Practicability Strength Assessment of a Bone Metallic Plate at the Femur Fixation (대퇴골(Femur)고정용 골 금속판 제작 및 강도 평가)

  • Study was developed the metallic plate for fixation in the femur fracture and plates has a firm place in fracture treatment. This plates can be stabilized for fracture fixation as well as biological and dynamical device. The device's designation and sizing has a optimization with bending structural stiffness and strength, known meaning that is reliable regardless of the plate by the short type and long type. The bending strength of the curved metallic long plate has to evaluate a 11,000N and the bending strength of the curved metallic short plate has to evaluate a 6,525N. This see the X-ray image of bending angle made certain of 15$^{\circ}$ at number 2 and same 82.87$^{\circ}$ at number 2, 4, 5, 7, 8, 9, 10 by outside angle, and confirmed 25.26$^{\circ}$ at number 3, 3.68$^{\circ}$ at number 6, 15.64$^{\circ}$ at number 9 by inside angle. This study shows that keep up the metallic plate for fixation in the femur fracture through X-ray Image and the device can be used to support Revision case of Hip Implant and to use a case of Hip screw compression of Hip Neck Fracture. Short plate have a wrapping of femur and long plate have to preserve a pole of femur.

The Effect of Graston Technique and Chuna manual therapy combined with Korean Medical Treatment for fibromyalgia: A Case Report

  • Objectives : The purpose of this study was to report the clinical effects of the Graston Technique and Chuna manual therapy, combined with Korean Medical Treatment for fibromyalgia. Methods : We treated a patient diagnosed with fibromyalgia. We used acupuncture, the Graston Technique, Chuna manual therapy, pharmacopuncture, herbal medicine, moxibustion and physical therapy. Outcomes were evaluated using the American College of Rheumatology Preliminary Diagnostic Criteria (ACR), the Fibromyalgia Impact Questionnaire (FIQ), and the Numeric Rating Scale (NRS). Results : The widespread pain index (WPI) scale score of the ACR decreased from 12 to 9, and the symptom severity scale (SS) score of the ACR decreased from 8 to 6. The FIQ score decreased from 63.69 to 50.15. On the NRS, lower back pain & lower limb pain decreased from 6 to 2; neck pain from 6 to 3; muscle tenderness & morning stiffness from 6 to 4; fatigue from 6 to 3; urticaria from 6 to 2. Conclusion : This case study suggests that the Graston Technique and Chuna manual therapy combined with Korean Medical Treatment may be effective treatments for fibromyalgia. However, further studies are needed.

Case Report; Tetanus, Differential Diagnosis with Trismus (개구장애 환자에서의 감별진단, 파상풍 환자의 증례보고)

  • Tetanus is fatal neurological disease caused by Clostridium tetani on contaminated wound that is characterised by muscle spasm, muscle pain, and autonomic dysfuction. C. tetani exist on contaminated wound frequently that developed clinical tetanus under low oxygen condition. Tetanus have four symptomatic form: generalized, localized, cephalic, and neonatal. The incubation period is about 7 days and mortality is high. The commonest presenting symptom is trismus and other is stiffness of neck and back(opisthotonos), muscle spasm, dysphagia, facial pain, risus sardonicus. Trismus is primary presenting symptom in 50~75% of the cases and this have high possibility of initial visit to dental office. This case report of a patient who visit in our department with trismus as chief complaint.

Visual Evaluation of Design Variations in Power Shoulder Jacket (파워 숄더 재킷의 디자인변화에 따른 시각적 평가)

  • The purpose of this study is to evaluate the differences of visual effects and visual images by variations in shoulder angle and shoulder width of power shoulder jacket. The stimuli are 5 samples: One control group, 2 variations of the angle of shoulder line and 2 variations of the width of shoulder line. The data has been obtained from 87 fashion design majors. The data has been analyzed by Factor Analysis, Anova, Scheffe's Test and the Correlation Analysis. The results of the study are as follows: The visual images by the angle and the width of shoulder line of power shoulder jacket are composed of 4 factors : attention, stiffness and softness, masculinity, and trendiness. Among these factors, attention factor is estimated by the most important factor. The visual effects by the angle and the width of shoulder line of power shoulder jacket are composed of 3 factors : whole silhouette, neck and arm figure, upper body figure. In these factors, the whole silhouette is estimated by the most important factor. The power shoulder jacket with extreme changes in shoulder angle has double images: it looks trendy and classy but also gives an awkward image. In case of changes in shoulder width, the visual evaluation shows big differences with the degree of expansion. The visual evaluation with variation in angle has stronger image and effect than that in width. The power shoulder jacket trend itself is also evaluated that variation in angle reflects the trend better than that in width.

