요양병원 간호사의 간호 스트레스 경험: 현상학적 접근 (Experience of Job Stress among Nurses Working in Long-term Care Hospital: A Phenomenological Approach)

  • Purpose: The purpose of this study was to explore the subjective experience of job stress among nurses working in long-term care hospitals. Methods: A phenomenological approach was used for the study. Data were collected from May to June, 2016 using open-ended questions during in-depth interviews. Participants were nurses working in long-term care hospitals and had reported experiences of stress. Six nurses participated in this study. Results: Six themes emerged from the analysis using Colaizzi's method: (a) Heavy workload and responsibility due to nurse shortage, (b) Getting exhausted by caring for cognitively impaired patients, (c) Feeling pressure due to conflict with patients' family, (d) compassion for patients who are getting worse, (e) Low value in being a long-term hospital nurse, and (f) Efforts to overcome stress. Conclusion: Sufficient labor supply, environmental improvements, program for improving interpersonal skills, education and counseling on end-of-life care, and recognition improvement about long term hospital are suggested to reduce the job stress of long-term hospital nurses.

Long-term prenatal stress increases susceptibility of N-methyl-D-aspartic acid-induced spasms in infant rats

  • Purpose: Infantile spasms, also known as West syndrome, is an age-specific epileptic seizure. Most patients with this condition also exhibit delayed development. This study aimed to determine the effect of long-term prenatal stress on susceptibility to infantile spasms. Methods: We subjected pregnant rats to acute or chronic immobilization stress. Resulting offspring received N-methyl-D-aspartic acid (15 mg/kg, intraperitoneally) on postnatal day 15, and their behaviors were observed 75 minutes after injection. The expression of KCC2 and GAD67 was also determined using immunohistochemistry. Results: Exposure to long-term prenatal stress increased the frequency of spasms and decreased the latency to onset of spasms compared with offspring exposed to short-term prenatal stress. Expression of KCC2 and GAD67 also decreased in the group exposed to long-term prenatal stress compared with the group exposed to short-term prenatal stress. Conclusion: Our study suggests that exposure to long-term prenatal stress results in increased susceptibility to seizures.

요양병원 간호사의 감정노동, 직무 스트레스 및 전문직 삶의 질 (Emotional Labor, Job Stress and Professional Quality of Life among Nurses in Long-term Care Hospital)

  • Purpose: This study was aimed to identify the level of emotional labor, job stress and professional quality of life and to identify the factors affecting on professional quality of life among nurses in long-term care hospitals. Methods: 136 nurses working at eight different long-term care hospitals were recruited from May 1 to June 30, 2016. Data were analyzed by descriptive statistics, t-test, ANOVA, Pearson correlation, and stepwise multiple regression using SPSS/WIN 22.0. Results: Professional quality of life is consisted of three subcategories as compassion satisfaction, secondary traumatic stress and burn-out. As for the factors affecting on compassion satisfaction, age, satisfaction on working ward and shift pattern of duties were significant factors. The three variables' explanation power on compassion satisfaction was 25.0%. As for factors affecting on secondary traumatic stress, emotional labor was a significant factor. The emotional labor's explanation power on secondary traumatic stress was 13.0%. Factors affecting on burn-out, emotional labor, age, and health condition were significant factors. The three variables' explanation power on burnout is 31.0%. Conclusion: On the basis of results, program development are required to relieve emotional labor and job stress for nurses at long-term care hospitals and to improve their professional quality of life.

국내 적용되고 있는 흙막이구조물의 축력에 대한 관리기준치 분석 (A Control Value Analysis on the Axial Force of Braced Excavation Walls Used In Korea)

  • This study aims to present a more reasonable control value than the exiting one by comparing and analyzing control values and field instrumentation values of the whole excavation depth of the four case sites using geometric averaging as a statistical method. The range of the study is confined to three things: (1) the axial force of the braced excavation walls among a variety of items prescribed in the control values by stress deformation of walls and adjacent structures; (2) by approximation of the allowable and design value; (3) and by safety factor. As a res it is desirable to revise "(Long term allowable stress + Short term allowable stress)/2 ~ Short term allowable stress," presented in the present control values by stress deformation of walls and adjacent structures, to "(Long term allowable stress + Short term allowable stress)/5 ~ (Short term allowable stress)/3." The result also shows that since there is a difference of about 3.5%, it is not necessary to revise 70, 90, and 100 percent of LEVEL I, II, and III, prescribed in the control values by the allowable and design value approximation. In addition, modifying the control value by the safety factor, now 1.07, is unnecessary, although it varies little difference from the present value.

