzed. Results: The marginal bone loss at 12 weeks were $1.28{\pm}0.51mm$, $1,32{\pm}0,57mm$ in the mesial and distal sides in group1, $1.94{\pm}0.75mm$, $1.81{\pm}0.84mm$ in group 2, respectively. In view of marginal bone loss, there was not a significant statistical difference between groups. Conclusions : Repeated dis/re-connection of implant abutment may not cause marginal bone loss around implant fixture although limited samples and short-term observation period. In spite of more bone loss in group 2, there was no statistical significant difference between groups. In context of those results, the clinical significance of the repeated dis/re-connection of implant abutment and the cleansing method of abutments is debatable when it comes to marginal bone loss during early healing period.

  • The purpose of this study is to identify the factors affecting job stress and relationship between dental hygienists personal characteristics and them. Final purpose of this study provides the basic data of dental hygienists interventions to help dental hygienists' job stress reduction program. Data were drawn from a convenient sample of 401 dental hygienists who were working for dental clinic, general hospitals, and university hospitals in Seoul and Kyungi areas. The results were analyzed with spsswin 10.0 and the conclusion is as follow: 1. General special quality distribution of dental hygienist is 25-59 and respondent who work in the dentist's office by maiden less than was most. 2. The dental hygienists' job stress was 2.68, it indicates considerably high compare to normal another groups. 3. 'By work, feel fatigue' appeared highest by 3.60 in 5 points full marks from item of business amount connection stress. 4. Only business amount connection stress was expose that is significant in relation with job stress and individual character type. 5. Age, dependent's number, faith, character type variable appeared to keep in mind for connection factor which influence in job stress. 6. Job stress connection factor by character type was expose by variable savages significant thing 'that do work year all as dental hygienist' in A type.

Radio Resource Management Modeling in IEEE 802.16e Networks (IEEE 802.16 망을 위한 무선 자원 관리 모델링)

  • In this paper, we develop radio resource management queueing model in IEEE 802.IS networks considering both connection and packet level. In the upper level connection, we model connection admission control depending on availability of bandwidth and priority queue in each service class. In the lower level packet, we model dynamic bandwidth allocation considering threshold and availability of bandwidth in each service class simultaneously. Hierarchical model is built using an extended Petri Nets, SRN (Stochastic Reward Nets). Bandwidth utilization and normal throughput as performance index for all service classes of traffic are calculated and numerical results are obtained.
