피해자보호원년을 선언한 경찰의 범죄피해자 보호·지원에 관한 연구 (Study on the police's protection·support of victims-declaring the Victims Protection years)

  • 우리나라도 이제는 범죄피해자의 보호 지원을 위한 질 높은 서비스를 제공할 만큼 경제적 성장을 이루었다고 본다. 지금까지는 법무부에서 범죄피해자 보호 지원업무를 전담해오다가 최근 경찰청에서도 범죄피해자 보호 지원을 경찰의 주요 업무로 인식하고 각종 서비스를 제공하고 있다. 사실 형사단계로 본다면, 검찰과 법무부보다는 사건직후 피해자를 처음 접하는 경찰에서 즉각적인 범죄피해자 보호 지원을 하는 것이 바람직하기는 하다. 문제는 법령과 제도 상 경찰은 서비스제공에 한계가 있지만, 경찰의 피해자 보호 지원의 중요성을 감안할 때, 민관협동 형태의 범죄피해자 보호 지원 모델을 탐색해볼 필요가 있다. 물론, 우선 법무부의 민관협동 체제와의 새롭게 관계정립이 필요하고, 경찰의 특성을 살려서 피해자에게 제공할 수 있는 서비스의 내용을 정비하고 민관협동 형태로 간다면, 어디까지 경찰에서 서비스를 제공할 것인지 방향도 정해질 것이다. 선진국의 경우 경찰과 민간기구의 상호 협조가 매우 긴밀하게 이루어지고 있으며, 이는 범죄피해자에게 실질적인 보호와 지원의 성과로 나타나고 있다. 이에 본 연구는 경찰단계에서 범죄피해자 보호 지원의 문제점을 검토해보고, 그 개선방안에 대하여 다음과 같이 제시하였다. 첫째는 범죄피해자 보호지원에 있어서 가장 먼저 접하는 경찰의 역할을 재정립하는 것이고, 둘째는 경찰서별로 피해자전담경찰관과 피해자 보호사 등의 상시 협력체계를 구축하는 것이며, 셋째는 경찰단계에서 피해자보호지원이 가능한 서비스의 종류를 개발하는 것이고, 넷째는 범죄피해자의 임시숙소 또는 신변보호를 위한 안전지대의 기능을 담당할 피해자종합지원센터를 전국적으로 개소하는 방안, 다섯째는 지역권내에 범죄피해자의 보호지원을 위한 유관기관 담당자로 구성된 솔루션회의의 정례화, 여섯째는 범죄피해자의 보호는 경찰 등의 국가기관에서, 범죄피해자에 대한 지원은 지방자치 단체의 복지부서에서 담당하고 종합적으로 사후관리하는 방안을 제시하였다.

강판채움벽을 갖는 RC 골조에 대한 반복가력 실험 (Cyclic Test for RC Frame with Infilled Steel Plate)

  • 철근콘크리트 골조에 얇은 강판채움벽을 접합한 합성벽의 내진 성능을 연구하기 위한 실험을 실시하였다. 실험체로서 얇은 채움강판을 사용한 3층 합성벽을 사용하였다. 주요 실험 변수는 기둥의 철근비와 채움벽의 개구부이다. 비교를 위하여 철근콘크리트 채움벽과 철근콘크리트 골조에 대한 실험을 실시하였다. 강판채움벽을 갖는 합성벽 실험체는 철근콘크리트 채움벽과 동일한 하중재하능력을 나타내면서도 변형능력이 크게 향상되었다. 또한 철골 골조를 사용한 강판벽 시스템과 마찬가지로 우수한 강도, 큰 변형능력 및 에너지소산 능력을 나타냈다. RC 골조에 대한 강판채움벽의 보강효과로 기둥-보 접합부의 전단균열과 손상을 방지할 수 있었다. 스트립 모델을 사용한 해석 연구를 통하여 합성벽 실험체의 강성 및 강도를 예측하였으며, 해석결과를 실험결과와 비교했다.

