여주지역 영유아 대상 영양플러스 사업 효과 - 영유아 영양보충 및 보호자 영양교육 실시에 따른 영유아의 영양개선효과를 중심으로 - (The Effect of Nutrition Plus Program among 0~5 Year Children in the Yeojoo Area - The Improvement in Nutritional Status of Children after Nutrition Supplement of Children and Nutrition Education in Parents/Guardians -)

  • This study evaluated the effect of nutritional improvement of 0~5 year children of financially poor families after nutrition supplement of children and nutrition education in parents/guardians. The subject selected among applicants for low-income family financing of the government included 198 children (0~12 months:92, 1~5 years:106), and the average ages were 5.3 months and 2.4 years respectively. Food that includes each nutrient factor was provided to the subject for six months. Nutrition education was performed in two ways, through class and private education, once a month for both. Nutritional improvement was evaluated by body measurement and anemia prevalence rate. As to the survey on breast-feeding, complete breast-feeding accounted for 58.7%, weaning food 27.3%, and combined feeding 14.1% respectively, which shows that breast-feeding accounted for the largest percentage. The education program improved knowledge and attitude of the parents/guardians, which brings in positive effects on growth of children as well. The nutrition knowledge score was improved after nutrition education in 0~12 month children $(8.5\pm1.75\;to\;8.9\pm1.69)$ and 1~5 year children $(7.3\pm1.39\;to\;7.8\pm1.30)$. The nutrition attitude score was significantly improved in 0~12month children $(52.0\pm6.99$ to $53.5\pm5.21$, p < 0.001) and 1~5year children ($45.1\pm7.30$ to $49.0\pm5.96$, p < 0.001). In evaluation on nutritional improvement based on body measurements, it turned out that the wasting was reduced as for children in consideration of the height and weight according to the subject's age. The concentration of hemoglobin after the nutrient treatment for six months was significantly improved, and the anemia prevalence rate as well was reduced from 40.6% to 13.5%, which indicates the significant improvement. Based on the findings above, it turned out that providing quality food to children and nutrition education to the parents/guardians poorly fed in low-income families improved knowledge and attitude of the guardians and prevents anemia and improves growth, which brings in positive effects on growth of children. In the results regarding attitude and knowledge on nutrient/dietary life; however, the improvement level was relatively low when the extent before the education was too small, which indicates the need to revise and complement the contents and methods. In addition, as shown in the result of body measurement, long-term and consistent investigation is necessary since it is difficult to judge the effect on growth only based on short-term nutrient supplement.

  • 우리나라 대도시 어디에서나 볼 수 있는 빈민지역이 어떻게 형성되었으며 도시내에서 어떻게 공간이동을 하였는가를 알기 위하여 대구를 사례로하여 고찰하였다. 연구결과를 요약하면 다음과 같다. 1) 대구의 빈민형성은 해방 및 6.25 동란을 겪으면서 그 수가 증대되어 1차적으로 도심가까이에 빈민지역을 형성하였고 그 후 도시와의 진전에 따라 도시주변지역의 무단점거에 의한 2차적 빈민지역이 형성되었다. 2) 최근에서 도시재개발사업에 환경이 개선되어 일반거주지역으로 전환되었거나 혹은 대규모 아파트단지가 건립되어 저소득층은 그 곳에 거주하지 못하고 도시근교의 영구 및 임대아파트 단지내에 대규모로 집단화하여 거주지의 수평적 이동이 이루어지게 되었다.

임대주택 커뮤니티시설의 거주자 이용현황 및 개선 요구 (Resident's Usage and needs for the improvement of Community Facilities in Public Rental Apartments)

