  • We developed a cuffless and noninvasive measurement technique of blood pressure using tonometric pressure sensor. With observation that the maximum value of pulse pressure is not obtained at mean arterial pressure(MAP), we have figured out MAP based on the physiological characteristic including the elasticity of wrist tisse. Detecting only one part of the body and using only one device are quite advantageous over other BP measurement techniques. Our technique makes new way for the cuffless BP measurement.

Effect of Kyungisan in on the Regional Cerebral Blood Flow and Mean Arterial Blood Pressure in Rats (균기산(勻氣散)이 흰쥐의 국소뇌혈류량 및 평균혈압에 미치는 효과)

  • Kyungisan (KGS) has been used in oriental medicine for many centuries as a therapeutic agent for treatment of stroke caused by deficiency of qi(氣虛). This study was performed to evaluate effects of KGS extract on the regional cerebral blood flow(rCBF) and mean arterial blood pressure(MABP) in rats. The result of this study were as follow ; 1. KGS significantly increased rCBF irrelevant to MABP in normal rats, 2. To prescribe KGS after pretreatment with indomethacin(IDN) decreased rCBF as compared with control group to administered only KGS in normal rats. But the change of MABP is not significantly as compared with control group. 3. To prescribe KGS after pretreatment with methylen blue( MTB) decreased MABP and rCBF as compared with control group to administered only KGS in normal rats. Especially, it significantly decreased rCBF. These results suggest that KGS increase rCBF by enlargement diameter of pial artery in brain. The active mechanism of KGS is related with prostaglandin activated by cyclooxygenase. So, I suggest that KGS has an anti-ischemic effect through the improvement of cerebral blood flow and can be used for stroke.

The Effects of Jujadokseo-hwan on the Activation of Brain and Neuroprotactive Effects (주자독서환의 뇌기능 활성 및 신경세포 보호효과)

  • Objectives This study is designed to investigate the effects of Jujadokseo-hwan on the brain ability and inducing oxidative stresses. Methods We measured the changes of regional cerebral blood flow and mean arterial blood pressure. Then we analyzed histological examination, immunohistochemistric response and anti-oxidant activity of Jujadokseo-hwan. Results 1. Treatment of Jujadokseo-hwan significantly increased regional cerebral blood flow but decreased mean arterial blood pressure. 2. Treatment of Jujadokseo-hwan-induced increase of regional cerebral blood flow was significantly inhibited by pretreatment with indomethacin (1 mg/kg, i.p.), an inhibitor of cyclooxygenase. 3. In histological examination through TTC stain, group I was no change, but group II showed that discolored in the most cortical part. Group III showed that decreased discolor in the cortical part. 4. In immunohistochemistric response of BDNF, group II showed that lower response effect. Group III showed that increase response effect. 5. Treatment of Jujadokseo-hwan increased proliferation rates of Glial cell effectively 6. Treatment of Jujadokseo-hwan accelerated proliferation rates of C6 cells in vitro. In addition, protective effects on cell death induced by paraquat, rotenone and hydrogen peroxide. In addition, activity of SOD were increased by treatment with Jujadokseo-hwan. Conclusions In conclusion, Jujadokseo-hwan can improve of the brain ability, learning ability, memory ability and induce ischemic brain injuries.

  • Objectives : The purpose of this study is to investigate the effects of $GB_{41}$ supplementation and $BL_{66}$ draining on changes in cerebral blood flow in normal rats. Methods : Regional cerebral blood flow(rCBF) and mean arterial blood pressure(MABP) in normal rats are observed, and those mechanisms were also investigated with pre-treatment of indomethacin(IDM) and methylene blue(MTB) each. Results : In this study, $GB_{41}$ supplementation and $BL_{66}$ draining elevated rCBF in time-dependent manner. Pre-treatment with indomethacin(IDM), an inhibitor of cyclooxygenase, inhibited increase of rCBF effectively. But pre-treatment with methylene blue(MTB), an inhibitor of guanylate cyclase, didn't affect rCBF levels. In addition, pre-treatment with IDM also decreased MABP levels. Conclusions : In conclusion, these results suggest that $GB_{41}$ supplementation and $BL_{66}$ draining is effective to increase rCBF, and the mechanisms are thought to be related to cyclooxygenase pathways.

