A Tenosynovial Giant Cell Tumor Arising from Posterior Cruciate Ligament of Knee Joint: A Case Report (슬관절 후방 십자 인대에서 기원한 건막 거대 세포종: 1예 보고)

  • Localized forms of giant cell tumor are known to arise commonly in the synovial membrane of the finger joints. Multinucleated giant cells are its characteristic pathology finding, giant cell tumor shows a low rate of recurrence after complete excision. When occurring at the knee joints, giant cell tumor manifests a wide form of symptoms, from no symptom at all, to intermittent locking. Complete excision is possible by arthroscopy, but if done incompletely, it is reported to recur in 45% of cases. We present here a case of giant cell tumor that has arisen from the anterior portion of the posterior cruciate ligament, excised by arthroscopy and followed by pathologic confirmation.

Surgical Treatment of the Giant Cell Tumors in the Spine (척추에 발생한 거대세포종의 수술적 치료)

  • Between 1992 and 1996, 5 patients with the giant-cell tumor of the spine were treated. Four were female and one was male. The mean age was 34 years old, and the mean follow-up time was 36 months. The locations of the lesions were the cervical spine in 1, the thoracic spine in 3, and the lumbar spine in 1. Pain was the predominant presenting symptom in all cases and four had a neurological deficit. A combined anterior and posterior surgical approach wds as performed in all cases, which were also treated with AIF(anterior interbody fusion) and anterior and/or posterior instrumentation. Adjuvant radiation therapy was performed in 1 case of cervical spine. At the final follow-up, the pain and neurologic symptoms were improved. Radiologic examination showed no evidence of local recurrence and no failure of instrumentation of the spine.

An Intensive Review On Clinical Thesis about Hwangryunhaedok-tang Pharmacopuncture Treatment: Focused on Case Reports and Controlled Studies in Korean Academic Journals (황련해독탕약침 치료에 대한 임상 논문 고찰: 국내 학술지의 증례보고와 대조군 연구 중심으로)

  • Objectives The purpose of this thesis is to compare and analyze outcomes of case studies and controlled studies about Hwangryunhaedok-tang (HHT) pharmacopuncture in clinical studies. Based on these studies reporting clinical effects by using HHT pharmacopuncture, we object to deduct the comparative review in applying clinical disorders limitedly. And conclusionally, by using this comparative review, we aim to make the fine evidence in treating clinical specific diseases. Methods We searched 106 studies about HHT pharmacopuncture from 6 Korean web databases by using in Korean word 'Hwangryunhaedok-tang pharmacopuncture'. This search had been progressed throughout 1 month (January, 2019). There was 106 thesis and we excluded studies that are not related to HHT pharmacopuncture, experimental research, not the case report or randomized controlled trials, non-randomized controlled trials, not using HHT pharmacopuncture, not using only one pharmacopuncture, and not using HHT pharmacopuncture for major treating method. As a result, 39 thesis are selected as a population. Results We analyzed 34 case reports and 5 controlled studies. As the final outcome, HHT pharmacopuncture was used in musculoskeletal diseases, dermatopathy, and 32 specific diseases. Conclusions After analyzing 39 studies, effective symptom changes clinically and statistically by using HHT pharmacopuncture herbal acupuncture were identified. However, these thesis designs were reported deficiently in many parts such as not excluding other confounding variables. After modifying defects of thesis design and conduct study, it can play a role as the evidence using HHT pharmacopuncture more precisely.

A Trend Analysis of Clinical Studies of Medical Gigong (formerly Journal of the Korean Academy of Medical Gi-Gong) (의료기공(구 대한의료기공학회지)에 실린 임상논문의 경향 분석)

  • Objective : The purpose of this study is to suggest the evidence of traditional Korean medicine by analyzing the clinical research trends of the Korean Academy of Medical Gi-Gong and to use it as reference data for future clinical studies. Methods : The clinical studies published in the Journal of the Korean Academy of Medical Gi-Gong from 2011 to 2019 were reviewed and the study design, yearly trends, patient's disease, therapeutic intervention, and outcome measure were analyzed. Results : Of the 24 clinical studies, there are 13 Case Reports, 9 Case Series Researches, and 2 Case-Control Studies. In terms of disease group, Musculoskeletal Diseases and female Diseases were the most common, followed by neurological Diseases, dermatological Diseases, and rare Diseases. In terms of treatment methods, acupuncture, herbal medicine, and Sugi therapy were mainly used. And cupping, thermotherapy, and Korean medical exercise therapy were used as a supplement. As an evaluation tool, clinical symptom change was the most common, followed by DITI, VAS, survey, and X-ray. In most reports, Korean medical treatment has been effective in treating diseases. Conclusions : More clinical studies including controlled clinical trials should be conducted to provide a basis for verifying the effectiveness of Korean medicine.

