운동선수의 구강악안면 외상 경험과 마우스 가드 착용과의 관련성 (The relationship between maxillofacial injury and the use of mouth-gards in athletes)

  • Objectives : This study was to investigate the use of mouth-guards and maxillofacial injuries and to examined the relationship among variables in athletes group. Methods : The subjects were 459(86.5%), who were 18 years or older with mean age of 20.7 years. Data was collected using a self administrated questionnaire from May 25 to June 15, 2009. We surveyed maxillofacial injuries(temporo-mandibular disorders contained) and mouth-guards usage and confidence concerning mouth-guards in athletes. The data were analysed with chi-square analysis, fisher's exact test and logistic regression using the SPSS 15.0 windows. Results : Prevalence of body or maxillofacial injuries experiences was 54.5% for athletes and 47.1% for non-athletes groups(p>0.05). The other hand, the incidence temporo-mandibular disorders was 67.0% for athletes and 49.8% for non-athletes groups(p<0.05). The experience of the use of mouth-guards was 83.3% for athletes and 78.8% for non-athletes, there were not significant differences between athletes and non-athletes groups (p>0.05). Risk factors found to be significantly related to the occurrence of maxillofacial injury were: unused mouth-guards(OR=2.83), no rules for use of mouth-guards(OR=2.46). Conclusions : Based on the findings, maxillofacial injuries experience is highly associated with mouth-guards usage, not only athletes also general people should be educated on the use of mouth-guard in order to reduce the incidence of maxillofacial injuries.

스포츠 선수들의 구강보호장치 사용실태 (Use of Mouth Guards among Sport Players)

  • 스포츠 선수는 경기 중 발생할 수 있는 몸싸움과 격렬한 동작으로 인해 항상 구강악안면 외상에 노출되어 있음에도 불구하고 구강보호장치는 불편하고 불필요하다는 인식이 강한편이다. 그러나 구강보호장치는 외부 충격 시 치아를 보호하고, 응력을 분산시키는 역할 등 스포츠 선수에게 없어서는 안 될 중요 예방대책이다. 그렇기 때문에 스포츠 선수들의 불편하고 불필요하다는 인식을 개선시켜줄 필요가 있고, 스포츠 선수의 구강 보호차원에서 구강보호장치의 사용을 일반화 하는 것이 중요하다 할 수 있다. 본 연구는 스포츠 선수 290명을 대상으로 구강보호장치 사용 실태를 조사하여 착용 경험율 6.6%, 현재 착용률 3.8%, 사용할 필요성 25.9%, 사용할 의사 28.3%인 결과를 얻어 스포츠 선수들의 구강보호장치 사용률과 인식모두 매우 낮은 실정을 알 수 있었고, 성별, 연령, 신체 접촉 정도, 구강악안면 외상경험, 외상 대처법 교육경험에 따라 구강보호장치 사용률에 변화가 있는지 분석한 결과, 사용률이나 인식이 남성과 고연령층이 높다는 것을 알 수 있었고, 신체 접촉이 많은 종목과 구강악안면 외상경험, 구강악안면 외상대처법 교육경험이 있는 스포츠 선수가 구강보호장치 사용률과 인식이 조금 더 높은 것으로 나타났다. 따라서 아직까지 스포츠 선수에게 구강보호장치에 대한 인식과 사용은 매우 낮은 실정이지만 경험과 교육을 통해 충분히 개선될 수 있고 향후 스포츠 선수들의 구강보호를 위해 많은 교육이 필요할 것으로 사료된다.

The sort of sports in which the player is majoring. 2. How long the players career is. 3. Whether the player has received prize or not. 4. The condition of stomatognathic system. 5. His or her eating habits. Results and conclusion: 1. Inconveniences should be minimized in wearing mouth guards. 2. The differential designs are thought to be needed for different sort of sports and for frequently affected regions 3. The changes in acknowledgment of athletes in mouth guards is necessary, that is mouth guard is very important & essential to prevent or minimize the traumatic damages. 4. Keeping in intimate and close contact with related athletic associations and putting athletes under the obligation to wear it for themselves, should be required.