Body Types and Its Relation with the Body Type Awareness, and Subjective Symptoms of Female High School Students in an Urban Area (도시지역 여자고등학생들의 체형과 체형에 대한 의식 및 자각증상과의 관계)

  • Purpose : This study investigated the relationship between body type, subjective symptoms and health awareness in female high school students Methods : The subjects were 393 students at the female high school in Taejon City by self-recorded questionnaires in May, 2006. Results : In the classification of body type based on the degree of obesity, low weight group was 20.9%, normal weight group was 58.0%, over weight group was 11.7% and obesity group was 9.4%. In the level of body type wareness, those who consider themselves to be obesity type 13.7%, standard type were 43.8%, and leptosomic type 8.1%, but there was a clear tendency to be leptosomic type that the rate was 76.3%. In the complaint rates of subjective symptoms, “fatigue” was the highest, followed by “neck pain(stiffness)”, “dizziness”, “stomachache” in the descending order. In the complaint rates of subjective symptoms by the degree of obesity, dizziness, irregular menstruation and anorexia were higher according to the lower obesity, but constipation and wearied eyes were the higher the more obesity. Conclusion : The study results showed that the recent female high school students were clear tendency to be leptosomic type, and leptosomic and obesity type were related with subjective symptoms. Considering these findings, it appeared that being excessively thin is related to symptoms and young women's thinness-oriented attitudes are unhealthful.

Relationship between Warning Signs and Sasang Constitution(SC) in Early Stroke Patients (급성기 중풍환자의 사상체질과 중풍 경고증상, 전조증상과의 상관성 연구)

  • Objective : This study investigated the relationship between warning signs and Sasang constitution (SC) in acute stroke patients. Methods : From October in 2005 to March in 2007, 629 acute stroke patients were studied. Patients were hospitalized within 14 days after the onset ofstroke at one of 3 oriental medical hospitals in Seoul and Kyong-gi province. We assessed the type of SC of acute stroke patients by Questionnaire for Sasang Constitution Classification II (QSCC II). We investigated warning signs (motor weakness, blindness, dysarthria, sensory disturbance, finger dullness, facial & eyelid spasm, neck stiffness), general characteristics, etc. Results : This study showed a higher proportion of Tae-eum & So-eum experience upper extremity sensory disturbance but So-yangexperience lower extremity sensory disturbance. Smoker So-eumexperience 1st & 2nd finger dullness more than non-smoker So-eum. So-eum women experience motor weakness more than So-eum men. Conclusion : Besides these results, we could observe almost no relationship between warning signs and Sasang constitution (SC) in acute stroke patients. More data from prospective cohort studies will help people better understand the relationship between warning signs and Sasang constitution (SC) in acute stroke patients.

Review of Acute Traumatic Closed Mallet Finger Injuries in Adults

  • In adults, mallet finger is a traumatic zone I lesion of the extensor tendon with either tendon rupture or bony avulsion at the base of the distal phalanx. High-energy mechanisms of injury generally occur in young men, whereas lower energy mechanisms are observed in elderly women. The mechanism of injury is an axial load applied to a straight digit tip, which is then followed by passive extreme distal interphalangeal joint (DIPJ) hyperextension or hyperflexion. Mallet finger is diagnosed clinically, but an X-ray should always be performed. Tubiana's classification takes into account the size of the bony articular fragment and DIPJ subluxation. We propose to stage subluxated fractures as stage III if the subluxation is reducible with a splint and as stage IV if not. Left untreated, mallet finger becomes chronic and leads to a swan-neck deformity and DIPJ osteoarthritis. The goal of treatment is to restore active DIPJ extension. The results of a six- to eight-week conservative course of treatment with a DIPJ splint in slight hyperextension for tendon lesions or straight for bony avulsions depends on patient compliance. Surgical treatments vary in terms of the approach, the reduction technique, and the means of fixation. The risks involved are stiffness, septic arthritis, and osteoarthritis. Given the lack of consensus regarding indications for treatment, we propose to treat all cases of mallet finger with a dorsal glued splint except for stage IV mallet finger, which we treat with extra-articular pinning.

