Purpose: The aim of this study was to show the clinical results of combination of Nd-YAP (1340nm) laser therapy with conventional endodontic and periodontal treatment. Materials and Methods: Four patients with chronic advanced periodontitis and endodontic infection were treated with conventional treatment and Nd-YAP laser therapy. Occlusal adjustment and splinting were done for stabilization of the teeth with severe horizontal and vertical mobility. The protocol for periodontal treatment was followed as scaling and root planing, pocket irrigation with 3% $H_2O_2$ and exposure of Nd-YAP laser using 320${\mu}m$ optical fiber with 160mJ/pluse, 30Hz. The other protocol for endodontic treatment was followed as access opening, canal preparation by hand and rotary instrument, canal filling, and exposure of Nd-YAP laser using 200${\mu}m$ optical fiber with 200mJ/pluse, 10Hz and 180mJ/pluse, 5Hz which were used respectively for disinfection and canal filling. The assessments of probing depth, mobility, and radiography were made prior to and after treatment. Result: All of these four clinical cases showed good healing of periodontium, which presented decrease of mobility and pocket depth, and increase of bone regeneration and bone density on the radiography. Conclusion: The bactericidal effect of Nd-YAP laser would provide benefits for improving clinical results that are obtained from conventional therapy.
This studies were to investigate the effects of low power wavelengths Helium-Neon Infra-Red(He-Ne 1R) laser on the changes of the serum mineral components in burn rats. The thirty Spraque-Dawley adult male rats were assigned to the 5 groups: the experimental groups(3), the bum control group (1) and the control group(1). There was made three degree bum by the 250mW IR on the back of each rats. from 3 days after being burned. the experimental laser groups were irradiated low power wavelengths(292 Hz. 1168Hz, 4672Hz) He-He IR laser far 5 minutes every day during the 7 days. The results were as tallows: 1. There were significantly not changes of the levels of serum sodium between all experimental groups. 2. The concentration of potassium in serum on the treated with 292Hz wavelengths group during 5 minutes were significantly increased ,but with 1168Hz wavelenghths group were decreased to the those of burn group. 3. The concentrations of serum calcium in serum 1168Hz wavelengths laser group on the treated with 5 minutes laser were significantly higher to normal level than those of bum groups. As above results. the changes of the 2 essentials mineral(calcium and potassium) in serum levels on the healing process have meaningful effected with the low power wavelengths role on the treated with He-Ne IR laser.
The present study was designed to investigate the effect of low power GaAlAs laser on spinal Fos expression related to the anti-nociceptive effect of laser stimulation. Low power GaAlAs laser was applied to either acupoint or non-acupoint for 2 hour under light inhalation anesthesia. Spinal Fos expression in the dorsal horn was compared to that obtained in inhalation anesthesia control group. Furthermore, we analyzed the effect of the local treatment of lidocaine on the spinal Fos expression evoked by low power GaAlAs laser stimulation. The results were summarized as follows: 1. In the normal animals, only a few Fos like immunoreactive(Fos-IR) neurons were evident in the lumbar spinal cord dorsal horn. Similarly, following prolonged inhalation anesthesia, Fos-IR neurons were absent in the dorsal horn of the lumbar spinal cord. In animals treated with laser stimulation, Fos immunoreactive neurons were increased mainly in the medial half of ipsilateral laminae I-III at lumbar segments L3-5. These findings directly indicated that prolonged anesthesia used in this study did not affect the Fos expression in the spinal cord dorsal horn of intact animals and low power laser stimulation dramatically produced Fos expression in the spinal cord laminae that are related to the anti-nociceptive effect of laser stimulation. 2. In acupoint stimulated animals, 10mW of laser stimulation, not 3mW and 6mW intensity, significantly increased the number of Fos immunoreactive neurons in the spinal dorsal horn(p<0.05). However, laser stimulation on acupoint more dramatically increased the number of Fos immunoreactive neurons in the spinal cord rather than laser stimulatin on non acupoint. These result suggested that laser stimulatin on acupoint was more effective treatment to activate the spinal neuron than non acupoint stimulation. 3. The local treatment of lidocaine totally suppressed the activity of spinal neurons that were induced by lower power 1aser stimulation. These data indicated that the anti-nociceptive effect of laser stimulation was absolutely dependent upon the peripheral nerve activity in the stimulated location. In conclusion, these data indicate that 10mW of low power laser stimulation into acupoint is capable of inducing the spinal Fos expression in the dorsal horn related to the anti-nociceptive effect of laser stimulation, Furthermore, the induction of spinal Fos expression was totally related to the peripheral nerve activity in the laser stimulated area.
