Feasibility of Bilateral Crossing C7 Intralaminar Screws : A Cadaveric Study

  • Objective : When the pedicle screw insertion technique is failed or not applicable, C7 intralaminar screw insertion method has been used as an alternative or salvage fixation method recently. However, profound understanding of anatomy is required for safe application of the bilaterally crossing laminar screw at C7 in clinic. In this cadaveric study, we evaluated the anatomic feasibility of the bilateral crossing intralaminar screw insertion and especially focused on determination of proper screw entry point. Methods : The C7 vertebrae from 18 adult specimens were studied. Morphometric measurements of the mid-laminar height, the minimum laminar thickness, the maximal screw length, and spino-laminar angle were performed and cross-sectioned vertically at the screw entry point (spino-laminar junction). The sectioned surface was equally divided into 3 parts and maximal thickness and surface area of the parts were measured. All measurements were obtained bilaterally. Results : The mean mid-laminar height was 13.7 mm, mean minimal laminar thickness was 6.6 mm, mean maximal screw length was 24.6 mm, and mean spinolaminar angle was $50.8{\pm}4.7^{\circ}$. Based on the measured laminar thickness, the feasibility of 3.5 mm diameter intralaminar screw application was 83.3% (30 sides laminae out of total 36) when assuming a tolerance of 1 mm on each side. Cross-sectional measurement results showed that the mean maximal thickness of upper, middle, and lower thirds was 5.0 mm, 7.5 mm, and 7.3 mm, respectively, and mean surface area for each part was $21.2mm^2$, $46.8mm^2$, and $34.7mm^2$, respectively. Fourteen (38.9%) sides of laminae would be feasible for 3.5 mm intralaminar screw insertion when upper thirds of C7 spino-laminar junction is the screw entry point. In case of middle and lower thirds of C7 spino-laminar junction, 32 (88.9%) and 28 (77.8%) sides of laminae were feasible for 3.5 mm screw insertion, respectively. Conclusion : The vertical cross-sectioned area of middle thirds at C7 spinolaminar junction was the largest area and 3.5 mm screw can be accommodated with 77.8 % of feasibility when lower thirds were the screw entry point. Thus, selection of middle and lower thirds for each side of screw entry point in spino-laminar junction would be the safest way to place bilateral crossing laminar screw within the entire lamina. This anatomic study result will help surgeons to place the screw safely and accurately.

  • Let R be a Krull domain with field of fractions K. By Br(R) we denote the Brauer group of R. Studying the Kernel of the homomorphism Br(R).rarw.Br(K), Orzech defined Brauer groups Br(M) for different categories M of R-modules [4]. In this paper we show that an algebra A in Br(D) is a maximal order in A K and that the map Br(D).rarw. Br(K) is one to one. We note here few conventions. All rings are Krull domains and all modules will be unitary. By Z we donote the set of height one prime ideals of a Krull domain.

The Effects of Knee Brace on the Knee Muscular Neuro-Biomechanical Variables during the Rebound in Female Highschool Basketball Players (여자 고등학교 농구 선수들이 리바운드 점프 후 착지할 때 무릎보호대가 무릎의 근신경 생체역학적 변인에 미치는 효과)

  • The purpose of this study were to investigate the effects of knee brace on the knee muscular neuro-biomechanical variables during the rebound in female highschool basketball players. Twelve high school female ($17.9{\pm}0.8years$) basketball players rebound jumped for maximal vertical height to sufficiently stress the anterior cruciate ligament with and without knee brace. Kinematic data were collected to estimate the knee flexion, abduction angles and jump height. The EMG data from the biceps femoris and rectus femoris was used to estimate the ratio of quadriceps muscle activity. Female athletes with knee brace showed more reduced the knee abduction angle and the ratio of quadriceps muscle activity at foot contact phase than without knee brace. In conclusion, Female athletes with brace reduced knee anterior cruciate ligament loads.

Regional Skin Maximal Elongation Rate for Appling E-textiles to Tight-Fit Clothing (전자섬유의 피부 밀착의복 적용을 위한 인체 부위별 피부 최대변형률)

  • The purpose of this study was to investigate the maximal elongation rate and area expansion ratio of human skin in various postures. Five males and five females (male: $23{\pm}2yr$ in age, $177.9{\pm}4.8cm$ in height, $76.7{\pm}8.8kg$ in body weight, $24.2{\pm}2.5$ in BMI, $16.2{\pm}3.4%$ in body fat; female: $22{\pm}1yr$, $163.2{\pm}3.6cm$, $51.4{\pm}2.7kg$, $19.3{\pm}1.6$, $27.4{\pm}6.7%BF$) participated in this study. Measurements were conducted using a pen and tape on the elbow, knee, wrist, shoulder, and neck. Subjects held postures so that each joint of the body regions was bent at its maximal level. The results were as follows: 1) The maximal elongation rate of skin showed a significant difference among the regions: $16.6{\pm}3.4%$ for the wrist, $22.4{\pm}5.5%$ for the neck (back), $37.6{\pm}11.3%$ for the shoulder, $42.6{\pm}10.0%$ for the knee, and $43.9{\pm}4.0%$ for the elbow (p<0.05). 2) The maximal expansion rate of the body surface area had the greatest values on the elbow ($93.7{\pm}6.4%$) and knee ($74.8{\pm}10.8%$). 3) No significant difference was found between males and females. In summary, maximal values of skin elongation and expansion rates in vivo were greater than in vitro values known from previous reports. These results can be applied to develop electronic fibers or textiles for wearable tight fit work clothing as well as fitness wear.

