α-COMPLETELY POSITIVE MAPS ON LOCALLY C*-ALGEBRAS, KREIN MODULES AND RADON-NIKODÝM THEOREM

  • In this paper, we study ${\alpha}$-completely positive maps between locally $C^*$-algebras. As a generalization of a completely positive map, an ${\alpha}$-completely positive map produces a Krein space with indefinite metric, which is useful for the study of massless or gauge fields. We construct a KSGNS type representation associated to an ${\alpha}$-completely positive map of a locally $C^*$-algebra on a Krein locally $C^*$-module. Using this construction, we establish the Radon-Nikod$\acute{y}$m type theorem for ${\alpha}$-completely positive maps on locally $C^*$-algebras. As an application, we study an extremal problem in the partially ordered cone of ${\alpha}$-completely positive maps on a locally $C^*$-algebra.

Moderate hypofractionated image-guided thoracic radiotherapy for locally advanced node-positive non-small cell lung cancer patients with very limited lung function: a case report

  • Patients with locally advanced lung cancer and very limited pulmonary function (forced expiratory volume in 1 second $[FEV1]{\leq}1L$) have dismal prognosis and undergo palliative treatment or best supportive care. We describe two cases of locally advanced node-positive non-small cell lung cancer (NSCLC) patients with very limited lung function treated with induction chemotherapy and moderate hypofractionated image-guided radiotherapy (Hypo-IGRT). Hypo-IGRT was delivered to a total dose of 45 Gy to the primary tumor and involved lymph nodes. Planning was based on positron emission tomography-computed tomography (PET/CT) and four-dimensional computed tomography (4D-CT). Internal target volume (ITV) was defined as the overlap of gross tumor volume delineated on 10 phases of 4D-CT. ITV to planning target volume margin was 5 mm in all directions. Both patients showed good clinical and radiological response. No relevant toxicity was documented. Hypo-IGRT is feasible treatment option in locally advanced node-positive NSCLC patients with very limited lung function ($FEV1{\leq}1L$).

  • This paper introduces a local non-positivity of two set-valued mappings (F,Q) and considers the existences and properties of solutions for set-valued mixed vector FQ-implicit variational inequality problems and set-valued mixed vector FQ-complementarity problems in the neighborhood of a point belonging to an underlined domain K of the set-valued mappings, where the neighborhood is contained in K. This paper generalizes and extends many results in [1, 3-7].

  • Sulphur-hexafluoride has extensively been used as an insulating gas of electric power equipment in substation for its high dielectric strength, non-toixity and non-flammable properties. But it is very expensive and is liquidizable under the condition of low temperature and high pressure. And it is very sensible to the locally high electric field strength. To resolve these problems, we studied 50% FOV and V-t characteristics of $SF_6-N_2$ mixtures under positive and negative lightning impulses using a 25mm rod-plane gap. 50% FOV of $SF_6-N_2$ 50% mixture was about 85% of pure $SF_6$ and it was known that $SF_6-N_2$ mixtures can be used as an economic substitution insulating gas for $SF_6$.

  • Currently the most important prognostic factor in lung cancer is the stage. In the current lung TNM classification system, N category is defined exclusively by anatomic nodal location though, in other type of tumours, number of lymph nodes is confirmed to be a fundamental prognostic factor. Therefore we evaluated the number of mediastinal lymph nodes as a prognostic factor in locally advanced NSCLC after multimodality treatment, observing a significant effect of the number of lymph nodes in terms of OS (p<0.01) and DFS (p<0.001): patients with a low number of positive mediastinal nodes have a better prognosis.

  • We study the characteristic Jacobi operator ${\ell}={\bar{R}({\cdot},{\xi}){\xi}$ (along the Reeb flow ${\xi}$) on the unit tangent sphere bundle $T_1M$ over a Riemannian manifold ($M^n$, g). We prove that if ${\ell}$ is pseudo-parallel, i.e., ${\bar{R}{\cdot}{\ell}=L{\mathcal{Q}}({\bar{g}},{\ell})$, by a non-positive function L, then M is locally flat. Moreover, when L is a constant and $n{\neq}16$, M is of constant curvature 0 or 1.

