Purpose: To assess the epidemiology and the clinical features of Glufosinate herbicide in Korea. Methods: Data was prospectively collected during 1 year since August 2005 from 38 hospital in Korea. We analyzed the epidemiologic characters and clinical manifestations of Glufosinate poisoning. In addition, the characteristics of patients with severe central nervous system toxicity were separately analyzed to find poor prognosis relating factors. Results: During study periods, there were 715 persons of poisoning of herbicides and insecticides. 6.3% (45 persons) of the agricultural chemicals poisoning had Glufosinate poisoning. There were 36 cases of suicide attempts and 7 cases of accidental exposure, The major of poisoning route was oral ingestion (44 cases). 28.9 % of the study patients had not toxic symptoms. The most common symptoms were gastrointestinal symptoms relating surfactant irritation. 67.7% of central nervous symptoms occurred lately. 10 persons showed severe central nervous system toxicity. 4 persons of them showed poor outcomes (1 death, 3 hopeless discharged). Complications of respiratory failure and renal failure related with poor outcome. Conclusion: Majority of patients ingested Glufosinate for suicide attempt. 22.2% of patients with Glufosinate poisoning showed delayed serious central nervous system toxicity. Early supportive care of altered mentality may prevent late respiratory complications and improve the outcomes.
A left ventricular rupture might be one of the most disastrous complications after a mitral valve replacement. An acute atrioventricular groove rupture (type I) was detected in a 54-year-old female diagnosed with a mitral stenosis combined with severe tricuspid regurgitation. She had a prior medical history of an open mitral commissurotomy in Japan at 30 years ago. The surgical findings suggested that the previous procedure was not a simple commissurotomy but a commissurotomy combined with a posteromedial annuloplasty procedure. After a successful mitral valve replacement and a measured (De Vega type) tricuspid annuloplasty, the weaning from a cardiopulmonary bypass was uneventful. However, copious intraoperative bleeding from the posterior wall was detected and the cardiopulmonary bypass was restarted. Exposure of the posterior wall of the left ventricle showed bleeding from the atrioventricular groove 3 cm lateral to the left atrial auricle. Under the impression of a Type I left ventricular rupture, epicardial repair (primary repair of the Teflon felt pledgetted suture, continuous sealing suture using auto-pericardial patch and application of fibrin-sealant) was attempted. Successful local control was made and the patient recovered uneventfully. The patient was discharged at 14 postoperative days without complications. We report this successful epicardial repair of an acute type I left ventricular rupture after mitral valve replacement.
This study evaluated the concentrations of urinary metabolites of polycyclic aromatic hydrocarbons (PAHs) in industrial waste incineration workers. The effect of genetic polymorphisms of xenobiotic metabolism enzymes on urinary concentration of PAH metabolites was assessed. And, aromatic DNA adduct levels were also determined in total white blood cells. Fifty employees were recruited from a company handling industrial wastes located in Ansan, Korea: non-exposed group (n=21), exposed group (n=29). Sixteen ambient PAHs were determined by GC/MSD (NIOSH method) from personal breathing zone samples of nine subjects near incinerators. Urinary 1-hydroxypyrene glucuronide (1-OHPG), a major pyrene metabolite, was assayed by synchronous fluorescence spectroscopy after immunoaffinity purification using monoclonal antibody 8E11 (SFS/IAC). Multiplex PCR was used for genotyping for GSTMI/TI and PCR-RFLP for genotyping of CYP1A1 (MspI and Ile/Val). PAH-DNA adducts in peripheral blood WBC were measured by the nuclease P1-enhanced postlabeling assay. Smoking habit, demographic and occupational information were collected by self-administered questionnaire. The range of total ambient PAH levels were 0.00-7.00 mg/㎥ (mean 3.31). Urinary 1-OHPG levels were significantly higher in workers handling industrial wastes than in those with presumed lower exposure to PAHs (p=0.006, by Kruskal-Wallis test). There was a statistically significant dose-response increase in 1-OHPG levels with the number of cigarettes consumed per day (Pearson correlation coefficient=0.686, p<0.001). Urinary 1-OHPG levels in occupationally exposed smoking workers were highest compared with non-occupationally exposed smokers (p=0.053, by Kruskal-Wallis test). Smoking and GSTMI genotype were significant predictors for log-transformed 1-OHPG by multiple regression analysis (overall model R²=0.565, p<0.001), whereas smoking was the only significant predictor for log-transformed aromatic DNA adducts (overall model R²=0.249, p=0.201). Aromatic DNA adducts was also a significantly correlation between log transferred urinary 1-OHPG levels (pearson's correlation coefficient=0.307, p=0.04). However, the partial correlation coefficient adjusting for Age, Sex, and cigarette consumption was not significant (r=0.154, p=0.169). The significant association exists only in individuals with the GSTMI null genotype (pearsons correlation coefficient=0.516, p=0.010; partial correlation coefficient adjusting for age, sex, and cigarette consumption, r=0.363, p=0.038). Our results suggest that the significant increase in urinary 1-OHPG in the exposed workers is due to higher prevalence of smokers among them, and that the association between urinary PAH metabolites and aromatic DNA adducts in workers of industrial waste handling may be modulated by GSTMI genotype. There results remain to be confirmed in future larger studies.

