The Effect of Adding Kami-guibi-tang to Acetylcholinesterase Inhibitor Treatment on the Cognitive Function of Mild Alzheimer's Disease Patients: Study Protocol of a Randomized, Placebo-Controlled, Double-Blind Pilot Trial

  • Background: Alzheimer's disease (AD) is a chronic neurodegenerative disease that causes disorientation, mood swings, problems with language, and difficulty remembering recent events. Acetylcholinesterase inhibitors (AchEIs) and memantine have been used to slow the course of the disease, but they can neither modify its progression nor prevent disease onset. Previous studies have suggested that Kami-guibi-tang (KGT) could be beneficial for supporting cognitive function in AD patients, but few clinical trials have been published. This pilot study aimed to evaluate the effect of KGT in improving cognitive function in AD patients. Methods: The study will be a randomized, placebo-controlled, double-blind, single-center trial conducted using subjects diagnosed with mild AD by neurologists. Study subjects will be randomly assigned to either a treatment or control group. The treatment group will receive KGT granules for 24 weeks, while the control group will receive placebo granules. AchEI administration will be maintained in both groups during the entirety of the study. Subjects will be assessed using the following exams: the Seoul Neuropsychologic Screening Battery (SNSB) for cognitive function; brain magnetic resonance imaging (MRI) for brain metabolite, neurotransmitter, and cerebral blood flow (CBF) measurements; the Korean version of Quality of Life-Alzheimer's Disease (KQol-AD) for quality of life; the Caregiver-Administered Neuropsychiatric Inventory (CGA-NPI) for neurobehavioral symptoms; blood tests for amyloid and tau proteins and general blood parameters; and electrocardiography (ECG) before and after taking the medication. Discussion: Our findings will provide insight into the feasibility of large-scale trials to consolidate evidence for the efficacy of KGT for dementia treatment. A Study on Improvement of 5G In-Building Quality using Antenna Orientation Principle (안테나 지향성 원리를 이용한 5G 건물 내 품질향상에 관한 연구)

  • This study is a study that designed in-building antennas with improved orientation to improve 5G quality in buildings as 5G is stabilized and more and more traffic is expected to occur in buildings. Instead of applying the forward arrangement of antenna elements, which is the Yagi antenna propagation orientation principle, the antenna design method of vertical arrangement applied to the base station antenna was proposed, and it was confirmed through experiments that antenna orientation increased. According to the experimental results, the directivity did not increase significantly within 10m of the separation distance from the antenna, but the directivity increased by about 3dB at the distance separated by more than 10m. Considering that the wireless environment in the building has various variables such as structure of internal structure, materials such as concrete and glass, closed space, and walls, it is expected that antenna with improved orientation can expand the scope of 5G quality improvement and maintain stable communication service in the building.

A Hybrid Forecasting Framework based on Case-based Reasoning and Artificial Neural Network (사례기반 추론기법과 인공신경망을 이용한 서비스 수요예측 프레임워크)

  • To enhance the competitive advantage in a constantly changing business environment, an enterprise management must make the right decision in many business activities based on both internal and external information. Thus, providing accurate information plays a prominent role in management's decision making. Intuitively, historical data can provide a feasible estimate through the forecasting models. Therefore, if the service department can estimate the service quantity for the next period, the service department can then effectively control the inventory of service related resources such as human, parts, and other facilities. In addition, the production department can make load map for improving its product quality. Therefore, obtaining an accurate service forecast most likely appears to be critical to manufacturing companies. Numerous investigations addressing this problem have generally employed statistical methods, such as regression or autoregressive and moving average simulation. However, these methods are only efficient for data with are seasonal or cyclical. If the data are influenced by the special characteristics of product, they are not feasible. In our research, we propose a forecasting framework that predicts service demand of manufacturing organization by combining Case-based reasoning (CBR) and leveraging an unsupervised artificial neural network based clustering analysis (i.e., Self-Organizing Maps; SOM). We believe that this is one of the first attempts at applying unsupervised artificial neural network-based machine-learning techniques in the service forecasting domain. Our proposed approach has several appealing features : (1) We applied CBR and SOM in a new forecasting domain such as service demand forecasting. (2) We proposed our combined approach between CBR and SOM in order to overcome limitations of traditional statistical forecasting methods and We have developed a service forecasting tool based on the proposed approach using an unsupervised artificial neural network and Case-based reasoning. In this research, we conducted an empirical study on a real digital TV manufacturer (i.e., Company A). In addition, we have empirically evaluated the proposed approach and tool using real sales and service related data from digital TV manufacturer. In our empirical experiments, we intend to explore the performance of our proposed service forecasting framework when compared to the performances predicted by other two service forecasting methods; one is traditional CBR based forecasting model and the other is the existing service forecasting model used by Company A. We ran each service forecasting 144 times; each time, input data were randomly sampled for each service forecasting framework. To evaluate accuracy of forecasting results, we used Mean Absolute Percentage Error (MAPE) as primary performance measure in our experiments. We conducted one-way ANOVA test with the 144 measurements of MAPE for three different service forecasting approaches. For example, the F-ratio of MAPE for three different service forecasting approaches is 67.25 and the p-value is 0.000. This means that the difference between the MAPE of the three different service forecasting approaches is significant at the level of 0.000. Since there is a significant difference among the different service forecasting approaches, we conducted Tukey's HSD post hoc test to determine exactly which means of MAPE are significantly different from which other ones. In terms of MAPE, Tukey's HSD post hoc test grouped the three different service forecasting approaches into three different subsets in the following order: our proposed approach > traditional CBR-based service forecasting approach > the existing forecasting approach used by Company A. Consequently, our empirical experiments show that our proposed approach outperformed the traditional CBR based forecasting model and the existing service forecasting model used by Company A. The rest of this paper is organized as follows. Section 2 provides some research background information such as summary of CBR and SOM. Section 3 presents a hybrid service forecasting framework based on Case-based Reasoning and Self-Organizing Maps, while the empirical evaluation results are summarized in Section 4. Conclusion and future research directions are finally discussed in Section 5.