  • 저자는 파노라마 방사선사진을 이용하여 악관절 진단시에 진단학적 유용성을 알아보기 위하여 5개의 건조 두개골을 대상으로 하악과두의 4부위에 자각 인위적으로 형성한 골편을 부착시켜 피사체를 통상적인 위치, 전방으로 25㎜ 이동시킨 위치 및 전후 반대방향으로 위치시킨 상태에서 촬영하여 그 상을 비교하여 다음과 같은 결론을 얻었다. 피사체가 통상적인 위치에 있을 경우에는 골증식체가 전외부와 전중부에 있을 때 전내부와 상방부에 있을 때보다 관찰이 용이하였다 (p<0.01). 피사체가 25㎜ 전방이동한 경우에는 골증식체가 전중부와 전내부에 있을 때 전외부와 상방부에 있을 때보다 관찰이 용이하였다. (p<0.05). 피사체가 반대방향으로 위치한 경우에는 골증식체가 전중부에 있을 때 타부위에서보다 관찰이 용이 하였다 (p<0.05)

  • Some odontogenic infections erode into fascial spaces directly and spread toward lymphatic tissues and blood streams. The principal maxillary primary spaces are the canine, buccal, and infratemporal space, the next secondary spaces are the masseteric, temporal and pharygeal space. As a result of the infection, trismus and orocutaneous fistula may be occurred. Trismus is owing to conditions not associated with temporomandibular joint itself and may be of myogenic, neurogenic, or psychogenic nature. Muscular trismus is due to infection adjacent to the elevator muscles of the jaw. The four principles of treatment of infection are as follows: (1) removal of the cause, (2) establishment of drainage, (3) institution of antibiotic therapy, and (4) provision of supportive care, including rest, nutrition and physiotherapy. Jaw physiotherapy is necessary to increase the amount of mouth opening and regain normal muscle tone. If proper care of odontogenic infection could be attained, the orocutaneous fistula will heal and close spontaneously by wound contraction mechanism of natural homeostatic response. This is a case report of the care of trismus and orocutaneous fistula due to fascial space abscess by advanced odontogenic infection in a physically disabled patient.

Additional Surgical Method Aimed to Increase Distractive Force during Occipitocervical Stabilization : Technical Note

  • Objective : Craniovertebral junctional anomalies constitute a technical challenge. Surgical opening of atlantoaxial joint region is a complex procedure especially in patients with nuchal deformity like basilar invagination. This region has actually very complicated anatomical and functional characteristics, including multiple joints providing extension, flexion, and wide rotation. In fact, it is also a bottleneck region where bones, neural structures, and blood vessels are located. Stabilization surgery regarding this region should consider the fact that the area exposes excessive and life-long stress due to complex movements and human posture. Therefore, all options should be considered for surgical stabilization, and they could be interchanged during the surgery, if required. Methods : A 53-year-old male patient applied to outpatients' clinic with complaints of head and neck pain persisting for a long time. Physical examination was normal except increased deep tendon reflexes. The patient was on long-term corticosteroid due to an allergic disease. Magnetic resonance imaging and computed tomography findings indicated basilar invagination and atlantoaxial dislocation.The patient underwent C0-C3-C4 (lateral mass) and additional C0-C2 (translaminar) stabilization surgery. Results : In routine practice, the sites where rods are bound to occipital plates were placed as paramedian. Instead, we inserted lateral mass screw to the sites where occipital screws were inserted on the occipital plate, thereby creating a site where extra rod could be bound.When C2 translaminar screw is inserted, screw caps remain on the median plane, which makes them difficult to bind to contralateral system. These bind directly to occipital plate without any connection from this region to the contralateral system.Advantages of this technique include easy insertion of C2 translaminar screws, presence of increased screw sizes, and exclusion of pullout forces onto the screw from neck movements. Another advantage of the technique is the median placement of the rod; i.e., thick part of the occipital bone is in alignment with axial loading. Conclusion : We believe that this technique, which could be easily performed as adjuvant to classical stabilization surgery with no need for special screw and rod, may improve distraction force in patients with low bone density.

