중풍환자에서 장기간의 한약과 양약의 병용투여가 간장 및 신장에 미치는 영향 (The effects of Contant Use of Herbal Medicine with Western Medicine On Liver and Kidney Functions)

  • Object : The purpose of this study was to evaluate whether herbal medication may injure the liver function and kidney function in stroke patients. Method : We observed the serum AST, ALT, r-GTP, Creatinine, BUN of 78 patient from May 1. 2008 to June 30. 2009 admitted in Oriental Hospital of Daejeon University. Liver function and Kidney function tests were done on admission and before discharge. And all patients take herbal medicine with Western medicine. Result : AST, ALT, r-GTP, Creatinine, BUN decreased on discharge compared with the value on admission. The liver function and kidney function test score(AST, ALT, r-GTP, Creatinine, BUN) by administration period of herbal medication was not significantly changed. Conclusion : This study suggests that general herbal medication does not injure liver function and kidney function of human.

Protective Effect of ACTIValoe N-931 Complex, a Mixture of Aloe vera and Silybum marianum, on Experimental Acute Liver Injury

  • The aim of this study was to investigate the hepatoprotective effect of $ACTIValoe^{(R)}$ N-931 complex, a mixture of Aloe vera and Silybum marianum, against acute liver injuries. Acute liver damages were induced by intraperitoneal injection of galactosamine (GalN, 700 mg/kg), naphthylisothiocyanate (ANIT, 40 mg/kg) and ethionine (500 mg/kg). $ACTIValoe^{(R)}$ N-931 (85, 170 and 340) was administered orally 48 h, 24 h, 2 h before and 6 h after the injection of hepatotoxins. At 24 h after GalN treatment the levels of serum aminotransferases and hepatic lipid peroxidation were significantly elevated, whereas hepatic glutathione, serum triglyceride (TG) and total cholesterol were decreased. These changes were attenuated by $ACTIValoe^{(R)}$ N-931 complex. The serum aminotransferase activities and total bilirubin significantly increased at 48 h after ANIT treatment, but were attenuated by $ACTIValoe^{(R)}$ N-931 complex. The bile flow was lower after ANIT treatment, which was restored by $ACTIValoe^{(R)}$ N-931 complex. $ACTIValoe^{(R)}$ N-931 complex reduced the ethionine-induced elevated hepatic TG contents. Histopathological analysis revealed that signs of liver injury were prominent at 24 h as result of ethionine injection, demonstrated by extensive areas of fatty change and microvesicular steatosis were observed around cells. These changes were attenuated by $ACTIValoe^{(R)}$ N-931 complex. Our results suggest that the $ACTIValoe^{(R)}$ N-931 complex has a protective effect on acute liver injury.

Hepatoprotective Effect of Aged Black Garlic Extract in Rodents

  • In this study, we investigated the hepatoprotective effects of aged black garlic (ABG) in rodent models of liver injury. ABG inhibited carbon tetrachloride-induced elevation of aspartate transaminase (AST) and alanine transaminase (ALT), which are markers of hepatocellular damage, in SD rats. D-galactosamine-induced hepatocellular damage was also suppressed by ABG treatment. However, ABG does not affect the elevation of alkaline phosphatase (ALP), a marker of hepatobilliary damage, in rats treated with carbon tetrachloride or D-galactosamine. We also examined the effect of ABG on high-fat diet (HFD)-induced fatty liver and subsequent liver damage. ABG had no significant effect on body weight increase and plasma lipid profile in HFD-fed mice. However, HFD-induced increase in AST and ALT, but not ALP, was significantly suppressed by ABG treatment. These results demonstrate that ABG has hepatoprotective effects and suggest that ABG supplementation might be a good adjuvant therapy for the management of liver injury.