BER of Rectangular QAM signals with MRC over Correlated Nakagami Fading Channels

  • The average bit error rate (HER) performance of a Gray coded arbitrary rectangular quadrature amplitude modulation (AR-QAM) signal with maximal ratio combining (MRC) diversity in an arbitrarily correlated Nakagami-m fading channel is derived and analyzed. The derived two types of general solutions are a simple closed-form and an integral form, depending on the types of the values (integer and non-integer) of the fading parameter. Using the derived equations in this paper, we analyzed the HER performances numerically based on the practical base station antenna configuration. The results show that MRC reception is a very effective scheme so far even though the combined signals are not independent each other because of the correlation values. The antenna height and separation of the MRC system relate to the correlation coefficient value between antennas, and go a long way with the diversity advantage. In particular, it is needed to be determined the antenna height that is carefully do for the diversity advantage because the correlation coefficient and the antenna height gain are contrary to each other from the aspect of the system performance. The expressions presented here can offer a convenient way to evaluate the exact HER performance of an arbitrary rectangular QAM signal with MRC diversity reception for various cases of practical interest on a correlated Nakagami fading channel.

Effect of Visual Block, Task Type, and Participation in an Exercise Program on Static Balance in the Elderly (시각 차단, 과제 유형, 및 운동프로그램 참여가 노인의 정적 균형에 미치는 영향)

  • The purpose of this study was to assess the effect of visual block (eyes open or closed), mental task type, and participation in an exercise program on static balance in the elderly. The subjects were 34 elderly (>65 years old) residents of a social welfare institute in Gyeonggi-do. We measured the following variables. Berg balance scale, mini mental state examination, balance performance monitor (sway area, path, and maximal sway velocity), age, weight, height and whether the subject participated in an exercise program. Scores for the Berg balance scale and mini mental state examination were evaluated to assess static balance ability either alone (single task paradigm) or while performing a mental task (dual task paradigm). Static balance variables that were measured included sway area, path, and maximal sway velocity. Each test was repeated three times. Multiple regressions analysis was used to examine the effect of each variable on static balance ability. For the dual task paradigm, static balance was affected by whether the subject participated in an exercise program. The Berg balance scale score for subjects with their eyes open was affected by whether they participated in an exercise program, while this variable in addition to the height and weight of subjects were determining variables in subjects with their eyes closed. As a result, whether subjects participated in an exercise program affected their static balance irrespective of whether their eyes open or closed. Therefore, regular exercise is recommended for elderly people and further research is needed to examine the relationship between static and dynamic balance while performing mental tasks such as cognition and attention.

  • The purposes of this study were to test the effects of the low power laser and exercise on the recovery in the cerebellar injured rats by 3-Acetylpyridine. Cerebellar injury was induced by 3-Acetylpyridine chemoablation of the inferior olive. Thirty Sprague-Dawley rats were assigned to the normal control and 3AP control and 3 experimental groups. Each experimental group was treated from 5 days after being injured for the 5 min(laser group), 10 min(exercise group) and 15min(exercise with laser) everyday during the 2 weeks. The Hindlimb splay test, Vestibular drop test, Hindlimb stride width test, Maximal Height Vertical Jump test were examined at pre-treatment on 1st day and 5th, 10th, 14th days after treatment on the cerebellar injured rats by 3AP. The results of this experiment were as follows; There were significantly increased exercise on the 3 experimental groups comparied with the 3AP control group, in the Hindlimb splay test, Vestibular drop test, Hindlimb stride width test, and Maximal Height Jump test(P<.05).

Effects of PNF Technique on Delayed Onset Muscle Soreness After Eccentric Exercise

  • This study examined the effects of hold-relax with agonist contraction (HR-AC) on the symptoms of delayed onset muscle soreness (DOMS) induced by intensive eccentric exercise of the non-dominant biceps brachii. Ten men (mean age=26.7 yrs, mean height=172.1 cm, mean weight=66.2 kg) and ten women (mean age=27.4 yrs, mean height=165.9 cm, mean weight=60.7 kg) who had not participated in a regular exercise program for the upper extremities in the previous six months were randomly assigned to one of two experimental groups: the HR-AC group, or the control group. We measured joint range of motion (ROM), maximal voluntary isometric contraction (MVIC), and muscle soreness before eccentric exercise, and 24, 48, and 72 hours after eccentric exercise. The subjects in the HR-AC group received the HR-AC technique in the non-dominant biceps brachii. The HR-AC technique was applied 24 and 48 hours after eccentric exercise. There was no significant difference between the HR-AC and the control group. However, the HR-AC group, compared to the control group, had a significant difference between the time points of the various parameters. Increased ROM (p<.05), decreased muscle soreness (p<.05), and reduced MVIC (p<.05) were found in the HR-AC group after 72 hours. Decreased ROM (p<.05) and MVIC (p<.05), and increased muscle soreness (p<.05) were observed in the control group. These findings suggest that the HR-AC technique effectively reduces muscle soreness and increases ROM 72 hours after eccentric exercise.

  • Loss of permanent molar by multiple dental caries traumatic influence or hereditable disease can cause loss of the vertical dimension in children. However traditionally reconstructive treatment to restore vertical dimension in children has been provided by using simple methods such as celluloid crown form and stainless steel crown. The presented case report describes an alternative treatment modality of vertical dimension by using removalbe appliance This appliance is made with average of facial height and maximal clenching force by using T-scan.