  • Nowadays, public rental apartments' community facilities emphasize only their quantity aspect without considering residents' characteristics, leading their installation regulations to be based on only the number of households, which is problematic. As a result, in many cases, residents, after moving in, alter the facility usage or remodel the facilities for their use. Hereby, this study conducted focus group interviews with public rental apartments' residents as to find out space conditions of and improvement demands for the community facilities. Two public rental apartment complexes in Seoul and Gyeongi-do which were built after 2007 were selected for this study. As for the survey method, workshop panel was employed. Four people in each age class of infants, children, adolescents, adults, and seniors were formed into one group, and a total of 40 people in 10 groups were surveyed. The survey was continuously conducted from February to March in 2009. The study findings can be summarized as follows. 1) Among community facilities, there were more satisfactory outdoor facilities than satisfactory indoor facilities. Residents expressed satisfaction with the facilities linked with natural environment. The causes of unsatisfactory facilities were mostly poor management. 2) The facilities demanded by the residents included ones relevant to study, exercise, and hobbies. As public rental apartments are resided mostly by low-income families, study and hobby activities bring them cost problems. Therefore, it is needed that the apartment complexes support diverse such activities 3) As for facilities demanded for usage alteration, a residents' meeting room accounted for the most popular opinions. As it was used only once a week, its usage was very low. It should be opened to the residents and allowed to be used in various ways. 4) As for demands of changing spatial characteristics, outdoor exercise facilities accounted for the most opinions. Although they were installed in most cases, they were damaged and poorly managed, therefore being in need of improvement in operation and management. In order to maximally utilize public rental apartments' community facilities, facility planning in consideration of various age groups from residents' viewpoint and sustainable systematic management are required.

유소년 약시환자의 생활환경 특성과 학교신체검사 안검진의 신뢰성 (The Reliability of Eye Screening Physical Examination in School and Living Environment Characteristics of Young Children with Amblyopia)

  • 유소년 약시환자의 생활환경특성을 알아보기 위해 2013년 9월 2일부터 11월 23일까지 약시진단을 받은 만 3세에서 12세의 안과 내원환자의 보호자 104명을 대상으로 설문조사하여 결과를 분석하였으며, 약시 진단시기는 만 4세(28.8%)때 가장 많이 진단을 한 것으로 나타났다. 또한 보호자 연령이 높을수록 약시 진단시기가 늦은 것으로 나타났으며(p<.01), 부모의 교육수준이 높을수록 약시 진단시기가 빠른 것으로 나타났다(p<.05). 위의 결과에 따라 유소년약시의 최초 진단시기와 생활환경 특성이 서로 밀접한 관련이 있음을 알 수 있으며 사회 환경적 요인과 생활환경 특성이 관련 있음을 알 수 있었다. 이러한 결과는 향후 지역의 소외계층이나 직업, 연령, 학력의 지형적인 분포에 따라 적극적인 계몽과 사회제도적 개선으로 국민 안건강 향상에 디딤돌이 되었으면 한다.

경남 일부 지역 여자 노인의 건강 및 영양 상태 조사 (A Study on Health Conditions and Nutritional Status of Elderly Women in Gyeongnam)

  • This study was performed to assess the nutritional status of low income elderly women aged ${\geq}$65 years residing in Gyeongnam Masan (n=124). Nutrition intakes, food intake frequency, and health-related behaviors including smoking, drinking, and exercise were investigated. Nutrition intake was calculated by the 24-hour recall method using CAN-pro (ver. 3.0). Average daily intakes of energy were $1,142.3{\pm}39$ kcal (71.4% of EER) in subjects aged 65~74 years and $1,071.0{\pm}41.7$ kcal (66.9% of EER) in subjects aged ${\geq}$75 years and the subjects consumed energy less than both 75% of estimated energy requirement (EER). The proportions of energy derived from protein, fat, and carbohydrate were 15.4:15.5:70.6 (aged 65~74 years), and 15.3:13.4:70.8 (aged ${\geq}$75). Nutrients consumed at less than estimated average requirements (EARs) were Ca (60.4%), P (98.4%), Zn (91%), vitamin E (48% of adequate intake, AI), vitamin $B_1$ (63.3%), vitamin $B_2$ (54%), niacin (87.7%), vitamin C (62.5%), and folate (50.5%). Especially, the intakes of Ca (58%), vitamin E (41% of AI), vitamin $B_1$ (60%), vitamin $B_2$ (50%), folate (46.5%), and vitamin C (54%) were 75% less than the EAR for people aged ${\geq}$75 years. According to the food intake frequency survey, the intakes of calcium, milk, fruits, and vegetables were very poor. In conclusion, this study suggests that a nutritional support program for elderly women of low socioeconomic class must be provided by the government to improve the quality of remaining life.

