빗자루병(病)에 감염(感染)된 대추나무 조직내(組織內)의 마이코플라스마 분포(分布) (Distribution of Mycoplasma in Witches'-broom Infected Jujube Tissue)

  • 형광색소(蛍光色素) DAPI(4'-6-diamidino-2-phenylindole, 2HCI)와 형광원미경(蛍光願微鏡)을 이용(利用)한 조직화학적(組織化學的) 기법(技法)으로 빗자루병(病)에 걸린 대추나무 조직내(組織內)의 마이코플라스마 분포(分布)를 조사(調査)한 결과(結果), 빗자루병징(病徵)이 나타나 있는 가지에서는 외관상(外觀上) 건전엽(健全葉)을 포함(包含)하여 모든 잎과 줄기에서 마이코 플라스마가 검출(檢出)되었으나 병징(病徵)이 나타나 있지 않은 가지의 잎과 줄기에서는 마이코플라스마가 검출(檢出)되지 않았다. 또한 감염(感染)된 나무의 뿌리조직(組織)에서도 뚜렷한 형광반응(蛍光反應)이 나타남으로써 마이코플라스마가 뿌리조직(組織)에도 많이 존재하고 있음을 알 수 있다.

Acute Necrotizing Pancreatitis Associated with Mycoplasma pneumoniae Infection in a Child

  • Mycoplasma pneumoniae is responsible for approximately 20% to 30% of community-acquired pneumonia, and is well known for its diverse extrapulmonary manifestations. However, acute necrotizing pancreatits is an extremely rare extrapulmonary manifestation of M. pneumoniae infection. A 6-year-old girl was admitted due to abdominal pain, vomiting, fever, and confused mentality. Acute necrotizing pancreatitis was diagnosed according to symptoms, laboratory test results, and abdominal computed tomography scans. M. pneumoniae infection was diagnosed by a 4-fold increase in antibodies to M. pneumoniae between acute and convalescent sera by particle agglutination antibody assay. No other etiologic factors or pathogens were detected. Despite the occurrence of a large infected pseudocyst during the course, the patient was able to discharge without morbidity by early aggressive supportive care. This is the first case in Korea of a child with acute necrotizing pancreatitis associated with M. pneumoniae infection.

Pediatric Mycoplasma pneumoniae Infection Presenting with Acute Cholestatic Hepatitis and Other Extrapulmonary Manifestations in the Absence of Pneumonia

  • Mycoplasma pneumoniae infections mainly involve respiratory tract; however, also can manifestate other symptoms by site involved. Extrapulmonary manifestations of M. pneumoniae infection are rarely known to occur without pneumonia. Herein we report a case of a 9-year-old boy who presented with acute cholestatic hepatitis in the absence of pneumonia. Rhabdomyolysis, skin rash, and initial laboratory results suspicious of disseminated intravascular coagulopathy were also observed in this patient. M. pneumoniae infection was identified by a 4-fold increase in immunoglobulin G antibodies to M. pneumoniae between acute and convalescent sera by enzyme-linked immunosorbent assay. This is the first pediatric case in Korea of M. pneumoniae infection presenting with acute cholestatic hepatitis in the absence of pneumonia.

  • Mycoplasma pneumoniae(M. pneumoniae) is a primary pathogene of the respiratory tract in children. Most studies of such pneumonia involve a group of admitted patients in hospital, usually with major medical illness. We retrospectively studied the epidemiologic and clinical feature of 105 patients with serologically proven M. pneumoigic pneumonia treated at Gwang-Yang Hospital during a epidemic period of Jun. 1993 to Dec. 1993. All cases of pneumonia developed in this period were also reviewed and compared with serologically proven group. The results were as follows. 1. There were 63 males and 42 females, and the male /female ratio was 1.5 : 1. 2. More than half(57%) of cases belonged to 5-9 years of age group, and mean age was 6.5 years old. 3. Mean age was steadily decreased as prevalence of mycoplasmal pneumonia had been subsided. 4. The date of index case was June 26th, and that of median case was 3 months after the index case. 5. A major determinant of the outbreak seemed to be the population density rather than the population size. 6. Recurrence and serious complication were not observed in this period. 7. All cases of pneumonia developed in this period might be attributed to M. pnemoniae. These epidemiologic and clinical characteristics would contribute to the prevention and diagnosis of mycoplasmal pneumonia.

  • 울릉도에서 채집(採集)한 위축(萎縮), 총생(叢生), 황화(黃化) 증상의 천궁, 전북 장수에서 채집한 위축(萎縮), 황화(黃化) 증상의 시호, 대관령에서 채집한 황화(黃化), 총생(叢生) 증상의 이병주(罹病株)로 부터 전자현미경 관찰 결과 마이코프라스마 병원체(病原體)가 발견(發見) 되었으며 천궁은 울릉도에서 발병률(發病率) 11.9%로써 피해가 심하였다.

Treatment of the Mycobacterium chelonae Infection after Fat Injection

  • For recent years, use of autologous fat injection has increased significantly in facial contouring surgery. Along with such increase in use, complications like atypical mycoplasma infection have been also on the increasing trend. The authors report two cases of Mycobacterium chelonae infection that occurred after autologous fat injection. Patients were treated as infection that resistant to common antibiotics and results were negative to routine culture and Gram staining. Acid-fast bacillus stain, polymerase chain reaction (PCR) test and mycobacterial cultures were conducted for diagnosis under suspicion of atypical mycoplasma infection. Then, combination antibiotics therapy, surgical treatment, and steroid injection were performed for treatment. Both patients were diagnosed with Mycobacterium chelonae in PCR test. They were positive to mycobacterial cultures. Combination antibiotics therapy was repeated to improvement of symptom. However, they could not be free from side effects such as deformation in facial contour, scar and pigmentation even after full recovery. When chronic wound infections after autologous fat injection, we must suspect atypical or mycobacterial infection and conduct examinations for a early diagnosis and proper antibiotic therapy that is effective to the nontuberculous mycobacteria.

