Study of the Drugs Prescribed on Alzheimer's Disease: from the Insurance Claims Data of Korea National Health Insurance Service (우리나라 건강보험 청구자료를 이용한 알츠하이머성 치매 치료제의 사용현황 분석)

  • Objective: The aims of this study are to investigate the total volume of prescribed medicines against Alzheimer's disease (AD) and the trends of usage by analyzing the claims-data from the Korea National Health Insurance Service. Method: The demographic and claims-data were included the major AD treating medicines such as donepezil, galantamine, rivastigmine and memantine, and analyzed during the period of 2010~2012. The assessing criteria were gender, age, habitation, types of medical institution, code of ingredients, outcomes of treatment, volume and amount of claims, and the numbers of patients with dementias. After trimming the data, it were analyzed by the market size, demographic traits, characteristics of medical service, characteristics of each anti-AD medicine, etc. Results: Among the chosen 4 medicines, donepezil had the top prescription volumes. Most prevalent prescribing preparations of donepezil were conventional types. However, among the non-conventional types, oro-dispersible formulation is the fast increasing one in both volume and growth rate. This specialized preparations to improve both toleration and adherence, tend to being prescribed generally at the tertiary medical institutions. While the younger patients with mild-to-moderate AD mostly treated by expensive medicines in resident at the tertiary hospitals, the rest older patients with severe AD have been treated non-expensive one at long-term care facilities. Conclusion: AD is a chronic illness therefore, long-term use of therapeutic medications are highly important. If an anti-AD treatment was applied steadily in the earlier stages, it would be achieved not only improving the quality of life of patient but also reducing the expenses in the medical and nursing cares. As the socioeconomical impacts of AD is expanding, healthcare professionals need to aware the importance of pharmacotherapy and to improve sociopolitical fundamentals.

Safety of Ojeok-san Extract Powder and Soft Extract in Healthy Male Volunteers, Single Center, Randomized Controlled, Cross-over Study (오적산 혼합단미엑스산제와 혼합단미연조엑스제의 안전성 평가를 위한 단일 기관, 무작위 배정, 교차 연구)

  • Objectives The purpose of this study is to evaluate the safety of Ojeok-san extract powder and soft extract in healthy male volunteers. Methods Randomized controlled, cross over study was carried out in healthy male volunteers. Total 27 of 31 subjects meeting the inclusion criteria were enrolled and three subjects for waiting were included. To each group 12 subjects were randomly allocated by random number table. Group A took the extract powder in the first period and then took the soft extract in the second period. Group B took the medicine in the opposite order. Trial was conducted through two times of hospitalizations and all subjects had a seven-days of wash out period. Vital sign and laboratory test were checked before and after the medication. The mean difference of safety evaluation variables were analyzed by paired t-test (p<0.001) or wilcoxon signed rank test (p<0.05). The mean difference between two groups were analyzed by independent t-test (p<0.05) or Mann whitney test (p<0.05). Results As a result of all data related to vital sign and laboratory test in both group, There were no significant differences associated with the clinical trial drug between before and after the medication. And there was no adverse event associated with the clinical trial drug. Conclusions Both Ojeok-san extract powder and soft extract were found to be safe for healthy male volunteers.

Analyzing the Projection of Physician Workforce Supply and Demand in Gyeongsangnam-Do, South Korea, through System Dynamics (시스템 다이내믹스를 활용한 경상남도 의사인력 수급추계를 통한 적정성 연구)

  • Background: The adequate provision of medical care relies on the availability of a suitable number of healthcare professionals. To ensure stability in healthcare delivery, it is crucial for a country to accurately estimate and address the supply of doctors. This study aims to contribute to the formulation of effective policies for securing and distributing doctor manpower, with a focus on medically underserved areas at both the national and local government levels. Methods: Employing the system dynamics methodology, this research utilizes stock and flow diagrams, including level and rate diagrams, to quantitatively analyze the cumulative structure of the doctor supply and demand system. Results: The analysis reveals a substantial shortage of clinical doctors in Gyeongsangnam-do, amounting to 15,477 as of 2021. Projections indicate a need for an additional 7,570 doctors by the year 2050 to maintain the current healthcare service level. Examination of medical treatment rights and distribution across cities and counties indicates an insufficiency in doctor supply relative to demand in the majority of regions. Alternative scenarios, such as increasing medical school enrollments and adjusting retirement ages, were explored, yet none provided a sufficient resolution to the shortage. Conclusion: The findings underscore an impending exacerbation of the doctor shortage in Gyeongsangnam-do if the existing system is perpetuated. Addressing this issue necessitates not only augmenting the number of medical school students and adapting retirement age policies but also implementing diverse strategies employed successfully in other countries. This study serves as a foundational step in informing evidence-based policies aimed at securing an ample and appropriately distributed doctor workforce for sustainable healthcare delivery.