The Fitness and Organizational performance Analysis of Hospital Specialization Strategy Types (병원전문화 전략유형의 적합성과 조직성과 분석)

  • This research classified hospital specialization strategy types through cluster analysis, analyzed fitness of hospital specialization strategy types for external environment or organizational structure, and examined relation between hospital specialization strategy types and organizational performance. This research surveyed 1,437 hospitals which have more than 30 patient's bed and practice national health service in Korea. Specifically, this research divided into two part : external fit - analysis of relation between external environment and specialization strategy, internal fit-analysis of relation between organizational structure factors and specialization strategy. also, as the organizational performance for achieving specialization strategic purpose, not only the productivity, efficiency, profit but also the medical quality was considered. In case of external fit, many hospitals chose integration type if there are a lot of competitive hospitals and regional population. Particularly, if there are many competitive hospitals, concentration type is chosen. In contrast, if there are many doctors in the region, differentiation type is chosen. In case of internal fit, according to organization type and patient's bed number, hospitals chose different types. If it is a general hospital and has a few bed number, generalization or concentration type is chosen. Tertiary hospital or the hospital with many patient's bed chose differentiation type. According to the number of specialists, if there are a few specialists, generalization or concentration type is chosen. If there are many specialists, differentiation type has high fitness for the hospital. In relation to strategy types and organizational performance, differentiation type has best result. Differentiation type has a good result in 7 items out of 11.

Need for Development of Oriental Medicine-derived Systemic Treatments against Chronic Fatigue-associated Symptoms (만성피로의 체계화된 한의학적 치료법 연구의 필요성)

  • Objectives: Fatigue is a prevalent symptom encompassing both acute and chronic manifestations. Most fatigue symptoms can be cured by taking a rest or removing underlying causes. However, chronic fatigue is frequently problematic due to its duration and effect on quality of life. There are no particularly effective therapies for chronic fatigue of unknown causes, and patients in Korea usually visit an Oriental clinic. This study aimed to analyze the current status of treatments and patients with chronic fatigue, and then map out of a strategy for development of generalized-treatments for chronic fatigue in Oriental Medicine. Methods: Clinical information related to chronic fatigue was selected from various different databases such as PubMed, KoreaMed, KStudy, DBPIA, OIM, and KOMS. Also, to understand current tendency of medical examination and treatment related with chronic fatigue, we requested Health Insurance Review & Assessment Service for clinical datum from 2003 to 2007. Results: The medical fees of National Health Insurance related with fatigue show an explosive year-on-year increase. On the other hand, it has been decreasing annually in the western medical fields. To take charge of clinical fields related with chronic fatigue by Oriental Medicine, we should make a unified diagnostic system. Then, we should also make standard evaluation tools and develop herbal drugs according to this unified diagnostic system. Conclusions: Fatigue-related symptoms will be a main target of Oriental medicine in the future. We expect that various studies related with chronic fatigue will be undertaken hereafter.

