3차원 가상모형을 이용한 하악 제2소구치 교두 변이 대칭성에 대한 융복합적 분석 - 전북지역 일부 대학생을 중심으로 (Convergence analysis of cusp variation symmetry of the mandibular second premolars using 3-dimensional virtual models - Focusing on college students in Jeollabuk-do)

  • 본 연구는 3차원 가상모형을 이용하여 하악 제2소구치 교두 변이 양상을 정성적으로 분석하고, 교두 변이에 따른 치아 표면적의 정량적 분석과 함께 좌·우측 대칭성을 분석하고자 하였다. 127개의 하악 제2소구치 가상치아를 준비한 후, RapidForm2004(INUS technology INC, Seoul, Korea)을 이용하여 각 교두의 절대적/상대적 단면적, 전체 치관 단면적, 교합면의 중심구 형태 등을 분석하였다. 통계방법은 Independent t-test, Kruskal-Wallis test 및 교차분석을 시행하였다. 연구 결과, 중심구의 형태는 양측간 높은 대칭성을 보였다. 좌측에서 U 형태이면 우측도 100.0% U 형태를 나타냈으며, 나머지 H 형태와 Y 형태는 각각 73.7%, 78.9% 비율로 양측간 동일한 중심구 형태를 나타냈다(p<.001). 본 연구에서 얻은 결과는 CAM/CAM을 이용한 보철물 제작 시 참고자료로 활용이 가능할 것이며, 향후 맹출 직후의 하악 제2소구치를 포함한 더 많은 샘플을 대상으로 한 후속연구가 이루어지기를 기대한다.

  • This study was undertaken to investigate the difference of chin morphology and mandibular form in relation to different mandibular growth direction. The subject was divided into three group i.e., control group, vertical group, and horizontal group, according to the criteria of $Bj{\ddot{o}}rk$ sum, and each group was composed of 15 females and 15 males. Medial axis analysis in addition to the routine cephalometric analysis using P.I.A.S. (personal image analysing system) was carried out to find out the differences of mandibular morphology on each group. The results were as follows: 1. The area of symphysis was larger in horizontal growth group than that of vertical growth group. 2. Protruding chin area was also larger in horizontal growth group than that of vertical growth group. 3. There was a close correlationship between protruding chin area and other form of mandible. 4. Antegonial notch depth and ramus posterior contour depth was deeper in vertical growth group than in horizontal growth group, and antegonial notch depth was more influenced by anterior part of notch than posterior part of notch. 5. Mental medial axis and incisal medial axis length, in relation to corpus medial axis length was larger in vertical growth group than in horizontal growth group.

하악 제3대구치가 하악 우각부 골절에 미치는 영향 (THE INFLUENCE OF MANDIBULAR THIRD MOLAR ON MANDIBULAR ANGLE FRACTURE)

  • Mandibular fractures have higher incidence rate compared to facial bone fractures because of relatively prominent form. Specially, mandibular angle fractures were known as common fracture site because of morphological anatomic structure. The mandibular third molar appears to be the most frequent impacted tooth. The mandibular third molar have influence on mandibular angle fractures according to it's eruption state. Also, the mandibular angle fracture including the third molar may influence on post operative infection whether the third molar is in impacted or extracted state when reduction of fracture site is operated. The presence or absence and degree of impaction of the mandibular third molar were assessed for each patient and were related to the occurrence of the mandibular angle fracture. The extraction or non extraction of the mandibular third molar were related to the occurrence of the post operative infection in the reduction of mandibular angle fractures. In the presence of mandibular third molar, the possibility of mandibular angle fractures were relatively common. When viewing the mandibular third molar at occlusal plane, the possibility of mandibular angle fractures were high in the class C group. The possibility of mandibular angle fractures were high in the mesial angulation and partial impaction. There was a statistically significant difference(P<0.05). In complete erupted mandibular third molar, the possibility of post operative infection occurs quite often, but there was no statistical significant difference(P>0.05). In the extracted or non extracted of mandibular third molar, the post operative infection showed no statistical significant difference(P>0.05). With the results mentioned above, mandibular third molar was significantly more susceptible to mandibular angle fracture. In the reduction of mandibular angle fracture, it was recommended that mandibular third molar should be extracted especially in case of pericoronitis, periodontitis and other infections.