A Study of Okchun-San Formulation for Freeze Drying Capsulation (동결 건조법을 이용한 옥천산 캡슐 제형 개발에 관한 연구)

  • Objective : Okchun-San(OCS) is known as an effective herbal medicine on Type 2 diabetes. We performed to change OCS formulation for freeze drying capsulation. Methods The mixtures of OCS were extracted with water. finally, the filtered solution were evaporated and lyophilized to dry granules. The various ratio of excipients were studied to determine the formation for capsulizing. The samples were inspected for any difference in color, taste and appearance. Results: The prepared form of OCS were dried and weighted 260kg. The lyophilized dry powder yielding 40kg. The suitable ratio of OCS-dry powder and excipient was 10:1. The average weights of On and placebo capsules were $440{\pm}5.28mg$, $465{\pm}7.95mg$, respectively. There was no notable change in color and appearance for both capsuled samples throughout the study period. Conclusions: Therefore in can be concluded that freeze drying capsulation is appropriate form of OCS.

Prescriptions Utilizing Vegetable Oils in 『Donguibogam』(『東醫寶鑑』) (『동의보감(東醫寶鑑)』중 식물성 기름(油)을 활용한 방제)

  • Object : To investigate which diseases the utilization of various vegetable oils were used and how they were used for treatment, and to change and develop vegetable oil combined prescriptions so that the Korean medical treatment effect in clinical practice can be enhanced. Methods : All words containing oil(油) were searched and selected oils used for simple prescription and oil combined prescription. Aii of animal oils such as butter(酥油), Suis oil(猪油), Anceris oil(鴈油), and Actonis caro oil(猯油) were excludIt is suggested to develop a new formulation by succeeding the utilization methods of various effects of sesame oil and other vegetable oils.ed. All selected oils were classified as sesame oil(麻油)(inclusive, 脂麻油, 胡麻油, 香油, 淸油) or other vegetable oils. Result : 1. In 『Donguibogam』(『東醫寶鑑』), 14 other vegetable oils were used to 23 prescriptions. 2. Sesame oil was used to 187 prescriptions over 41 disease categories as of 20 effects described in the 『Zhenghebencao』(『政和本草』). 3. In 28 prescriptions, sesame oil was also used for hygiene and prevention, emetic, preparating herbs and so on. Conclusion : The 101 prescriptions used for medicinal auxiliary materials are due to the efficacy of treatments of malignant furuncles, sores, scabies, promoting regeneration of tissue and flesh, reliving pain, dispersing abscess and swelling and supplementing fissured skin. In 『Donguibogam』(『東醫寶鑑』), the unique efficacy of each vegetable oil and sesame oil was used in various ways and preparation forms to treat various diseases. It is suggested to develop a new formulation by succeeding the utilization methods of various effects of sesame oil and other vegetable oils.

Scientific Analysis of Formulation Theory of Chungpesagan-tang; The purgative Action of Chungpesagan-tang

  • To analyze scientifically the fundamental formulation theory and drug interaction on the purgative action of Chungpesagan-tang, which is one of the traditional Korean medical prescriptions that has been most frequently used for stroke, water extracts of herbal medicines with Rhei Rhizoma were administered into mice and the purgative action was measured. Water extracts of Raphani Semen or Cimicifugae Rhizoma with Rhei Rhizoma had more potent purgative activity than Rhei Rhizoma alone, although Raphani Semen alone displayed no purgative action. However, Platycodi Radix and Puerariae Radix inhibited the purgative action of Rhei Rhizoma. When Rhei Rhizoma with Raphani Semen or Cimicifugae Radix was extracted with water, sennoside A content in the water extract was increased. However, Puerariae Radix and Platycodi Radix decreased the content of sennoside A in the extracted water. The transforming rate of sennoside A of Rhei Rhizoma was inhibited by Puerariae Radix and Platycodi Radix. The transforming rate of sennoside A of Chungpesagan-tang to rheinan-throne by human intestinal microflora and its purgative activity were similar to those of Rhei Rhizoma. Finally we thought that the purgative action of Chungpesagan-tang could be controlled by the addition and subtraction of Chungpesagan-tang-composing herbal medicines.