Effects of HACCP Implementation on an Industry Foodservice Operation in Daegu (대구지역 사업체급식소에 대한 HACCP 적용 효과)

  • The purpose of this study was to evaluate the effects of HACCP implementation. The HACCP education was provided twice within one month to 20 employees of a contracted food service operation in Daegu. Critical control points (CCP) were determined based on food preparation processes: non-heating, preparation after heating, and heating. We evaluated the effects of HACCP implementation by checking microbiological quality, time and temperature at each of the CCPs during the receiving, preparation, cooking, and serving stages. After HACCP implementation, the biggest changes in microbiological qualities were in heated foods. At the cooking and serving stages, the microbiological qualities of heated foods improved to the standard levels. HACCP education helped employees ensure that the internal temperatures of the heated foods were kept higher than the standard (74 $^{\circ}C$) and the food holding temperature avoided the dangerous zone (5-6$0^{\circ}C$), thus lowering microbiological levels. At the serving stage, the microbiological levels of utensils also improved after HACCP education. This result strongly suggests that it is essential to educate employees in managing the temperature to treat foods safely. However, HACCP education didn't affect the microbiological levels of non-heated foods and foods prepared after heating, which continued to be higher than the standard. The reason for this was that poor microbiological quality seasonings were added to those types of foods. This indicates that seasoning factories as well as food service operations should implement HACCP to reduce hazards.

  • Applying Information Technology (IT) in practical engineering has become one of the most important issues in the past few decades, especially on internal solitary wave, intelligent robot interaction, artificial intelligence, fuzzy Lyapunov, tension leg platform (TLP), consumer and service quality. Other than affecting the traditional teaching mode or increasing the inter-relation with users, IT can also be connected with the current society by collecting the latest information from the internet. It is apparently a fashion-catching-up technology. Therefore, the learning of how to use IT facilities is becoming one of engineers' skills nowadays. In addition to studying how well engineers learn to operate IT facilities and apply them into teaching, how engineers' general capacity of information effects the results of learning IT are also discussed. This research introduces the "Combined TAM and TPB mode," to understand the situation of engineers using IT facilities.

A Comparative Study on the Concepts of the Chuna(推拿) (추나(推拿)의 개념 비교연구)

  • The Chuna(推拿) in Korea has been developed since 1990's. Korean Chuna which is the only one manual therapy done by medical doctor is riched by absorbing other nation's manual therapy like Chinese Tuina(中國推拿) and Chiropractic therapy of U.S.A. So, We find that Korean Chuna's peculiarity and development by comparing Chinese Tuina, Chiropractic of U.S.A and Japanese manual therapy which influenced establishment of Korean Chuna. We compared each manual therapy's history, development, diagnosis, correction and treatment of human body, frequently contacting disease and medical service of the present state. Korean Chuna has absorbed other nation's manual therapy and advantage of preserving muscle-skeleton and spinal disorder disease. But, for more prosperity, it needs union of various conception and diagnosis and more research and application to more various disease, for example in internal medicine, gynecology or pediatry and founding system as regular academic work inside the college the possibility which objection and it will practice more completeness. And as one of specialty apprentice doctor subject inside the hospital, the possibility of raising a quality improvement of study and research environment.

Changes of Rice Storage Proteins Affected by Dry and Storage Temperature (건조 및 저장조건에 따른 쌀 저장단백질의 변화양상)

  • Quality of rice grain changes during dry storage with internal physiological changes and external injury by organism. Storage rice changes by condition with respiration via variable temperature, hydrolysis enzyme reaction, lipid peroxidation occurs with change of palatability. During dry storage, physiological change with protein variation pattern was examined by image analysis on proteomic technology. Analysis revealed that protein activity had no change store at room temperature and store at $40^{\circ}C$, but decreased store at $60^{\circ}C$. Analysis of variable hydrophobic protein pattern revealed that protein activity of beta-tubulin, protein disulfide isomerase, vacuolar ATPase b subunit, globulin was not significantly decreased all dry and store condition. However, heat shock protein 70, and glutathione transferase was significantly decreased when rice dried at $60^{\circ}C$ compared with room temperature and $40^{\circ}C$ dry condition.

Flexural strengthening of continuous unbonded post-tensioned concrete beams with end-anchored CFRP laminates

  • This paper provides the results of an experimental investigation into the flexural behavior of continuous two-span unbonded post-tensioned high strength concrete (HSC) beams, strengthened by end-anchored CFRP laminates of different configurations in the hogging region. Implementing two different configurations of end-anchorage systems consisting of steel plates and bolts and carefully monitoring the development of strains throughout the load history using sufficiently large number of strain gauges, the response of beams including the observed crack propagations, beam deflection, modes of failure, capacity enhancement at service and ultimate and the amount of moment redistribution are measured, presented and discussed. The study is appropriate in the sense that it covers the more commonly occurring two span beams instead of the simply supported beams investigated by others. The experiments reconfirmed the finding of others that proper installation of composite strengthening system is most important in the quality of the bond which is essential for the internal transfer of forces. It was also found that for the tested two span continuous beams, the capacity enhancement is more pronounced at the serviceability level than the ultimate. This is an important finding as the design of these beams is mostly governed by the serviceability limit state signifying the appropriateness of the suggested strengthening method. The paper provides quantitative data on the amount of this capacity enhancement.