한국인 정상교합자의 하악 치열궁 형태 (Mandibular Clinical Arch Forms in Koreans with Normal Occlusions)

  • 본 연구의 목적은 한국인 정상교합자에서 하악치열궁의 형태적 차이를 알아보고자 하였다. 한국인 102명의 정상교합자를 대상으로 하악 모형의 교합면을 복사한 후 13개의 접촉점중에서 가장 협측으로 위치한 부분을 digitize하였고 각 치아의 브라켓위치에 해당하는점을 하악치아의 두께에 의거하여 4개의 선계측과 2개의 비율을 측정하였다. 치열궁의 형태를 square, ovoid, tapered from으로 분류하여 그 빈도를 남녀성별에 따라 비교한 결과 특이한 차이점을 보이 지 않았으나 대구치간 폭경에서는 남녀 성별의 차이를 보였다. 정상교합자에서 치열궁 형태분포는 ovoid, square, tapered순 이었고 tapered arch from은 $10\%$미만이었다. 치열궁 형태의 빈도분포에 있어서 남녀간의 차이는 없었다.

Comparison of arch form between ethnic Malays and Malaysian Aborigines in Peninsular Malaysia

  • Objective: To determine and compare the frequency distribution of various arch shapes in ethnic Malays and Malaysian Aborigines in Peninsular Malaysia and to investigate the morphological differences of arch form between these two ethnic groups. Methods: We examined 120 ethnic Malay study models (60 maxillary, 60 mandibular) and 129 Malaysian Aboriginal study models (66 maxillary, 63 mandibular). We marked 18 buccal tips and incisor line angles on each model, and digitized them using 2-dimensional coordinate system. Dental arches were classified as square, ovoid, or tapered by printing the scanned images and superimposing Orthoform arch templates on them. Results: The most common maxillary arch shape in both ethnic groups was ovoid, as was the most common mandibular arch shape among ethnic Malay females. The rarest arch shape was square. Chi-square tests, indicated that only the distribution of the mandibular arch shape was significantly different between groups (p = 0.040). However, when compared using independent t-tests, there was no difference in the mean value of arch width between groups. Arch shape distribution was not different between genders of either ethnic group, except for the mandibular arch of ethnic Malays. Conclusions: Ethnic Malays and Malaysian Aborigines have similar dental arch dimensions and shapes.

Three-dimensional assessment of the temporomandibular joint and mandibular dimensions after early correction of the maxillary arch form in patients with Class II division 1 or division 2 malocclusion

  • Objective: This study aimed to assess three-dimensional changes in the temporomandibular joint positions and mandibular dimensions after correction of dental factors restricting mandibular growth in patients with Class II division 1 or division 2 malocclusion in the pubertal growth period. Methods: This prospective clinical study included 14 patients each with Class II division 1 (group I) and Class II division 2 (group II) malocclusions. The quad-helix was used for maxillary expansion, while utility arches were used for intrusion (group I) or protrusion and intrusion (group II) of the maxillary incisors. After approximately 2 months of treatment, an adequate maxillary arch width and acceptable maxillary incisor inclination were obtained. The patients were followed for an average of 6 months. Intraoral and extraoral photographs, plaster models, and cone-beam computed tomography (CBCT) images were obtained before and after treatment. Lateral cephalometric and temporomandibular joint measurements were made from the CBCT images. Results: The mandibular dimensions increased in both groups, although mandibular positional changes were also found in group II. There were no differences in the condylar position within the mandibular fossa or the condylar dimensions. The mandibular fossa depth and condylar positions were symmetrical at treatment initiation and completion. Conclusions: Class II malocclusion can be partially corrected by achieving an ideal maxillary arch form, particularly in patients with Class II division 2 malocclusion. Restrictions of the mandible in the transverse or sagittal plane do not affect the temporomandibular joint positions in these patients because of the high adaptability of this joint.

Proliferative periostitis of the mandibular ramus and condyle: a case report

  • Proliferative periostitis is a rare form of osteomyelitis that is characterized by new bone formation with periosteal reaction common causes of proliferative periostitis are dental caries, periodontitis, cysts, and trauma. While proliferative periostitis typically presents as a localized lesion, in this study, we describe an extensive form of proliferative periostitis involving the whole mandibular ramus and condyle. Because the radiographic findings were similar to osteogenic sarcoma, an accurate differential diagnosis was important for proper treatment.