Objectives: We measured the density of smoke and harmful gases emitted from burning adhesive indirect moxa. Through the test we aimed to find out if there was an excessive amount of smoke emitted and if it included harmful gases. Methods: 9 types of adhesive indirect moxa were chosen. The buffer layers which do not burn during treatment were removed and 10g of each moxa were made into powder and put into a holder. A smoke density chamber (Smoke Density Chamber FTT. U.K) and Fourier transform infrared spectroscopy (FT-IR I 4001. MIDAC U.S.A.) were used to measure the density of smoke and harmful gases emitted from burning moxa by ISO 5659-2 test. Results: The result of measuring maximum smoke density showed that the regular indirect adhesive moxa (A-F) emitted high density smoke of 172.1-291.4Ds. The smokeless moxas, Seoam moxas, emitted the least amount of 3.4-5.5Ds. Concentrations of 7 typical harmful gases (CO, HCl, HCN, HBr, HF, SO2, NOx) were measured and all of the moxas emitted CO due to incomplete combustion. 4 types of moxa emitted NOx and all smokless moxas emitted NOx. HBr, HCN, HCl, HF, SO2 were not found in any of the moxas. Conclusions: The amount of harmful gases emitted from burning moxa was much lower than short-term exposure standards of chemical and physical factors (Ministry of Labor 2010-44). Further experiments measuring gases from moxa combustion should be done in larger environments similar to normal medical clinics.
Despite the government operating various preventive programs to ward off accidents and diseases on business sites, diseases rates are not decreasing, unlike accident rates. In many cases, diseases caused by work have a latent period before symptoms appear or progress over a longer term, making it difficult for workers to prove the causal relation between their work and the diseases. Moreover, data related to the business site are mostly owned by the employer. Even if the employee has access to parts of such data, his lack of medical expertise limits his ability to identify the characteristics of the diseases and how it appears. In August, 2017 the Supreme Court did an about-face with its ruling on the case involving diseases caused by exposure to harmful substances in work environments, by easing the burden of proof on the employees. As such, this study focuses on the case to analyze cases involving diseases that have occurred in work environments and present their implications. In doing so, the study seeks to provide a basic set of data that can help secure the employees' labor rights and rights to health by complementing the current law in relation to recognizing industrial incidents caused by rare diseases and making work environments safer for employees.
A social phobia is an anxiety disorder characterized by extreme fear and phobic avoidance of social and performance situations. Medications or cognitive-behavior methods have been mainly used in treating it. These methods have some shortcomings such as being inefficient and difficult to apply to treatment. Lately the virtual rcality technology has been applied to dcal with the anxiety disorders in order to compcnsate for these defects. A virtual environment provides a patient with stimuli which cvokes a phobia. and the patient's exposure to the virtual phobic situation make him be able to overcome it. In this study, we suggested the public speaking simulator based on a personal computer for the treatment of social phobia. The public speaking simulator was composed of a position sensor. head mount display and audio system. And a virtual environment for the treatment was suggested to be a seminar room where 8 avatars are sitting. The virtual environment includes a tracking system the trace a participant's head-movement using a HMD with position sensor and 3D sound is added to the virtual environment so that he might fcel it realistic. We also made avatars' motion and facial expression change in reaction to a participant's speech. The goal of developing public speaking simulator is to apply to treat fear of public speaking efficiently and economically. In a future study. we should get more information about immergence and treatment efficiency by clinical test and apply it to this simulator.