Diagnostic Usefulness of Digital Infrared Thermal Image in Carpal Tunnel Syndrome (수근관 증후군에서 적외선 체열 검사의 진단적 유용성)

  • Objective: The purpose of this study is to evaluate the usefulness of infrared thermography in patients with carpal tunnel syndrome by comparing with electrodiagnostic and ultrasonographic findings. Method: From January 2014 to October 2017, electrodiagnosis, ultrasound, and digital infrared thermal image (DITI) of unilateral carpal tunnel syndrome diagnosed in a single hospital were retrospectively analyzed. The subjects with bilateral symptoms of carpal tunnel syndrome, peripheral vascular disease, diabetes, thyroid disease, fibromyalgia, rheumatic disease, systemic infection, inflammation, malignant tumor, and other musculoskeletal disorders such as finger osteoarthritis, peripheral neuropathy, cervical radiculopathy, and the previous history of surgery were excluded. Results: Of 53 patients diagnosed with carpal tunnel syndrome, 11 were male and 42 were female. The visual analogue scale was 4.9 ± 1.9, and the duration of symptom was 11.8 ± 12.5 months. There was no statistically significant difference in the body surface temperature between the unaffected and affected sides. The severity of symptoms, electrodiagnostic findings, and cross-sectional area of the median nerve significantly correlates to each other. The temperature difference between the second fingers of the affected and unaffected sides showed a weak correlation with the amplitude of sensory nerve action potential and onset latency of compound muscle action potential, when there was no significant correlation with the other parameters. Conclusion: The difference in temperature on the surface of the body, which can be confirmed by DITI, is little diagnostic value when DITI is performed in unilateral carpal tunnel syndrome patients, especially when compared with ultrasonography.

A Literature Study on the Diagnostic Factors and Value as a Syndrome of Damjeok (담적의 진단요소 및 증후군으로서의 가치에 대한 문헌고찰)

  • Objectives: The purpose of this study is to investigate the diagnostic factors of Damjeok in the literature. Methods: This study used the databases of five (Mediclassics, RISS, KMbase, OASIS, CNKI) to analyze diagnostic factors of Damjeok. Literatures published by May 2023 were assessed and we classified 91 literatures dealing with diagnostic factors by 5 categories, 'characteristics', 'location', 'symptom', 'pulse wave & tongue feature', and 'eating habits & lifestyle'. Results: Damjeok is mainly formed at the stomach in the form of lump or hard seed, and it can be observed in other regions or even the whole body by its trait of floating. Damjeok mainly shows symptoms of the digestive system and also causes various symptoms associated with the respiratory system and infection, musculoskeletal system, urogenital system, neuropsychiatric system and cardiovascular system. Regarding pulse wave and tongue feature, a slippery wave and greasy coated tongue are typical features of Damjeok. Additionally, bad eating habits and a lack of exercise have been described as a factor that cause and aggravate Damjeok. Conclusions: The two most important factors in the diagnosis of Damjeok are abdominal stiffness by physical examination and digestive symptoms, and these correspond to the definition of a syndrome. Diagnostic factors based on literature evidence will provide clues to the clinical diagnosis of Damjeok syndrome.

Physiological and Psychological analysis of musculoskeletal symptoms (근골격계질환에 대한 물리적/심리적요인에 대한 연구)

  • The object of this study is to evaluate the prevailing physical and psychosocial conditions regarding occupational low back injury. This study consists of two parts. In the first part of the study, analytic biomechanical model and NIOSH guidelines are applied to evaluate risk levels of low back injury for automobile assembly jobs. Total of 246 workers are analysed. There are 20 jobs having greater back compressive forces than 300kg at L5/S1. Also, there are 44 jobs over Action Limit with respect to 1981 NIOSH guidelines. The relationship between psychosocial factors and low back injury was examined in the second part of the study. A battery of questionnaires concerning the psychosocial stress based on PWI (Psychosocial Well-being Index) and musculoskeletal pain symptoms at low back was completed by 246 workers at the same plant. Results showed that 207 out 246 workers experienced the symptoms and 27 workers were diagnosed as patients. Two groups(low stressed, high stressed) based on PWI score had no significant relationships with both symptoms and results of diagnosis. The relationships between physical work load and psychosocial stress were also analysed. Specifically, some postural factors(vertical deviation angle of forearm, horizontal deviation angle of upperarm, vertical deviation angle of thigh, etc) were highly correlated with psychosocial stress. The results illustrated that PWI scores were associated with some physical workloads. However, psychosocial stress levels couldn't be well related with the pain symptom as well as the actual incidence of low back injury since pain or discomfort regarding low back injury were more complex than that of other musculoskeletal disorders.

