  • Purpose: As the medical environment changes, the size of medical facilities continues to change. Of course, it is not easy to analyze the correlation between changes in the medical environment and changes in facility size in a rigorous scientific method. However, the results of investigating and analyzing changes in the size that occurred for any cause be able to serve as important references for future medical facility architectural plans. Methods: This study surveyed 68 general hospitals in Seoul for 15 years from 2005 to 2020. The change in the number of beds was investigated with data from the Ministry and Welfare. The gross area change was confirmed by analyzing the of all hospitals. Results & Implications: As a result of the survey, general hospitals in Seoul show a common phenomenon in which all hospitals except one case have expanded their size in the direction of increasing the gross area per bed over the past 15 years. However, in the method, a relatively small hospital with less than 500 beds mainly chose to reduce the number of beds. Large hospitals increased both the number of beds and the gross area, however showed a significant increase in the gross area.

우리나라 병원의 마케팅 활동수준과 재무성과 (Marketing Activities and Financial Performance of Korean Hospitals)

  • The main objective of this paper is to perform an empirical analysis on the relationship between various marketing activities and financial performance of Korean hospitals. A survey was conducted through structured questionnaire for 495 hospitals, and data from 218 hospitals were utilized in the final anaylsis.(response rate: 44%) Survey items include general characteristics of the hospitals(size, type, location), degree of competition, financial performance. marketing organization! budget, and level of various marketing activities in service development, access improvement, promotion, and pricing. We examine descriptive statistics of the response scores on marketing activities to evaluate the current status of marketing management of Korean hospitals, compare the results across hospital size, type and location, and perform regression analysis to investigate the relaionship between marketing and financial performance. Major findings are as follows: 1) About 46% of the responding hospitals have marketing departments although they are named as 'planning' or 'PR' departments, and the marketing budget on average represents 1.74% of the total expenditures. 2) Average level of marketing activities is calculated to be about 3.32 on 5-point scale, meaning that Korean hospitals implement their marketing programs 'somewhat actively'; however, the scores on the areas of marketing plannning and strategy are relatively low. 3) Large hospitals tend to be more active in marketing than small hospitals, and public hospitals' activities in marketing are not lower compared to private hospitals. 4) Level of overall marketing activities is positively related with financial performance measured by various finacial indicators except for profitability, implying that marketing is successful in revenue generation but needs to be more cost-effective. Also, when the marketing variables are separately included in the regression, no significant relationship is found, which means that various marketing activities are more effective when they are collectively implemented.

한 도시 대학병원 자료를 이용한 입원환자의 의료서비스 이용량 변이에 관한 연구 (Variation in resource utilization for inpatients among university teaching hospitals in a city)

  • The variation in resource utilization for hospitalized patients who had a group of similar diseases -- a Korean Diagnosis Related Group (KDRG) -- among the same type of hospitals was studied to assess the utillization variation due to the practice pattern of hospitals. Information about inpatients who were beneficiaries of the medical insurance for teachers and government officials discharged from 20 large university teaching hospitals in Seoul during 1986 and information about the hospitals were analyzed to achieve the study objective. A total of 20,223 non-outlier patients in 100 most frequent KDRGs were included in the analysis. Case charges after the review and length of stay (LOS) were used as measures of resource utilization during a hospitalization. A substantial variation among hospitals was found in most KDRGs : o the ratio of the maximum and the minimum among the mean case charges of hospitals was greater than 2 in 83 KDRGs ; o the difference between the maximum and the minimum among the mean case charges of hospitals was greater than 100,000 Won in 94 KDRGs : o the ratio of the maximum and the minimum among the mom LOS of hospitals was greater than 2 in 82 KDRGs ; o the difference between the maximum and the minimum among the mean LOS of hospitals was greater than 3 days in 94 KDRGs. The practice pattern of hospitals explained more than 20% of charge variation in 49 KDRGs and more than 20% of LOS variation in 43 KDRGs. The study results indicated need for a new health policy initiative for cost containment and quality assurance.

농어촌 지역병원 의료이용률 제고방안 (A Methodology for The Improvement of Rural Hospital's Utilization)

  • Although Korea joined OECD in 1996, there has still seen much unbalance in medical care and welfare standard between urban and rural area. The unbalance of medical demand between urban and rural area deepened low utilization of rural hospitals. So it caused many hospital's failure and conversion in rural area. Many rural hospitals are in difficulty managing business because of low medical demand along with the shortage of medical manpower, medical equipment and facilities. The objectives of this study were to reveal the cause of low utilization of hospitals in rural area, and to increase utilization of those hospitals. In this study the improvement methods of rural hospital's utilization were presented by examining were placed in difficult management condition, in respect of hospital's management conditions, manpower input, patient medical treatment record, financial record, and actual output. The causes of rural hospital's low utilization were as follows; 1) changes in number and structure of population 2) rural people's preference for large hospitals and hospitals which located in urban area 3) rural hospitals lacking in hospital management skill. Consequently rural hospital's operation condition got more and more deteriorated. To raise rural hospital's utilization, method for social policy, method for health policy, and intrinsic method of hospitals were presented in turn. For rural residents to utilize medical service conveniently, it is necessary for rural hospitals operated normally. So government must insist that rural hospitals solve the problems which come out from their internal management problems. And also these rural hospitals should be supported and nurtured by the government until their management is operated normally.

