가정형 호스피스 환자의 가족지지와 죽음준비가 삶의 질에 미치는 영향 (Influence of Family Support and Death Preparation on the Quality of Life in Home Care Hospice Patients)

  • Purpose: This study aimed to examine the influence of family support and death preparation on the quality of life in home care hospice patients. Methods: The study recruited 117 patients in home care hospice in four general tertiary hospitals and three general hospitals in three cities. Data were collected using self-reported questionnaires from September 1, 2019 to March 31, 2020 and analyzed using the statistical package IBM SPSS software version 22.0. Results: The quality of life according to the participants' general characteristics of the subjects shows a statistically significant difference between patients who live with supporters and those who do not(Z=2.96, p=.003). A statistically significant correlation was found between predictors such as family support, death preparation, and quality of life. Family support and death preparation affect the quality of life in home care hospice patients and these variables could explain 33.7% of it. Conclusion: To improve the quality of life in home care hospice patients, we should develop an intervention to enhance family support and death preparation.

  • Purpose: This study aimed to determine the moderating effect of self-esteem on the perception of death and the meaning of life among hospital nurses. Methods: The participants were 167 nurses working in two tertiary general hospitals located in city B. Data were collected from September 8 to 21, 2022. SPSS/WIN 29.0 was used for analysis along with t-test, ANOVA, Pearson correlation coefficients, and hierarchical multiple regression. Results: The study showed that the meaning of life was significantly correlated with the perception of death (r=.34, p<.001) and self-esteem (r=.31, p<.001). Self-esteem significantly moderated the relationship between the perception of death and the meaning of life (F=16.00, p<.001). Conclusion: Self-esteem may moderate the relationship between the two factors. This means that nurses with higher self-esteem have a greater tendency for their perception of death to positively impact their meaning of life. Therefore, for hospital nurses to discover the meaning of life, it is necessary to develop and apply a self-esteem enhancement program specialized for each clinical career.

한국인 기대여명의 한계추정에 관한 연구 (A Study on the Estimation of Limits to Life Expectancy)

  • The purpose of this study is estimate limits of Korean life expectancy at birth by 'Gompertz growth curse Model', 'Cause-Elimination Model' and Multidimensional models of Senescencee and Mortality'. Data used in Gompertz curve were obtained from all life tables published from 1905 to 1990 in Korea, and life expectancies at birth of eighteen groups were selected at five-year interval in consideration of time-series changes. Data used in Cause-Elimination Model are 'Cause of Death statistics in 1991' published in 1992 by National Bureau of Statistics of Korea and 'life table of 1989' published in 1990 by National Bureau of Statistics, Economic Planning Board of Korea. The materials are all classifiable death data, 119, 253 cases of male and 82, 420 cases of female, which is from 1991 Causes of Death statistics. The cases of death analyzed belong to one of 8 categories; i.e., Infectious and Parasitic Diseases(001-139; with notation of Infectious Diseases), Malignant Neoplasms(140-208), Hypertensive Diseases(401-405), Ischemic Heart Dieases and Diseases of Pulmonary Circulation and Other Forms of Heart Diseases(410-429;with notation of Heart Disease), Cerebrovascular Diseases(430-438), Chronic Liver Diseases and Cirrhosis(571; with notation of Liver Diseases), Injury and Poisoning(800-999) and all other disease. Data used in 'Multidimensional models of senescence and mortality' were life table of 1989 published by National Bureau of statistics, Economic Planning Board of Korea and life table of 1970, 1978-79, 1983, 1985 and 1987. The major findings may be summarised as follows: 1. Estimate equations of Gompertz growth curve using life expectancy at birth during the 1905-1990 period are as the following. Male : y = 88.047697 $\times$ $0.199690^{0.903381x}$ Female : y = 95.632828 $\times$ $0.199690^{0.903381x}$ Limits of life expectancy at birth, which were estimated by Gompertz growth curve, are 88.05 for male and 95.63 for female. 2. The effect on life expectancy at birth eliminationg all causes death is 14.04 years(for male) and 10.86 years(for female). Astonishingly, eliminating the malignant neoplasms increase life expectancy at birth by 2.85 years for male 2.03 years for female in 1991. In table 8 we show the effect on life expectancy at birth of separately eliminating each of the 8 categorical causes of death. The theoretical limit to life expectancy by Cause-Elimination Model is 80.96 for male and 85.82 for female. 3. If the same rate of delay [0.376 year(male), 0.435 year(femable) per calendar year] continued, then life expectancy at birth would reach 74.82(male) years and 84, 10(female) years in 2010. With 14.04-years(male) and 10.86-years(female) effect attributable in 2010 would be 88.86 years(male) and 94.96(femable) years. 4. 'Multidimensional models of senescence and death' permits calculations of the value of the attribution coefficient (B), percent of loss per year of physiologic function. The results of Ro and B during the 1970-1989 period are listed in table 9. Estimate of limit to Korean life expectancy at birth by 'Multidimensional models of senescence and death' is 99.47 years for male and 104.74 years for female in 1989.

