Recanalization Rate and Clinical Outcomes of Intravenous Tissue Plasminogen Activator Administration for Large Vessel Occlusion Stroke Patients

  • Objective : Stroke caused from large vessel occlusion (LVO) has emerged as the most common stroke subtype worldwide. Intravenous tissue plasminogen activator administration (IV-tPA) and additional intraarterial thrombectomy (IA-Tx) is regarded as standard treatment. In this study, the authors try to find the early recanalization rate of IV-tPA in LVO stroke patients. Methods : Total 300 patients undertook IA-Tx with confirmed anterior circulation LVO, were analyzed retrospectively. Brain computed tomography angiography (CTA) was the initial imaging study and acute stroke magnetic resonance angiography (MRA) followed after finished IV-tPA. Early recanalization rate was evaluated by acute stroke MRA within 2 hours after the IV-tPA. In 167 patients undertook IV-tPA only and 133 non-recanalized patients by IV-tPA, additional IA-Tx tried (IV-tPA + IA-Tx group). And 131 patients, non-recanalized by IV-tPA (IV-tPA group) additional IA-Tx recommend and tried according to the patient condition and compliance. Results : Early recanalization rate of LVO after IV-tPA was 12.0% (36/300). In recanalized patients, favorable outcome (modified Rankin Scale, 0-2) was 69.4% (25/36) while it was 32.1% (42/131, p<0.001) in non-recanalized patients. Among 133 patients, non-recanalized after intravenous recombinant tissue plasminogen activator and undertook additional IA-Tx, the clinical outcome was better than not undertaken additional IA-Tx (favorable outcome was 42.9% vs. 32.1%, p=0.046). Analysis according to the perfusion/diffusion (P/D)-mismatching or not, in patient with IV-tPA with IA-Tx (133 patients), favorable outcome was higher in P/D-mismatching patient (52/104; 50.0%) than P/D-matching patients (5/29; 17.2%; p=0.001). Which treatment tired, P/D-mismatching was favored in clinical outcome (iv-tPA only, p=0.008 and IV-tPA with IA-Tx, p=0.001). Conclusion : The P/D-mismatching influences on the recanalization and clinical outcomes of IV-tPA and IA-Tx. The authors would like to propose that we had better prepare IA-Tx when LVO is diagnosed on initial diagnostic imaging. Furthermore, if the patient shows P/D-mismatching on MRA after IV-tPA, additional IA-Tx improves treatment results and lessen the futile recanalization.

Compact UWB Band-pass Filter with Open-Stub Using Impedance Mismatching and SIR (임피던스 부정합 및 SIR을 적용한 개방형 광대역 소형 스터브 대역통과 여파기)

  • In this paper, the UWB (Ultra Wide Band) band-pass filter (BPF) with compact-size using impedance mismatching in transmission line and SIR (Stepped Impedance Resonator) instead of open stubs is presented. The proposed BPF have 103 % of bandwidth and 11.2 GHz of center frequency, respectively. In additional, the operation frequencies of the suggested BPF are 4.8 GHz to 16 GHz. In this structure, the length of the transmission line is reduced to half compared with the original one by impedance mismatching technique with low frequency band (sub harmonics) and harmonic components. Also, the open stub can be used for SIR due to reduced size. Experimental results show that the insertion and return losses are 0.35 dB and 15.1 dB, respectively and the filter size is $8.92{\times}10.6mm^2$. The proposed BPF is in good agreement.

  • An algorithm for the threshold decision from the maximum mismatching correlation value in a direct-sequence spread-spectrum system is presented. This algorithm is named the TDMMC(Threshold Decision from the Maximum Mismatching Correlation value). The purpose of the algorithm is to set the decision threshold in the system which will provide large probability of signal detection. Using this algorithm, the proper setting of the threshold for various SNRs is possible. An additional block called the Threshold Block is used to improve the system performance. The result from the computer simmulation has shown that appling the TDMMC to the noncoherent DS/SS-CSK system can achieve performance improvement.