한국인 정상교합자의 하악치열궁 형태에 관한 연구 (A STUDY OF MANDIBULAR DENTAL ARCH FORM OF THE KOREAN WITH NORMAL OCCLUSION)

  • 이 연구는 한국인 하악치열궁 형태를 분류하고, 교두정에서 브라켓 높이의 협면점까지 거리를 계측하여 이상적인 호선 제작에 도움을 얻기위하여 시행되였다. 13세에서 25세사이의 정상적인 교합자 159명의 석고모형을 대상으로 순측 및 협측 교두정을 지나는 하악치열궁선에서 견치간, 제1대구치간 및 제2대구치간 폭경과 이들의 고경을 계측한 다음, 유클리안 거리를 이용한 평균군집분석을 시행하여 치열궁형태를 5가지로 분류하고, 각각의 6차 다항식과 결정계수를 구하였으며, 교두정에서 브라켓 높이의 협면점까지의 거리를 계측하고 비교검토하여 다음의 결과를 얻었다. 1. 5가지 하악치열궁선을 실제의 크기로 작도하고, 1형, 2형, 3형, 4형 및 5형으로 명명하였다. 2. 각각의 치 열궁선은 하악 제2대구치간 폭경으로 표현되는 6차다항식과 결정 계수를 산출하였으며 이 식으로 $97\%$이상 설명이 가능하였다. 3. 각 치열궁선의 분포는 5형이 $24.5\%$로 가장 많았으며, 2형과 3형이 각각 $20.8\%$ 1형은 $17.6\%$, 4형은 $16.3\%$의 분포를 보였다. 4. 한국인의 하악치열궁선은 프랑스인에 비하여 폭경이 넓고 고경이 낮은 특성을 보였다. 5. 교두정에서 브라켓높이의 협면점까지의 거리는 절치가 1.0, 견치는 1.9, 제1소구치는 2.6, 제1 및 제2 대구치는 2.7 이였다.

Application of Plate and Circumferential Wiring with Resin to Bilateral Mandibular Fractures in a Calf: Case Report

  • A 2-month-old, 50 kg male Korean native calf was referred with bilateral mandibular fractures. The bilateral mandible was hanging loose, with inability to use the jaw, indicated by excessive salivation. Radiography revealed a long oblique fracture in the right mandible, and a short oblique fracture in the left mandible. Computerized Tomography, a different form of radiography, revealed the fracture in the left mandible to be a comminuted fracture. In order to stabilize the bilateral mandible fractures, a combination of bone plate and wiring was applied; in addition, resin was applied to enhance stabilization of the mandible. Six weeks after surgery, the calf was able to masticate and ruminate well. In this case, application of the plate and wiring with resin to bilateral mandibular fractures presented a successful functional recovery. Accordingly, the combination of bone plate and wiring with resin can be an effective technique for treating mandibular fractures.

Comparison between dental and basal arch forms in normal occlusion and Class III malocclusions utilizing cone-beam computed tomography

  • Objective: The purpose of this study was to investigate the relationship between the mandibular dental and basal arch forms in subjects with normal occlusion and compare them with those of Class III malocclusion using cone-beam computed tomography (CBCT). Methods: CBCT images of 32 normal occlusion (19 males, 13 females; 24.3 years) and 33 Class III malocclusion subjects (20 males, 13 females, 22.2 years) were selected. Facial axis and root center points were identified from the left to right mandibular first molars. Distances between the facial axis and root center points for each tooth were calculated, and 4 linear and 2 ratio variables were measured and calculated for each arch form. The variables were compared between groups by independent t-test. Pearson correlation coefficient was applied to assess the relationships between dental and basal variables within each group. Results: The mandibular dental and basal intercanine widths were significantly greater in the Class III group than in normal occlusion subjects (p < 0.05). The dental and basal intercanine widths as well as the dental and basal intermolar widths were strongly correlated in normal occlusion and moderately correlated in Class III malocclusion. Conclusions: The dental arch form demon strated a strong positive correlation with the basal arch form in the normal occlusion group and moderate correlation in the Class III malocclusion group. These results might be helpful for clinicians to have a better understanding of the importance of basal arch form in the alveolar bone.