  • Purpose : This study aims to explore nursing activities after the decision to discontinue life-sustaining treatment, awareness of a good death, and perception of life-sustaining treatment decisions among nurses in intensive care units (ICUs) at tertiary general hospitals. Methods : Participants were 173 nurses working in two tertiary general hospitals. The data were collected using structured questionnaires and analyzed using an independent t-test, paired t-test, one-way ANOVA, Scheffé's test, and Pearson's correlation coefficient. Results : Participants were 173 nurses working in two tertiary hospitals. The nursing activity increase was the greatest in the spiritual domain, and the physical domain was where the activities decreased the most. There were significant associations between Awareness of good death (Clinical) and Perception of life-sustaining treatment decision(r=.26, p <.001), Awareness of good death (Closure) and Perception of life-sustaining treatment decision(r=.36, p <.001), and Awareness of good death (Personal control) and Perception of life-sustaining treatment decision(r=.49, p <.001). Conclusion : Based on the results, systematic education programs and job training are required to improve the awareness regarding good death and perception of life-sustaining treatment decision for nurses in ICUs where discontinuing life-sustaining treatment decisions are made.

대학생의 죽음에 대한 주관성 연구 (Subjectivity toward Death among College Students)

  • The purpose of this study was to classify the subjectivity type toward death among college students and to understand the characteristics of attitudes and orientations toward death of each type. Since attitudes and orientations toward death is very subjective and unique, Q-methodology was employed in this study. Q-methodology explains the respondent's subjectivity by objectifying his subjectivity for himself. The P-sample consisted of 63 students of K university in D city. Forty statements concerning attitudes and orientations toward death, which developed by Yeun(1999) were utilized for Q-sample. Forty Q-statements were sorted according to the level of agreement or disagreement by forced normal distribution. The Q-sorts by each subjects were coded and analysed with the PC-QUANL program. Five types of subjectivity toward death were identified and labeled. Type 1 'the death- preparation' think frequently about his own death and talk openly about the problem of death with a positive attitude. Type 2 'the life-esteemed' respect the dignity of life most of all. Type 3 'the realty-oriented' do not believe the afterlife and is very concerned about the present life itself instead of thinking about death. Type 4 'the ambivalent' think importantly the aspect of present life and orient the afterlife at the same time. This type reveals opposite attitude of preparing and scaring the death at the same time. Type 5 'the destiny-recipient' attribute death to the destiny and deny suicide. On the basis of this result, the studies about applying and developing educational program on death and hospice care for nurses who care terminal cancer patients and families are needed.

호스피스 자원봉사자 교육이 죽음에 대한 성향에 미치는 효과

  • The purpose of this study was to promote the popularization of hospice services by providing the information about the influences of hospice training on participants. We compared differences of attitudes toward death between those of pre-training and post-training by use of questionnaire. This study involved 137 volunteers participating in the hospice training held by one hospice center located in K-city. The questionnaire was composed of 46 items, 21 items of general information and 25 items of information about attitudes toward death. We applied Park's translated version(1992), originally distributed by Thorson and Powell(1988), in the assessment of participants' changed attitudes toward death. Participants were asked to fill out the questionnaire before and after the hospice training. The data were analged by frequency. ANOVA t-test, paired t-test. The results were as follows, 1.Pre-training items holding a high rank were 'The subject about the future life after death is distressing'(F=5.20), 'No worry about any occurrences around the body after a funeral'(F=4.75), 'Having little interest in being laid in a coffin after death'(F=4.56); items holding a low rank were 'Having little interest in any occurrences around the body after death'(F=2.22), 'Willing to deal with the aftermath of the death in detail'(F=1.94), 'No fear to have an idea to be cancerous'(F=1.72). 2.Post-training items holding a high rank were 'No fear to be dead as a result of sufferings from long-lasting illness'(F=3.18), 'No fear to have an idea to be cancerous'(F=3.16)', Having little interest in the future life after death'(F=3.09); items holding a low rank were 'Unpleasant to be not able to move after death(F=1.74), 'Distressing not to know about the future life after death'(F=1.61), 'The subject about the future life after death is distressing'(F=1.60). 3.There were significant difference in participants' attitudes toward death. Compared with pretraining(F=3.97), more positive were developed in post training(F=2.30). The results of this study indicate that hospice training renders positive regards toward death to the participants. Therefore every human being, who is able to speculate about the meaning of death, has to be exposed to the hospice training. In conclusion, hospice training should be popularized in the near future.

