Progress of Pruritus Research in Atopic Dermatitis

  • Atopic dermatitis is a common skin disease affecting up to 10% of children and approximately 2% of adults. Atopic dermatitis exhibits four major symptoms, including intense itching, dry skin, redness and exudation. The "itch-scratch-itch" cycle is one of the major features in atopic dermatitis. The pathophysiology and neurobiology of pruritus is unclear. Currently there are no single and universally effective pharmacological antipruritic drugs for treatment of atopic dermatitis. Thus, controlling of itch is a very important unmet need in patients suffering from atopic dermatitis. This article will update progress during the past 10 years of research in the field of pruritus of atopic dermatitis, focusing on aspects of pruritogens (including inflammatory lipids, histamine, serotonin, proteinases, proteinase-activating receptors, neurotransmitters, neuropeptides, and opioid peptides), antipruritic therapies, and emerging new targets. Based on recent progress, researchers expect to identify exciting possibilities for improved treatments and to develop new antipruritic drugs acting through novel targets, such as histamine H4 receptor, gastrin-releasing peptide receptor, MrgprA3, thromboxane A2 receptor and the putative SPC receptor.

  • International Hull Clauses(IHC(2003)) which have replaced the old system, Institute Time Clauses, Hull(ITCH), used more than 100 years in the shipping industry that did not fit the reality of the market and customs was announced in 01/11/2003 and was intended to supplement the existing issues to reflect the terms that had been used. IHC(2003) was composed of systematical and logical phrases to minimize the conflict which happened between the insured and the insurer after the incident. But IHC(2003) is being ignored by both the insured and the insurer in the shipping industry due to the differences in both positions. In addition, the studies about IHC(2003) in local academics are very low and many companies in the shipping industry are using both systems, IHC(2003) and ITCH so the usage of IHC(2003) is not so activated. Thus, this study will examines the main features and the compositions of IHC(2003) and compare IHC(2003) with ITCH(1983) in some provisions in both systems and then derive some similarities from the both systems and finally would suggest the necessity and validity of active use of IHC(2003).

A Case Report of Atopic Dermatitis treated by Soshiho-tang based on Shanghanlun Provisions (『상한론(傷寒論)』 변병진단체계(辨病診斷體系)에 근거하여 소시호탕(小柴胡湯)을 투여한 양명병(陽明病) 아토피 피부염 치험 1례)

  • Objective : This case report presents the effect of Soshiho-tang on Atopic Dermatitis. The patient was diagnosed with Yangmyung-byung based on Shanghanlun provisions. Methods : According to Shanghanlun six meridian patterns diagnostic system, we dianosed the patient as Yangmyung-byung and he had trouble getting out of bed and nighttime itch. The patient was treated with Soshiho-tang. We evaluated the results of treatment by Objective SCORAD Index and pictures. Results : After the treatment, Objective SCORAD Index decreased from 61 to 21.5. And he became to have no trouble in getting up and also nighttime itch vanished. Conclusions : This case report sowed and effectiveness of using Soshiho-tang according to Diagnostic system based on Shanghanlun provisions on Atopic Dermatitis.

  • A literature study was progressed for oriental medical treatment for atopic dermatitis. Atopic dermatitis is a chronic or chronically relapsing, pruritic dermatitis. In oriental medical treatment is classified into the treatment of inside(herb tea), treatment of outside(medicine for external application), treatment of inside & outside. The method of inside treatment is improving in health for the spleen and clearing wetness(健脾利濕), extinguishing wind and refreshing fever(消風淸熱), nourishing blood and making dryness gloss(養血潤燥). The method of outside treatment is refreshing fever and removing wetness(淸熱燥濕), counteracting the poison and getting rid of itch(解毒止痒), removing the boil and forming proud flesh(消腫生肌). The method of inside & outside is improving in health for the spleen and clearing wetness(健脾利濕), refreshing fever and getting rid of itch(淸熱止痒). refreshing blood and extinguishing wind(凉血祛風).

A Clinical Study of Urticaria and an Assessment Guideline (두드러기에 대한 임상 연구 및 평가 지침)

  • Objectives: This study was designed to assess traditional Korean medical therapy for urticaria and establish a clinical assessment guideline. Methods: Duration of symptoms, personal and family history of allergic diseases, serum IgE level, suspected causes and Provocation factors were investigated in 56 patients. Frequency of wheals, duration of wheals, number of wheals, itch severity and antihistamine counts on a 6-point (0-5) scale were assessed in 34 patients. The change of total scores and each criterion between baseline and follow up were analyzed using paired t-test (p<0.05). Results: 1. Acute urticaria within 6 weeks was $26.8\%$ and chronic urticaria over 6 weeks was $73.2\%$. The mean duration of symptoms was $19.2\pm38.8$ months. 2. $57.1\%$ of patients had a personal history of allergic diseases and $50\%$had a family history. $29.1\%$ had over 200 IU/mL serum IgE level. 3. The suspected causes were none $(39.3\%)$, foods $(32.1\%)$, drugs $(12.5\%)$, contact $(5.4\%)$ and maternity$(5.4\%)$. 4. The suspected provocation factors were foods $(41.1\%)$, none $(26.8\%)$, fatigue $(12.5\%)$, heat and sweating $(12.5\%)$, pressure $(10,7\%)$ and stress $(7.1\%)$. 5. The total scores between baseline and follow up of 34 patients were lowered from $15.38^{\circ}\;{\ae}3.75\;to\;8.82^{\circ}\;{\ae}5.21$, which was significant statistically (p=0.000). 6. Itch severity, frequency of wheals and number of wheals were the most significantly lowered of all criteria. Conclusions: Traditional Korean medical therapy was proven to be effective for urticaria, while itch severity, frequency of wheals and number of wheals should be assessed clinically with greater priority.