An Epidemiologic Study on the Frequency of Stress Symptoms in the Orofacial Region (스트레스에 의한 구강안면증상의 발혀에 관한 역학적 연구)

  • Stress is recognized as a major predisposing and/ or precipitating factor in long-lastig intractable chronic pain, such as temporomandibular disorders, headache, and other psychophysiological disorders. So it is necessary to detect physical and psychological changes induced by stress as soon as possible for positive treatment outcome. This study was performed to investigate the occurrence rate of stress symptoms according to anatomic region, type of symptom, and other personal and social factors. 859 subjects from general population answered the stress symptom questionnaire devised by the author and composed of 50 items. Data from the questionnaire were analyzed statistically with SPSS program and the results obtained were as follows : 1. Oral symptom which showed the highest frequency rate of 38.8% was vesicular lesion of the lip and cheek. The other symptoms with more 20% occurrence rate were ulcerative leion of lip and cheek, toothache, paresthesia of teeth, eruption of tongue, tongue coating and taste change in descending order. 2. In extraoral symptoms, ?데 disturbance was the item which showed the highest frequency rate of 62.0%, and the items for stiffness of suboccipital region and neck, headache, facial swelling, furuncle of face were answered more than 45% of the subjects whereas only 14.0% of the subjucts complained jaw pain under stress. 3. The better one who thought his or her health status was, the fewer items were answered and the difference of symptom frequency by dwelling place and by having hobby were shown in extraoral symptoms only. 4. For relief of stress symptoms, 79.2% of the subjects replied only to take a rest whereas not more than 13, 5% of the subjects visited dental clinic. Correlationship between symptom sites were very high.

Influence on Vertebral Artery and Basilar Artery Blood flow by Cervical CHUNA Manual Therapy (경추 추나요법이 추골동맥과 기저동맥 혈류에 미치는 영향)

  • Objectives : CHUNA therapy that removes compression of dislocated vertebral bones has positive effect, but sometimes improper CHUNA manual therapy may give rise to negative effect. The aim of this study is to make sure that cervical CHUNA manual therapy give positive effect or negative effect to the blood flow velocity of vertebral artery(VA) and basilar artery(BA) by Trancranial Doppler sonography(TCD). Methods : We performed TCD study on 20patients(male 5, female 15, mean ages 38.5 years) with diagnosis like cervical movement related disorder, headache or dizziness. After we measured mean blood flow velocity(Vm) of VA and BA before cervical CHUNA therapy(Pre-CCT) and after cervical CHUNA therapy(Post-CCT), statistically evaluated the results. Results: The patients received cervical CHUNA therapy for TA sequel, HIVD of cervical spine, headache, dizziness, neck stiffness etc. VA Vm was $31.9{\pm}8.0cm/sec$ before CHUNA therapy, but significantly increased $35.0{\pm}8.7cm/sec$ after CHUNA therapy (p < 0.05). But, there was no significant variation of BA Vm between $41.8{\pm}7.5cm/sec$ Pre-CCT and $41.2{\pm}8.5cm/sec$ Post-CCT(p>0.05). Though VA Vm slightly increased after CHUNA therapy in normal range group, there was no significant variation between VA Vm Pre-CCT and VA Vm Post~CCT. In VA Vm decrease group, VA Vm significantly increased after CHUNA therapy(p<0.05). But, there was no significant variation of BA Vm between Pre-CCT and Post-CCT in BA Vm normal range group and BA Vm decrease group(p>0.05). Conclusions: These findings suggest that cervical CHUNA manual therapy have positive effect on blood flow velocity of VA and BA.

Galgeun-tang, an Herbal Formula, Ameliorates Atopic Dermatitis Responses in Dust Mite Extract-treated NC/Nga Mice

  • Objectives: Galgeun-tang (GGT, gegen-tang, kakkon-to), an herbal formula, is used to treat the common cold, fevers, headaches, hangovers and neck and upper back stiffness. The drugs currently used to treat atopic dermatitis (AD) are limited by the significant adverse effects associated with their long-term usage. The need to efficiently manage the AD response while reducing side effects has led to the development of alternative remedies. Methods: To assess the effects of GGT on AD, the anti-inflammatory and anti-AD properties of GGT were evaluated in both in vitro and in vivo systems. Results: Nitric oxide (NO) and histamine production on lipopolysaccharide (LPS)-treated RAW264.7 cells and phorbol-12 myristate 13-acetate (PMA)/A23187-treated MC/9 cells, respectively, were inhibited by GGT. GGT reduced thymus and activation-regulated chemokine (TARC/CCL17) release on TNF-${\alpha}$/IFN-${\gamma}$ stimulated HaCaT cells in a dose-dependent manner. GGT reduced both plasma levels of IgE and histamine and the dermatitis score in house dust mite induced atopic dermatitis-like lesions on NC/Nga mice. However, there were no significant histopathological differences observed between the GGT group and the AD-induced group, such as AD-like lesions in the dorsal skin or ear or mast cell infiltration in the dorsal skin. Conclusions: These results indicate that GGT inhibits chemokine production by keratinocytes and the atopic dermatitis response in NC/Nga mice, suggesting that GGT may be useful as a therapeutic remedy for treating AD and allergic inflammation-related diseases.
