본 연구는 뇌졸중 환자에게 제공한 음악요법이 우울과 재활 동기에 미치는 효과를 규명함으로써 음악요법을 뇌졸중 환자의 간호중재방법으로 활용하기 위한 근거를 마련하고자 시도되었다. K병원에 뇌졸중 진단으로 입원한 33명을 대상으로 취침 전 30분 동안 사전에 선정된 10곡의 음악을 3일간 듣게 하였다. 그 결과 인구사회학적 특성에서 남성 참여자(25명)가 여성(8명) 참여자에 비해 많았고 연령대는 40대와 50대가 25명(75.8%)에 달했다. Wilcoxon-signed rank 검정을 사용하여 분석한 결과 우울과 재활 동기에는 영향이 없는 것으로 나타났으나, 인지수준이 낮은 MMSE 1그룹(≤ 29)에서 음악요법이 우울감소에 효과가 있는 것으로 확인되었다. 이 결과로 음악 요법의 효과성을 높이기 위한 다양한 간호중재로 적용하는데 기초자료로 제공될 것을 기대한다.
Purpose: This study was conducted to describe the risk of dysphagia and to evaluate the effects of a singing intervention for women elderly in community. Methods: One-group pre test-post test design was used. The subjects were 29 women over the age of sixty and residing in a local area. A singing intervention consisted of breathing, vowel production and singing. The one hour intervention occurred once a week for eight weeks. The variables of dysphagia risk, a swallowing test, and nutritional status were measured. Analysis was done by paired t-test. Results: There was a significant decrease in the score of dysphagia risk (p<.001). There were significant increases in the swallowing test scores [modified water swallowing test (p=.032), food test (p=.001)]. There were no significant differences in nutritional status (triceps skin fold thickness, mid arm muscle circumference). Conclusion: The findings support that a singing intervention can be helpful in reducing the risk of dysphagia may improve deglutition for the women elderly. These results suggest that this study may contribute to the use of a singing intervention for women elders with dysphagia risk.
본 연구는 국내 음악치료 및 물리치료 분야에서 리듬청각자극(RAS)을 적용하여 수행된 연구를 비교분석하였다. 이를 위해 1999년부터 2018년 11월까지 발표된 학위 논문과 학술지 게재 연구 중 RAS 및 보행기능 관련 주제어를 검색하여 기준에 부합한 45개의 연구를 선정하였고, 세부중재방법과 리듬자극의 유형, 기술된 치료 원리의 측면에서 분석하였다. 분석 결과, 두 영역에서의 연구 모두 일정한 간격의 리듬을 핵심적인 치료 자극으로 사용하고 RAS의 기본 절차를 따른 점에서는 공통적이었지만, 목표기능이나 구체적인 리듬제공방법에 있어서는 차이가 있었다. 음악치료 연구에서는 모두 보행기능이 목표 변인이었던 데 비해, 물리치료 연구에서는 균형이나 근력, 고유수용성감각 등 관련 신체기능이 목표에 포함되었다. 또한 음악치료 연구에서는 다양한 요소를 포함한 '음악'이 리듬자극으로 활용된 반면, 물리치료 연구에서는 메트로놈을 리듬자극으로 활용한 연구가 음악을 활용한 연구보다 많았다. RAS 원리와 관련하여 단순한 기법 설명 외에 동조화와 같이 치료의 원리가 충분히 기술된 연구는 물리치료보다는 음악치료 분야에서 많이 확인되었다. 이러한 결과는 음악치료 연구에서는 리듬과 더불어 음악의 다양한 요소가 보행에 미칠 수 있는 영향이 고려되어 음악의 역할이 적극적으로 활용된 반면, 물리치료 연구에서는 보행에 영향을 미치는 다양한 신체기능에 대한 개입이 포함된 경향이 있음을 보여준다. 두 가지 측면 모두 재활에 있어 중요한 부분임을 고려할 때, 본 연구는 다학제간 연구를 기반으로 한 RAS 연구의 기법의 확장 적용에 대한 시사점을 제시한다고 할 수 있다.