Transoral $CO_2$ Laser Microsurgery of glottic Carcinoma is replacing external approaches. The qualify of life after surgery for laryngeal cancer may be as important as complete resection of the tumor for patient. Transoral $CO_2$ laser cordectomy for the management of early laryngeal cancer has advantages with regard to oncological results, preservation of laryngeal functions, morbidity and cost in comparison to those of open surgery or radiation therapy. Moreover, transoral laser surgery can be a useful choice as a salvage surgery in radiation therapy failed early glottic cancer. A classification of laryngeal endoscopic cordectomies which included eight different types was described by the European laryngological Society in 2000. We will also introduce type VI which was newly proposed recently.
The perioral region, which is one of the most conspicuous parts of the face, plays a significant role in interpersonal communication. However, the perioral region is highly susceptible to scarring and pigmentation and is difficult to treat. The causes of perioral hyperpigmentation are many, including physiologic reasons, systemic diseases, drugs, and infections. In the treatment of perioral hyperpigmentation, the cause of the disease must be diagnosed first. Management includes lifestyle modifications such as reducing the UVA exposure time, use of topical treatments including hydroquinone and retinoids, and physical therapy including laser therapy. We report a case of perioral hyperpigmentation treated with 1,064-nm Q-switched neodymium:yttrium-aluminum-garnet laser toning.
Laser therapy is becoming an accepted procedure for tissue coagulation and ablation and is especially useful in treating tumors. The laser energy is applied to the tissue of interest through various delivery systems which are introduced percutaneously, via blood vessels, through body openings, or during surgical exposure of the tissue. One of the major obstacles to effective application of lasers has been the lack of reliable method to determine the extent of tissue involvement in real time. Several methods have been proposed for monitoring the tissue response and controlling the laser in real time during laser therapy. Among them, magnetic resonance imaging(MRI) has been introduced to monitor laser-tissue interactions because laser irradiation induces changes not only in the thermal motions of the hydrogen protons within the tissue but also in the distribution and mobility of water and lipids. The buttocks of New Zealand rabbits were treated by KTP and $CO_2$laser(power : 10 watts, exposure time:10 seconds). m images were taken at immediately after lasering, 1 week later, 2 weeks later, and at the same time, tissues were harvested for histopathologic study. We analyzed MR images and histopathologic findigs of laser-treated tissues. The MR images taken immediately after laser treatment showed 3 layer pattern and which was correlated with histopathologic changes. We suggest MRI may become a useful monitoring tools for laser-tissue interaction.
In this study, we evaluated the muscle tension in hemiplegia patients using a real-time monitoring system combined with high-intensity laser treatment (HILT) device. Five hemiplegia patients were treated with HILT in the left forearm muscle for 8 minutes, and the muscle was relaxed for 8 minutes following the treatment. Both the gradient of the force-displacement curves and the muscle hardness decreased during the therapy, and the effects were maintained following the treatment. The results show that muscle tension decreased rapidly during laser irradiation, and the efficacy of the treatment depended on the duration of the illness. Consequently, we conclude that this combined HILT and real-time muscle tension monitoring system is useful for evaluating the therapeutic procedure and for treatment of muscle pain.