노인장기 요양시설 종사원의 서비스 지향성 결정요인에 관한 연구 (Determinant Factors of Service Orientation for Human Resources of Long Term Care Facility)

  • Purpose - This study is to propose discriminative management strategies to long-term care facilities based on the empirical analysis after reviewing the effects of social support, perceived by long-term care facility employees, to service orientation. Research design, data, and Methodology - The research model designed social support, job stress, organizational commitment, and service orientation. The survey collected data from 453 customers in a long-term care facility in jeju. The SPSS 18.0 package was used for analysis. Results - First, social support for long-term care facility employees has a negative(-) effect to job stress. Test results, social support factors except appraisal support had a negative impact on job stress. Second, social support has a positive(+) effect to organizational commitment. Test results, informational support, tangible support and appraisal support had significant effects on organizational commitment. However, emotional support had a positive impact on affective commitment and normative commitment. Third, social support has a positive(+) effect to service orientation. Test results had a positive impact. Fourth, job stress has a negative(-) effect to organizational commitment. In the test results, employee's continuance commitment and normative commitment had significant negative effects in job stress. However, affective commitment had no significant impact. Fifth, job stress has a negative(-) effect to service orientation. Test results showed a negative impact. Conclusions - The study implies the following. First, that there should be a change in the social perception of long-term care facilities. 'Long-Term Care Insurance for The Elderly' was enacted to emphasize this responsibility for the elderly problems as a new system. Enactment of this Act was expected to improve the quality of life of the people by stabilizing the elderly life and reducing the burden of families. Therefore, long-term care facility system should be as efficient as possible for making plans for systematic and organizational support. Second, the efforts of facility managers to minimize job stress of employees is necessary. Accordingly, performing spontaneous work is required for a comfortable working environment and management. Third, the systematic education and training to employees for service oriented behavior of the facility will be required in the long term.

노인장기요양보험에서 1등급 노인과 3등급 치매노인을 케어하는 요양보호사의 직무스트레스와 직무만족에 관한 비교연구 (A Comparative Study on Job Stress and Job Satisfaction of Long-Term Care Staffs for Physically Disabled Seniors with Rating 1 and for Demented Seniors with Rating 3)

  • 본 연구는 노인장기요양시설에 입소한 노인 중 1등급 노인과 3등급 치매노인을 케어하는 요양보호사들의 직무스트레스(전체, 서비스대상 관계스트레스, 서비스제공 스트레스)를 비교하고 직무만족의 하위변인(대우만족, 상사관계 만족, 동료관계만족, 성장만족)에 영향을 미치는 변수를 알아보기 위한 것으로, 경기북부 권 파주, 고양, 의정부시의 노인장기요양시설에 근무하는 요양보호사 212명을 대상으로 이루어졌다. 독립표본 t-검증과 다중회귀분석을 실시한 결과는 다음과 같다. 첫째, 두 집단(1등급 노인을 케어하는 요양보호사 집단, 3등급 치매노인을 케어하는 요양보호사 집단)의 직무스트레스는 '직무스트레스 전체', '서비스대상 관계스트레스', '서비스제공 과정스트레스'에서 통계적으로 유의미한 차이를 보였다. 둘째, 집단별 직무만족에 직무스트레스 하위변인이 어떻게 영향을 미치는지 회귀 분석을 실시한 결과 오직 3등급 치매노인을 케어하는 요양보호사 집단에만 직무스트레스가 '대우만족', '동료관계만족'에 영향을 미치는 예측변수로 나타났다. 구체적으로 '대우만족'에는 '과정스트레스'가, 그리고 '동료관계만족'에는 '관계스트레스', '과정스트레스', 그리고 '학력'이 유의한 변수로 나타났다. 본 연구의 의의는 기존 선행연구에서 다루지 못한 시설 내 요양보호사의 직무스트레스의 차이와 직무만족의 하위변인에 영향을 미치는 예측변수를 비교·분석한 연구의 시작이라는 것이며, 비록 같은 시설 내 근무하는 요양보호사라 할지라도 그들이 케어하는 노인의 등급과 상태에 따라 직무스트레스에 차이가 존재한다는 것을 주목할 필요가 있다. 그리고 직무만족의 하위변인에 영향을 주는 중요한 예측변수로 발견된 서비스대상 관계와 서비스제공 과정에서 발생하는 스트레스를 고려한 근무환경 개선과 스트레스를 경감시킬 수 있는 방안의 모색이 필요하다.