병원산업에서 시장지향성이 성과에 미치는 영향과 규모와의 관계 (The Relationship between Hospital Size and the Impact of Market Orientation on Performance in Korea)

  • There is general consensus in the research literature that market orientation is related to organizational performance. The study examines this relationship in the Korean hospital industry. One feature of this study is to examine the differences between large and small hospitals in terms of their market orientation, performance and the relationship between these constructs. The other feature is that both market orientation and performance are conceptualized as being multi-dimensional constructs. Hence a structural equations modeling (SEM) technique is used to examine the dimensionality of market orientation and performance and to examine the nature of this relationship. Data for this study are collected using a questionnaire that is mailed to the top marketing-related managers of 1,048 hospitals. Usable responses are obtained from 230 hospitals for a response rate of 21.9%. The SEM results confirm the multi-dimensional nature of both market orientation and performance, and the strong relationships between the constructs. Interestingly, this relationship is found to be much stronger for smaller hospitals than for larger hospitals. For smaller hospitals, this study shows that market orientation has a tremendous influence on performance, with almost 73.9% of the variance in performance being attributed to market orientation.

병원규모에 따른 간호사의 간호업무환경, 전문직업성과 직무만족의 비교 (A Comparative Study on Nursing Practice Environment, Professionalism, and Job Satisfaction according to Hospital Size)

  • Purpose: This study was conducted to investigate and compare the characteristics and relations of nursing practice environment, professionalism and job satisfaction among nurses in general hospitals according to hospital size. Methods: The participants included 314 staff and charge nurses who were working in the general medical/surgical nursing units in one large hospital, three medium sized hospitals, and four small hospitals. Data collected through using self-report questionnaire were analyzed using the SPSS and SAS statistical programs. Results: Nursing practice environment and job satisfaction had significant differences according to hospital size. Both of these scales were highest for medium hospitals and lowest for small hospitals. For all hospital sizes there were positive correlations between each of the variables. Multiple regression analysis showed that both nursing practice environment and job satisfaction were affected by hospital size, but professionalism was not. Conclusion: The results of this study indicate that nursing practice environment and job satisfaction vary with the size of the hospital. Therefore, further study is necessary to identify the work environment variables of nurses for performance management and to implement appropriate policies.

현대 종합병원 공용공간구성의 사용자중심적 실내디자인 특성 연구 - 해외사례를 중심으로 - (A Study on Characteristics of User-Focused Interior Design in Common Space Composition of Contemporary General Hospitals - Focused on Overseas Case -)

  • As patients prefer large scale hospitals recently, the interior design of general hospitals are conspicuously improving. In the past, most patients visit hospitals to get treatment on diseases; however, recently patients visiting hospitals for the early prevention of disease is rapidly increasing. Accordingly, contemporary hospitals should meet the wish of patients who want to get medical service in more clean and comfortable environment. The existing studies on hospital interior design were mostly limited on analyzing the functionality of each space; however, studies on the interior design of common use space, which is open to all hospital users, are required now. Accordingly, this study classified the common use space in contemporary general hospitals into lobby space, corridor space, waiting space and resting space. The interior design characteristics of each space were classified into; connectivity with external environment, level change by void, introduction of natural elements, dynamics of space and variety of color. Then a case analysis was done by selecting 12 common use spaces in overseas general hospitals. According to the analysis result, the interior design characteristics were more conspicuous in the sequence of corridor space, lobby space, resting space and waiting space. The interior design elements such as overall space arrangement and material-finishing were relatively acceptable; however, the aspect of a user's environmental behavior, in other words, the psychological aspect, which can lead a patient to natural healing, was relatively insufficient. It is believed that an in-depth interior design focusing on practical users would enable various spatial behaviors. It is hoped that this study would be a help to the interior design of domestic general hospitals as a guideline.