호스피스 환자들은 질병으로 인한 심리 정서적 문제를 동반하는데 이에 대한 적절한 치료적 개입이 이루어지지 않으면 이러한 문제가 신체화 현상으로 나타나면서 삶의 질을 방해하는 요인으로 작용한다. 본 연구는 음악이 부정적인 자기 은폐(self-concealment)를 감소시키고 언어적 비언어적 표현(self-expression)을 촉진시키는 지를 보고자하는 데에 그 목적을 두었다. 호스피스 기관에 있는 전문가들로부터 추천받은 3명의 말기 환자들을 대상으로 4개월간의 음악치료 세션을 실시하였으며, 음악치료는 각 환자들의 상태에 따라 총 9회에서 11회에 걸쳐 진행되었다. 자기은폐척도(Self-Concealment Scale: SCS)와 자기서술문(Personal Descriptive Essays)을 이용하여 음악치료프로그램 사전, 사후 검사를 실행한 후 양적 및 질적 자료를 산출하여 결과를 분석하였다. 연구 결과 다양한 음악적 개입 중에서 특히 노래를 중심으로 구성된 프로그램이 삶과 관련된 여러 가지 주제와 감정적 내용들에 대한 환자들의 반응을 이끌어 내는 데에 효과적임을 보여주었다. 사후 검사에서 환자들의 자기은폐 수준이 낮아졌으며, 자기서술문에서 보고된 자료에는 사실적인 내용 대신 감정적 내용이 증가하였음을 보여주었다. 본 연구는 여러 가지 심리 정서적 어려움을 가지고 있는 말기 환자들에게 감정표현과 의사소통 매개체로서 음악이 효과적이라는 결과를 보여주었다. 또한, 자기감정과의 만남과 이에 대한 표현은 삶의 질을 향상시킨다는 관점에서 감상과 같은 수동적 참여를 기본으로 환자의 활동 수준에 맞추어 음악프로그램을 구성한다면 환자들의 삶의 질을 향상시킬 수 있다는 점을 시사한다.
본 연구는 음악과 심상 음악치료의 과정에서 치료사와 내담자간의 상호 성장의 경험을 치료사의 관점에서 조망하고자 하였다. 치료사-내담자 간의 상호 성장의 과정을 치료사의 관점으로부터 심층 인터뷰를 통해 질적 자료를 취합하였으며, 이를 근거이론의 방법으로 분석하였다. 이 연구의 연구 문제는 음악과 심상 치료사가 느끼는 '상호 성장 경험(the experience of mutual maturity)'은 어떤 과정이며, 그 경험은 어떤 의미를 갖는가'이다. 상호 성장 경험과 관련하여 치료사들은 반구조화된 심층인터뷰를 통해 상호 성장에 대한 개인적 정의와 그 예들, 상호 성장 경험에 관한 통찰과 깨달음, 상호성장 경험의 의미와 해석에 관하여 대답하였다. 본 연구의 인터뷰에 참여한 4명의 치료사들은 Lisa Summer가 인증하는 음악과 심상치료사(MIT, Music & Imagery Therapist)의 자격을 획득한 음악과 심상치료사이며, 현재 GIM(Guided Imagery & Music) 훈련과정에 있다. 분석된 심층 인터뷰의 내용은 개방 코딩의 결과 총 81개의 개념들이 추출되었고, 이들 개념들은 20개의 하위범주로 나뉘었으며, 축코딩을 통해 9개의 범주로 정리되었다. 근거이론의 패러다임 모형에 입각하여 분석된 범주들로부터 음악과 심상 치료사들의 성장 과정이 도출되었는데, 결론적으로 내담자-치료사 간 상호 성장 경험에서 '역전이를 다루는 것'은 중심현상을 이루고 있다. 역전이를 작업한 결과로서 치료사들은 자신에 대한 앎과 수용을 넓혀 스스로의 성장을 경험하며, 다른 한 축으로는 치료사로서의 유능감을 확인받아, 내담자의 성장을 이끈다고 하겠다.
Purpose: This study compared the effect of music and noise blocking on the vital signs, postoperative pain, analgesic use, length of stay in the Post Anesthesia Care Unit (PACU) and satisfaction after a laparoscopic colectomy. Methods: This randomized controlled trial was performed in a 555-bed National Cancer Center, from February 13 through May 31, 2012. Subjects consisted of 69 patients who underwent a laparoscopic colectomy under general anesthesia, and were recruited by informed notices. The inclusion criteria were patients between the ages of 35-75, with an American Society Anesthesiologist physical classification I or II. The subjects were randomly allocated to three groups; music therapy group (MTG), noise blocking group (NBG) and control group (CG). Collected data were analyzed using Repeated measures ANOVA, one-way ANOVA and Kruskal-Wallis test through IBM SPSS (Version 19.0). Results: There were no significant differences in vital signs among the three groups. Postoperative pain in MTG (p<.05) and NBG (p<.05) was significantly decreased compared to CG. The amount of analgesics (p=.030) and length of stay at PACU (p=.021) in MTG was significantly decreased compared to NBG or CG; satisfaction in MTG and NBG was significantly higher compared to CG. Conclusion: Music seems to reduce postoperative pain, the amount of analgesics, and the length of stay at PACU. Therefore, music therapy is considered to be included in nursing intervention for postoperative patients at PACU.
