Limited Unilateral Decompression and Pedicle Screw Fixation with Fusion for Lumbar Spinal Stenosis with Unilateral Radiculopathy : A Retrospective Analysis of 25 Cases

  • Objective : Lumbar spinal stenosis is conventionally treated with surgical decompression. However, bilateral decompression and laminectomy is more invasive and may not be necessary for lumbar stenosis patients with unilateral radiculopathy. We aimed to report the outcomes of unilateral laminectomy and bilateral pedicle screw fixation with fusion for patients with lumbar spinal stenosis and unilateral radiculopathy. Methods : Patients with lumbar spinal stenosis with unilateral lower extremity radiculopathy who received limited unilateral decompression and bilateral pedicle screw fixation were included and evaluated using visual analog scale (VAS) pain and the Oswestry Disability Index (ODI) scores preoperatively and at follow-up visits. Ligamentum flavum thickness of the involved segments was measured on axial magnetic resonance images. Results : Twenty-five patients were included. The mean preoperative VAS score was $6.6{\pm}1.6$ and $4.6{\pm}3.1$ for leg and back pain, respectively. Ligamentum flavum thickness was comparable between the symptomatic and asymptomatic side (p=0.554). The mean follow-up duration was 29.2 months. The pain in the symptomatic side lower extremity (VAS score, $1.32{\pm}1.2$) and the back (VAS score, $1.75{\pm}1.73$) significantly improved (p=0.000 vs. baseline for both). The ODI improved significantly postoperatively ($6.60{\pm}6.5$; p=0.000 vs. baseline). Significant improvement in VAS pain and ODI scores were observed in patients receiving single or multi-segment decompression fusion with fixation (p<0.01). Conclusion : Limited laminectomy and unilateral spinal decompression followed by bilateral pedicle screw fixation with fusion achieves satisfactory outcomes in patients with spinal stenosis and unilateral radiculopathy. This procedure is less damaging to structures that are important for maintaining posterior stability of the spine.

Bluetooth Ad hoc 망에서 멀티 슬롯 SAR-QT 알고리즘을 고려한 WAP 패킷의 성능 분석 (Performance Analysis of WAP Packet Considering Multi-Slot SAR-QT Algorithm in Bluetooth Ad hoc Network)

  • 본 논문은 Bluetooth 환경에서 SAR-QT (Segment And Re-assembly Quick Transfer) 알고리즘을 사용하여 WAP (Wireless Application Protocol) 성능을 향상시키기 위한 WAP 패킷의 전송 시간을 분석하였다. 이러한 WAP의 전송 능력을 향상시키기 위한 한 방법으로 SAR-QT의 과정은 WTP (Wireless Transaction Protocol) 상위 계층에서 내려온 전체 메시지를 분할한 다음 베이스밴드에서 패킷을 전송하게 된다. 그리고 Bluetooth 피코넷 환경에서 SAR-QT 알고리즘을 사용하여 Bluetooth 패킷 타입 중 DM (Data-Medium rate) 1, DM3, DM5에 따른 WAP over Bluetooth의 패킷 전송 시간을 분석하였다. 이 SAR-QT 알고리즘은 멀티 슬롯으로 전송할 경우 L2CAP (Logical Link Control And Adaptation Protocol) 에이스밴드 패킷 전송 시간을 감소시킨다. 결과로부터, WAP over Bluetooth 환경에서 WAP 패킷의 전송 시간을 줄이기 위해서 WTP 패킷 크기가 증가해야 한다는 점을 알 수 있었다 또한, 이러한 무선 채널을 고려한 Rician 페이딩 환경에서 WAP over Bluetooth의 적당한 패킷 크기를 구할 수 있었다.

NFlex Dynamic Stabilization System : Two-Year Clinical Outcomes of Multi-Center Study

  • Objective : Pedicle-based dynamic stabilization systems, in which semi-rigid rods or cords are used to restrict or control spinal segmental motion, aim to reduce or eliminate the drawbacks associated with rigid fusion. In this study, we analyzed the two-year clinical outcomes of patients treated with the NFlex (Synthes Spine, Inc.), a pedicle-based dynamic stabilization system. Methods : Five sites participated in a retrospective study of 72 consecutive patients who underwent NFlex stabilization. Of these 72 patients, 65 were available for 2-year follow-up. Patients were included based on the presence of degenerative disc disease (29 patients), degenerative spondylolisthesis (16 patients), lumbar stenosis (9 patients), adjacent segment degeneration (6 patients), and degenerative lumbar scoliosis (5 patients). The clinical outcome measures at each assessment were Visual Analogue Scale (VAS) to measure back pain, and Oswestry Disability Index (ODI) to measure functional status. Radiographic assessments included evidence of instrumentation failure or screw loosening. Results : Sixty-five patients (26 men and 39 women) with a mean age of 54.5 years were included. Mean follow-up was 25.6 months. The mean VAS score improved from 8.1 preoperatively to 3.8 postoperatively, representing a 53% improvement, and the ODI score from 44.5 to 21.8, representing a 51% improvement. Improvements in pain and disability scores were statistically significant. Three implant-related complications were observed. Conclusion : Posterior pedicle-based dynamic stabilization using the NFlex system seems effective in improving pain and functional scores, with sustained clinical improvement after two years. With appropriate patient selection, it may be considered an effective alternative to rigid fusion.

