Non-epileptic paroxysmal events during sleep - Differentiation from epileptic seizures - (수면시 발생하는 비간질성 발작 - 간질 발작과의 비교 -)

  • This review describes the wide spectrum of paroxysmal events during sleep in infancy and childhood. The differential diagnosis between sleep-related non-epileptic paroxysmal events and epileptic seizures is difficult in special occasions. The nocturnal frontal lobe seizure and of the more common nonepileptic paroxysmal events during sleep are described. The main differentiating features characterizing parasomnias are: onset in early childhood, rare episodes of long duration, relatively lower frequency per night, absence of stereotypy, gradual disappearance of older age. Video-polysomnography is the gold standard to diagnosing and differentiating parasomnias from nocturnal frontal lobe seizures.

On Study on Chatacteristics of Nocturnal Meteorological Parameter at Mountain Slope (연구노트 산사면에서의 야간 기상요소의 특성에 관한 연구)

  • A series of meterological observation using automation weather station(AWS) carried out to investigate characteristics of nocturnal meteorological parameters for 16~17 June 1998 at Buljeongdong mountain slope, Kyungbuk. Dry temperature at valley was lower than mountain because of high lapse rate at valley, so the strong inversion layer occurrenced at mountain slope for nighttime. Contrary of dry temperature, relative humidity of valley was higher than mountain for nighttime. Wind speed at valley from sunset to next day morning was lower than mountain, but that of valley after sunrise was higher than mountain. Wind direction at valley for all observation time were southeasterlies(SE), that of mountain for nighttime were northeasterlies(NE) or northnorthwesterlies(NNW), and that of mountain after sunrise were irregular. Vapor pressure at valley for all observation time was higher mountain, particularly the difference was high for nighttime.

Polysomnographic Assessment of Nocturnal Enuresis in Adults: A Case Study of Parasomnia Overlap Syndrome With Obstructive Sleep Apnea

  • Enuresis nocturia is more prevalent in children than in adults. Using polysomnography (PSG), we investigated the causes of adult enuresis nocturia in a 20-year-old female patient with nighttime bedwetting. In spite of normal urological examinations, her detailed medical history disclosed frequent sleep paralysis and urination during dreams. During PSG, two electromyograms were attached to her anus to assess the tone of her bladder's sphincter while she slept. During REM sleep, the EMG tone of the mandible decreased, but the anal and bladder sphincter tones did not. The polysomnogram revealed moderate obstructive sleep apnea. Consequently, she was diagnosed with adult parasomnia (nocturnal enuresis) overlap syndrome with OSA. This study demonstrates the value of PSG with simultaneous anal tone EMG for diagnosing NREM parasomnia and nocturnal enuresis.

The Analysis of the Nocturnal Ozone Variations over Kangreung and Wonju (강릉과 원주지역의 야간 오존 변화에 대한 분석)

  • This paper analyzed the characteristics of daily ozone variations over Kangreung and Wonju. It was found that the diurnal cycle of ozone over Wonju has a primary ozone peak in the afternoon and a minimum around sunrise, which is a typical diurnal ozone cycle observable in the urban area. However, the cycle over Kangreung shows a primary peak in the afternoon and secondary peak around 3 a.m. The amounts of ozone in the secondary peak is occasionally higher than that in the primary peak. This nocturnal ozone peak is frequently observed year-round, and the highest frequency and extent are observed in spring. The possible cause of this nocturnal ozone increase was investigated using meteorological parameters and the HYSPLIT trajectory model. It was found that the nocturnal ozone peak is highly correlated with strong wind speed, which has led to positive temperature anomaly. The trajectory model revealed that when the secondary peak occurred, the air was originated from the west and a sinking motion subsequently followed. These findings suggested that when the westerly wind is strongest in spring, the polluted airs from urban areas are transported to the upper boundary layer over Kangreung area. In the case of strong wind during the night, nocturnal ozone peaks were produced by active vertical mixing between lower boundary and upper boundary layers.

Effects of exercise on sleep EEG following caffeine administration (카페인 투여 후 운동이 수면에 미치는 효과)

  • The purpose of this experiment was to examine influence of acute exercise on nocturnal sleep which had been disrupted by caffeine(400mg$\times$3) thought the daytime. Six healthy young males aged 21.0$\times$0.2 yr with a history of low caffeine use. Subjects completed three conditions in a within-subject. At three conditions Sleep EEG were investigated: (1) nocturnal following quiet rest, (2) nocturnal sleep following the consumption of 1200mg of caffeine (3) nocturnal sleep following cycling at 60 min of 60% V $O_{2peak}$ with 1200mg of caffeine consumption. Sleep data were calculated for REM sleep, REM latency, sleep onset latency, sleep efficiency, sleep stages, SWS. Those data were analyzed using repeated-measures ANOVA of change scores. A main effect to, drug(caffeine) indicated that caffeine elicited sleep disturbance that is, TST and sleep onset latency increase and sleep efficiency and stage 4 decrease. The effects of exercise on sleep following caffeine intake generally improve sleep that is, stage 2, 3 and SWS increase and sleep onset latency decrease. A condition effect for sleep indicated sleep improvement after exercise Therefore The data supported a restorative theory of slow-wave sleep and suggest that acute exercise may be useful in promoting sleep and reducing sleep disturbance elevated by a high dose of caffeine.