Thioacetamide로 유발한 간손상 모델에서 계혈등(鷄血藤)의 간보호 효과 (Protective Effect of Spatholobi Caulis in Thioacetamide induced Acute Liver Injury of Rat)

  • Objectives : This study was undertaken to investigate the hepatoprotective effect of Spatholobi Caulis water extract (SC) to thioacetamide (TAA)-induced acute liver injury (ALI) in rats. Methods : The rats were injected intraperitoneally with TAA (200 mg/kg body weight) and orally administered SC (100 or 200 mg/kg b.w.) daily for 3 days. Liver biomarkers were assessed by serum glutamic oxaloacetic transaminase (GOT), glutamic pyruvic transaminase (GPT), and ammonia levels. Malondialdehyde (MDA) was measured both serum and liver tissue. In addition, nicotinamide adenine dinucleotide phosphate (NADPH) oxidase, anti-oxidant, and inflammation-related proteins were investigated by western blot analysis. Histological examination further confirmed though hematoxylin and eosin stain. Results : The SC treatment reduced liver function markers like GOT and GPT and also remarkably decreased ammonia level. Moreover, the elevated MDA level in TAA-induced group was significantly reduced by SC treatment. NADPH oxidase expression associated with oxidative stress including NOX2, NOX4, and p47phox markedly inhibited by SC administration. SC treatment exerted anti-oxidant effect through the increase of anti-oxidant enzyme including superoxide dismutase (SOD), Catalase, and heme oxygenase-1 (HO-1). The protein expressions of inflammatory cytokines such as tumor necrosis factor-�� (TNF-��), IL-6, and IL-1�� induced by nuclear factor-kappa B (NF-��B) activation were modulated through blocking the phosphorylation of inhibitor of nuclear factor ��B�� (I��B)��. SC treatment also improved histological alterations. Conclusion : These findings suggested that SC administration may be a potential candidate for the prevention or treatment of ALI.

인진삼물탕(茵蔯三物湯) 약침처리(藥針處理)가 백서(白鼠)의 간손상(肝損傷) 회부(回復)에 미치는 영향(影響) (Effects of Herbal-Acupuncture with Inzinsammultang on the Recovery of Liver Injury in Rats)

  • These studies were carried out to investigate the effect of Herbal-acupuncture with Inzinsammultang extract on the recovery from liver injury of rats. The liver injury of rats induced with 0.3ml/ea carbon tetrachloride. The Herbal-acupunture with Inzinsam multang extract solution inserted into corresponding focus of Kansu(BL18) in rats. In this study, SD-Rats(Sprague-Dawley) were divided into 4 groups ; Normal group (None treated group), Control-group(The group not treated after $CCI_4$-intoxication), Treated group; Example I-group(Saline-injected groups after $CCl_4$-intoxication) and Example II-group(Inzinsammultang-injected groups after $CCl_4$-intoxication). Through histological observation, Example II-group sho-ws that liver injury is weaker than Control group.(p<0.05) Biochemical assays for each serum enzyme activities of AST, ALT and LDH, levels of albumin, ${\gamma}$-GT, TG, total cholesterol were performed. The results were summarized as follows 1. AST(Aspartate aminotransferase) activity in serum significantly decreased in the Inzinsammultang-injected groups after $CCl_4$-intoxication. Inzinsammultang-injected groups after $CCl_4$-intoxication showed significantly higher AST activities, compared with the Saline-injected groups after $CCl_4$-intoxication.(p<0.05) 2. ALT(Alanine aminotransferase) activity in serum significantly decreased in the Inzinsammultang-injected groups after $CCl_4$-intoxication. The Inzinsammultang-injected groups after $CCl_4$-intoxication showed significantly higher AST activities, compared with the Saline-injected groups after $CCl_4$-intoxication(p<0.05) 3. Level of Albumin in serum significantly increased in the Inzinsarnmultang-injected groups after $CCl_4$-intoxication. 4. LDH(Lactate dehydrogenase) activities in serum significantly decreased in the Inzinsammultang-injected groups after $CCl_4$-intoxication.(p<0.05) 5. ${\gamma}$-GT(Glutamyl transferase) in serum significantly decreased in the Inzinsammultang-injected groups after $CCl_4$-intoxication(p<0.05) 6. As for the TG(Triglyceride) levels in serum, Inzinsammultang-injected groups after $CCl_4$-intoxication is no significant differences compared with the Saline-injected groups after $CCl_4$-intoxication.(p<0.05) 7. Total cholesterol in serum significantly increased in the Inzinsarnmultang-injected groups after $CCl_4$-intoxication.(p<0.05) Those results indicate that the Inzinsammultang Herbal-acupuncture have significant effects on the liver injury induced by $CCl_4$. So it is expected that the Inzinsammultang Herbal-acupuncture can be used to cure inflammations and recover the functions of damaged liver cells.

