근접방사선치료 시 몬테카를로 전산모사를 이용한 인체전산팬텀의 우측 폐와 주변 장기 선량평가 (Evaluation of Absorbed Dose for the Right Lung and Surrounding Organs of the Computational Human Phantom in Brachytherapy by Monte Carlo Simulation)

  • This study is to evaluate absorbed dose from right lung for brachytherapy and to estimate the effects of tissue heterogeneities on dose distribution for Iridium-192 source using Monte Carlo simulation. The study employed Geant4 code as Monte Carlo simulation to calculate the dosimetry parameters. The dose distribution of Iridium-192 source in solid water equivalent phantom including aluminium plate or steel plate inserted was calculated and compared with the measured dose by the ion chamber at various distances. And the simulation was used to evaluate the dose of gamma radiation absorbed in the lung organ and other organs around it. The dose distribution embedded in right lung was calculated due to the presence of heart, thymus, spine, stomach as well as left lung. The geometry of the human body was made up of adult male MIRD type of the computational human phantom. The dosimetric characteristics obtained for aluminium plate inserted were in good agreement with experimental results within 4%. The simulation results of steel plate inserted agreed well with a maximum difference 2.75%. Target organ considered to receive a dose of 100%, the surrounding organs were left the left lung of 3.93%, heart of 10.04%, thymus of 11.19%, spine of 12.64% and stomach of 0.95%. When the statistical error is performed for the computational human phantom, the statistical error of value is under 1%.

신체자세 안정도의 정량분석에 관한 연구 (Quantitative analysis of body postural stability)

  • Posturography stands or quantitative assessment of body postural stability analysis. The present study developed a balance plate system to monitor patient's center of pressure (COP) movement and to analyze its stability. An equilateral triangular shaped plate was made of duralumin and forces were measured on the three vertices of the plate using industrial load cells. Specially designed electronic circuit picked up force signals ed into data acquisition system to calculate the cartesian coordinates of COP. COP calculation error was less than 2%. The force signals enabled to compute stability measures, which consisted of a variety of clinical parameters related to postural stability. Clinical experiments were carefully designed and performed on 40 normal subjects. The results were that 1) postural stability decreased with age and 2) the best parameters were those of posture deviation measures. A customized PC-based software package was developed to apply the present technique with a great convenience to monitoring and analyzing postural stability in an accurate and quantitative way.

『승정원일기(承政院日記)』의 진료기록 연구 (A Study on the Clinical Records of 『The Daily Records of Royal Secretariat of Chosun Dynasty』)

  • "承政院日記" is a journal written by the scribes belonging to the Royal Secretariat and consists of objective and detailed records about events, dialogs, and actions that happened in the presence of the king and also collections of all the documents output by the Royal Secretariat. The medicine-related records in "承政院日記" are mainly records of the medical examination and treatment of the king and the royal family and related documents of operation. Of the many different entries involved with clinical practices, this study focuses on the medical examination and treatment of the king. Through the case studies displayed in the clinical records of "承政院日記", trial and error of its time as well as clinical results can be verified. Sorting out of affirmative tradition that could not be handed down due to institutional interruption is also made possible through comparison of effective treatment methods of late Chosun dynasty including patterns or distinctive methods of treating specific diseases against their counterparts in Traditional Korean Medicine of today.

『어약원방(御藥院方)』 이본(異本)에 대한 연구 - 「담음문(痰飮門)」을 중심으로 - (A Study on Different Versions of Eoyakwonbang Based on the Phlegm-fluid Chapter)

  • Eoyakwonbang is a collection of prescriptions of Yuyaoyuan, an imperial medical bureau of China. While the first edition of this book does not exist at present, two versions printed in Korea, and one printed in Japan, which is the well-known Gyesasingan Eoyakwonbang have been passed down. Eoyakwonbang is a meaningful text for the history of medical communication between China, Japan and Korea, but research on the different versions and contents of Eoyakwonbang have been insufficient. Out of the 2 versions published in Korea, Eulheja Eoyakwonbang is different from Gapjinja Eoyakwonbang [another version in Korea] and Gwanjeong Eoyakwonbang of Japan, in that the prescriptions are organized, the ingredients are organized according to the amount of each ingredient for each formula, and the amounts are recorded in an accurate manner. On the other hand, the Gapjinja version has many mistakes in the characters and content. The Gwanjeong version has lesser mistakes in characters, but repeats the content error of the Gapjinja version. Eulheja was printed after correction based on the original version or unknown version from China. Gapjinja was re-printed based on the Gyesasingan version, while Gwanjeong seems to have used the Gapjinja version as its original script, as the mistakes made in the Gapjinja version are repeated in the Gwanjeong version as well.

