Effect of impression coping and implant angulation on the accuracy of implant impressions: an in vitro study

  • PURPOSE. The purpose of this study was to compare the accuracy of the implant master cast according to the type (pick-up, transfer) and the length (long, short) of the impression copings. MATERIALS AND METHODS. The metal master cast was fabricated with three internal connection type implant analogs (Osstem GS III analog), embedded parallel and with $10^{\circ}$ of mesial angulation to the center analog. Four types of impression coping were prepared with different combinations of types (transfer, pick-up) and lengths (long, short) of the coping. The impressions were made using vinyl polysiloxane (one step, heavy + light body) with an individual tray, and 10 impressions were made for each group. Eventually, 40 experimental casts were produced. Then, the difference in the distance between the master cast and the experimental cast were measured, and the error rate was determined. The analysis of variance was performed using the SPSS (v 12.0) program (${\alpha}$= .05), and the statistical significance was set at P < .05. RESULTS. The ANOVA showed that the pick-up type impression coping exhibited a significantly lower error rate than the transfer type. However, no significant difference was observed with respect to the length of the impression coping. Additionally, no significant difference was observed between the parallel and mesial angulated groups. CONCLUSION. Within the limitations of this study, the pick-up type impression coping exhibited a more accurate implant master cast than the transfer type in parallel group. The accuracy of the implant master cast did not differ for different lengths of impression coping of at least 11 mm. Additionally, the accuracy of the implant cast was not different for the parallel and $10^{\circ}$ mesial angulated groups.

사진 주사(走査)를 이용한 치과용 모형재의 정확도에 관한 연구 (A STUDY ON THE ACCURACY OF DENTAL CAST AND DIE MATERIALS USING PHOTO-SCANNING)

  • Dental cast and die materials are essential material using in almost dental prsthodontic procedure and it's most important requirement is accuracy for reqorducing the oral anatomical structures. In this study, 5 abutments A, B, C, D, E were fabricated on the metal master model to simulate the arch form and specimens were poured with 4 cast materials. Inter-abutment distances, A-B, A-C, A-D, A-E, B-C, B-D were calculated using the photo-scanning and the deviations from the metal master model were also evaluated. The results were as follows; 1. The distance between A-B, A-C, A-D, A-E, B-C, B-D of the abutments A, B, C, D, E of each cast material was calculated. And after comparing the deviations between the metal master model. $Fujirock^{(R)}$ showed the lowest value with $0.20{\pm}0.22mm$, and the deviation increased in the order of $Suprastone^{(R)}$, Epoxy $Die^{(R)}$, Die $Keen^{(R)}$. There was significant difference between $Fujirock^{(R)}$ and Epoxy $Die^{(R)}$, Die $Keen^{(R)}$. 2. In each calculation area, the difference in measurements between cast material and metal master model showed singificant difference between A-B and Cross arch measure-ments of A-D, B-D, A-E(p<0.05). 3. The difference in measurements between cast material and metal master model in the A-B area showed $Fujirock^{(R)}$ to be the lowest with $0.05{\pm}0.04$mm and increased in the order of Die $Keen^{(R)}$, $Suprastone^{(R)}$, Epoxy $Dies^{(R)}$. There was significant difference between $Fujirock^{(R)}$ and $Suprastone^{(R)}$, Epoxy $Die^{(R)}$ (p<0.05). 4. The difference in measurements between cast material and metal master model in the B-C area showed $Fujirock^{(R)}$ to bo the lowest with $0.17{\pm}0.11$mm and increased in the order of $Suprastone^{(R)}$, Die $Keen^{(R)}$, Epoxy $Dies^{(R)}$. There was significant difference between $Fujirock^{(R)}$ and Die $Keen^{(R)}$, Epoxy $Die^{(R)}$(p<0.05). 5. The difference in measurements between cast material and metal master model in the B-D area showed $Fujirock^{(R)}$ to bo the lowest with $0.13{\pm}0.07$mm, Epoxy $Dies^{(R)}$and increased in the order of $Suprastone^{(R)}$, Die $Keen^{(R)}$. There was significant difference between $Fuji-rock^{(R)}$ and Die Keen(p<0.05). 6. In this experiment, Epoxy $Dies^{(R)}$ showed mean contraction in every calculation area. And when reconstruction cross arch restorations it is thought that distortion should be considered in every cast material.