피부화상이 간의 유해산소 생성계 및 해독계 효소 활성에 미치는 영향 (Effect of Skin Burn on the Activities of Free Radical Generating and Scavenging Enzymes in Rat Liver)

  • In order to investigate a pathogenesis of liver damage induced by skin burn, thermal injury was induced by scald burn on entirely dorsal surface in rats (total body surface area 30%) except for inhalated injury. At 5 and 24 h after scald burn, biochemical assay in skin tissue, serum and liver tissue were examined. The effects of burn injury on the levels of glutathione, lipid peroxide and on the activities of oxygen of histologic and ultrasound changes, measuring. the protein concentration in plasma, and counting the number of intravascular polymorphonuclear leukocytes. Post burn 24 h, the content of glutathione was decreased (47.50%), whereas that of lipid peroxide was increased (37.01%), and the activity of superoxide dismutase was diminished (p<0.001). Thus decreasing the capacity of oxygen free radical scavenging enzymes led to oxidative injury in skin tissue. In liver tissue, at 24 h after scald burn, both the content of glutathione and the activity of catalase were markedly decreased (p<0.01, p<0.05), thus the imbalance between free radical generating and scavenging capacities has been induced.

Trichostatin A Protects Liver against Septic Injury through Inhibiting Toll-Like Receptor Signaling

  • Sepsis, a serious clinical problem, is characterized by a systemic inflammatory response to infection and leads to organ failure. Toll-like receptor (TLR) signaling is intimately implicated in hyper-inflammatory responses and tissue injury during sepsis. Histone deacetylase (HDAC) inhibitors have been reported to exhibit anti-inflammatory properties. The aim of this study was to investigate the hepatoprotective mechanisms of trichostatin A (TSA), a HDAC inhibitor, associated with TLR signaling pathway during sepsis. The anti-inflammatory properties of TSA were assayed in lipopolysaccharide (LPS)-stimulated RAW264.7 cells. Polymicrobial sepsis was induced in mice by cecal ligation and puncture (CLP), a clinically relevant model of sepsis. The mice were intraperitoneally received TSA (1, 2 or 5 mg/kg) 30 min before CLP. The serum and liver samples were collected 6 and 24-h after CLP. TSA inhibited the increased production of tumor necrosis factor (TNF)-${\alpha}$ and interleukin (IL)-6 in LPS-stimulated RAW264.7 cells. TSA improved sepsis-induced mortality, attenuated liver injury and decreased serum TNF-${\alpha}$ and IL-6 levels. CLP increased the levels of TLR4, TLR2 and myeloid differentiation primary response protein 88 (MyD88) protein expression and association of MyD88 with TLR4 and TLR2, which were attenuated by TSA. CLP increased nuclear translocation of nuclear factor kappa B and decreased cytosolic inhibitor of kappa B ($I{\kappa}B$) protein expression, which were attenuated by TSA. Moreover, CLP decreased acetylation of $I{\kappa}B$ kinase (IKK) and increased association of IKK with $I{\kappa}B$ and TSA attenuated these alterations. Our findings suggest that TSA attenuates liver injury by inhibiting TLR-mediated inflammatory response during sepsis.

간손상 관련 한약 안전성 연구의 개선을 위한 한약인성 간손상 조사표 제안 (Suggestion of Herbal Medicine-Induced Liver Injury Investigation Forms for Improving the Research Quality of Herbal Safety)

  • It is important to establish the safety of herbal medicine because of its frequent and widespread use in Korea. Several studies on the safety of herbal medicine have been performed and there have been rare serious adverse drug reactions from those reports in Korea. However, the results are not strongly supported because of not adopting appropriate enough research methodology as to make the safety issue clear. For improving the quality of the safety research on herbal medicine. including investigations of drug induced liver injury (DILl). the aim of this study was to suggest herbal medicine-induced liver injury investigation forms for performing reasonable safety research. After a systematic review of the preceding studies regarding herbal safety in Korea was performed in 2008, we assessed the quality and the limitations of the primary studies. Two investigation forms for herbal safety research were made as a following step. one a basic investigation form for herbal safety research and the other an advanced investigation form for suspected DILl cases, Those forms include the essential informations and data needed to make an appropriate assessment of whether DILl occurred during or after the use of herbal medicine. Guidelines for using those forms and other recommendations were also suggested. More rigorous studies are required for answering the safety issue of herbal medicine as well as the efficacy issue. We hope for wide use and improvement of those investigation forms in the study of herbal safety by many researchers for establishing better evidences in Korea.