Researches on the Enhancement of Plasticity of Bulk Metallic Glass Alloys

  • Bulk metallic glass (BMG) shows higth strength, high elastic limit, corrosion resistance and good wear resistance and soft magnetic properties and has been considering as a candidate for new structural materials. But they show limited macroscopic plasticity and lack of tensile ductility due to highly localized shear deformation, which should be solved for real structural application. In this paper researches on the enhancement of plasticity of BMG were reviewed briefly. Introducing heterogeneous structure in glass is effective to induce more shear transformation zones (STZs) active for multiple shear band initiation and also to block the propagating shear band. Several methods such as BMG alloy design for high Poisson's ratio, addition of alloying element having positive heat of mixing, pre-straining BMG and variety of BMG composites have been developed for homogenous distribution of locally weak region, where local strain can be initiated. Therefore enhancement of plasticity of BMG is normally accompanied with some penalty of strength loss.

  • The false-positive FDG uptakes on head and neck areas are common due to benign lesion, iatrogenic and physiologic changes. The Schneiderian papilloma is uncommon benign tumor arising from the mucosa of the paranasal sinuses and nasal cavity. The findings of paranasal papilloma on conventional modality such as CT and MRI are non-specific and they could be confused with inflammatory polyp or retention cyst. Despite of benign tumor, the papilloma usually shows locally aggressive growth with malignant potential, therefore the FDG can be actively accumulated in this lesion. We describe the case of 18F-FDG PET/CT finding in a 77-year-old woman who demonstrates oncocytic papilloma in maxillary sinus.

Serum Tumor Marker Levels might have Little Significance in Evaluating Neoadjuvant Treatment Response in Locally Advanced Breast Cancer

  • Background: To determine the potential value of serum tumor markers in predicting pCR (pathological complete response) during neoadjuvant chemotherapy. Materials and Methods: We retrospectively monitored the pro-, mid-, and post-neoadjuvant treatment serum tumor marker concentrations in patients with locally advanced breast cancer (stage II-III) who accepted pre-surgical chemotherapy or chemotherapy in combination with targeted therapy at Fudan University Shanghai Cancer Center between September 2011 and January 2014 and investigated the association of serum tumor marker levels with therapeutic effect. Core needle biopsy samples were assessed using immunohistochemistry (IHC) prior to neoadjuvant treatment to determine hormone receptor, human epidermal growth factor receptor 2(HER2), and proliferation index Ki67 values. In our study, therapeutic response was evaluated by pCR, defined as the disappearance of all invasive cancer cells from excised tissue (including primary lesion and axillary lymph nodes) after completion of chemotherapy. Analysis of variance of repeated measures and receiver operating characteristic (ROC) curves were employed for statistical analysis of the data. Results: A total of 348 patients were recruited in our study after excluding patients with incomplete clinical information. Of these, 106 patients were observed to have acquired pCR status after treatment completion, accounting for approximately 30.5% of study individuals. In addition, 147patients were determined to be Her-2 positive, among whom the pCR rate was 45.6% (69 patients). General linear model analysis (repeated measures analysis of variance) showed that the concentration of cancer antigen (CA) 15-3 increased after neoadjuvant chemotherapy in both pCR and non-pCR groups, and that there were significant differences between the two groups (P=0.008). The areas under the ROC curves (AUCs) of pre-, mid-, and post-treatment CA15-3 concentrations demonstrated low-level predictive value (AUC=0.594, 0.644, 0.621, respectively). No significant differences in carcinoembryonic antigen (CEA) or CA12-5 serum levels were observed between the pCR and non-pCR groups (P=0.196 and 0.693, respectively). No efficient AUC of CEA or CA12-5 concentrations were observed to predict patient response toward neoadjuvant treatment (both less than 0.7), nor were differences between the two groups observed at different time points. We then analyzed the Her-2 positive subset of our cohort. Significant differences in CEA concentrations were identified between the pCR and non-pCR groups (P=0.039), but not in CA15-3 or CA12-5 levels (p=0.092 and 0.89, respectively). None of the ROC curves showed underlying prognostic value, as the AUCs of these three markers were less than 0.7. The ROC-AUCs for the CA12-5 concentrations of inter-and post-neoadjuvant chemotherapy in the estrogen receptor negative HER2 positive subgroup were 0.735 and 0.767, respectively. However, the specificity and sensitivity values were at odds with each other which meant that improving either the sensitivity or specificity would impair the efficiency of the other. Conclusions: Serum tumor markers CA15-3, CA12-5, and CEA might have little clinical significance in predicting neoadjuvant treatment response in locally advanced breast cancer.
