집단 인지정서행동요법 프로그램이 간호사의 직무스트레스, 소진, 직무만족, 조직몰입 및 이직의도에 미치는 효과 (Effects of Group Rational Emotive Behavior Therapy on the Nurses' Job Stress, Burnout, Job Satisfaction, Organizational Commitment and Turnover Intention)

  • Purpose: This study aimed to develop a Group REBT program with group counseling for nurses and test the effect of group counseling on their job stress, burnout, job satisfaction, organizational commitment, and turnover intention. Methods: A quasi-experimental study with nonequivalent control group design was employed to identify the effect of the Group REBT program on nurses' job stress, burnout, job satisfaction, organizational commitment, and turnover intention. Data were collected from 47 participants from two hospitals. The data from the experimental (n=23) and control (n=24) groups were analyzed from January 5 to April 3, 2015. The Group REBT program was conducted eight tmes in all, once a week, with each session lasting 180 minutes. The effect of experimental intervention was measured for each group using a series of structured questionnaires at each of the phases: Pre-intervention, post-intervention (immediately after intervention), and post-intervention (four weeks after intervention). Following this, the significance of the changes in the scores was tested. Results: The scores of the experimental group, which received the Group REBT program, were compared with those of the control group; the hypotheses were supported in terms of job stress (F=8.85, p<.001), burnout (F=5.62, p=.022), job satisfaction (F=2.70, p=.042), organizational commitment (F=2.97, p=.048), and turnover intention (F=4.60, p=.012). Conclusion: The Group REBT program was shown to be an effective intervention that could reduce nurses' job stress and burnout and increase job satisfaction and organizational commitment. Therefore, the Group REBT program can be adopted by nursing organizations to strategically decrease nurses' turnover intention.

암생존자를 위한 심리교육중재의 효과에 대한 체계적 고찰 및 메타분석 (Effects of Psychoeducational Intervention for Cancer Survivors: A Systematic Review and Meta-Analysis)

  • Purpose: This study was a systematic review and meta-analysis designed to investigate effects of psychoeducational intervention for cancer survivors. Methods: Ten databases were searched. Two reviewers independently performed the selection of the studies, data extraction and assessment. The risk of bias was assessed using Cochrane Collaboration's tool. To estimate the effect size, meta-analysis of the studies was performed using Comprehensive Meta-Analysis and RevMan programs. Results: Of 18,781 publications identified, 35 met inclusion criteria, and 25 studies were used to estimate effect size of psychoeducational intervention. Effect sizes (standardized mean difference [SMD]) were heterogeneous and random effects models were used in the analyses. Psychoeducational intervention was effective for quality of life (n=2,410, ES=0.23; 95% CI: 0.09~0.37), coping and self-efficacy (n=179, ES=0.68; 95% CI: 0.26~1.11), anxiety (n=1,786, ES=-0.26; 95% CI: -0.37~-0.15), depression (n=1,910, ES=-0.28; 95% CI: -0.37~-0.18), and psychological distress (n=2,242, ES=-0.31; 95% CI: -0.46~-0.17). Subgroup analysis showed that counseling was the most effective intervention for quality of life, and behavioral therapy was an effective intervention for all positive and negative outcomes. Publication bias was not detected except for psychological distress. Conclusion: Psychoeducational intervention appears to be effective in improving quality of life and coping and self-efficacy, and it is effective in reducing psychological symptoms in cancer survivors. Behavioral therapy, especially, is commonly effective in improving psychosocial outcomes. However, low-quality evidence, variability in the designs of existing studies, and publication bias suggest that additional high-quality trials should be conducted in the future.

산부를 위한 마사지 중재 효과에 대한 메타분석 (Meta Analysis of the Effects of Massage Intervention for the Laboring Women)

  • Objectives : The study was to examine the empirical evidence concerning the effects of massage intervention for the laboring women and to investigate the effect sizes related to the characteristics of intervention. Methods : Meta analysis was performed. A total of 11 trials related to massage intervention between 2000 and 2007 were reviewed. Results : Massage intervention showed a beneficial effect on length of labor(1st stage : U=46.75, p=0.00, 2nd stage : U=6.13, p=0.01), perception of childbirth experience(U=12.57, p=0.00), intensity of uterine contraction at the active(U=6.34, p=0.01) and the transition phases(U=24.83, p=0.00), and interval of uterine contraction at the active phase(U=4.83, p=0.03). Massage decreased systolic(U=10.81, p=0.00) and diastolic blood pressure at the transition phase(U=10.60, p=0.00). However, massage had no effect on subjective and objective labor pain and anxiety. No differences were found in effectiveness of massage according to characteristics of intervention(massage material, massager, and massage site) except for anxiety(QB=4.82, p=0.03) and diastolic blood pressure at the active phase by massage site(QB=4.52, p=0.03). Conclusions :We found that massage intervention for the laboring women had an empirical evidence on improving perception of childbirth experience and shortening length of labor. These results should be interpreted with caution due to the lack of studies. More full-scale randomized clinical trials with reliable designs are recommended to further warrant the effectiveness of massage.