스포츠전공 남학생의 구강악안면 외상과 보호구 착용 및 스트레스와의 관련성 (Correlation between maxillofacial injury, use of mouth guards and stress in physical education majoring male students)

  • Purpose : The purpose of this study was to investigate the correlation between stress and maxillofacial injuries in male students. Methods : The subjects were 386 male students who were 18 years or more. Mean age was $20.99{\pm}2.80$ years. Data were collected using a self-reported questionnaire from March 7 to March 28, 2013. We surveyed maxillofacial injuries, mouth guards use and stress in male students majoring physical education. The data were analyzed by Cochran's Mantel-Haenszel, McNemar test and logistic multiple regression. Results : Those who had clenching habit and maxillofacial pain accounted for 48.7%. The pain was 3.23 folds higher in clenching habit than those who had not (OR=3.23, p <.001). The more stress they had, the more clenching habit (OR=2.13) and pain(OR=1.68) did they have. Within 2 years, those having maxillofacial injury accounted for 53.2% and 78.6% of them put on maxillofacial protection guard. In rule for mouth guard use, 39.9% had no maxillofacial injury. Maxillofacial injury was 2.41 folds higher in those who had no mouth guard usee (OR=2.41). Conclusion : Maxillofacial injury had a close correlation with mouth guard use and stress. Therefore, it is very important to establish the rule for mouth guard use in sports activities.

마우스 가드가 신체 균형에 미치는 영향 (Effect on Body Balance due to mouth guard)

  • Purpose: This study, it was observed on the effect of material and thickness of the mouth guard on postural balance in order to assess the influence of the use of a mouth guard. Methods: The mouth guards of 10 adults selected as the experiment subject was sorted into each of hard and soft material, and these were made as thickness of 2.0 mm, 2.5 mm, 3.0 mm, 3.5 mm by each of the material. Results: The results of this study are expected to wear the mouthguard hard to influence positive influence on the improvement of the muscle strength, static balance. Conclusion: Suggest the need for additional research on dynamic balance.

스포츠 마우스가드의 기능성과 안정적인 착용감을 위한 제작 증례 (Fabrication of a sport mouse guard for performance and comfortable wearing)

  • Here, we studied the sports mouse guard as an oral device system, to minimize the sports related facial and dental injuries, jawbone fracture and brain injury, and by layering the hardened sheets for improving the activity performance and stable wearing. By pressuring and layering 2 soft- and 1 hard-layers of ethylene vinyl acetate (EVA) thermoplastic materials, for a category of martial art, record sports and leports, here we introduce a methodology for thickness control of layers to protect the teeth and oral structure. A personally customized mouse guard optimized for sports by layering a mixture of soft and hardened sheets is not easily detached during the sporting activity, easy to breathe through, comforts to wear, and also improves the sporting record. A designed EVA thermoplastic material for individual sports is used as the mouth guard, which is stably attached, easily removed, and convenient for breathing through the mouth.

체육전공 대학생의 구강악안면 외상 예방법 교육경험에 영향을 미치는 요인 (Influencing factors on oral and maxillofacial trauma prevention education experience of students majoring in physical education)

  • Objectives: The aim of this study was to investigate the influencing factors on oral and maxillofacial trauma prevention education experience of students majoring in physical education. Methods: A self-reported questionnaire was filled out by 268 students majoring in physical education in Busan. All statistical analyses were performed using SPSS 21.0. The questionnaire consisted of general characteristics(4 questions), TMJ symptoms(9 questions), oral habit(8 questions), and mouth guard awareness(5 questions). The questionnaire was carried out by 5 Likert scale. Higher points of Likert scale showed the negative tendency except the mouth guard awareness. The higher points of mouth guard awareness showed the positive tendency to use the mouth guard. Results: Male students had higher scores of 2.75 points in bad oral habit than the female students(p<0.05). Female students higher score of 1.30 points in mouth guard awareness than male students(p<0.05). TMJ symptoms experience in oral and maxillofacial trauma was 3.15 points which was higher than the prevention education experience without maxillofacial trauma(p<0.001). TMJ symptoms and Mouth guard awareness showed 2.71(p<0.01) and 1.20 points(p<0.001) respectively in students with maxillofacial trauma prevention education experience. These score were higher than those without education experience of trauma prevention education experience. The influencing factors on trauma prevention education experience are mouth guard awareness(p<0.001), oral habits(p<0.01), and temporomandibular joint symptom(p<0.01). Conclusions: The necessity of mouth guards should be emphasized to prevent the serious oral trauma. It is very important to establish the rule for mouth guard use in sports activities. Therefore, oral and maxillofacial trauma prevention education program is needed.