가철식 국부의치 하부구조의 적합성에 대한 기공과정에 관한 고찰 (Laboratory procedures for fitting removable partial denture frameworks)

  • 앞에서 말씀드린 기술은 가철식 국부의치 하부구조(removable partial denture frame work)를 구강 내에 맞추기 전에 master cast에 맞추는 것을 기술한 것이다. 이 과정에서는 본 하부구조(frame work)의 외형을 유지하며 적합성을 높이는 것에 중점을 두어야 한다. 또한 제거된 금속의 양은 모형(cast)이 긁히지 않게 하기 위한 것이며, master cast 위에 주조체를 안착시키기에 필요한 양 만큼이어야 한다. 주조체의 치아 접촉면의 적합이 끝나면 가벼운 압박을 주며 광택을 낸다. 이러한 과정이 기공소에서 주의깊게 완수되면 치과의사는 하부구조를 최소한 짧은 시간에 구강 내에 맞출 수 있게 된다(그림 7).

치과 임플랜트용 고정체 인상법과 지대원주 인상법간의 정확성 비교 (A COMPARISON OF ACCURACY BETWEEN FIXTURE IMPRESSION AND ABUTMENT IMPRESSION FOR DENTAL IMPLANTS)

  • The purpose of this study was to compare the accuracy between future impression and abutment impression using strain gauges. The master model used in this study was a partially edentulous mandibular metal cast with two fixture analogs on both sides. On the left, two future analogs were parallel, whereas right side, posterior future analog exhibiting a 15-degree lingual inclination. From master cast, 10 impressions were made for each of the three impression methods. The master frameworks was fabricated on the master model, and two-element strain gauge was attached to a master framework. The master framework was seated on each cast, and gold screws were tightened to 10 Ncm using a torque controller AI-1600 strain measurement system was used for strain measuring. Impression methods studied were : Group 1:abutment impression Group 2:fixture impression Group 3:combined impression (anterior:fixture impression, posterior:abutment impression) The results were as followed. 1. The strain values on X-axis and Y-axis according to the three impression methods showed no significant difference. 2. The strain values on parallel and angulated groups according to the three impression methods showed no significant difference. 3. The parallel group exhibited significantly higher accuracy in adaptation than angulated group for all experimental groups (p<0.05). In conclusion, it is considered that accuracy of implant prostheses is more affected by implant angulation than impression methods.

The effects of custom tray material on the accuracy of master cast reproduction

  • The accuracy of master cast reproduction by a polyvinylsiloxane impression material using two visible-light curing resin and autopolymerizing polymethyl methacrylate resin custom tray material was investigated. Custom trays were fabricated from a master cast that had three index points marked on both inner and outer vestibules and then poured in yellow stone. The distance between the reproduced index points were measured to be ${\pm}0.001mm$ with a measuring microscope and the algebraic norms calculated for each tray material. No differences were found in the algebraic norms of inner and outer dimensions for upper tray impressions by ANOVA(p>0.05). However, T-test revealed that there were differences between upper and lower impressions and Tukey's hsd test revealed that in lower tray impressions, the Palatray in inner, the Lightplast in outer dimensions respectively were different from other materials. The index points reproduced on the casts compared with the master cast, were closer together for upper tray impressions. All four tray materials produced acceptable casts, 1. Algebraic norms of inner and outer dimensions of the test casts for upper trays were not statistically different irrespective of materials.(P>0.05) 2. T-test showed that there were differences between means with upper and lower trays especially in outer dimension.(P>0.05) 3. But, algebraic norms of inner and outer dimensions of the test casts for lower trays were statistically different between materials. 4. Palatray XL in inner, Lightplast-platten in outer dimensions respectively for lower trays were different from other materials, but, the nearest to the original model.

가철성 국소의치 제작을 위한 인상채득 (Impression procedures for removable partial dentures)

  • 저작기능의 회복과 더불어 환자에게 편안함을 제공하며 잔존치아와 치조점막을 건강하게 유지할 수 있는 가철성 국소의치 제작을 위해서는 구강내의 형태적, 기능적 상태를 재현하는 모형제작이 필수적이다. 국소의치 제작을 위해서는 진단모형과 주모형의 제작이 필요하며 이를 위해서 예비인상과 최종인상 채득 과정을 거치게 된다. 국소의치의 인상채득은 고정성이나 총의치 인상과 다르게 치아부위와 조직부위 모두가 잘 인기되어야 하고 치아 결손 부위에 따라 교합을 지지하는 부위가 달라지므로 증례에 따른 적절한 인상재와 인상채득 방법에 대한 이해를 바탕으로 정확한 임상적 적용이 요구된다.