새로운 하이브리드 스테레오 정합기법에 의한 3차원 선소추출 (3D Line Segment Detection using a New Hybrid Stereo Matching Technique)

  • We present a new hybrid stereo matching technique in terms of the co-operation of area-based stereo and feature-based stereo. The core of our technique is that feature matching is carried out by the reference of the disparity evaluated by area-based stereo. Since the reference of the disparity can significantly reduce the number of feature matching combinations, feature matching error can be drastically minimized. One requirement of the disparity to be referenced is that it should be reliable to be used in feature matching. To measure the reliability of the disparity, in this paper, we employ the self-consistency of the disunity Our suggested technique is applied to the detection of 3D line segments by 2D line matching using our hybrid stereo matching, which can be efficiently utilized in the generation of the rooftop model from urban imagery. We carry out the experiments on our hybrid stereo matching scheme. We generate synthetic images by photo-realistic simulation on Avenches data set of Ascona aerial images. Experimental results indicate that the extracted 3D line segments have an average error of 0.5m and verify our proposed scheme. In order to apply our method to the generation of 3D model in urban imagery, we carry out Preliminary experiments for rooftop generation. Since occlusions are occurred around the outlines of buildings, we experimentally suggested multi-image hybrid stereo system, based on the fusion of 3D line segments. In terms of the simple domain-specific 3D grouping scheme, we notice that an accurate 3D rooftop model can be generated. In this context, we expect that an extended 3D grouping scheme using our hybrid technique can be efficiently applied to the construction of 3D models with more general types of building rooftops.

Low Bit Rate을 고려한 8kbps FBD-MPC 방식에 관한 연구 (A Study on 8kbps FBD-MPC Method Considering Low Bit Rate)

  • 유성음원과 무성음원을 사용하는 음성부호화 방식에 있어서, 같은 프레임 안에 모음과 무성자음이 있는 경우에 음질저하현상이 나타난다. 본 연구에서는 연속음성에서 무성자음을 포함한 천이구간을 탐색, 추출하고 주파수대역에서 근사합성하는 8kbps의 멀티펄스 음성부호화 방식(FBD-MPC)를 제안하였다. 기존의 8kbps MPC와 FBD-MPC의 SNRseg를 평가한 결과, FBD-MPC의 남자음성에서 0.5dB, 여자음성에서 0.2dB 개선된 것을 확인할 수 있었다. 결국, MPC에 비해 FBD-MPC의 SNRseg가 개선되어 음성파형의 일그러짐을 제어할 수 있었으며, 본 방법은 셀룰러폰이나 스마트폰과 같이 Low Bit Rate의 음원을 사용하여 음성신호를 부호화하는 방식에 활용할 수 있을 것으로 기대된다.

모폴로지 연산과 가우시안 혼합 모형에 기반한 컬러 영상 분할 (Color Image Segmentation Based on Morphological Operation and a Gaussian Mixture Model)

  • 본 논문에서는 수학적 모폴로지 연산과 가우시안 혼합 모형에 기초한 새로운 칼라 영상 분할 알고리즘을 제안한다. 우리는 혼합 모형에서 구성 성분의 수를 결정하고, 각 구성 성분의 중심값을 계산하는데 모폴로지의 연산과 라벨링 연산을 이용한다. 그리고 칼라 특징 벡터의 확률 모형으로 가우시안 혼합 모형을 사용하고, 이들의 모수 값들을 추정하는데 결정적 어닐링 EM알고리즘을 사용한다. 최종적으로 혼합 모형으로부터 계산된 사후 확률을 이용하여 칼라 영상을 분할한다. 실험 결과를 통하여 모폴로지 연산이 혼합모형의 수를 자동으로 결정하고 각 성분의 모드를 계산하는데 아주 효율적인 방법임을 보였고, 또한 결정적 어닐링 EM 알고리즘에 의하여 추정된 가우시안 혼합 모형을 사용하여 계산된 사후 확률에 의한 영상 분할 방법이 기존의 분할 알고리즘보다 정확한 분할 방법임을 보였다.

A Knowledge-based Model for Semantic Oriented Contextual Advertising

  • Proper and precise embedding of commercial ads within Webpages requires Ad-hoc analysis and understanding of their content. By the successful implementation of this step, both publishers and advertisers gain mutual benefits through increasing their revenues on the one hand, and improving user experience on the other. In this research work, we propose a novel multi-level context-based ads serving approach through which ads will be served at generic publisher websites based on their contextual relevance. In the proposed approach, knowledge encoded in domain-specific and generic semantic repositories is exploited in order to analyze and segment Webpages into sets of contextually-relevant segments. Semantically-enhanced indexes are also constructed to index ads based on their textual descriptions provided by advertisers. A modified cosine similarity matching algorithm is employed to embed each ad from the Ads repository into one or more contextually-relevant segments. In order to validate our proposal, we have implemented a prototype of an ad serving system with two datasets that consist of (11429 ads and 93 documents) and (11000 documents and 15 ads), respectively. To demonstrate the effectiveness of the proposed techniques, we experimentally tested the proposed method and compared the produced results against five baseline metrics that can be used in the context of ad serving systems. In addition, we compared the results produced by our system with other state-of-the-art models. Findings demonstrate that the accuracy of conventional ad matching techniques has improved by exploiting the proposed semantically-enhanced context-based ad serving model.