Percutaneous Radiofrequency Therapy of Benign Bone Tumors in the Femoral Head (대퇴골두 부위에 발생한 양성 골 종양에 대한 경피적 고주파치료 (증례보고))

  • Purpose: To report two cases of bone tumors other than osteoid osteoma in the proximal femur and treated with percutaneous high frequency radioablation method. Cases: We reviewed two cases with intracortical chondroma and enchondroma in the femoral head retrospectively. The patient with intracortical chondroma was a thirty one year old woman and had suffered right hip pain of 1 year duration. The lesion was located in the head of right femur and treated with CT guided percutaneous high frequency radioablation after needle biopsy under general anesthesia. The symptom was gone immediately after the procedure and was discharged postop. 1 day. 15 months has passed without symptom recurrence. Second case having enchondroma, was 56 year old woman complaining of gluteal area pain for 3 months. Radiologic evaluation showed osteolytic lesion with sclerotic rim on the inferior portion of the left femoral head. She received a same therapy with CT guided radiofrequency ablation following needle biopsy. She reported dramatic pain relief after the procedure and was discharged postop. 1 day. No symptom has occurred for 3 months until now. Conclusion: We present 2 cases of bone tumor occurred in the hip joint area other than osteoid osteoma which were treated with CT guided radiofrequency ablation.

Giant Cell Tumor of Tendon Sheath in Hand (수부에서 발생한 건초의 거대 세포종)

  • Purpose : The giant cell tumor of tendon sheath is the second most common tumor of the hand, but recurred frequently although excision was performed. Authors analyzed and would report clinical findings and postoperative results of it. Materials and Methods : Between January 1991 and December 1998, 38 patients, 41 cases which the authors had performed excisional biopsy to the mass in the hand and diagnosed with the giant cell tumor of tendon sheath, was analyzed with age, sex, chief complaint, symptom duration, involved finger, involved tendon, frequently developed site in fingers, size, multiplicity, radiologic findings and recurrence. The mean duration of follow-up was 13.1 months (5~40 months). Results : Of 38 patients, twenty-nine were female. It is frequent in the fourth decade and mean age was 40.1 years old. The neurological compression symptom was found in 5 cases. The mean duration of symptom was 23.4 months. Flexor tendon was involved in 24 cases. The distal interphalangeal joint area in digit was involved most frequently in 20 cases. Index finger was the most common involved finger (14 cases), and long finger was the second most common (9 cases). All tumors were unilateral. The majority of patients had solitary lesion but one case had multiple lesion. In the radiologic findings, erosion or pressure indentation of bone was seen in 3 cases. All patients were operated by marginal excision. Recurrence rate was 5.1%. Conclusion : The risk factors in giant cell tumor of tendon sheath were female, forth decade, index finger, flexor tendon, and distal interphalangeal joint area. The recurrence was increased in marginal excision of recurred cases, in cases with multiple developed lesions or in multilobular lesion, so wide surgical excision is necessary to prevent recurrence.

Smartphone Overuse and Upper Extremity Pain, Anxiety, Depression, and Interpersonal Relationships among College Students (대학생의 스마트폰 중독사용 정도에 따른 상지통증, 불안, 우울 및 대인관계)

  • This study aimed to survey the impact of smartphone overuse on upper extremity pain, anxiety, depression, and interpersonal relationships among college students. Subjects completed a structured questionnaire consisting of the Smartphone Addiction Inventory, the Musculoskeletal Symptom Checklist, the State-Trait Anxiety Inventory, the Beck Depression Inventory-II, and the Relationship Change Scale from May to June 2012. We analyzed the survey data from 525 responses, excluding unreturned or incomplete surveys. Data were analyzed using the $x^2$ test and t-test to determine the differences in smartphone overuse and its impact on upper extremity pain, anxiety, depression, and interpersonal relationships between two groups: the overuse and normal use groups. Moreover, Pearson's correlation coefficient was used to examine the correlation between smartphone overuse and upper extremity pain, anxiety, depression, and interpersonal relationships. The results placed 62 people (11.8%) in the smartphone overuse group. The extent of smartphone overuse was more severe among female than male college students, and longer time spent using smartphones per day was directly related to smartphone overuse. The smartphone overuse group evidenced higher shoulder pain than the normal use group did, but no differences were found in other sites of the upper extremities. State anxiety, trait anxiety, and depression were higher in the smartphone overuse group than in the normal use group. Subjects with a higher extent of smartphone overuse experienced increased state-anxiety, trait-anxiety, and depression. Moreover, subjects with higher state-anxiety, trait-anxiety, and depression scores were more likely to have poor interpersonal relationships. Therefore, early screening for smartphone overuse should be evaluated, because it can be useful in developing addiction prevention programs to improve posture, stress coping, positive mental health, and effective interpersonal relationships.