하악 과두부에 발생한 골연골종의 치험례 (OSTEOCHONDROMA OF THE MANDIBULAR CONDYLE: A CASE REPORT)

  • Osteochondroma is one of the most common benign tumors of the axial skeleton, but is rarely found in the facial bones. Osteochondroma shows an irregular radiopaque lesion and chondromatic area surrounded by osteoma. It may appear different findings as calcification levels. When it develops in the long bone, it has a marked tendency in the ages from 10 to 20 years and ceases with the end of pubertal growth. However, when it develops in the condyle, it is prevalent in the third decades (average 39.2 years) and continues to develop. Lesions developed in the long bone have a predilection for men (M:F = 2:1), but for women in the mandible. Osteochondroma is differentiated from chondroma, osteochondromatosis and osteoma. Mandibular condyle osteochondroma presents asymptomatic facial swelling, rarely posterior openbite, pain during mouth opening and internal derangement of the temporomandibular joint disc due to condylar lengthening and condylar hyperplasia. The first choice of treatment of the massive osteochondroma is the surgical removal. We report osteochondroma of the mandibular condyle showing good result to treat the lesion.

Influence of Guide Vane Setting in Pump Mode on Performance Characteristics of a Pump-Turbine

  • Performance characteristics in pump mode of pump-turbines are vital for the safe and effective operation of pumped storage power plants. However, the head characteristics are different under different guide vane openings. In this paper, 3-D steady simulations were performed under 13mm, 19mm and 25mm guide vane openings. Three groups of operating points under the three GVOs were chosen based on experimental validation to investigate the influence of guide vane setting on flow patterns upstream and downstream. The results reveal that, the guide vane setting will obviously change the flow pattern downstream, which in turn influences the flow upstream. It shows a strong effect on hydraulic loss (power dissipation) in the guide and stay vanes. It is also found that the hydraulic loss mainly comes from the flow separation and vortices. In addition, in some operating conditions, the change of guide vane opening will change the flow angle at the runner inlet and outlet, which will change the Euler momentum (power input). The joint action of Euler momentum and hydraulic loss results in the change of the head characteristics.

SP시험에 의한 TMCP강의 방향성 및 용접부의 파괴인성에 관한 연구 (A study on fracture toughness of welded joint and orientation in TMCP steel by th SP test)

  • In this paper, the fracture toughness evaluation of the various microstructures such as HAZ, F.L and W.M in weldment of TMCP steel which has the softening zone owing to high heat input welding was carried out by using of the small punch(SP) test. In addition, the fracture toughness with the specimen orientation of rolled TMCP steel was investigated by means of SP test and the crack opening displacement (COD) test and then was compared with that of conventional SM50YB steel. From the results of SP test for TMCP steel, it could be seen that the SP energy transition curves of three different orientation were shifted to higher temperature side in order of S, T and L. But the {TEX}$DBTT_{SP}${/TEX} of each orientation specimen did not show a lot of differences and were quite lower than those of conventional SM50YB steel. The mechanical properties of HAZ structure in weldment of TMCP steel such as hardness, SP energy at room temperature and -196$^{\circ}C$ and the upper shelf energy of SP energy transition curve were lower than those of base metal due to softening. The {TEX}$DBTT_{SP}${/TEX} of each microstructure in weldment of TMCP steel increased in order of HAZ, F.L and W.M against base metal, but all microstructures showed a quite lower {TEX}$DBTT_{SP}${/TEX} than those of SM50YB steel.

은 나노 연고가 측두하악관절낭염의 환자의 치료에 미치는 영향에 관한 연구 (THE THERAPEUTIC EFFECT OF SILVER NANOCRYSTALLINE OINTMENT ON TMJ CAPSULITIS)

  • The silver nanocrystalline is widely used for biological field because of its biocompatibility and anti-microbial effect. The objective of this study was to evaluate the therapeutic efficacy of the silver nanocrystalline ointment on the temporomandibular joint (TMJ) capsulitis. Total 39 patients were included in this study and all patients were received single topical application of the silver nanocrystalline ointment (group A, n=30) or placebo ointment (group B, n=19). Measured variables were maximum mouth opening (MMO), visual analog scale (VAS) for pain, and VAS for function. In results, we could not assess any therapeutic efficacy of single application in the chronic TMJ capsulitis (p>0.05). However, the single application of silver nanocrystalline ointment showed significant improvement in MMO and VAS for pain compared to placebo effect in the acute TMJ capsulitis (p<0.05). We could not find any complications related to ointment application in both groups. In conclusion, the single application of silver nanocrystalline ointment was effective in improving patient's symptom in acute TMJ capsulitis without any noticing complications.