미류나무 성숙 절간조직으로부터 식물체 증식 (Shoot Regeneration of Mature Nodal Segments in Poplar (Populus deltoides))

  • 미류나무 절간조직(2년생)을 이용하여 식물생장조절물질이 함유된 MS배지에서 대량의 식물체를 증식시켰다. 대부분의 식물체는 BA 1.0 + TDZ 0.1 mg/L가 함유한 배지에서 줄기의 형성층 부분에서 절편당 $24.6\;{\pm}\;4.6$개의 줄기가 유기되었으며, 몇몇 줄기는 피목 부분의 조직에서 유기되었다. 신장된 줄기는 NAA 0.01 mg/L가 포함된 배지에서 $94.6\%$의 발근이 이루어졌다. 신장된 줄기의 발근은 식물생장조절물질에 따라 차이를 보여 NAA와 IBA에서 $90\%$ 이상의 발근이 촉진되었으나, cytokinin류를 처리한 시료에서는 $50\%$ 이하의 낮은 발근율을 나타냈다. 발근된 유식 물체는 Cell-tray의 인공상토(peatmoss : vermiculite : perlite = 1 : 1 : 1)에 이식하여 8주간 생육후 $98\%$의 높은 생존율을 나타냈다. 본 연구결과, 2년생 미루나무류의 줄기에서 직접 대량의 줄기를 유도하여 대량증식과정을 단순화 시켰을 뿐만 아니라 조직배양묘 경화시 multi-cap을 이용하여 생존율을 향상시킬 수 있었다.

달리기 시 체간의 골반-척추구조변형이 동적안정성에 미치는 연구 (Kinematic Analysis of Dynamic Stability Toward the Pelvis-spine Distortion during Running)

  • The purposes of this study were to assess dynamic stability toward pelvis-spine column distortion during running and to compare the typical three-dimensional angular kinematics of the trunk motion; cervical, thoracic, lumbar segment spine and the pelvis from the multi-segmental spine model between exercise group and non-exercise group. Subjects were recruited as exercise healthy women on regular basis (group A, n=10) and non-exercise idiopathic scoliosis women (group B, n=10). Data was collected by using a vicon motion capture system (MX-T40, UK). The pelvis, spine segments column and lower limbs analysiaed through the 3D kinematic angular ROM pattern. There were significant differences in the time-space variables, the rotation motion of knee joint in lower limbs and the pelvis variables; obliquity in side bending, inter/outer rotation in twisting during running leg movement. There were significant differences in the spinal column that is lower-lumbar, upper-lumbar, upper-thoracic, mid-upper thoracic, mid-lower thoracic, lower thoracic and cervical spine at inclination, lateral bending and twist rotation between group A and group B (<.05, <.01 and <.001). As a results, group B had more restrictive motion than group A in the spinal column and leg movement behaved like a 'shock absorber". And the number of asymmetry index (AI) showed that group B was much lager unbalance than group A. In conclusion, non-exercise group was known to much more influence the dynamic stability of equilibrium for bilateral balance. These finding suggested that dynamic stability aimed at increasing balance of the trunk ROM must involve methods and strategies intended to reduce left/right asymmetry and the exercise injury.

神經網을 利用한 韓國語 數字音 認識에 관한 硏究 (A Study on the Spoken KOrean-Digit Recognition Using the Neural Netwok)

  • 한국어 숫자음이 단음절인 특성을 이용하여 각 숫자음에 대해 시간정합을 필요로 하지 않으면서 일정한 수를 갖는 특징벡터를 추출하여 다층구조 신경망으로 인식실험을 하였다. 음성신호의 시작점/끝점과 더불어 모음의 최대 피크점을 기준으로 해석구간을 초성, 중성, 종성의 세 부분으로 나누었으며, 음성신호의 특징벡터로는 반사계수, 켑스트럼, ${\Delta}$켑스트럼, ${\Delta}$에너지 등을 이용하여, 각 특징벡터 및 입력층과 은닉층의 노드 수에 따른 인식율 및 학습속도 등을 비교하였다. 신경망의 입력층의 특징벡터로서 반사계수를 사용한 경우보다 켑스트럼을 사용했을 때가 더 좋은 인식율을 보였다. ${\Delta}$켑스트럼의 특성이 전체 인식율에 미치는 영향이 그다지 크지 않았는데, 이는 한국어 숫자음이 단음절로 구성되어 있는 특징을 이용해 분석 구간을 stationary한 특성을 갖는 세 부분으로 구분하였기 때문이라 생각된다. 각 숫자음에 대해 150개의 켑스트럼을 사용한 경우에 97.8%의 인식율을 얻었다.