부자(附子) 및 천오(川烏)를 포함한 처방이 혈중 Aspartate Aminotransferase, Alanine Aminotransferase 수치에 미치는 영향 (The Study of Dosages of Herbal Medicine Including Aconiti Lateralis Preparata Radix and Aconiti Tuber on Aspartate Aminotransferase, Alanine Aminotransferase)

  • Objectives : Aconiti Lateralis Preparata Radix (附子) and Aconiti Tuber (川烏) are not commonly prescribed, but are necessary for some clinical conditions, despite of the fact that some negative effects have been known to occur with these medicines. This study shows the consequences for aspartate aminotransferase (AST), alanine aminotransferase (ALT) due to herbal medicines, including Aconiti Lateralis Preparata Radix (附子) and Aconiti Tuber (川烏). Methods : From 1st June 2007 to 10th May 2009, the results were analyzed for 64 patients belonging to the OO Oriental Medical Hospital who took herbal medicine, including Aconiti Lateralis Preparata Radix (附子) and Aconiti Tuber (川烏), more than 20 days. This is the study about the comparison and the investigation of AST, ALT. And the results were taken upon their hospitalization and to their departure. The Criteria of Diagnosis in Liver Injury of CIOMS was used as a standard for their examinations. Results : AST, ALT (total 64). The number of patients who were within normal reference value : 49. The number of patients who exceed the normal reference value after dosage, without satisfying the Criteria of Diagnosis in Liver Injury : 5. The number of patients who were recovered in normal reference value after dosage : 9. The number of patients who satisfying the Criteria of Diagnosis in Liver Injury after dosage : 1. Conclusions : This results suggest that these herbal medicines, including Aconiti Lateralis Preparata Radix (附子) and Aconiti Tuber (川烏), didn't cause adverse side-effects on AST, ALT of the patients who are taking them.

흰쥐의 알코올 유발성 간손상에 실비음(實脾飮)이 미치는 보호 효과 (The Protective Effects of Silbi-um Extract on the Alcoholic Liver Injury in Rats)

  • Objectives: The objective of this study is to investigate the effects of Silbi-um (SBU) extract on the alcoholic fatty liver induced by EtOH administration for 8 weeks. Methods: Male Sprague Dawley rats were used. All animals were randomly divided into 3 groups; Normal, EtOH and EtOH+SBU. The rats of EtOH group were daily treated with ethanol of 25% (v/v) for 8 weeks (n=10). EtOH+SBU group was orally treated with SBU water extract after ethanol administration (n=10). The rats of Normal group were treated with saline (n=10). After 8 weeks, the mean body weight, liver weight, and liver-body weight ratio were calculated. The serum alanine aminotransferase (ALT) and aspartate aminotransferase (AST) of all groups were measured. The morphological alterations were observed using hematoxylin and eosin (H&E) and Oil Red O staining. Moreover, the alteration of tumor necrosis $factor-{\alpha}$ ($TNF-{\alpha}$) levels were analyzed immunohistochemistrically. Results: The histological data showed that liver sections from EtOH group displayed severe steatosis. SBU extract significantly inhibited the progression of the alcoholic liver injury. The increased serum level of ALT and AST induced by ethanol administration were decreased by SBU extract. Furthermore, SBU extract significantly decreased the liver concentrations of $TNF-{\alpha}$. Conclusions: SBU water extract attenuated the alcohol induced fatty liver by improving hepatic lipid metabolism via suppression of $TNF-{\alpha}$ protein. SBU could be effective in protecting the liver from alcoholic fatty liver.
