High Yielding [18F]Fluorination Method by Fine Control of the Base

  • New [$^{18}F$]F-fluorination methods using a minimized amount of precursor has been developed by controlling the base concentration. In the first method, pre-conditioning of the anion exchange cartridge with $K_2CO_3$ solution or water was carried out. The trapped [$^{18}F$]fluoride on the cartridge was then eluted by KOMs or KOTf solution. [$^{18}F$]F-Fluorination could be performed without additional base. In the second method, the QMA cartridge was preconditioned with KOMs solutions. Trapped [$^{18}F$]fluoride on the QMA was then eluted with KOMs and additional base, such as KOH, $K_2CO_3$, and $KHCO_3$, was added into the reaction vessel. Method 1 showed a [$^{18}F$]F-incorporation yield of 20.9% for [$^{18}F$]FLT synthesis with 5 mg of precursor. Unlike method 1, a [$^{18}F$]F-incorporation yield of 91.4% was achieved from the same amount of precursor in method 2.

  • Flavonoids with oxygen atoms are known to have potent biological effect.They have been studied long as major antioxidants which protect cell membranes. Recent medical surveys show that increased intake of selenium decreases the risk of breast, colon, lung and prostrate cancer by preventing free radical generation. The flavonoids, isoflavonoids, and coumarins which form the bulk of these compounds are very polar and have limited use as drugs which have to pass through BBB(Brain Blood Barrier)The non-polar property is increased by exchange oxygen to selenium as a part of heterocyclic compound.   • Medical robot has four fields. Surgery assistant robot, robotic surgery, Surgery Simulator, rehabilitation robot. Thus, medical robots is often high precision and reliability requirements for operations are being developed. Medical service robot's another sector is care for service robots. Care services robot is the hospital's reception work and biometric data acquisition of patients, the hospital in location and content information provide to patients. But now medical service robot practical acceptance process failed to progress. In this paper were the medical service robot systems design and implementation. Implemented the robot system is using the standard protocols for the exchange of medical information and can be linked with hospital information system. The hospital's patient reception and processing, to provide care waiting number information.

Mirror Syndrome Resulting from Metastatic Congenital Neuroblastoma to Placenta

  • Congenital neuroblastoma is a rare disease. Placental metastasis is extremely rare and poor prognosis has been reported in neonates. Mirror syndrome could occur in mother with placental metastasis with possibilities of hypertension and edema. We report a case of detection of left suprarenal mass in fetus at $31^{+5}weeks^{\prime}$ gestation. Mother presented with palpitation, edema, headache, and hypertension. Maternal 24 hours urine vanillylmandelic acid (VMA) and normetanephrine (NME) level at 34 weeks' gestation were elevated. Consequently, emergent cesarean section was done. Based on abdominal ultrasonography and whole body magnetic resonance imaging, left adrenal tumor with liver metastasis was suspected. Neuroblastoma was confirmed by liver and placenta biopsy. Chemotherapy was started with Pediatric Oncology Group 9243 at day 7 and changed into Children's Oncology Group 3961 due to cholestasis and poor response during 2nd cycle. Plasma exchange was done for aggravated direct hyperbilirubinemia. The baby expired at 73 days due to multi-organ failure. Maternal symptoms were completely resolved in 2 weeks after delivery along with normalization of the elevated level of VMA and NME. We report a first case of mirror syndrome in Korean mother and fetus resulting from metastatic congenital neuroblastoma to placenta.

A Study on Legal Protection, Inspection and Delivery of the Copies of Health & Medical Data (보건의료정보의 법적 보호와 열람.교부)

  • In a broad term, health and medical data means all patient information that has been generated or circulated in government health and medical policies, such as medical research and public health, and all sorts of health and medical fields as well as patients' personal data, referred as medical data (filled out as medical record forms) by medical institutions. The kinds of health and medical data in medical records are prescribed by Articles on required medical data and the terms of recordkeeping in the Enforcement Decree of the Medical Service Act. As EMR, OCS, LIS, telemedicine and u-health emerges, sharing and protecting digital health and medical data is at issue in these days. At medical institutions, health and medical data, such as medical records, is classified as "sensitive information" and thus is protected strictly. However, due to the circulative property of information, health and medical data can be public as well as being private. The legal grounds of health and medical data as such are based on the right to informational self-determination, which is one of the fundamental rights derived from the Constitution. In there, patients' rights to refuse the collection of information, to control recordkeeping (to demand access, correction or deletion) and to control using and sharing of information are rooted. In any processing of health and medical data, such as generating, recording, storing, using or disposing, privacy can be violated in many ways, including the leakage, forgery, falsification or abuse of information. That is why laws, such as the Medical Service Act and the Personal Data Protection Law, and the Guideline for Protection of Personal Data at Medical Institutions (by the Ministry of Health and Welfare) provide for technical, physical, administrative and legal safeguards on those who handle personal data (health and medical information-processing personnel and medical institutions). The Personal Data Protection Law provides for the collection, use and sharing of personal data, and the regulation thereon, the disposal of information, the means of receiving consent, and the regulation of processing of personal data. On the contrary, health and medical data can be inspected or delivered of the copies, based on the principle of restriction on fundamental rights prescribed by the Constitution. For instance, Article 21(Access to Record) of the Medical Service Act, and the Personal Data Protection Law prescribe self-disclosure, the release of information by family members or by laws, the exchange of medical data due to patient transfer, the secondary use of medical data, such as medical research, and the release of information and the release of information required by the Personal Data Protection Law.