Review of Clinical Researches for Herbal Medicine Treatment on Nocturnal Enuresis in Children (소아 야뇨의 한약치료에 대한 임상연구 동향 -중의학 논문을 중심으로-)

  • Objectives The purpose of this study is to evaluate the efficacy and safety of herbal medication for the treatment of nocturnal enuresis in children by analyzing randomized controlled trials conducted in China. Methods We searched literatures from China National Knowledge Infrastructure published up to 19 January, 2018. Selected literatures were collected and analyzed in order of publication year, and then demographic information, treatment method, duration of illness, duration of treatment, follow-up period, outcome measurement and adverse events. Results A total of 34 studies were selected for the analysis. In most studies, the total efficacy of the treatment group was reported to be higher than that of the control group, and the recurrence rate and complete cure rate were also found to be more effective in the herbal medicine treatment group. The most frequently used medical herb was Alpiniae Fructus (益智仁), and the use of medical herbs belong to tonifying yang (補陽藥), tonifying qi (補氣藥), and astringing essence strengthening collapse medicine (澁精縮尿止帶藥) were relatively high. The adverse events rate for the herbal medicine treatment group were significantly lower than those in the Western medicine treatment group. Conclusions This study showed that a herbal medicine treatment can be effective and safe option for treating pediatric nocturnal enuresis. However, additional well-designed clinical studies need to be performed to establish a basis.

  • Paroxysmal nocturnal hemoglobinuria(PNH) is an acquired hematologic disorder characterized by intravascular hemolysis, nocturnal hemoglobinuria, thrombotic events and bone marrow failure. It rarely occurs in children and can be complicated by acute renal failure(ARF). Here, we present two cases of ARF complicating PNH which has not been reported yet in Korean children. We suggest that PNH should be considered in differential diagnosis of ARF in children associated with intravascular hemolysis.

  • The purpose of this study is to make a clinical investigation of 14 patients of Male Sex Clinic of Kyung Hee Oriental Medical Hospital whose chief complaint is nocturnal emission. The results are as following. 1. The 14 unmanied male patients, whose average age is 25.7, have had unsound emission for more than two years. 2. Symptoms which they complain of generally correspond to Shinheojeong in Oriental Medicine. And the fact that those who take Boshinseopjungjun grow better in 2.9 weeks on the average seems to show the excellence of the treatment of Oriental Medicine for nocturnal emission.

Elderly Sleep Pattern and Disturbing Factors Before and After Hospitalization (노인환자의 입원 전ㆍ후 수면양상과 수면장애요인에 관한 연구)

  • Sleep is a necessity for survival. Disruption of sleep leads to numerous adverse physiological and psychological consequences. These could be particularly undesirable for older patients, who are subject to many additional factors. But there is limited research related to hospitalized elderly in Korea. The purpose of the study is to explore sleep patterns and disturbing factors of before and after hospitalization, in order to present basic information regarding elderly sleep to develop nursing intervention. The sample consisted of 32 elderly men and women between the ages of n and 87 years. Data collection was done from September to November 1997. Measures of sleep patterns and related factors were obtained from self-reported sleep questionnaires. Analysis of data was done by use of t-test, paired t-test, ANOVA, and Pearson Correlation Coefficient. The results of this study were summarized as follows : 1. In comparision between before and after admission of their sleep pattern, “sleep onset” tends to be delayed and nocturnal sleep time was significantly reduced. So, hospitalized elderly reported less total sleep time than before admission. 2. Regarding the sleep disturbing factors, medication(hypnotics ; 37.5%), physiological factor (snoring ; 59.4%) environmental factor (pillow ; 78.1%), emotional factor(anxiety related to disease ; 37.5%), and illness factor(fatigue ; 34.7%) were reported. 3. Significant differences in gender were found. Men had more disturbances in sleep than women owing to difficulty in falling a sleep and lack of nocturnal sleep. Women consumed more sleep inducing drugs. Significant increase was reported in napping during the day with increasing age. 4. Significant differences between good sleepers and poor sleepers were found for the following variables : nocturnal sleep time, total sleep time, bed time, sleep onset latency time, sleep latency time after nocturnal awakening, time spent in bed upon arousal, environmental factors, and emotional factors. In conclusion, it was found that the quantity and quality of sleep were significantly altered in hospitalized elderly, but adequate strategies for better sleep were not practiced. Further research is needed to develop Intervention strategies to promote sleep and to prevent sleep problems.

An Analysis on Factors Influencing Nocturnal Sleep Duration in 4-year-olds (만 4세 유아의 야간수면시간에 영향을 미치는 변인분석)

  • The study aims to investigate nocturnal sleep duration in 4-year-olds and relative effects of various factors including the personal factor (temperament), the familial factor (mother's employment status), the time-use factors (hours spent in childhood educare institutions, on napping in the institutions, and on using electronic media). The data of 937 4-year-olds collected by the Korea Children and Youth Panel Survey in 2012 was analyzed. Using SPSS 21.0 program, frequency analysis, descriptive statistics, Pearson's correlation analysis and hierarchical regression were conducted. The results are as follows. First, there were significant negative correlations among the personal factor, familial factor and time-use factors. Finally, the time-use factors showed largest explanatory power on variance of children's nocturnal sleep duration, followed by the familial factor. But the personal factor was not statistically significant. These results suggested that parents and childhood educare institutions need to cooperate in order for children to sleep well. It is also necessary to implement and spread the social systems to support healthy development of children.