작업기반 중재와 전산화인지재활치료의 결합 중재가 뇌졸중 환자의 웰니스를 위한 우울감에 미치는 영향 (The Effect of Combination Intervention of Occupation - Based Intervention and Computerized Cognitive Rehabilitation on Depression in Stroke Patients for Wellness)

  • 본 연구의 목적은 작업기반 중재와 전산화인지치료의 결합 중재를 통해 뇌졸중 환자의 심리사회적 기능과 인지 기능 및 우울감에 어떠한 영향을 미치는지 알아보고자 하는 것이다. 뇌졸중 환자 23명을 실험집단 11명, 대조집단 12명으로 나누어 진행하였으며, 실험집단과 대조집단은 4주간, 30분씩의 CoTras를 받았으며, 실험집단의 추가적으로 CoTras 시작 이전에 30분간 작업기반 중재를 받았다. 총 4주간, 20회 진행되었다. 평가는 자기 효능감 척도, 스트레스 척도, 우울척도, 신경행동학적 인지 상태 검사를 사용하였다. 연구 결과, 실험집단은 자기 효능감과 스트레스, 우울의 개선에 대조집단보다 더 유의미한 개선을 나타냈으며, 인지 기능에서 실험집단은 대조집단과 작은 효과의 크기차이를 보였다. 본 연구를 근거로 뇌졸중 환자의 심리사회적 요인과 우울증, 인지 기능의 개선을 위해 CoTras와 함께 작업기반 중재가 동시에 수행되는 것을 활용할 수 있을 것이다.

암환자의 통증에 적용한 중재효과의 메타분석 (A Meta-Analysis of Intervention Studies on Cancer Pain)

  • Purpose: This study was to analyze the characteristics and effect size of intervention studies in reference to cancer pain. Methods: In order to conduct a meta-analysis, a total of 208 studies were retrieved from search engine. And 29 studies published from 2000 to 2010 were selected upon their satisfaction with the inclusion criteria. The data was analyzed by the RevMan 5.0 program of Cochrane library. Results: 1) Intervention studies included 7 studies on reflexology (24.1%), 5 for pain management education (17.2%), 3 studies for each music therapy, spiritual care and hand massage (10.3%, respectively), and 2 studies for each hospice and horticultural therapy (6.7%, respectively). 2) The effect size of the intervention studies were high in hand massage (d=-0.98), reflexology (d=-0.74), spiritual care (d=-0.72), pain management education (d=-0.66), music therapy (d=-0.41), and horticultural therapy (d=-0.32). Conclusion: This study suggest that non-drug therapy can reduce the levels of cancer pain intensity, even though the numbers of intervention studies and randomized controlled trials are very rare.

Effects of a School-based Intervention Program on Attitude and Knowledge of Household Members Towards a Smoke-free Home: a Cluster Controlled Trial

  • Background: A school-based smoke free home (SFH) program is useful in empowering the mother and child to reduce secondhand smoke exposure but the effects of pretesting on knowledge and attitude has been largely ignored. We aimed to test whether such a program can be effective in Southern Thailand with an additional assessment of the net effect of the pretest. Materials and Methods: A Solomon four-group design was used. Twelve rural primary schools were assigned to one of the four conditions (each with 3 schools): intervention with and without a pretest, control with and without the same pretest. The intervention was performed in the classroom and home over a period of 1 month. Outcomes were assessed at baseline and 3 months after the intervention on whether the home was smoke free and related knowledge and attitude. Results: The intervention could lead to a smoke-free home without statistical significance. Attitude, knowledge and self-confidence on creating a smoke-free home, and self-confidence in avoidance of secondhand smoke exposure and persuading smokers to not smoke in their home were significantly improved. No pretest effect was observed. Conclusions: Gain in attitude, knowledge and self-confidence among family members from the brief school-based education should be enhanced by other measures.

