  • Purpose: To assess the clinical usefulness of Transfer Insensitive Labeling Technique (TILT) in t evaluation of ischemic cerebrovascular disease. Method: Arterial spin labeling (ASL) is a method of perfusion weighted imaging usin endogenous water as a tracer. To avoid MT-related artifacts, which is common in usual A technique, a transfer insensitive labeling technique (TILT) was used, which globall manipulate macromolecular spins in the same way by both labeling and reference preparatio while free water is labeled in one case and left unchanged in the other. Philips Interal 1.5 T system was used. 40cm FOV and 32 repeated measurements were done because of the wea perfusion signal. 5 slices of supratentorial brain were obtained in 5 patients {MCA infar (n=3), moyamoya disease (n=2)}. We simultaneously obtained contrast enhanced T2*-weighted perfusion MRI and correlate to TILT images.

  • In this paper, we infer the onset of the brain infarction from the MR image using evaluate signal intensities on diffusion weighted and turbo spin echo T2-weighted and FLAIR images. Infarcts were divided into four stages (hyperacute, acute, subacute, chronic) depending on period of onset. DWI is useful for the detection of early ischemic infarct, and stages of ischemic infarctions can be estimated by evaluating CR(conspicuity ratio) and CNR(contrast to noise ratio) on DW, T2, FLAIR images Hyperacute infarcts were visualized DWI. Acute infarcts were visualialized both DWI and T2 Weighted image.

  • We report a case of aortic dissection masquerading as acute ischemic stroke followed by intravenous thrombolysis. A 59-year-old man presented with dizziness. After examination, the patient had a seizure with bilateral Babinski signs. Soon after identifying multiple acute infarctions in both hemispheres on diffusion-weighted brain magnetic resonance (MR) imaging, tissue plasminogen activator (t-PA) was administered. Both common carotid arteries were invisible on MR angiography, and subsequent chest computed tomography revealed an aortic dissection. The emergency operation was delayed for 13 hours due to t-PA administration. The patient died of massive bleeding.

  • Cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL) is a single-gene disease caused by mutations in the neurogenic locus notch homolog protein 3 (NOTCH3) gene. The spectrum of clinical manifestations is broad, ranging from asymptomatic to typical ischemic stroke, and mainly depends on the location of the mutations. We describe the case of a 76-year-old female without apparent neurological deficits. However, brain magnetic resonance imaging revealed confluent lesions in the white matter. Direct sequencing of the NOTCH3 gene revealed a novel pathogenic mutation, c.811T>A, which results in a mild phenotype. Therefore, this report will expand the current knowledge in regards to the mutations that can cause CADASIL.

  • We will present various MR findings of hyperacute ischemic stroke with our own experiences in the management of the patients according to the findings. 대상 및 방법: A total of 441 patients were underwent 'acute stroke MR' imaging protocol between Mar. 2001 and Jun. 2003. The protocol included initial T2-weighted image (WI), diffusion WI (DWI, b=2000), time-of-flight (TOF) MR angiography (MRA), and pefusion WI(PWI), and follow-up T2WI, DWI, TOF MRA, and neck vessel contrast-enhanced MRA obtained three to five days after the insult. Among them, we retrospectively reviewed the MR findings and clinical courses of 193 patients with anterior circulation territorial infarction. Those ICA and MCA lesions were divided into six and five groups respectively according to the level and mechanism of the occlusion. PWI findings can be another factor in the management planning.

Rapid progression of large intracranial cerebral artery involvement in a patient with myeloperoxidase antineutrophil cytoplasmic antibody-associated vasculitis

  • Antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV) is a systemic necrotizing vasculitis that predominantly affects small vessels of the body. The two most common ANCAs are myeloperoxidase ANCA and proteinase 3 ANCA. Neurological manifestations are frequent in patients with AAV, including peripheral neuropathy, meningitis, and stroke. AAV-associated ischemic stroke usually affects small vessels supplying the white matter or brainstem. This case report details the presentation and treatment course of a 70-year-old man with rapidly progressive multiple intracranial large artery involvement attributed to myeloperoxidase ANCA-associated vasculitis. Despite treatment with high-dose steroids and a rituximab infusion, the patient developed new speech difficulties and respiratory distress, and brain imaging confirmed new stroke lesions with progressive multiple intracranial large cerebral artery involvement. The patient died from SARS-CoV-2 infection 4 months after the diagnosis. This case emphasized the rare presentation of rapidly progressive large vessel involvement in a patient with myeloperoxidase ANCA-associated vasculitis despite active immunotherapy.