  • Cheonghunhwadam-tang(CHT) have been used in oriental medicine for many centuries as a therapeutic agent of vertigo by wind, fire and phlegm. CTG was CHT adding Aurantii FructusㆍGastrodae Rhizoma. CTG was significantly increased regional cerebral blood f1ow(rCBF) in a dose-dependent, and CTG was decreased mean arterial blood pressure(MABP) compared with normal MABP(100%) in a low dosage, but was increased MABP in a dose-dependent, was aliked with normal MABP in a high-dosage. Therefore, purpose of this Study was to investigate experimental mechanism of CHTAG on the cerebral hemodynamics(rCBF, MABP) in rats. The changes of rCBF and MABP were determinated by Laser-Doppler Flowmetry(LDF). The results were as follows ; Pretreatment with indomethacin(3㎎/㎏, i.v.) was significantly inhibited CTG induced increase of rCBF and pretreated with propranolol(3㎎/㎏, i.v.) was inhibited CTG induced increase of rCBF, but pretreatment with methylene blue(10㎍/㎏, i.v) was increased CTG induced increase of rCBF. Pretreatment with indomethacin was decreased CTG induced increase of MABP, but pretreatment with propranolol and methylene blue were increased CTG induced increase of MABP. This results suggest that the mechanism of CTG is mediated by cyclooxygenase.

Implementation of Impedance Method to Estimate Blood Flow Variation with Cuff Pressure Change (커프 압력 조절에 따른 혈류량 변화 평가를 위한 임피던스법의 구현)

  • In this study, we measured the blood flow on arm by non-invasive method and implemented a system to measure variation of the blood flow by estimating bio-electrical impedance and arterial pressure according to cuff pressure. The implemented system measured impedance variation according to pressure variation applied by artificial cuff pressure on the measuring position. The system consisted of pressure measuring part and impedance measuring part using 4-electrode method. Pressure measuring part was composed of semiconductor pressure sensor and electronic circuit for signal processing of sensor output signal. In addition, impedance measuring part was composed of constant current source circuit and lock-in amplifier for detecting impedance signal. We conducted experiments of impedance measuring part using standard resistance for performance evaluation of the implemented system. In addition we experimented to estimate variation of the blood flow by measuring impedances of the experimental group. We estimated ratio of the blood flow resistance using mean arterial pressure and variation of the blood flow. As a result the ratio of the blood flow resistance and variation of blood flow were in an inverse relationship with each other and the correlation coefficient was -0.96776.

Pharmacological Evidence that Calcitonin Gene-Related Peptide is Implicated in Cerebral Autoregulation

  • In the present study, it was aimed to asses the possibility that calcitonin gene-related peptide (CGRP) released in response to transient hypotension may contribute to the reflex autoregulation of cerebral blood flow as a putative modulator. Changes in pial arterial diameter (mean, 33.0 ${\pm}$ 1.1 $\mu\textrm{m}$) with changes in systemic arterial blood pressure (mean, 101.9 ${\pm}$ 2.7 mmHg) were observed directly through a closed cranial window in anesthetized normotensive rats. Image of the pial vessels was captured with a stereoscope connected to a CCD video camera and the diameter was measured with a microscaler. In the capsaicin-treated rats (one day prior to experiment, 50 nmol capsaicin injected intracisternally), both vasodilater and vasoconstrictor responses evoked by a transient hypotension and the reverse of blood pressure were markedly attenuated or almost abolished. When changes in pial arterial diameter were plotted as a function of changes in blood pressure, the slopes of both regression lines (for vasodilators and vasoconstrictors ) were markedly reduced. Similar reductions were evidenced under treatment wi th the CGRP antibody serum (1:1,000) and following CGRP receptor desensitization. However, the autoregulatory mechanics were neither affected by treatment wi th spantide (1 ${\mu}$M), substance P antagonist, nor by substance P receptor desensitization. Suffusion wi th mock cerebrospinal fluid containing CGRP and cromakalim caused a vasodilatation in a concentration-dependent manner, respectively and their effects were antagonized by glibenclamide. Substance P produced a vasodilatation, which was, however, little affected by glibenclamide. These observations indicate that the CGRP released from the perivascular sensory fibers in response to a hypotension is implicated in the modulation of the autoregulation of cerebral blood flow.