간호사의 임종간호 태도에 미치는 영향요인 (Factors Influencing Nurses' Attitudes Toward Terminal Care)

  • Purpose : The purpose of this study was to identify the extent to which nurses' death perceptions, death anxiety, and stress associated with terminal care affects their attitudes toward terminal care. Method : A total of 214 nurses were asked questions about characteristics, death perceptions, death anxiety, stress associated with end-of-life care, and terminal care attitudes, using structured questionnaires. The data were analyzed using descriptive analysis, t-test, ANOVA, Scheffé's test, Pearson's correlation coefficients, and stepwise multiple regression. Results : Factors influencing terminal care attitudes in participants were positive meaning associated with death, respect for life, final education, end-of-life nursing education experiences, stress associated with end-of-life care, death anxiety, and position. These factors explained 38.7% of variance in participants' terminal care attitudes (F=20.18, p<.001). Conclusion : In order for nurses to have a positive attitude toward terminal care, it is necessary to raise positive awareness about death and respect for life, and it is necessary to develop various educational programs and strengthen their capacity through continuous education.

특정사인제거정도가 부분노동력여명에 미치는 연장효과에 관한 연구 (The Study on Potential Gains in Working Life Expectancy according to the Degree of Reduction of Specific Causes of Death)

  • This study was carried out to calculate working life expectancy and its potential gains according to the degree of reduction in the specific causes of death. It sought to ascertain what potential gains in labor force longevity might be reasonably achieved through efforts to reduce mortality from injuries and poisoning, diseases of circulatory system, neoplasms and the other causes of death. The data were drawn from the three sources such as "The 1982 Causes of Death Statistics," "Abridged Life Table for Korea 1978~'79" and "The 1982 annual Report of Economically Active Population" issued by Economic Planning Board. Analytical tools used in this study were the cause-deleted life table and the Wolfbein Wool's working life table method. Partial life expectancy was adopted as an index of this study, This application will be widely used as a good demographic tool for analyzing the dynamics of labor force and causes of death. Some of the findings are summerized as follows. 1. Partial life expectancies from initial age 15, 25, and 45 respectively to terminal age 65 are 44.86, 35.59 and 17.41 year in life expectancy itself, 37.08, 32.83 and 15.21 year in working life expectancy, 7.78, 2.76 and 2.20 years in inactive life years. 2. Potential gains in working expectancy from initial age 15, 25 and 45 to terminal age 65 by the complete elimination of the specific causes of death are 1.36, 0.94 and 0.27 years in injuries and poisoning, 0.88, 0.83 and 0.54 years in diseases of circulatory system, 0.56, 0.54 and 0.37 years in neoplasms, 1.13, 1.02 and 0.58 years in the other causes of death. The relationship between degree of reduction in causes of death and potential gains in working life years is in direct proportion. The prime orders of reduction effectiveness in age groups by the causes of death are injuries and poisoning in age 15-24, the other causes of death in age 25-49 and diseases of circulatory system in age 50 and over. 3. If it were possible to reduce 25%, 50% and 75% in mortality condition 1978~'79, the average length of working life would be 38.08, 39.13 and 40.17 years in age 15, and 33.68, 34.57 and 35.44 years in age 25, and 15.67, 16.14 and 16.63 years in age 45.

일부 간호대학생들의 죽음에 관한 인식유형 분석 (An Analysis of Factors about Nursing Students' Attitudes Toward the Perception of Death : Q-sort method)

  • The aim of this study was to identify the attitude of nursing students' toward death using Q-methodology and to obtain baseline data to improve nursing students' education program on death. Twenty-nine participants at a college classified 40 Q-statements on a one to nine scale. The resultant Q-sort was a matrix representing the participant's operant subjectivity on the issue under consideration. The results of above procedures were analyzed by PQ Method. The results revealed that there are three types of perception about nursing students' attitudes toward the death. The categories were labeled positive-perspective, pain-avoid and preparation-deficiency. Positive-perspective individuals have a positive sense of their life and death. Thus, they would not fear death, believing in an after-life world, while being positive towards donation of intestines after death. Pain-avoid individuals hope life and death without pain. Preparation-deficiency individuals are unprepared to die. In conclusion, this study discovers three types of the perception about nursing students' attitudes toward the perception of death. By identifying the nature of each of these types, this study can be useful to develop efficient strategies for education program on death.

  • Purpose: The purpose of this study was to examine the influences of emotional intelligence and attitudes about death on hospice volunteers' perception of life as meaningful. Methods: In this study, 232 hospice volunteers who were serving at 3 university hospitals and 7 general hospitals for 6 months or longer. Data were collected using self-report questionnaires from January 20 to February 15, 2016. Data were analyzed using descriptive statistics, independent-tests, one way ANOVAs, Pearson's correlation coefficients, and multiple regression analyses with SPSS 21.0. Results: Hospice volunteers' perception of life as meaningful was positively correlated with their attitudes toward death and emotional intelligence. The significant predictors of perception of meaning were emotional intelligence, attitudes toward death, religion and perceived health status. These variables explained 41.0% of the variance in hospice volunteers' perception of life as meaningful. Conclusion: These results suggest that hospice volunteers perceptions of life as meaningful can be changed positively by increasing emotional intelligence and positive attitudes about death.