Clostridium perfringens (C. perfringens) and Mycobacterium fortuitum (M. fortuitum) are associated with considerable diseases in animals and human. In this study, the disinfection efficacy of a commercial disinfectant composed to povidone-iodine (PVI) was evaluated against C. perfringens and M. fortuitum. A bactericidal efficacy test by broth dilution method was used to determine the lowest effective dilution of the disinfectant following exposure to C. perfringens and M. fortuitum for 30 min at $4^{\circ}C$. The disinfectant and test bacteria were diluted with hard water (HW) or organic matter suspension (OM) according to treatment condition. On HW condition, the bactericidal activity of the disinfectant against C. perfringens and M. fortuitum was 50 and 80 fold dilutions, respectively. On OM condition, the bactericidal activity of the disinfectant against both C. perfringens and M. fortuitum was 15 fold dilutions. As the disinfectant composed to PVI possesses bactericidal efficacy against C. perfringens and M. fortuitum, the disinfectant solution can be used to control the spread of bacterial diseases.
Manufacturing facilities of the pharmaceuticals must meet certain level of the cleanness required so that foreign substances such as dust, moisture, heat, microorganism, or virus do not contaminate the product. In case of radiopharmaceuticals for medical treatment and diagnosis, not only should the operators and environment be protected from radiation but also need to be isolated from the foreign contaminant. Therefore, manufacturing facilities for radiopharmaceuticals must satisfy the design standards of both hot cell and clean room which are specified by GMP. However, standards of maintaining negative pressure for preventing spread of radioactive contaminant in isolated facilities conflict with the standards of maintaining positive pressure for keeping cleanness. To solve this problem, air pressure of hot cell was designed lower than in the adjacent area to meet standards of the radiation safety. To keep higher cleanness in certain part of the hot cell for filling, minimal relative positive pressure allows. In order to effectively maintain the cleanness that is required for production of Tc-99m generator, which takes 70% of whole demand of radiopharmaceuticals, the rooms placed in each side of production room are used as a buffer area and three lead hot cells are installed in production room. In this research, we established the appropriate engineered design concept for Tc-99m generator manufacturing facility, which satisfies both GMP cleanness standard for preventing particles, bacteria, other contaminants and the regulations of radiation safety for supervising and controlling the amount of radiation exposure and exhausted radioactivity. And the concept of multi-barrier buffer zones is introduced to apply negative air pressure for hot cell with first priority and to continue relative positive air pressure for clean room.

The aim of this study was to investigate vitamin D status and factors associated with vitamin D deficiency among Koreans in their using Korean National Health and Nutrition Examination Survey (KNHANES) V. The subjects were 1,837 people who were 20-29 years old. The data were analyzed by ${\chi}^2$ test and multiple logistic regression analysis using weighted composite sample analysis. Vitamin D deficiency was reveled in 84.2% of subjects; 88.6% of women and 79.4% of men showed deficiency. In the multiple logistic regression analysis, the risk for vitamin D deficiency was higher in women (OR 1.84; 95% CI 1.31-2.56), non-married (OR 2.12; 95% CI 1.41-3.19), poor subjective health status (OR 1.94; 95% CI 1.16-3.25), and in the absence of moderate to vigorous physical activity (OR 3.38; 95% CI 1.85-6.16). Continued research on vitamin D levels at this time is needed since vitamin D deficiency among Koreans in their twenties will exacerbate health problems, both individually and socially. In addition, vitamin D has been shown to be closely related to time of exposure to sunlight and physical activity. Therefore, it is thought that long-term and planned efforts should be made at the national level, such as establishing schools and workplaces in which those in their twenties can perform outdoor activities and moderate to vigorous physical activity.

Diagnosis X-ray equipment localized at 1950's but it is developed suddenly at 1960's with demand together. Manufacture of Diagnostic X-ray equipment is controled by the KS regulation and the Ministry of Health and Welfare because of hazardous element etc. exposure by radiation. Most of diagnostic X-ray equipment ware single phase and three phase full-wave rectification but from 1980's it transforms it was exchanged in inverter type X-ray equipment. Inverter type X-ray equipment produces approximately 50~80% more average photon intensity then single phase full-wave rectification and the accuracy is high. But from a clinic it dose not use because expensive therefor the efficiency improvement of single phase full-wave rectification is necessary. We produced single phase full-wave rectification X-ray equipment control unit, high tension transformer, filament heating transformer, rectification circuit, high tension cable and others and evaluated efficiency, in result which is excellent compare with Rule of Safety Management and KS regulation.