Induction of Neuron-derived Orphan Receptor-1 in the Dentate Gyrus of the Hippocampal Formation Following Transient Global Ischemia in the Rat

  • Neuron-derived orphan receptor (NOR-1) is a member of the thyroid/steroid receptor superfamily that was originally identified in forebrain neuronal cells undergoing apoptosis. In addition to apoptotic stimuli, activation of several signal transduction pathways including direct neuronal depolarization regulates the expression of NOR-1. In this study we tested whether the expression of NOR-1 is changed following transient ischemic injury in the adult rat brain. NOR-1 mRNA increased rapidly in the dentate gyrus of the hippocampal formation and piriform cortex 3 h after transient global ischemia and returned to basal level at 6 h. On the other hand, oxygen-glucose deprivation of cultured cerebral cortical neurons did not alter the expression of NOR-1. These results suggest that expression of NOR-1 is differentially regulated in different brain regions in response to globally applied brain ischemia, but that hypoxia is not sufficient to induce its expression.

광화학적 뇌경색 백서 모델에서 황련의 항염증 및 운동기능 회복에 미치는 효과 (Effects of Coptidis Rhizoma on the Anti-inflammation and Motor Recovery in Photothrombotic Brain Infarction Model in Rats)

  • Objectives : Coptidis Rhizoma (Coptis japonica MAKINO; CR) is a well known crude drug as antimicrobial, antibacterial, anti-inflammatory, antioxidant activity. However, there is no study of the effect of CR on brain infarction and it's mechanism. The aim of this study was to investigate the effects on ischemic stroke induced by photothrombotic infarction by evaluating the functional & neuronal recovery after brain infarction. Materials & Methods : Male Sprague-Dawley rats (250-300 g) were induced photothrombotic brain infarction on sensorimotor cortex, and brain infarction volume by image J software (NIH, USA) after Nissl stain, also single pellet reaching task as a functional motor recovery were observed. After orally pretreated by CR (500 mg/kg) or normal saline as a sham control before 7 days from the time of photothrombotic infarction, rats were sacrificed. After then we analysed anti-inflammatory cytokines (TNF-$\alpha$, IL-6, IL-1$\beta$), by RT-PCR and ELISA method, and immunohistochemistry (GFAP, connexin-43) as a marker of neural plasticity. Results : CR (100, 250, 500 mg/kg) decreased the infarction volume dose-dependently, however the effect of 500mg/kg of CR (CR 500) showed the best (P=0.051). Also, CR 500 decreased the infarction volume time-dependently, the most effective time was 3-7 days after stroke. Photothrombosis increased inflammatory cytokines after infarction, CR 500 suppressed significantly mRNA expression of IL-1$\beta$, IL-6 and TNF-$\alpha$. In serum, CR 500 decreased the amount of IL-1$\beta$, 12h, 24h and 48h respectively (p < 0.05), also decreased that of IL-6 and TNF-$\alpha$, 12h respectively (p < 0.05) after infarction. The more astrocytes were observed and neural plasticity was facilitated in the rat brain of CR 500 than that of sham control in immunohistochemistry. Conclusions : This results suggest that CR decrease infarction volume and improve functional motor recovery in acute stage in photothrombotic ischemic infarction model in the mechanism of anti-inflammation and promoting neural plasticity.

  • 고혈압은 소아 청소년시기에 발생하는 흔한 만성질환 중 하나이다. 고혈압을 치료하지 않으면 여러가지 소아의 장기에 악영향을 미칠 수 있다. 여기서는 고혈압이 중추신경계에 미치는 영향에 대해 알아 보았다. 혈압의 변화에 따라 대뇌가 받는 영향은 다른 장기에 비해 덜 하다. 그것은 대뇌 자동 조절 기전때문이다. 즉 혈압의 변화에 따라 뇌혈관의 저항이 변하기 때문에 뇌혈류량이 일정하게 유지 된다. 그러나 이런 자동 조절 기전의 항상성이 파괴되면 급성으로 또는 만성으로 급성 고혈압 뇌병증, 허혈성, 출혈성 뇌경색, 학습 장애와 인지 기능 장애 등이 발생한다. 이에 고혈압이 대뇌에 미치는 영향에 대해 숙지하고 빠른 시일 내에 적절한 치료를 하면 이런 합병증을 예방할 수 있다.

Laser Speckle Contrast Imaging for Measuring Cerebral Blood Flow Changes Caused by Electrical Sensory Stimulation

  • Recently laser speckle contrast (LSC) imaging has become a widely used optical method for in vivo assessment of blood flow in the animal brain. LSC imaging is useful for monitoring brain hemodynamics with relatively high spatio-temporal resolution. A speckle contrast imaging system has been implemented with electrical sensory stimulation apparatus. LSC imaging is combined with optical intrinsic signal imaging in order to measure changes in cerebral blood flow as well as neural activity in response to electrical sensory stimulation applied to the hindlimb region of the mouse brain. We found that blood flow and oxygen consumption are correlated and both sides of hindlimb activation regions are symmetrically located. This apparatus could be used to monitor spatial or temporal responses of cerebral blood flow in animal disease models such as ischemic stroke or cortical spreading depression.