Development and evaluation of semi-nested PCR for detection of the variable lipoprotein haemagglutinin (vlhA) gene of Mycoplasma Synoviae in chicken

  • This study aimed to develop a semi-nested polymerase chain reaction assay for the direct detection of Mycoplasma synoviae (M. synoviae) from clinical samples using three newly designed oligonucleotide primers specific to the variable lipoprotein haemagglutinin (vlhA) gene and differentiate M. synoviae field strains based on a nucleotide deletion or the insertion of the proline-rich repeat (PRR) region of the vlhA gene. The developed semi-nested polymerase chain reaction (PCR) assay revealed positive results in 12 out of 100 clinical samples collected from chickens showing lameness and joint swelling. Six positive samples were selected randomly for sequencing, and sequence analysis revealed 96.3-100% nucleotide identities compared to the reference sequences. Phylogenetic analysis showed that sequences of the strains in this study were closely related to WVU1853 (Spain), CK.MS.UDL.PK.2014.2 (Pakistan), and F10-2AS (USA) strains, but they were distinct from the M. synoviae-H vaccine strain sequence. M. synoviae obtained from these samples were identified as types A and C with a length of 38 and 32 amino acids, respectively. These results indicated that the specific and sensitive semi-nested PCR could be a useful diagnostic tool for the direct identification of clinical samples, and the sequence analysis of the partial vlhA gene can be useful for typing M. Synoviae.

닭의 호흡기성(呼吸器性) Mycoplasma 균병(菌柄)에 대한 Furamizole 의 예방시험(豫防試驗) (Field Tests of Furamizole for Preventive Control of Avian Mycoplasmosis)

  • A number of chemotherapeutic agents, namely antibiotics, sulfonamides and nitrofuran derivatives have been used, as a fred additive, for the purposes of growth improvement of chicken, increase of feed efficiency, decrease of animal mortality, and disease prevention. In these experiments, furamizole which is one of nitrofuran derivatives and feed additive was test, in field, its antibacterial activity against Mycoplasma gallisepticum causing air sac disease, its effect on pullorum disease control, its effect on the lowering the mortality of chicken and finally the increase of feed efficiency. Throughout the studies, furamizole, in concentration of 0.025% in feed fed to baby chicken continuously resulted as following: 1. Tested chicken showed no avian mycoplasma infection compared to 3.7% outbreak in control chicken. 2. Tested chicken showed a low degree of outbreak of pullorum disease. However, its outbreak was much more surpresed compared to that of control chicken. 3. Total mortality rate of 5.5% and 30.8% were obtained in test and control chicken respectively. 4. Feed efficiency were 2.83 and 2.97 in test and control chicken respectively.

Consideration in treatment decisions for refractory Mycoplasma pneumoniae pneumonia

  • Mycoplasma pneumoniae (MP) is the most common cause of childhood bacterial pneumonia. Although macrolide is known to be effective as a first-line therapy, the proportion of macrolide resistance in MP pneumonia has strikingly increased during recent 2 decades in East Asia. This is challenging to physicians since they have to decide more often whether to use secondary treatment. Diagnostic methods to detect macrolide-resistance of MP are currently not available in Korean hospitals. Even in the diagnosis of MP infection, both serologic and molecular test have limitation: inability to differentiate current illness from carriage or asymptomatic infection. Combining these 2 diagnostic methods and excluding infection caused by other respiratory pathogens allow a more reliable diagnosis. This effort is even more demanding in recent years to keep children from unnecessary exposure to secondary antibiotics. Although several observational studies have reported that tetracycline and fluoroquinolone, which are considered in the treatment of refractory MP pneumonia, have efficacy of shortening the duration of fever and respiratory symptoms, those findings need to be proven by well-designed prospective studies. The use of tetracycline and fluoroquinolone in children is generally tolerable, as supported by many observational data. However, since concerns about side effects still remain, careful consideration about benefits and risks is needed to decide their use.

Benefits and risks of therapeutic alternatives for macrolide resistant Mycoplasma pneumoniae pneumonia in children

  • Although Mycoplasma pneumoniae pneumonia (MPP) has been generally susceptible to macrolides, the emergence of macrolide-resistant MPP (MRMP) has made its treatment challenging. MRMP rapidly spread after the 2000s, especially in East Asia. MRMP is more common in children and adolescents than in adults, which is likely related to the frequent use of macrolides for treating M. pneumoniae infections in children. MRMP is unlikely to be related to clinical, laboratory, or radiological severity, although it likely prolongs the persistence of symptoms and the length of hospital stay. Thereby, it causes an increased burden of the disease and poor quality of life for the patient as well as a societal socioeconomic burden. To date, the only alternative treatments for MRMP are secondary antimicrobials such as tetracyclines (TCs) or fluoroquinolones (FQs) or systemic corticosteroids; however, the former are contraindicated in children because of concerns about potential adverse events (i.e., tooth discoloration or tendinopathy). A few guidelines recommended TCs or FQs as the second-line drug of choice for treating MRMP. However, there have been no evidence-based guidelines. Furthermore, safety issues have not yet been resolved. Therefore, this article aimed to review the benefits and risks of therapeutic alternatives for treating MRMP in children and review the recommendations of international or regional guidelines and specific considerations for their practical application.