Photoprotective effects of topical ginseng leaf extract using Ultraflo L against UVB-induced skin damage in hairless mice

  • Background: Abnormal activation of matrix metalloproteinases (MMPs) plays an important role in UV-induced wrinkle formation, which is a major dermatological problem. This formation occurs due to the degeneration of the extracellular matrix (ECM). In this study, we investigated the cutaneous photoprotective effects of Ultraflo L treated ginseng leaf (UTGL) in hairless mice. Methods: SKH-1 hairless mice (6 weeks of age) were randomly divided into four groups (8 mice/group). UTGL formulation was applied topically to the skin of the mice for 10 weeks. The normal control group received nonvehicle and was not irradiated with UVB. The UV control (UVB) group received nonvehicle and was exposed to gradient-UVB irradiation. The groups (GA) receiving topical application of UTGL formulation were subjected to gradient-UVB irradiation on $0.5mg/cm^2$ [GA-low (GA-L)] and $1.0mg/cm^2$ [(GA-high (GA-H)] of dorsal skin area, respectively. Results: We found that topical treatment with UTGL attenuated UVB-induced epidermal thickness and impairment of skin barrier function. Additionally, UTGL suppressed the expression of MMP-2, -3, and -13 induced by UVB irradiation. Our results show that topical application of UTGL protects the skin against UVB-induced damage in hairless mice and suggest that UTGL can act as a potential agent for preventing and/or treating UVB-induced photoaging. Conclusion: UTGL possesses sunscreen properties and may exhibit photochemoprotective activities inside the skin of mice. Therefore, UTGL could be used as a potential therapeutic agent to protect the skin against UVB-induced photoaging.

Therapeutic Effect of Majoon Mundi and Qairooti Karnab in Dā al-Ṣadaf (Psoriasis): A Case Series

  • Introduction: Psoriasis is a major health concern around the world. Physicians of the Unani system of medicine have been treating psoriasis for centuries. Aim: The purpose of our study was to assess the effect of Majoon Mundi (a semisolid Unani medication intended for oral intake used as blood purifier) and Qairooti Karnab (a Unani medication in paste form intended for topical application used as emollient) in the treatment $na{\ddot{i}}ve$ psoriasis cases and to collect data to warrant further clinical trials. Material and Methods: Psoriasis cases were diagnosed clinically. Data were collected during treatment of five patients of psoriasis treated with the Majoon Mundi (oral intake of 5 gm twice daily with 200 ml of water for 12 weeks) and Qairooti Karnab (topical application on affected sites twice a day for 12 weeks). Patients were treated for 12 weeks. Treatment response was seen with clinical improvement in skin lesions and measurement of Psoriasis Area and Severity Index (PASI Scoring) before and after treatment. Results: Reduced PASI Score was observed in all five patients after 12 weeks of treatment [PASI before and after treatment was (mean${\pm}$SD) $20.7{\pm}4.6$ vs. $3.2{\pm}1.8$; p-value <0.05.]. Clinical improvement was noticed within an average of 4 weeks of treatment. Conclusion: Preliminary findings indicate the potential therapeutic role of Majoon Mundi and Qairooti Karnab in the treatment of psoriasis. Clinical trials based on this Unani pharmacopeial formulation should be conducted to explore the therapeutic potential of this formulation in psoriasis

Anti-inflammatory Effects of Ojeok-san in LPS-induced Inflammatory Rat Model (LPS유도 염증 동물모델에서 오적산의 항염증 효과)

  • Objectives: The aim of this study is to investigate the anti-inflammatory effects of Ojeok-san and compare the therapeutic effects according to its formation. Methods: We evaluated the anti-inflammatory effects of Ojeok-san using lipopolysaccharide (LPS) induced inflammatory animal model. Male SD rats were administered intra-orally with two different formulation types of Ojeok-san according to prescribed dosage. One hour later, to induce inflammatory responses, subsequent intra-peritoneal injection of LPS was conducted. After 5 hours later, serum TNF-α, IL-1β, IL-6 and PGE2 levels were measured by ELISA to assess the alteration of pro-inflammatory markers. Results: In our experiment, regardless of its formation, administration of Ojeok-san decreased TNF-𝛼, IL-1𝛽, IL-6 and PGE2 level in serum. Furthermore, LPS-induced toxicity of liver and kidney was not detected by Ojeok-san administration. Conclusions: The anti-inflammatory effect of Ojeok-san was shown in LPS-induced inflammatory model by decreasing pro-inflammatory markers, and there would be no significant difference in therapeutic effect between two formulation types of Ojeok-san.

On Modeling and Analyzing Signaling Pathways with Inhibitory Interactions Based on Petri Net

  • In this paper we discuss the formulation and the analysis of a signaling pathway by Petri nets. In order to explicitly and formally describe the molecular mechanisms and pathological characteristics of signaling pathways, we propose a new modeling method to construct signaling pathways on the basis of formal representation of Petri net. Our proposed extended algorithm effectively finds basic enzymic components of signaling pathways by employing T-invariants of Petri nets with considering the origination leading to an occurrence of inhibition functions than existing methods. An application of the proposed algorithm is given with the example of Interleukin-1 and Interleukin-6 signaling pathways.