일부 다빈도 질환에서 의료기관 유형별 의약품 사용의 변이 (Variations on Drug Utilization between the Types of Hospital in Some Frequent Diseases)

  • This study presents the variations on drug utilization for outpatients' URI, gastritis. and hypertension by the type of hospital- tertiary hospital. general hospital. hospital. clinic. It investigated drug expenses. daily drug expenses. days of medication. the highest price of the drugs used. and the number of the different drugs used for each disease and type of hospital. This study also performed analysis to see how much the variations of variables related to drug use affect the variations of drug expenses. The dependent variable was drug expenses and the independent variables were days of medication. the average price of the drugs used. and the number of the different drugs used. Analysis of the drug utilization was performed on NFMI(National Federation of Medical Insurance) 1994 medical expense claim data. Patients with secondary diseases were excluded. In this study. 379 patients with URI, 386 patients with gastritis. 1.257 patients with hypertension were included. It was founded that there were large variation on drug utilization between the types of hospital for same diseases. Days of medication were longest in tertiary hospitals and shortest in hospitals or clinics. Clinics showed the lowest daily drug expenses in all of the diseases investigated. Daily drug expenses were highest in general hospitals or hospitals. which also tended to use drugs of higher price than other types of hospital. General hospitals and hospitals had larger variations in daily drug expenses and the highest price of drugs. It suggested that drug might be utilized overly in general hospitals and hospitals and some other factors might influence on drug utilization in these hospitals. It was found that the variations of drug expenses were affected by the variations of drug price and days of medication rather than the number of the different drugs. Then the strategy to reduce the variations of drug utilization and to improve the quality of drug utilization should focus on the drug price and days of medication. Further study is needed to assess the quality as well as the variation of drug utilization and to show the factors which affect them.

일부 병원 실내에서의 공기중 미생물 오염에 관한 연구 (A Study on Airborne Microorganism in Hospital)

  • To assess biological indoor air quality in hospital, concentrations of viable airborne microbes were determined at intensive care unit(ICU), patient room (PR), outpatient waiting room(OPWR) in hospitals of large(1000 beds), middle(500 beds), small(100 beds) hospitals, respectively. Gram positive bacteria, gram negative bacteria, fungi were sampled using suctional sampling method by RCS sampler (Reuter centrifugal air sampler) and RCS GK-A agar plate. In gram positive bacteria groups, CNS(Coagulase Negative Staphylococcus), Micrococcus, Lactobacillus, S. aureus, Enterococcus, St. viridans identified. In gram negative bacteria groups, A. baumannii, Kl. peumoniae and E. coli were identified, and Penicillium was identified in fugi groups. Results of the study were as follows. 1. The highest concentrations of airborne microbes was $971CFU/m^3$ at 5:00 PM in small hospital patient room, and average concentrations of airborne microbes in large, middle and small hospitals were $282CFU/m^3$, $289CFU/m^3$ and $625CFU/m^3$, respectively. Average concentrations of airborne microbes in office(control) was $90CFU/m^3$. Thus, the small hospital showed the worst condition. 2. Representatives of 8 different genera were identified in 150 samples. The most frequently isolated organisms were Staphylococcus (73.0%), Micrococcus (20.7%) and Lactobacillus (4.7%), respectively. Pathogenic microbes isolated were A. baumannii, E. coli, Enterococcus, Kl. peumoniae, S. aureus, St. viridans and Penicillium as fungi. In office, no pathogenic microbes were identified. Average concentrations of airborne pathogenic microbes in large, middle and small hospital were $5CFU/m^3$ (2%), $11CFU/m^3$ (4%) and $12CFU/m^3$ (2%), respectively. Thus, condition in a large hospital was better than those in a middle and a small hospital.

신종 감염병 환자의 효율적 처치를 위한대형병원 응급실 관리요소의 중요도 도출 (Deriving the Effective Improvement Elements of the Emergency Room in Large General Hospitals)

  • Purpose: This study aimed to derive the elements for effectively improving the management of emerging infectious diseases in the emergency room in large general hospitals. Methods: This research involved an analytic hierarchy process analysis of 10 experienced nurses working in a large general hospital located in the Seoul metropolitan area. The weighted values of the criteria for evaluating the importance of the management elements of patients with emerging infectious diseases in the emergency room of large general hospitals were derived using an analytic hierarchy process survey. Results: The results of the analytic hierarchy process analysis showed that experts considers responsiveness (0.291) as most important with respect to the criteria for the policy for isolation and control of infectious patients. The order of importance of the criteria for the policy for isolation and control of infectious patients were as follows: responsiveness (0.291), economical efficiency (0.257), feasibility (0.242), and sustainability (0.209). Subsequently, the weighted values of the management of the policy elements were determined based on the derived importance of the criteria mentioned above. These were, in order of priority, the construction of communication channels with the government authorities when dealing with emerging infectious diseases (15.9%), the equipment and provision of personal protective equipment to protect nurses from infection and dissemination of material (14.6%), and the operation of the isolation room and patient separation systems (14.1%). Conclusion: This study suggests critical strategies for infection control during a pandemic of emerging infectious disease.