전자포탈영상장치(EPID)와 GafChromic EBT3 film을 이용한 다엽콜리메이터 정도관리 시 갠트리 각도 변화에 따른 엽의 위치 정확성 비교 평가 (Comparative evaluation for leaf position accuracy according to gantry angle variation in MLC quality assurance using electronic portal imaging device(EPID) and GafChromic EBT3 film)

  • 목 적: 본 연구에서는 EPID와 GafChromic EBT3 film을 이용하여 picket fence test를 시행하여 갠트리 각도에 따른 중력 효과로 인한 다엽콜리메이터의 엽의 위치 오차를 분석하여 정확성을 평가하고자 하였다. 대상 및 방법: 테이블에 5 cm의 고체 팬텀을 놓고 SAD를 100 cm이 되도록 설정하였다. EBT3 film을 고체 팬텀 위에 정확하게 놓이게 한 후 1.5 cm의 고체 팬텀을 놓고 picket fence test를 시행하였다. EPID는 선원 검출기간 거리 100 cm에서 EBT3 film과 같은 조건으로 측정했다. 갠트리 각도에 따른 다엽콜리메이터 이동 위치를 알아보기 위해 갠트리 각도 $0^{\circ}$, $90^{\circ}$, $180^{\circ}$, $270^{\circ}$에서 각각 측정하였다. 다엽콜리메이터의 기하학적 평가를 위해 분석 프로그램을 이용하여 다엽콜리메이터의 엽의 위치 정확성을 분석하였다. 결 과: EPID의 경우 갠트리 각도 $0^{\circ}$, $90^{\circ}$, $180^{\circ}$, $270^{\circ}$로 갠트리 각도를 변경했을 때 엽의 위치 오차는 각각 평균 0.18 mm, 0.31 mm, 0.20 mm, 0.26 mm였고, 오차의 최댓값은 각각 0.44 mm, 0.54 mm, 0.34 mm, 0.44 mm였다. EBT3 film은 갠트리 각도 $0^{\circ}$, $90^{\circ}$, $180^{\circ}$, $270^{\circ}$로 갠트리 각도를 변경했을 때 엽의 위치 오차는 각각 평균 0.19 mm, 0.21 mm, 0.19 mm, 0.31 mm였고, 오차의 최댓값은 각각 0.35 mm, 0.45 mm, 0.36 mm, 0.48 mm였다. 결 론: 본 연구는 갠트리 각도에 따른 다엽콜리메이터의 엽의 위치 오류를 분석해본 결과 중력효과에 의한 엽의 위치 오류를 확인하였고, EPID와 EBT3 film을 이용하여 갠트리 각도 변화에 따른 엽의 정확성을 비교한 결과 EPID을 이용한 오차 분석방법에서 EBT3 film보다 더 큰 오차가 발생했다. 따라서 다엽콜리메이터를 기반으로 하는 세기조절방사선치료의 경우 정확한 선량 조사에 대한 검증뿐만 아니라 다엽콜리메이터의 정확하고 정밀한 작동에 대한 정도관리와 분석방법에 대한 비교 및 검증이 필요할 것으로 사료된다.