He-Ne, IR Laser가 급성 화농성 슬관절염 치유에 미치는 효과를 알아보기 위해 Sprague-Dawley계 흰쥐를 포도상 구균으로 화농성 슬관절염을 유발 4일 후 부터 1일 1회 4분 및 10분 동안 7일간 레이저로 점조사를 하였다. 레이저 점조사를 한 동물 혈액의 효소활성과 IgG의 변화를 분석하여 얻은 결과는 다음과 같다. ALT활성은 대조군과 각 실험군이 비슷한 수준을 보였다. ALT활성은 관절염군에 비해 10분 레이저군은 유의한 증가를 보였으나 4분 레이저군은 유의하게 감소하여 대조군과 비슷하였다. LDH활성은 대조군에 비하여 10분 레이저군은 유의하게 증가하였으나 관절염군과 4분 레이저군은 유의한 감소를 보였다. ALP활성은 대조군에 비하여 관절염군, 4분 및 10분 레저군이 유의하게 감소되었으나, 그러나 관절염군에 비해 10분 레이저군은 유의한 증가를 보였다. CK활성은 대조군과 관절염군이 비슷하였으나, 관절염군에 비하여는 10분 레이저군은 유의하게 증가하였고, 4분 레이저군은 유의하게 감소되었다. IgG의 변화는 관절염군에 비해 10분 레이저군은 유의한 증가를 보였으며 4분 레이저군은 비슷한 수준을 보였으나 대조군에 비하여 각 실험군이 유의한 증가를 보였다. 이상의 결과를 종합해볼 때 레이저 치료로 근육에 많이 함유하고 있는 효소들이 염증이 있는 관절염군에 비하여 10분 레저군에서 AST, LDH, ALP 및 CK활성과 IgG 농도는 유의하게 증가하였으나,4분 레이저군에서는 AST, LDH, 및 CK활성이 관절염군에 비해 유의하게 감소된 것으로 봐 화농성 슬관절염 치유에 효과가 있는 것으로 생각된다. 앞으로 화농성 관절염에 대해 얻어질 수 있는 레이저 치료효과와 치유기전, 그리고 적절한 조사시간에 대해서는 더 연구되어야 할 것으로 생각된다.
Current therapeutic methods for suppressing muscle spasticity are intensive functional training, surgery, or pharmacological interventions. However, these methods have not been fully supported by confirmed efficacy due to the aggravation of the muscle spasticity in some patients. In this study, a combined system was developed to treat with a low-level laser and to monitor the region of the treatment using an optical spectroscopic probe that measures oxygen saturation and deoxygenation during low-level laser therapy (LLLT). The evaluation of the wavelength dependence for LLLT was performed using a Monte Carlo simulation and the results showed that the greatest amount of heat generation was seen in the deep tissue at ${\lambda}$ = 830 nm. In the oxy- and deoxygenation measurements during and after the treatment, oxygen-Hb concentration was significantly increased in the laser-irradiated group when compared to the control group. These findings suggest that LLLT using ${\lambda}$ = 830 nm may be of benefit in accelerating recovery of muscle spasticity. The combined system that we have developed can monitor the physiological condition of muscle spasticity during the laser treatment in real time and may also be applied to various myotonia conditions such as muscle fatigue, back-pain treatment/monitoring, and ulcer due to paralysis.
This paper performed the basic study for fabricating the low level laser therapy apparatus, and one of the goals of this paper was to make this apparatus used handily. The apparatus has been fabricated using the 655nm laser diode and microprocessor unit. The apparatus used a 655 nm laser diode for laser medical therapy and was designed for a pulse width modulation type to increase stimulation effects. And then, each experiment was performed to irradiation group and non-irradiation group for cells. MTT assay method was chosen to verify the cell increase of two groups and the effect of irradiation on cell proliferation was examined by measuring 590nm transmittance of ELISA reader. As a result, the cell increase of cells was verified in irradiation group as compared to non-irradiation group.