The Influence of Maternal Educational Level on the Oral Health Behavior of Korean Adults

  • Background: Parental attention is crucial for preventing childhood oral diseases. Mothers play a significant role in maintaining their families' oral health, and their educational level influences their children's oral health behaviors. This study investigates the impact of mothers' educational levels on adult oral health behaviors using data from a national survey. Methods: This study employed a cross-sectional analysis of secondary data. The data used were obtained from the 8th Korea National Health and Nutrition Examination Survey. Descriptive statistics were calculated to identify participant characteristics. Next, t-tests and one-way analysis of variance were conducted to examine the effects of the explanatory variables on the distribution of the dependent variable. Finally, logistic regression analysis was used to investigate the influence of the explanatory variable on the dependent variable, using "no education" as the reference value, and calculate the odds ratios. Results: Children of mothers with a college education or higher had a 1.13 times higher likelihood of receiving oral examinations than those whose mothers had no education. Children whose mothers graduated from college or higher had a 2.23 times higher probability of receiving preventative dental treatment than those whose mothers had no education. Children whose mothers graduated from college or higher had a 1.92 times higher probability of receiving scaling than those whose mothers had no education. Children whose mothers graduated from high school had a 1.35 times higher probability of receiving scaling than those whose mothers had no education. Conclusion: Developing oral health programs is important for low-educated and low-income parents to change theirs and their children's oral health behaviors/attitudes. This will help reduce oral health disparities among adults raised by parents of higher and lower socioeconomic statuses. Therefore, a comprehensive approach is essential for adults to maintain good oral health, regardless of variations in their parental educational levels during childhood.

중증질환자의 민간의료보험 가입 현황 및 관련요인 (Status and related factors of private health insurance for severely ill patients)

  • 이 연구의 목적은 중증질환자들을 대상으로 민간의료보험 가입자와 미가입자간의 특성을 파악하고, 가입 요인을 분석하기 위함이다. 연구도구는 2015년도 국민건강영양조사 자료를 이용하였으며, SPSS ver 23.0을 사용하였고, 연구 대상자는 19세 이상 중증질환자(암, 심장질환, 뇌혈관질환) 417명을 선정하였다. 분석방법은 민간의료보험 가입 현황과 차이를 파악하기 위해 교차분석을 실시하였고, 민간의료보험 가입에 미치는 요인을 분석하기 위해 이항 로지스틱 회귀분석을 실시하였다. 분석 결과 사회 인구학적 특성에서는 성별, 거주지, 혼인상태와 관계없이 나이가 젊을수록 높았다. 그리고, 사회 경제적 상태에서는 교육수준과 관계없이 가구소득이 높을수록, 의료급여대상자에 비하여 건강보험대상자의 가입률이 높았다. 건강행태에서는 주관적 건강 인식 및 걷기 실천과 무관하게 활동제한이 있는 집단의 민간의료보험 가입률이 낮았으며, 설명력은 51.7%이었다. 그러므로, 중증질환자 중 민간의료보험에 가입하지 못한 집단을 선별하여 생활환경 개선 및 지방자치단체의 지원을 촉구해야 할 것이다. 또한, 사회적 취약계층 및 건강행태에 제한 있는 집단을 대상으로 중증질환 예방을 위한 정부의 보건교육 또는 프로그램 제공 등 다양한 보건정책을 고려해야 할 것이다.

관공호텔 조리직무의 분업과 통합에 따른 문제점과 개선방안에 관한 연구 (A Study on Effective Management & Administration System for Deluxe Hotel Kitchen in Seoul Area.)