  • Massive hemoptysis is a life-threatening condition and sometimes leads to death due to airway obstruction rather than exsanguinations. In a critical hemoptysis, endotracheal intubation may be necessary to maintain adequate gas exchange and protect the unaffected side of the lung. Bronchial blockers (BBs), commonly used technique for one-lung ventilation in thoracic or cardiac surgeries, are valuable devices for protecting the airway in massive endobronchial bleeding. We report three cases intubated with BBs, Univent$^{(R)}$, in massive hemoptysis. We suggest that BBs are one of the indispensable equipments for respiratory specialized wards and intensive care units.

  • Malaria is one of the most common infectious diseases in the world. Plasmodium falciparum, accounting for nearly all malaria mortality, kills an estimated 1 to 2 million persons yearly and has several features that make it deadlist of malarias. While cerebral malaria is the most common presentation of severe disease, acute lung injury associated with malaria is uncommon but serious and fatal complication. We report two cases of severe malaria with ARDS and multi-organ failure. All two patients traveled to foreign countries, Kenya, Papua New Guinea where choroquine-resistant malaria is distributed. The first case, which developed cerebral malaria, hypoglycemia, multi-organ failure, and ARDS, treated with quinine and mechanical ventilator, but expired due to oxygenation failure. Autopsy showed acute necrotizing infiltration, diffuse eosinophilic fibrinoid deposits along the alveolar space, and alveolar macrophage with malaria pigment The second case also developed multi-organ failure, followed by ARDS, and was treated with quinine, exchange transfusion, plasmapheresis, and mechanical ventilator. He recovered with residual restrictive lung change after treatment.

  • Bonchoyuham Nyoryeong(本草類函要領) shows how Donguibogam(東醫寶鑑) -Mirror of Eastern Medicine- is understood, digested into Korean medicine, and at the same time how it participated in the historical shaping of Korean medicine since the publishing in the early 17th century. The author, Hyeon Jae-deok, internalized the structure, content, and significance of Donguibogam and drew out a novel, concise, but comprehensive type of medical manual, while many other medical books since the 17century in Korea are estimated to have been an abridged edition of the Donguibogam. It may well be estimated as exemplar of the extent to which Donguibogam informed Korean medicine since the 17 century. The book shows as well how Bencao Gangmu(本草綱目) -Compendium of Materia Medica- and Donguibogam are merged and set a new medical stream in the 19th century Korea. Hyeon Jae-deok looked at the Bencao Gangmu from the lens of clinical treatment and prescription such that he focused on sections, elements, or parts of procedures, treatments, and prescriptions, not on the herbal taxonomic knowledge of the book. This perspective was embodied in Bonchoyuham(本草類函), which cited simple remedies, prescriptions, or treatments from Bencao Gangmu.

Unexpected Restart Failure of Durable Left Ventricular Assist Devices: A Report of Two Cases

  • The HeartWare Ventricular Assist Device (HVAD) was widely used for mechanical circulatory support in patients with end-stage heart failure. However, there have been reports of a critical issue with HVAD pumps failing to restart, or experiencing delays in restarting, after being stopped. This case report describes 2 instances of HVAD failure-to-restart during heart transplantation surgery and routine outpatient care. Despite multiple attempts to restart the pump using various controllers and extensions, the HVAD failed to restart, triggering a hazard alarm for pump stoppage. In one case, the patient survived after receiving a heart transplantation, while in the other, the patient died immediately following the controller exchange. These cases highlight the rare but life-threatening complication of HVAD failure-to-restart, underscoring the importance of awareness among clinicians, patients, and caregivers, and adherence to the manufacturer's guidelines and recommendations for HVAD management.

Bio-information Monitoring System Based on embedded-MDEC (내장형 MDEC 모듈 기반의 생체정보 모니터링 시스템)

  • 본 논문에서는 다중 생체계측 장비를 CMCIS(Central Monitoring and Cardiology Information System) 기반의 통합 시스템에 연동하기 위한 내장형 MDEC 모듈을 소개하고 바이탈 사인 장비와 결합하여 무선 네트워킹을 통하여 원격지에 생체정보를 전송하는 시스템을 제안한다. 내장형 MDEC 모듈로 전송된 데이터는 실시간 뷰에 표시됨으로서 환자정보를 원격지에서 감시 및 관리할 수 있는 유효성을 가지게 된다. 본 연구의 결과는 CMCIS 기반의 통합 시스템을 구축하는 기반기술로 향후 효율적 의료서비스에 기여할 것으로 사료된다.