집단지지 중재가 뇌졸중 환자 가족원의 제 적응에 미치는 효과 (The Effect of Support Group Intervention on Various Adaptations of Primary Family Caregivers Caring for Cerebro-Vascular Accident Patients)

  • The purpose of this research was to examine the effect of support group intervention on the various adaptations of primary family caregivers caring for Cerebro- Vascular Accident patients. The nonequivalent control group pretest-posttest design within the framework of Lazarus & Folkman's stress-adaptation model was used for this experimental study. The subjects were 86 primary family caregivers caring for Cerebro- Vascular Accident patients at K hospital in Taegu, D herbal hospital in Kyung Ju, H hospital in Pohang from March, 1998 to July, 1998. Among 86 subjects, 43 were placed in an experimental group and 43 in a control group. The experimental group was treated by researcher who administered informational and emotional support group intervention once a week over a five weeks period. The data were collected through interviews. Collected data was analized by means of a chi-square test, t-test, ANCOVA, and Pearson correlation coefficient. The results of this research were as follows: 1. Physical, emotional, and social adaptation scores in the experimental group were revealed to be significantly higher than those of the control group. 2. There was significant positive correlation among physical health, subjective burden, depression and objective burden. Accordingly, it is concluded that informational and emotional support group intervention was a useful nursing intervention on the various adaptations of primary family caregivers caring for Cerebro-Vascular Accident patients.

Impact of an Information Leaflet on Knowledge of Hepatocellular Carcinoma and Hepatitis B among Chinese Youth

  • Background: To assess the effect of an information leaflet on the level of Chinese youth's knowledge about hepatitis B and hepatocellular carcinoma (HCC), the most common type of primary liver cancer (PLC). Materials and Methods: A total of 500 students, from two universities in the Chaoshan area of China, were randomly divided into an intervention group of 280 participants and a control group of 220. Baseline knowledge of HCC and hepatitis B was evaluated by questionnaire interview. Subsequently, only the intervention group was given an information leaflet of HCC and hepatitis B. Three months later, the two groups were contacted for a second interview. Changes in knowledge from baseline of HCC and hepatitis B were compared between the two groups. Results: There was no statistically significant difference in mean PRE-questionnaire scores between the intervention and control groups. However, the mean POST-questionnaire score was significantly higher in the intervention group after the intervention. The leaflet had the greatest effect on the participants' questionnaire score, and raised their level of knowledge about HCC and hepatitis B. Conclusions: The information leaflet intervention is significantly effective in improving the knowledge of HCC and hepatitis B among the youth.

Gestalt Group Meditation Therapy Effect on Occupational Stress and Anxiety in Working Women

  • The purpose of this research was to measure the effect of a gestalt group meditation therapy intervention program on occupational stress and anxiety in working women aged 30 to 49 in Korea. Scientific methods were employed to test a gestalt group meditation therapy intervention program as treatment for working women in 30-40s to reduce their level of occupational stress and anxiety. The study involved 18 volunteer subjects ages of 30 to 49 years, randomly divided into two groups: (1) gestalt group meditation therapy intervention program, and (2) no program. Both groups completed a pre-test and a post-test of occupational stress scale and trait anxiety inventory. The outcome data was analyzed as qual1litative information using the SPSSPC+ computer program. Mann-Whitney U test was used to analyze the result in order to verify the sameness between experienced group and controlled group of pre-test scores within the groups. Also U-test comparisons were used to analyze the treatment effects. The results of this investigation indicated that (1) occupational stress scores decrease after gestalt group meditation therapy intervention program on the treatment group, and (2) trait-anxiety scores decrease after gestalt group meditation therapy intervention program on the treatment group. In summary, the gestalt group meditation therapy intervention program for career women, aged 30 to 49, in Korea could reduce the subjects' occupational stress and anxiety.

전신조정술의 맨손 중재와 자가교정운동 모형이 휜 다리의 교정에 미치는 영향 (The Effects of the Manual Intervention and Self Corrective Exercise Models of General Coordinative Manipulation on the Distorsional Leg)

  • Purpose: The purpose of this study was to analyze the effect of the manual intervention and self corrective exercise models of GCM(General Coordinative Manipulation) on the groups bow-knee and knock-knee. Methods: GCM Center of 23 members were divided into the two different groups. 12 members of group bow-knee and 11 members of group knock-knee applied to each manual intervention and self corrective exercise models of GCM. Two different groups were applied to 1 cycle a day for 4 weeks, 3 times a week. Results: The effect of manual intervention and self corrective exercise models of GCM on the groups bow-knee and knock-knee was significant(z<.05). The relationship between groups bow-knee and knock-knee was no significant(z>.05). Conclusion: the manual intervention and self corrective exercise models of GCM was contributed in the Correct recovery of bow-knee and knock-knee(z<.05).