재가요양보호사의 직무스트레스와 이직의도의 관계에서 직업의식 매개효과 검증 (The Mediating Effort of Vacational Consciousness on the Relationship between Job Stress and Turnover Intention in Long-Term Care Worker)

  • 본 연구는 재가요양보호사의 직무스트레스와 이직의도와의 관계에서 직업의식 매개효과를 검증하고, 그 결과를 기초하여 재가요양보호사의 이직의도를 감소시키려는 목적으로 수행하게 되었다. 연구대상은 서울·경기지역에서 일하고 있는 재가요양보호사 447명을 대상으로 하였다. 연구목적을 위해 자료분석은 SPSS 통계패키지를 활용하여 기술분석과 상관관계 분석 및 다중공선성 확인 후 매개효과를 검증하였다. 분석결과 첫째, 재가요양보호사가 연령이 낮고, 월가구소득이 높을수록 이직의도가 높을 것으로 나타났다. 둘째, 재가요양보호사가 직무스트레스를 높게 인식할수록 이직의도가 높아지는 것으로 나타났다. 셋째, 직업의식이 높을수록 이직의도는 낮게 나타났다. 넷째, 재가요양보호사의 직무스트레스는 직업의식을 부분 매개하여 이직의도에 통계적으로 유의미한 영향을 주는 것으로 나타났다. 이상의 결과를 토대로 재가요양보호사의 이직을 예방하고 노인돌봄 서비스의 질을 향상시키기 위한 임상적·정책적 실천에 대한 제언을 하였다.

요양병원간호사의 윤리적 딜레마와 임종간호스트레스가 소진에 미치는 영향 (The Influence of Ethical Dilemma and End-of-life Care Stress on Burnout in Long­term Care Hospital Nurses)

  • 본 연구의 목적은 요양병원 간호사를 대상으로 윤리적 딜레마, 임종간호스트레스 및 소진의 관계와 윤리적 딜레마와 임종간호스트레스가 소진에 미치는 영향을 파악하고자 함이다. 본 연구의 대상자는 경상남도에 위치한 7개 노인요양병원에 근무하고 임종간호 경험이 한 번 이상 있으며 연구의 목적을 이해하고 연구 참여에 동의한 간호사 143명을 대상으로 구조화된 설문지를 이용하여 설문조사하였다. 본 연구결과 연령과 직위, 임종간호교육, 임종간호스트레스가 요양병원 간호사의 소진에 영향을 미치는 요인으로 확인되었다. 따라서 요양병원간호사의 소진을 예방하고 완화시키기 위해 임종간호 관련 교육과 함께 스트레스를 관리하는 체계적인 중재프로그램이 필요할 것이다.

주간보호시설과 요양시설 치매 환자 가족의 부양 스트레스와 시설간호서비스 만족도 (Family Member's Caregiving Stress and Satisfaction of Care for Patients with Dementia in Long Term Care Facilities and Day Care Centers)

  • Purpose: This study was to explore family member's caregiving stress and satisfaction of care for patients with dementia in long term care facilities and day care centers. Methods: The subjects were 129 family members of elders with dementia from ten long term care facilities and eight day care centers in Daegu and Kyungsangbukdo. Data were collected from August to October, 2007. The instruments were self administered questionnaires and included. The Family Perceptions of Caregiving Role (FPCR) and the Family Perceptions of Care Tool (FPCT) which were developed by Maas and Buckwalter (1990) and translated by Park (2002). Results: Family member's care giving stress in day care centers were higher than that of long term care facilities (t=-2.89, p=.005) especially in the categories of captivity (t=-3.27, p=.001), guilty (t=-2.93, p=.004), and loss (t=-2.44, p=.016). Family member's satisfaction of care in day care centers was higher than that of long term care facilities (t=-3.21, p=.002) in the (use - categories or measures since you are referring to the instrument and delete aspects) aspects of effective management (t=-3.69, p=.000) and activity (t=-2.00, p=.045). Conclusion: The results of this study showed that family members' perceptions toward their care giving roles and satisfaction of care differ whether the facilities are long term care or day care centers. This study provides baseline data that could be used for improving the quality of long term care services.

Experimental investigation of long-term characteristics of greenschist

  • The greenschist in the Jinping II Hydropower Station in southwest China exhibits continuous creep behaviour because of the geological conditions in the region. This phenomenon illustrates the time-dependent deformation and progressive damage that occurs after excavation. In this study, the responses of greenschist to stress over time were determined in a series of laboratory tests on samples collected from the access tunnel walls at the construction site. The results showed that the greenschist presented time-dependent behaviour under long-term loading. The samples generally experienced two stages: transient creep and steady creep, but no accelerating creep. The periods of transient creep and steady creep increased with increasing stress levels. The long-term strength of the greenschist was identified based on the variation of creep strain and creep rate. The ratio of long-term strength to conventional strength was around 80% and did not vary much with confining pressures. A quantitative method for predicting the failure period of greenschist, based on analysis of the stress-strain curve, is presented and implemented. At a confining pressure of 40 MPa, greenschist was predicted to fail in 5000 days under a stress of 290 MPa and to fail in 85 days under the stress of 320 MPa, indicating that the long-term strength identified by the creep rate and creep strain is a reliable estimate.