  • Despite prologed business stagnation of both international and domestic economy, hotel business as well as tourist industry has continuously been keeping growing, owing to increase of surplus income and world flowing population. During recent 4 years, growth rate of yearly mean in domestic hotels reached 9.9% and especially that of the superior class hotels 15.2%. In the composition of domestic tourist hotel's revenue, the earnings of guest rooms form 37.4%, on the other hand those of food & beverage 39.9%. This result is that our hotel business is concentrated on its interest in FOOD & BEVERAGE of which productivity per unit dimension can be increased to an unlimited extent and extent and superior class hotels strengthened in F&B are increasing in comparison with European or American hotels which are focused on guest rooms in their management. For value added rate of F&B is low as compared with increase of their earnings, they are interested in the management techniques which focus on rising the rate. As for the cost of Food & Beverage, personnel expenditure forms 36.5% and the direct materials 31.5%. Therefore how to manage personnel and materials costs which compose as much as 68% of total revenue will greatly affect net profit. We can say that an effective management technique in cost of Food & Beverage is one of the most important know-hows in hotel management. Especially management know-how for the Kitchen Department where the most of foods come out makes a great effects on various expenses, productivity and it is the achievement from hotel management. For the most of the hotel's top managers, they don't seriously take the fact that KITCHEN SYSTEM affects greatly total expenditure. This study starts from the point of recognizing the question of fundamental cause affecting tow largest cost elements incurred in Food & Beverage and trying to present an effective kitchen system. To settle the questions raised, I compared and analyzed productivity and cost of food & beverage and unit kitchen centered around superior class hotels in Seoul, which vary in Kitchen Systems. In order to attain the aforementioned study effectively purpose of this study, I compared Room-Service and Coffee-Shop Menu, flow of basic food in the kitchen, extent and result of division of labor and integration in the kitchen, scale of outlet kitchen, productivity, the turnover rate of food in store, food cost rate one another which all vary in Kitchen Systems. All these elements are compared and analyzed each other being divided into two main groups such as①. Main Production kitchen and Banquet Kitchen, and ②. coffee-shop kitchen and Room-service Kitchen. Therefore this study is to point out the problems in managing kitchens of superior class hotels which are different in systems. An effort was made to find out the better Kitchen System for superior deluxe hotels. I emphasize the followings on the proper scale of division of labor and integration of unit kitchen and a disposition plan for outlet kitchens of restaurant. First, KITCHEN SYSTEM as a sub-system of Hotel Management System is composed of sub-systems of outlet unit kitchen. Basic food materials are cooked and served for the guests while support kitchen and out restaurant kitchen interact organically each other. So Kitchen should be considered as a system composed of integrated sub-systems. Second, support and banquet kitchens should be integrated to be managed. And these unit kitchens have to be designed to be placed in the back of banquet rooms area. Third, coffee-shop kitchen and room-service kitchen should be integrated to be managed. Fourth, several unit business kitchens should be place on the same floor. Fifth, main production kitchens ought to be located near the loading duck, food store and large refrigerator. Sixth, considering the limits of supervision, duties should be adjusted as 12-20 cooks in two shifts a day for a sub-kitchen, and 18-30 cooks in three shifts a day so that labor division can be made. Last, I would like to two points for direction and task of future study. Firstly, I compare the effective income and increasing costs each other, which are incurred by increasing the use rate of the second processing materials for foods perched outside and through the results. I can find out the better points of the processing production and circulation system, and then I study this effects made on hotel kitchen system. Secondly, I can point out that more efficient kitchen system shall be established through comparing and analyzing the matter of amount of indirect costs and flow of food in different kitchen systems.

대구시민의 의료기관 이용률과 연관요인 (Utilization Rate of Medical Facility and Its Related Factors in Taegu)