구강보호장치의 재료적인 특성이 하악골 충격 시악골 및 두부에 미치는 영향에 관한 유한요소분석 (FEM Analysis of the Effects of Mouth guard material properties on the Head and Brain under Mandibular Impact)

  • 연구목적: 본 연구의 목적은 하악골 충격 시 구강보호장치의 재료적 성질에 따른 악골과 두부에 미치는 영향을 조사하는 것이다. 연구재료 및 방법: 경질과 연질의 서로 다른 성질을 가지는 구강보호장치를 두부외상이나 뇌질환 병력이 없고 정상교합과 자연치열을 가진 한국 성인 여성에게 제작하고, CT를 이용하여 악골과 두부의 3차원 유한요소모델을 제작하였다. 이 유한요소모델은 두개골, 뇌, 상악골, 하악골, 관절원판, 치아와 구강보호장치를 포함하며 407,825개의 elements와 82,138개의 node로 구성되어 있다. 0.1초동안 하악골 3부위에서 800 N으로 사선 방향의 힘을 가하였을 때 두 가지 재료에 따른 상악 치아, 상악골 및 두개골의 응력분포를 평가하였고 동일한 조건하에서 두 재료에 따른 뇌의 변위량을 비교하였다. 결과: 이 연구를 통해 다음과 같은 결과를 얻었다. 1. 상악치아에서 유효응력 값을 비교하였을 때 연질의 구강보호장치가 경질의 구강보호 장치보다 응력값이 유의차 있게 작게 나타났다 (P < .05). 2. 상악골과 두개골에서 응력값을 비교하였을 때 연질의 구강보호장치가 경질의 구강보호장치보다 응력값이 유의차 있게 작게 나타났다 (P < .05). 3. 경질의 구강보호장치에서 연질의 구강보호장치보다 상악치아 및 상악골과 두개골에 더 많은 응력집중부위를 보였다. 4. 경질의 구강보호장치와 연질의 구강보호장치 간에 하악 충격 시 뇌의 변위량은 연질의 구강보호장치 쪽이 크게 나타났으나 유의차는 없었다. 본 연구를 통하여 악골 충격 시에 경질 및 연질 구강보호장치가 상악골과 두개골에서의 응력분포에 미치는 양상을 알 수 있었지만, 아직도 이의 상관관계를 모두 파악하기에는 불충분한 점이 많으리라 생각되며, 스포츠 외상에 따른 구강보호장치의 재료에 따른 상관관계에 대하여 보다 더 많은 연구가 필요할 것으로 사료된다.

Lesch-Nyhan Syndrome in Two Brothers: Dental Approach to Prevent Injuries Caused by Self-mutilative Behaviors

  • Lesch-Nyhan syndrome is a rare X-linked recessive disorder characterized by a deficiency of the enzyme hypoxanthine-guanine phosphoribosyl transferase, which is important in the metabolism of purines. Prevalence of Lesch-Nyhan syndrome is 1:100,000 to 1:380,000 live births, and Lesch-Nyhan syndrome mainly affects males. It is characterized by neurologic dysfunction, uric acid overproduction, and cognitive and behavioral disturbances. The self-mutilation is the most distinctive symptom in Lesch-Nyhan syndrome, appearing about 1 year of age. It is expressed as persistent bites in the oral mucosa, tongue, lips, and fingers, which leads to the total or partial destruction of the affected tissues. Several medical and dental management methods might be performed in Lesch-Nyhan syndrome patients. Dental approaches to prevent injuries caused by self-mutilation include treatments with oral appliances such as mouth guards or lip bumpers, extraction, and orthognathic surgery. This report described two brothers with Lesch-Nyhan syndrome, who injured themselves on oral tissues. The methods of tooth reduction and extraction were performed to prevent injuries caused by self-mutilation.