가변형 치과 인상용 트레이로 제작된 모형의 재현성에 관한 연구 (ACCURACY OF STONE CAST PRODUCED BY ADJUSTABLE DENTAL IMPRESSION TRAY)

  • Purpose: This study was performed to measure the accuracy of adjustable dental impression trays by a scanning laser three-dimensional digitizer. Materials and methods: The metal stock, individual, and adjustable stock trays were used for 60 stone casts(10 casts each) duplicated a resin master model of mandible and maxilla. The type IV dental stone was poured in a vinyl polysiloxane impressions and allowed to set for one hour. The master model and the duplicated casts were digitized using an optical digitizer. The distance between the reference points were measured and analyzed on the graphic image of 3-D graphic software(CATIA version 5.0). The statistical significance of the differences between the groups was determined by a two-way ANOVA. Results : There were no significant differences between the accuracies of the adjustable stock tray and the master model except only anterior arch width on the upper arch and the diagonal arch length and arch length on one side of the lower arch. Conclusion: The adjustable stock trays showed clinically acceptable accuracies of the study cast produced by them.

인상채득방법이 임플란트 주모형의 정확성에 미치는 영향 (EFFECT OF IMPRESSION TECHNIQUE ON THE ACCURACY OF MASTER CAST FOR IMPLANT PROSTHESIS)

  • Statement of problem: Major objective in making on implant-supported prosthesis is the production of superstructure that exhibits a passive fit when connected to multiple abutments. One requirement to ensure passive fit is to make an accurate impression. Purpose : The purpose of this study was to compare the accuracy of master cast fabricated by using different impression methods at the different impression levels. Material and method: The master model used in this study was resin block having low implant analogs. Impression method studied were 1) direct method on fxiture level (Group FIX-D), 2) indirect method on fixture level(Group FIX-I), 3) modified indirect method on fixture level(Group FIX-M), 4) direct method on abutment level(Group AB-D) and 5) indirect method on abutment level(Group AB-I). Each of the five groups took 10 impressions. Fifty impressions were made for master cast by using Impregum $F^{(R)}$ impression material loaded on individual tray. Three dimensional measuring microscope was used to measure the inter-implant distance. Error rate of each inter-implant distance were calculated and evaluated. Results : The results were as follows. 1. Group FIX exhibited higher accuracy than group AB. 2. In group FIX, modified indirect method showed the highest accuracy, while indirect method showed the lowest accuracy. In group Ab, indirect method showed the higher accuracy than direct method. 3. Group FIX showed larger horizontal error than group AB. But, group AB showed the larger vertical error than group FIX. 4. Group Fix-M showed smallest vertical and horizontal error. Conclusion: An impression method have more effect on accuracy of master model than an impression level. A modified indirect method showed smallest vertical and horizontal error.

임상적 방법을 이용한 내부연결 임플랜트에서 고정체수준 인상법의 정확도 평가 (EVALUATION OF THE ACCURACY OF FIXTURE-LEVEL IMPRESSION TECHNIQUE FOR INTERNAL CONNECTION IMPLANT USING CLINICAL METHODS)

  • Statement of problem : Accurate impression is essential to success of implant prostheses. But there have been few studies about the accuracy of fixture-level impression technique in internal connection implant system. Purpose: This study evaluated the accuracy of splinted fixture-level impression technique using clinical methods and the effect of internal hex on fit of superstructure in internal connection implant system (Astra Tech). Material and method : Two metal master frameworks made from two abutments (Cast-to Abutment ST) each for parallel and divergent conditions and a corresponding. passively fitting, dental stone master cast with four future replicas (Fixture Replica ST) were fabricated. Ten dental stone casts were made with vinyl polysiloxane impressions from the master cast by acrylic resin splinted fixture-level impression technique. To evaluate the accuracy of impression technique, the fit of master frameworks for test models was evaluated using screw resistance test (SRT) and one-screw test. The results of SRT were recorded as SRT values from grade 1 to grade 5 by 1/4 turn. And to evaluate the effect of hex on fit of superstructure, the same tests were performed after removing hexes of master frameworks. Results: 1. There was only one case (2.5%) showing SRT value of test model below ade 2 in total before and after removing hexes of master frameworks. And, by removing hexes. SRT values decreased in only one test model (5%) and did not change in 17 test models (85%). 2. SRT values of the 1$^{st}$ screws were grade 2 in 80% of cases before, and grade 1 in 80% of cases after removing hexes. And, by removing hexes, SRT values decreased in 72.5% of cases. 3. SRT values of the 2$^{nd}$ screws were grade 3 in 85% of cases before, and grade 3 in 95% of cases after removing hexes. And, by removing hexes, SRT values did not change in 85% of cases. 4. There were only 2 cases regarded as acceptable fit by one-screw test, and SRT values of 2$^{nd}$ screws of both cases were grade 2. Conclusion. Within the limitations of this study, future-level impression of internal connection implant system is considered to obtain inaccurate working cast, even using acrylic resin splinted impression technique. And, it is considered to be unable improve the fit to remove the hexes of implant restoration.