The Effects of Wig Wearing on Scalp Disorders in Female Patients with Cancer (여성암환우의 가발착용이 두피질환에 미치는 영향)

  • This study is intended to look into the effects of wig wearing on scalp disorders in female patients with cancer. To achieve this, a survey was empirically carried out to female patients with cancer. The findings are as follows. The effect of wig wearing for hair loss on scalp itching in female patients with cancer showed that larger inconvenient factors did not cause the itch when wearing wig. And the adhesion, perspiration and psychological shrinking of wig wearing did not directly spark the scalp itch. The effect of wig wearing for hair loss on scalp trouble in female patients with cancer showed that higher inconvenient status and psychological shrinking significantly caused scalp trouble when removing wig, while the adhesion and perspiration did not directly correlated with scalp trouble when removing wig. As for the findings, it suggests that the simple adhesive status and perspiration do not just cause scalp disorders in that wig wearing for hair loss causes the scalp itch and trouble in female patients with cancer.

  • Objective : To find the main symptoms associated with atopic dermatitis and to get the utmost out of information about cause, treatment are contribute in the treatment of atopic dermatitis rather than looking for ways through how to connect modern atopic dermatitis and a listed symptom of classic literature. Method : In this research, we recorded about the main symptoms of atopic dermatitis such as itch, erythema, edema, pus and furuncle. Also we interpreted original text by extracting a sentence containing the terms like red[赤], pus[膿], flare[發赤], dermatosis[溞], swelling[洋], light red[紅], itch[瀁], worm running[蟲行], juice[汁], rash[癮疹], variegation[疹斑] and summarized cause of symptoms, disease sign, prescription. Results : There are the cause of many symptoms accompanied by atopic dermatitis in the body that recorded unpreparedness[虛], Wind[風], Fever[熱], unpreparedness & interruption of vitality hole[氣穴不通], unpreparedness & Wind, Wind Fever[風熱], Wind Coldness[風寒], Wind Humidity[風濕], Humidity Fever[濕熱], Wind Humidity Fever, Wind of Lungs[肺風], Wind of Lungs and Spleen[脾肺風], the Kidney Wind poison[腎臟風毒], Wind of Lungs Fever, Worm[蟲], miscellaneous vitality[雜氣], unpreparedness and Fever of Heart[心虛熱], old sore of kidney poison[久瘡腎臟風毒].

Antipruritic Effect of DA-5018m A Capsaicin Derivative, in Mice

  • The antipruritic effect of DA-5018m a capsaicin derivative, was examined in mice. Male ICR mice were topically pretreated with Zostrix-HP (0.075% capsaicin cream), 0.1%, 0.3% DA-5018 cream or cream base (control) twice daily for 4 days. One hour after the last application, itch was induced either by compound 48/80 ($50{\mu}g$, s.c.) or leukotriene B4 (0.03 nmol, i.d.) injection into the rostral back of the animals, and the number of scratches made by the animals at the injection site was counted for 60 min post-injection. DA-5018 cream (both 0.1 and 0.3%) significantly inhibited compound 48/80-induced scratching when compared with the cream base control (p<0.01), which Zostrix-HP showed minimal inhibition of the scratching behavior. In leukotriene B4-induced itch model, Zostrix-HP and 0.3% DA-5018 cram significantly inhibited the scratching during the first 10-min period (p<0.01). The results suggest that DA-5018 cream can be used as an antipruritic agent and warrant clinical evaluation.

  • Dermatitis herpetiformis (DH) is a chronic disease of the skin marked by groups of watery, itch blisters. It is characterized by urticarial plaques and blisters on the elbows, buttocks, and knees, although other sites may also be involved. The ingestion of gluten (the proteins gliadin and prolamin contained in wheat, rye, oats, and barley) triggers an immune system reponse that deposits a substance, IgA (immonuglobin A), under the top layer of skin. IgA is present in affected as well as unaffected skin. DH is a hereditary autoimmune disease linked with celiac disease. Treatment for DH is twofold. (1) Remove the cause: gluten. (2) Suppress the skin response with drugs such as Dapsone or some other sulphones. The latter is the most common treatment used as it is rapidly relieves the itch. However there are some side effects associated with these medications and they need to be taken under medical monitoring with blood tests to detect side effects. Recently, we experienced a DH and that was successfully treated by the herbal medication and external therapy. The medications taken by the patient were yongdamsagantanggami and external therapy were gosam and gumunhwa. So we report this case with a bief review of the oriental medical and medical literatures.

  • Many of the patients visit the doctors because of itching sensation. Itching is an unpleasant sensation. In the epidermal keratinocytes, various neurotransmitters and receptors are related itching. The itch signal is mainly transmitted through the lateral spinal ganglion-derived nerve fibers extending to the lower epidermis. Many mediators such as histamine are involved in the itching pathway. It can be helpful in the treatment of patients having itching sensation with a lot of new therapies from the basic medication such as antihistamines. Also, many drugs are currently under study.