Spatial Compounding of Ultrasonic Diagnostic Images for Rotating Linear Probe with Geometric Parameter Error Compensation

  • In ultrasonic medical imaging, spatial compounding of images is a technique where ultrasonic beam is steered to examine patient tissues in multiple angles. In the conventional ultrasonic diagnostic imaging, the steering of the ultrasonic beam is achieved electronically using the phased array transducer elements. In this paper, a spatial compounding approach is presented where the ultrasonic probe element is rotated mechanically and the beam steering is achieved mechanically. In the spatial compounding, target position is computed using the value of the rotation axis and the transducer array angular position. However, in the process of the rotation mechanism construction and the control system there arises the inevitable uncertainties in these values. These geometric parameter errors result in the target position error, and the consequence is a blurry compounded image. In order to reduce these target position errors, we present a spatial compounding scheme where error correcting transformation matrices are computed and applied to the raw images before spatial compounding to reduce the blurriness in the compounded image. The proposed scheme is illustrated using phantom and live scan images of human knee, and it is shown that the blurriness is effectively reduced.

Preoperative estimation of hemi-liver volume using standard liver volume and portal vein diameter ratio in living donor liver transplantation

  • Backgrounds/Aims: Although body surface area (BSA)-based standard liver volume (SLV) formulae have been used for living donor liver transplantation and hepatic resection, hemi-liver volume (HLV) is needed more frequently. HLV can be assessed using right or left portal vein diameter (RPVD or LPVD). The aim of this study was to validate the reliability of using portal vein diameter ratio (PVDR) for assessing HLV in living liver donors. Methods: This study included 92 living liver donors (59 males and 33 females) who underwent surgery between January 2020 and December 2020. Computed tomography (CT) images were used for measurements. Results: Mean age of donors was 35.5 ± 7.2 years. CT volumetry-measured total liver volume (TLV), right HLV, left HLV, and percentage of right HLV in TLV were 1,442.9 ± 314.2 mL, 931.5 ± 206.4 mL, 551.4 ± 126.5 mL, and 64.6% ± 3.6%, respectively. RPVD, LPVD, and main portal vein diameter were 12.2 ± 1.5 mm, 10.0 ± 1.3 mm, and 15.3 ± 1.7 mm, respectively (corresponding square values: 149.9 ± 36.9 mm2, 101.5 ± 25.2 mm2, and 237.2 ± 52.2 mm2, respectively). The sum of RPVD2 and LPVD2 was 251.1 ± 56.9 mm2. BSA-based SLV was 1,279.5 ± 188.7 mL (error rate: 9.1% ± 14.4%). SLV formula- and PVDR-based right HLV was 760.0 ± 130.7 mL (error rate: 16.2% ± 13.3%). Conclusions: Combining BSA-based SLV and PVDR appears to be a simple method to predict right or left HLV in living donors or split liver transplantation.

다기능 선량계로서의 Cadmium sulfide의 X-선에 대한 특성 평가 (The Measurement and Evaluation of X-ray Characteristics of Cadmium Sulfide as a Multi-function Dosimeter)

  • 다기능 X-선 선량계로서의 cadmium sulfide의 X-선에 대한 특성을 평가하기 위해 cadmium sulfide 기반의 X-선 선량계를 진공 증착법을 사용하여 제작하였으며 X-선 노출에 따른 cadmium sulfide의 반응을 측정하였다. Cadmium sulfide의 X-선 관전압, 관전류, 조사시간에 따른 전압의 변화를 측정하여 이를 분석하였다. X-선의 관전압에 따른 cadmium sulfide의 반응을 회귀분석을 통해 통계처리 한 결과 y=0.0995x-0.1146 ($R^2$=0.9595, $\sigma$=0.08, standard error=2%) 이고 X-선 관전류에 따른 cadmium sulfide의 출력신호를 분석한 결과 y=0.0439x+1.1891 ($R^2$=0.9021, $\sigma$=0.04, standard error=1.8%)였다. 또한 X-선 조사시간에 따른 cadmium sulfide의 반응을 같은 통계 방법으로 통계처리한 결과 y=8.2853x+5.5878 ($R^2$=0.7287, $\sigma$=0.06, standard error=1.9%) 였다. 결론적으로 cadmium sulfide의 X-선에 대한 반응은 X-선의 에너지가 증가됨에 따라 선형적으로 반응함을 알 수 있었으며 제작된 cadmium sulfide의 X-선의 다기능 선량계로서의 그 사용 가능성을 제시하고자 한다.