  • 도시지역 주민의 의료기관 이용률과 그 관련요인을 조사분석 하기 위하여 1988년 7월 3일부터 7월15일까지 1단계 단순집락 표본추출을 통하여 선정한 대구시 남구 지역의 450가구를 대상으로 면접조사를 실시하여 431가구 1,723명에 대한 자료를 분석하여 다음과 같은 결과를 얻었다. 대상자 1,000명당 조사기간 2주간의 급성이환율은 101이었고 9세이하군에서 202로 가장 높았고, 지난 1년간 만성이환율은 77이었으며 고연령층, 저소득층 그리고 의료보호 적용군 등에서 유의하게 높았다(p<0.01). 대상자 1,000명당 2주간의 의료기관 이용률은 689이었고, 이중 병의원이용률이 294로 가장 높았고, 약국 보건소 그리고 한의원 및 한약방 순이었다. 성별로는 여자가 785회로 남자 591보다 높았으며, 연령별로는 70세이상 군이 2,022회로 가장 높았다(p<0.01). 그외 특성에서는 의료보호 적용군 2,057(p<0.01), 월30만원미만 소득군 346, 자영업이 907로 가장 높았다. 2주간의 의료기관 이용자 1인당 평균 방문회수는 3.25회이었고, 이중 병의원이 3.26회로 가장 많았으며, 한의원 및 한약방, 약국 그리고 보건소 순이었다. 성별로는 여자가 3.47회로 남자보다 많았다. 그외 50대연령군이 5.02회, 의료보호적용군 6.41회, 월30만원미만 소득군 3.78회, 그리고 생산직이 3.64회로 가장 많았다. 대상자 1,000명당 연간입원율은 27.6이었고, 여자 38.9 남자 16.3으로 여자가 높았고 연령별로는 70세이상군이 133.3으로 가장 높았다. 의료보장상태별로는 의료보험적용군이 비적용군보다 2배이상 높았으며, 의료보호적용군에서는 한건도 없었다. 월 30만원미만 소득군이 20.8로 가장 낮았으며 소득이 증가할수록 입원율이 높아졌다. 직업별로는 무직 및 기타직이 35.9로 가장 높았고, 전문, 관리 및 사무직이 가장 낮았다. 입원의료 이용자 1인당 연평균 입원일수는 총 22.5일이었으며, 성별로는 남자가 28.1일로 여자보다 많았다. 연령별로는 40대가 72.3일로 가장많았다. 직장, 직종 및 지역의료보험적용군이 28.8일, 월 30만원미만 소득군이 44.5일 그리고 무직 및 기타직이 21.9일로 가장 많았다. 대상자 1,000명당 연간 총 입원일수는 560일이었으며, 여자가 661일로 남자보다 많았으며, 연령별로는 70세이상군이 2,800일로 가장많았다. 의료보험 적용군이 비적용군보다 3배이상 많았으며, 직업별로는 무직 및 기타직이 789일로 가장 많았다. 2주간 병의원 이용여부를 종속변수로 한 지수형회귀분석에서 유의한 계수는 9세이하군(+), 70세이상군(+), 급성질병(+), 만성질병(+), 공 교의료보험적용(+), 직장, 직종 및 지역의료보험적용(+) 그리고 전문, 관리 및 사무직(-) 등이었다. 약국이용여부 분석에서는 9세이하군(+), 50-69세군(+), 70세이상군(+), 금성질병(+), 만상질병(+) 그리고 공.교의료보험적용(-) 등이 유의하였다. 2주간의 병의원 외래이용회수에 대한 중회귀분석에서는 급성질병(+), 만성질병(+) 직장, 직종 및 지역의료보험적용(+), 전문, 관리 및 사무직(-) 등이 유의한 변수였으며, 약국이용회수 분석에서는 급성질병(+), 만성질병(+), 공.교의료보험적용(-) 그리고 직장, 직종 및 지역의료보험적용(-)등이 유의하다. 연간 입원의료 이용여부에 대한 지수형 회귀분석에서 통계적으로 유의한 변수로는 9세이하군(+), 70세이상군(+), 만성질병(+), 공.교의료보험적용(+) 그리고 직장, 직종 및 지역의료보험적용(+) 등이었다.

산촌(山村)의 노동력(勞動力) 변화(變化)에 관(關)한 연구(硏究) (A Study on the Labor Change in Mountainous Villages)

  • 본(本) 연구(硏究)는 임업(林業) 생산(生産)의 주축(主軸)을 형성(形成)하고 있는 산촌(山村)이란 특수지역(特殊地域)을 대상(對象)으로 산촌입구(山村入口)의 대량(大量) 유출(流出) 현상을 고찰하고 산촌(山村) 노동력(勞動力)의 변화(變化)가 농가경영(農家經營)과 임업생산(林業生産)에 미치고 있는 파급(波及) 양상(樣相)을 분석(分析) 검토(檢討)하고자 시도 되었으며 그 결과(結果)는 다음과 같다. 1. 산촌내(山村內) 입구(入口)의 대량(大量) 유출(流出)은 70년대(年代) 중반(中半)부터 표면화(表面化)되고 있으며 전출(轉出) 속도(速度)는 점차 가속화(加速化) 하고 있다. 2. 입구(入口) 유출(流出)은 임업생산(林業生産)이 활발(活發)치 못한 지역(地域)에서 임업생산(林業生産)이 활발(活發)한 지역(地域)보다 더욱 심하다. 3. 임업(林業)이 선진(先進)한 산촌(山村)에서는 임업생산(林業生産) 활동(活動)을 통(通)해 농가(農家)의 잉여축적(剩余蓄積)이 가능(可能)하였고 농민층(農民層)의 분화(分化) 현상(現象)이 나타나고 있으며 농촌업(農村業)을 중심(中心)으로 하는 농가(農家)의 겸업화(兼業化)가 진행(進行)중에 있다. 4. 산촌(山村)의 입구유출(入口流出)은 16~30재(才) 계층(階層)이 82~85%를 차지하고 있으며 결과적(結果的)으로 노동력(勞動力)의 노령화(老令化) 부녀화(婦女化) 등 질적(質的) 저하(低下) 현상이 현재화(顯在化)하고 있다. 5. 임업노동(林業勞動) 시장(市場)의 성격(性格)은 유촌지역내(由村地域內)에서 수급(需給)이 조절되던 과거(過去)의 폐쇄적(閉鎖的)인 모습에서 개방적(開放的)인 모습으로 변화(變化)되어 가고 있다.