  • The transform domain communication system (Hereinafter referred to as TDCS) takes on numerous advantages, inclusive of anti-jamming and low probability of detection. Yet its application is confined by the consistent spectrum in the transmitter and receiver, which is not possible in the case of a huge distance exsits between them. In this paper, a TDCS based modified weighted fractional fourier transform (WFRFT) is proposed to solve the problem resulting from spectrum mismatching for TDCS application. The amplitude and phase information are incorporated with the TDCS signals and transmit to the receiver together in the wake of a modified WFRFT. The basic function and the TDCS signals shall be accessible to the receivers in the wake of an inverse WFRFT transform, which make sure that the original information can be demodulated properly. The system's reliability while transmitting signals with different modulation methods and with spectrum mismatching is demonstrated by bit error rate (BER). In the meantime, the constellations of the signals and the BER performances at the eavesdropper demonstrate the proposed system is better secured.

  • The purpose of this research is to find a solution for matching service between recruiting and job seeking of software developers. An efficient matching service solution is essential for acquiring solid global competitiveness in software industry and IT academia. An in-depth analysis on deep-rooted problems on mismatching has been performed. Past and present policies on mismatching solutions have been reviewed based on desirable co-growth structure. A new matching solution has been derived and a triple-helix strategy has been suggested. A survey result for 14 domain experts and 85 employees of large and small software companies shows that the suggested matching solution is useful. The results of this research can be used for finding a better matching service solution and policies.

  • A computerization project was completed in Korea to computerize the paper cadastral maps in sheet and tile format in 2003. There were errors, which had not been found during the computerization project, as a part of efforts to implement the cadastral database. The drawing mismatching one of those errors, which has caused inconvenience to surveyors in providing the survey results in the cadastral survey. This study suggested the plan for the preparation of the accurative continuous cadastral mapping by field surveying eliminating the drawing mismatching, which may cause inconvenience to surveyors during the field surveying.

  • The present work is concerned with three-dimensional finite element analysis of the hollow section extrusion process using a porthole die. The effects of related design parameters are discussed through the finite element simulation for extrusion of a triply-connected rectangular tubular section. For economic computation, mismatching refinement, an efficient domain decomposition method with different mesh density for each subdomain, is implemented. In order to obtain the uniform flow at the outlet, design parameters such as the hole size and the hole position are investigated and compared through the numerical analysis. Comparing the velocity distribution with that of the original design, it is concluded that the design modification enables more uniform flow characteristics. The analysis results are then successfully reflected on the industrial porthole die design.

  • The present work is concerned with three-dimensional finite element analysis of the hollow section extrusion process using a porthole die. For economic computation, mismatching refinement, an efficient domain decomposition method with different mesh density for each subdomain, is implemented. The proposed method improves the computational efficiency significantly, especially fur the three-dimensional analysis of extrusion problems. As a numerical example, extrusion of the underframe part of a railroad vehicle are analyzed. For three-dimensional mesh generation of a complicated shape with hexahedral elements, a modified grid-based approach with the surface element layer is utilized. The analysis results are then successfully reflected on the industrial porthole die design.

  • The rigid-plastic finite element analysis requires a large amount of computation time due to its non-linearity. For economic computation, mismatching refinement, and efficient domain decomposition method with different mesh density for each sub domain, is developed. A modified velocity alternating scheme for the interface treatment is proposed in order to obtain good convergence and accuracy. As a numerical example, the axisymmetric extrusion process is analyzed. The results are discussed for the various velocity update schemes form the viewpoint of convergence and accuracy. The three-dimen-sional extrusion process with rectangular section is analyzed in order to verify the effectiveness of the proposed method. Comparing the results with those of the conventional method of full region analysis, the accuracy and the computational efficiency of the proposed method are then discussed.

  • In this paper, I'm tying to define the connection of the Device Characteristics and the Error Terms, and yield maximum estimated value of the mismatching errors by modeling the individual devices with signal-flow graph, and by seeking the transfer function of the system with the mismatching error in the combination of the devices using the Signal-flow Graph Gain Formula.