Purpose: The aim of the investigation was to identify the effects of purposeful activities combined with music on upper extremity function, motivation, and mood in acute stroke patients. Methods: For the study, 19 acute stroke inpatients were recruited at a general hospital in Korea. The patients were allocated between an experimental group (n=9) and a control group (n=10). The experimental group performed purposeful activities with their preferred music, and the control group carried out purposeful activities without music. The 2 groups received the treatments 30 min per day, 5 days per week, for 4 weeks. Measurements were taken before and after intervention, including a manual function test (MFT) for measuring upper extremity function, a volitional questionnaire (VQ) for assessing motivation, and the Beck depression inventory (BDI) and Beck anxiety inventory (BAI) for assessing mood. Results: In post-treatment measurement, both groups showed greater MFT, VQ, and BDI scores than in pre-treatment. Significantly greater VQ and BDI changes were represented in the experimental group as compared to the control group. Conclusion: Based on the findings of this study, purposeful activities combined with music have a positive effect on motivation and depression in acute stroke patients.

The purpose of the present study is to provide exercises, together with music, that are helpful for elderly people's balancing ability, and to determine their effects in order to provide data for the promotion of elderly persons' health. Thirty elderly persons were randomly assigned to a balance training group(BTG) of 15 subjects, or a music and balance training group (music therapy + balance training [MTBTG]) of 15 subjects; intervention was implemented three times per week for six weeks. To measure the changes in their balancing ability before and after the experiment, the limit of stability, the "Timed Up and Go"(TUG) test, and the Berg Balance Scale (BBS) were measured. Changes in the limit of stability before and after the experiment were shown to be significant in both the BTG and the MTBTG. Changes in the limit of stability of the right side before and after the experiment showed statistically significant differences between the BTG and the MTBTG. Changes in the TUG test and the BBS before and after the experiment were shown to be statistically significant in both the BTG and the MTBTG. The application of music during balance training thus is considered to have a positive effect on elderly persons' balancing ability.
Purpose: This study examine the effects of music therapy and phone counseling on postpartum depression and maternal identity in high risk women. Methods: A nonequivalent control group time series research design was employed. Eighteen women comprised the experimental group and seventeen women, the control group. Experimental group received 4 weeks of music therapy and phone counseling. Postpartum depression and maternal identity were measured prior to intervention and on the 1st, 2nd, and 4th weeks after its introduction. Results: The first hypothesis (i.e., "the degree of postpartum depression would be lower in the experimental group than in the control group in a month's time") was not supported (z=-0.47, p=.641). The second hypothesis (i.e., "the degree of maternal identity would be lower in the experimental group than in the control group in a month") was not supported as well (z=1.08, p=.285). Conclusion: There is the need to monitor long-term effects of music therapy and phone counseling on high risk postpartum women, beyond 4 weeks observed in this study. Additionally, the development of music therapy tailored to high risk individuals and systematic phone counseling protocols for postpartum depression is required.
Purpose: This study examined the effects of music therapy on pain, anxiety and length of stay of patients undergoing laparoscopic hysterectomy in the postanesthesia care unit (PACU). Methods: Sixty patients who received laparoscopic hysterectomy under general anesthesia from a PACU in a university hospital located in Cheonan city participated in this study. The experimental group (n=30) was offered the option to listen to their preferred music by using MP3 players and headphones for 30 minutes. The control group (n=30) received routine postoperative nursing care. Visual analogue scale was used to measure participants' pain and anxiety, length of stay in the PACU was examined by using their medical records. Data were collected from December 2013 to February 2014 and analyzed by descriptive statistics, ${\chi}^2$-test, Fisher's exact test, and independent t-test using SPSS version 21.0. Results: The result showed that the level of post-operative pain (t=2.44, p=.018), anxiety (t=2.37, p=.021), and the length of stay in PACU (t=3.06, p=.004) significantly decreased in the experimental group as compared to the control group. Conclusion: This study indicated that music therapy with the patients' preferred music showed positive effects. Therefore, it can be used as a therapeutic intervention for postoperative pain management of patients with laparoscopic hysterectomy.