Relationship of Mean Arterial Pressure with the Adverse Outcomes in Adult Blunt Trauma Patients: Cross-sectional Study (성인둔상환자에서 평균동맥압과 위해사건발생의 관련성:단면 조사 연구)

  • Purpose: Non-invasive blood pressure measurement is widely used as a pre-hospital triage tool for blunt trauma patients. However, scant data exits for using the mean arterial pressure (MAP), compared to the systolic blood pressure, as a guiding index. The aim of this study was to determine the association between adverse outcomes and mean arterial pressure (MAP) and to exhibit the therapeutic range of the MAP in adult blunt trauma patients. Methods: The electronic medical records for all trauma patients in a single hospital from January 2010 to September 2012 were retrospectively reviewed. Patients below 17 years of age, patients with penetrating injuries, and patients with serious head trauma (injuries containing any skull fractures or any intracranial hemorrhages) were excluded. Adverse outcomes were defined as one of the following: death in the Emergency Department (ED), admission via operating theater, admission to the intensive care unit, transfer to another hospital for emergency surgery, or discharge as hopeless. Results: There were 14,537 patients who met entry criteria. Adverse outcomes occurred for MAPs in range from 90 to 120 mmHg. Adverse outcomes were found, after adjusting for confounding variables, to occur increasingly as the MAP declined below 90 mmHg or rose above 120 mmHg. Conclusion: Not only lower but also higher mean arterial pressure is associated with increased adverse outcomes in adult blunt trauma patients. Thus, patients with a MAP above 120 mmHg should be considered as a special group requiring higher medical attention, just as those with a MAP below 90 mmHg are.

Effects of Siegesbckiae Herba on the Brain Ischemia (희렴 이 뇌허혈에 미치는 효과)

  • This experimental study was designed to investigate the effects of SIEGESBECKIAE HERBA extract (SHE) on the change of cerebral hemodynamics 〔regional cerebral blood flow (rCBF) and mean arterial blood pressure(MABP)〕 in normal condition and cerebral ischemic rats, and to determine the mechanism of action of SHE. This study was designed to investigate whether or not SHE inhibit lactate dehydrogenase (LDH) activity in neuronal cells and cytokines production in serum of cerebral ischemic rats. The results were as follows SHE increased rCBF significantly in a dose-dependent manner, but MABP was not changed by SHE in normal rats. The SHE-induced increase in rCBF was significantly inhibited by pretreatment with indomethacin (IDN), an inhibitor of cyclooxygenase but was increased by methylene blue (MTB), an inhibitor of guanylate cyclase. SHE inhibited lactate dehydrogenase (LDH) activity significantly in neuronal cells. rCBF was increased significantly and stably by SHE(10㎎/㎏, i.p.) during the period of cerebral reperfusion, which contrasted with the findings of rapid and marked increase in control group in ischemic rats. In serum by drawing from femoral arterial blood after middle cerebral arterial occlusion(MCAO) for 1hr and reperfusion for 1hr, the sample group was decreased IL-1β production significantly compared to that of the control group. In serum by drawing from femoral arterial blood after MCAO 1hr and reperfusion 1hr, sample group decreased TNF-α production significantly compared to that of the control grolilp. In serum by drawing from femoral arterial blood after reperfusion 1hr, sample group increased TGF-β production significantly compared to that of the control group. In serum by drawing from femoral arterial blood after MCAO for 1hr and reperfusion for 1hr, IL-10 production of the sample group was similar to that of control group. These results suggested that SHE had inhibitive effect on the brain damage by inhibited LDH activity, IL-1β and TNF-α production, but accelerated TGF-β production.

An Automatic Control System of the Blood Pressure of Patients Under Surgical Operation

  • We developed an automatic blood pressure control system to maintain the blood pressure of patients at a substantially low level during a surgical operation. The developed system discharges two functions, continuous feedback control of the mean arterial pressure (MAP) by a state-predictive servo controller and risk control based on the inference by fuzzy-like logics and rules using measured data. Twenty-eight clinical applications were made beginning in November 1995, and the effects of the automatic blood pressure control on the operation time and on bleeding were assessed affirmatively by means of Wilcoxon testing. This paper essentially reports the engineering details of the control system.
